RESUMO

Objective To compare the characteristics of coronary artery lesions and ventricular systolic, diastolic and synchronous function of non-ST-segment elevation (NSTEMI) with those of ST-segment elevation myocardial infarction (STEMI) in Chinese. Methods One hundred and sixty-eight consecutive patients with NSTEMI 116 and STEMI 52 were enrolled into this study. Coronary angiography (CAG) was performed within 72 hours in all patients, while the parameters of left ventricular function were measured by equilibrium radionuclide angiography (ERNA) with ventricular phase analysis (PA) within 1 week after acute myocardial infarction. RESUMO

The index on myocardial contractility of pure mitral regurgitation(MR) after mitral valve replacement is believed to be useful in determining proper operation time before irreversible myocardial damage by volume overload. Thus the authors examined pre and post-operative echocardiographic results of 20 cases of pure MR patients who had been admitted to Pusan National University hospital and compared the usefulness of each index. Pre and post-operative echocardiographic results were as follows respectively ; 1) Left ventricular internal dimension in diastole(LVIDd) were 6.49+/-0.19cm and 5.51+/-0.17cm. 2) Left ventricular internal dimension in systole(LVIDs) were 4.26+/-0.12 and 3.79+/-0.18cm. 3) Ejection fraction were 71.28+/-1.57% and 59.24+/-3.05%. 4) Fractional shortening(FS) were 34.09+/-1.18% and 27.21+/-1.84%. 5) Mean Vcf were 1.53+/-0.08cm/sec and 1.18+/-0.09cm/sec. 6) Left ventricular end-diastolic volume index(EDVI) were 196.35+/-18.33cc/m2 and 98.46+/-9.96cc/m2. 7) Left ventricular end-systolic volume index(ESVI) were 55.28+/-5.12cc/m2 and 41.88+/-6.07cc/m2. 8) Left ventricular end-systolic wall stress/ESVI(ESS/ESVI) were 2.3+/-0.21 and 3.91+/-0.83. ESS/ESVI showed significantly(p6cm, LVIDs>4cm, and ESS/ESVI<2, the prognosis was poor due to myocardial damage by mitral regurgitation.

RESUMO

In order to study the characteristics of the early cardiovascular responses after oleic acid induced lung damage, oleic acid (OA) 0.1ml/kg body weight was infused intravenously into 11 anesthetized and artificially ventilated dogs, and the effects were observed for 3 hours. After OA infusion, PaO2 decreased progressively to a fairly low level with a coincident increase of the alveolar-arterial PO2 gradient and decrease of the oxygen delivery. Cardiac index gradually decreased and reached only 49% of the baseline value at 180 minutes after OA injection. Mean systemic arterial pressure decreased transiently at 60 minutes after OA injection without concomitant increase of the pulmonary arterial pressure. The left ventricular stroke work, +dp/dtmax and - dp/dtmax, and the impedance contractility index were inhibited throughout the experiment. The postmortem lung-body ratio was significantly increased.These results suggest that the dogs following OA injection exhibit severe hypoxemia, reduced cardiae output, suboptimal cardiac performance and reduced oxygen delivery; the reduced cardiac outpur is associated with early and sustained inhibition of the myocardial contractility; and the pulmonary arterial pressure does not change significantly in the whole course of the experiment, so the edema formation does not result from the increase of the capillary hydrostatic pressure.

RESUMO

To find out the method of assessing the car-diac reserve function under the resting condition,we observed the changes of left ventricular con-tractility, relaxation and compliance, as well ascardiac pump function under the resting and over-loaded conditions with intact hearts of male rats invarious states of heart failure induced by variousadriamycin cumulative doses. The results of thepresent study indicated that the degree of my-ocardium injury was aggravated along with the in-crease of adriamycin cumulative doses. Mean-while, all of the parameters of left ventricular con-tractility, relaxation, compliance and cardiac re-serve function decreased significantly. However,the indexes of basic cardiac pump function in-creased to a certain extent because of the influenceof non-cardiac factors. The discriminant analysisshowed that the degree of heart failure assessed bymeans of the discriminant function, or synthesizingthe changes of left ventricular contractility, relax-ation and compliance under the resting condition,was almost consistent with that by means of thecardiac reserve function. In addition, the multipleregression indicated that the contractility, relax-ation and compliance had different effects on thecardiac reserve function, corresponding to the different states of heart failure.
