RESUMO

Toxicology, is a slow acting less potent poison, which does not kill the person immediately, but gets manifested in the form of many diseases. Skin diseases (Kustha) are one among the main symptoms of Dooshivisha. Vicharchika, is one among Kshudra kustha, which is highly prevalent, common and distressing condition. Recurrence and chronicity are the main threat faced by patients suffering from Vicharchika. Most of the features of Vicharchika show similarity with symptoms of the disease entity, eczema of modern dermatology. In India, the point prevalence of dermatological cases are 9.25%, and that of eczema is 6.75%. Arkatailam is the formulation prescribed in Yogaratnakaram, which reflects the therapeutic effect of toxic plant Arka, which is directly indicated for Vicharchika. The aims and objective of the study is to evaluate the combined effect of Arkataila and Patoladi gana kashaya in Dooshivishajanya Vicharchika. The study design was an interventional pre-post study with sample size 30. Patients with signs and symptoms of Vicharchika, having history of Dooshivisha, of age group between 20-60 satisfying the diagnostic criteria selected from the OPD of Government Ayurveda College Panchakarma, Hospital, Poojappura, Thiruvananthapuram, were included in the study. The study group was given Avipathychoorna internally on first day for Virechana. On 2nd day onwards Patoladi gana Kashaya was given internally and Arkataila externally for 14 days. Symptomatic assessments were done before treatment, on 8th day, after the treatment and after the follow up period of 7 days. Results were analyzed statistically. The results show that the effect of Arkataila and Patoladiganakashaya is very effective in Dooshivishajanya vicharchika.

Panchakarma is a specialized branch of Ayurveda, which deals with both aspects of Ayurvedic treatment which are purification as well as pacification of vitiated Doshas. Virechana (Purgation) is considered as Agra Chikitsa (main treatment) in all types of Kushtarogas (skin diseases) and also given Pradhanyata in Pittaja type of Kushtas. Vicharchika is a type of Kshudra kushta with Pitta and Kapha dosha pradhanyata and has the symptoms like Kandu (itching), Srava (discharge), Pidaka (eruption) and Shyavavarna twacha (blackish discoloration of skin). Vicharchika is often correlated with Eczema based on its clinical presentation. Acharya Kashyapa mentioned it as Sadhya vyadhis (curable disease). The main line of treatment of Vicharchika is Shodhana (purification) and Shamana (pacification). Among the Shodhana karma, Virechana is chiefly advised along with Shamanaushadi. We are presenting a case of Vicharchika, who made a remarkable improvement in the condition with these lines of treatment.

Skin disorders are most common occurring diseases in places where humidity prevails throughout the year. Vicharchika and Eczema both having resemblance in set of their clinical signs. Rasapushpadi Malhara (RPM) is Rasa preparation described in Rasa tarangini which is indicated in some skin disorders like Phirang, Vrna, Vicharchaika etc. In present study two samples of Rasa Pushpa (RP) has been prepared by two methods i.e. Kupipakva method and Damru yantra method. Both the samples were further used to prepare two samples of Rasapushpadi Malhara using Siktha taila as base and ratio of Siktha & Til taila was taken in ratio of 1: 5 according to the season. The randomized control clinical trial was carried out in between two groups. Both groups showed significant results on different clinical parameters like Kandu, Daha, Srava, Rukshta, Pidika, Vaivaranyata, Pidika, Raji, but group B showed highly significant result in EASI Score, which indicates severity of eczema. Level of significance in Group B for EASI Score is, P<0.00001. Rasapushpadi Malahara is very potent in treatment of Vicharchika as it contains mineral and herbal drugs like Parada, Kasisa, Saindhava, Sphatika, Wax, Tila taila, all having good antibacterial and antifungal properties. So this formulation showed significant results only by external application.
