Mechanism of Wenxin Granule Inhibits Myocardial Cell Apoptosis After Myocardial Infarctionby Regulating Endoplasmic Reticulum Stress in Rats / 中国实验方剂学杂志

Objective:To explore the intervention effect of Wenxin granule on endoplasmic reticulum stress apoptosis pathway and its related mechanism in rats with myocardial infarction. Method:Ligating the left anterior descending coronary artery to establish the rat model， the rats were randomly divided into 5 groups， namely sham group， model group， betaloc group and low， high-dose Wenxin granule groups，10 in each group. The sham group and the model group were given 10 mL∙kg^-1∙d^-1 deionized water， the low， high-dose modified Wenxin groups were given 1.35， 2.7 g∙kg^-1∙d^-1 aqueous solution respectively， and the betaloc group was given 2.25 mg∙kg^-1∙d^-1 aqueous solution. After 14 days，the catheter method was used to detect the cardiac hemodynamics and hematoxylin-eosin staining （HE） was used to observe the pathological morphological changes. The levels of endoplasmic reticulum stress protein glucose regulatory protein 78（GRP78）， protein kinase R like endoplasmic reticulum kinase（PERK）， phosphorylated activated PERK（p-PERK）， activated transcription factor 6 （ATF6）， nuclear transcription factor X cassette binding protein（XBP1） and apoptosis protein C/EBP homologous protein（CHOP）， B cell lymphoma/leukemia-2（Bcl-2） and Bcl-2 associated X protein（Bax） were detected by Western blot. Deoxyribonucleotide end-transferase-mediated notch end labeling （TUNEL） was also used to detect the myocardial cell apoptosis. Result:Compared with control group， the levels of the maximum ascending rate of left ventricular pressure（+dp/dt_max）， the maximum descending rate of left ventricular pressure（-dp/dt_max） and the left ventricular systolic blood pressure（LVSP） were decreased significantly（<italic>P</italic><0.05），but the level of the left ventricular end diastolic blood pressure （LVEDP） was significantly increased（<italic>P</italic><0.05）. The expressions of GRP78， p-PERK， PERK， ATF6， XBP1， CHOP， Bax and apoptosis index were increased significantly （<italic>P</italic><0.05，<italic>P</italic><0.01）， while Bcl-2/Bax and Bcl-2 were decreased significantly（<italic>P</italic><0.01） in model group. Compared with the model group， the levels of -dp/dt_max and Bcl-2 were increased significantly （<italic>P</italic><0.05），while the level of the apoptosis index was decreased significantly（<italic>P</italic><0.01） in low-dose Wenxin granule group. In high-dose Wenxin granule group， the levels of +dp/dt_max and -dp/dt_max were increased significantly（<italic>P</italic><0.05，<italic>P</italic><0.01）， the levels of endoplasmic reticulum stress pathway protein GRP78， p-PERK， PERK， ATF6， XBP1，apoptosis-related protein CHOP， Bax and apoptosis index were decreased significantly（<italic>P</italic><0.01）， but Bcl-2 and Bcl-2/Bax were increased significantly （<italic>P</italic><0.01）. Conclusion:Wenxin granule has effect in inhibiting excessive endoplasmic reticulum stress， reducing myocardial cell apoptosis and improving cardiac hemodynamics. Its molecular mechanism may be related to decrease the levels of GRP78， PERK， p-PERK， ATF6， XBP1， CHOP， Bax and increasing the expression of Bcl-2.

Effects of Wenxin granules and bisoprolol fumarate combined with aerobic exercise on serum matrix metallo-proteinase-1 and N-terminal pro-brain natriuretic peptide levels in patients with unstable angina pectoris / 中国基层医药

Objective To investigate the influence of Wenxin granules and bisoprolol fumarate combined with aerobic exercise on serum matrix metalloproteinase－1(MMP－1) and N－terminal pro－brain natriuretic peptide (NT－proBNP) levels in patients with unstable angina pectoris.Methods From January 2016 to February 2018, 92 patients with unstable angina pectoris in Lishui Hospital of Traditional Chinese Medicine were selected and randomly divided into control group(n=46) and study group(n=46) according to the digital table.On the basis of routine intervention,the control group was given bisoprolol fumarate for 2 weeks and aerobic exercise for 1 month, while the study group was given bisoprolol fumarate,aerobic exercise and Wenxin granules for 2 weeks.The frequency and duration of angina pectoris before and after 1 month of treatment,clinical efficacy,serum MMP－1 and NT－proBNP levels,blood rheology index(plasma viscosity,whole blood high shear viscosity,total blood low－cut viscosity,fibrinogen) and incidence of adverse reactions were observed.Results After 1 month of treatment,the frequency of angina pectoris was lower than that before treatment,and the duration was shorter than that before treatment,which in the study group [(2.09 ± 0.65)times/week,(2.24 ± 0.83)min/time] were better than those in the control group(t1=10.898,t2= 9.302,all P<0.05).The total effective rate of the study group(91.30% ) was higher than that of the control group (73.91% )(χ2 =4.842,P<0.05).After 1 month of treatment,the serum levels of MMP－1 and NT－proBNP in the two groups were lower than those before treatment,which in the study group [(196.10 ± 55.01) g/L,(403.82 ± 114.15)ng/L] were lower than those in the control group ( t1 =3. 619, t2 =4. 185, P <0. 05 ). At the end of treatment,the plasma viscosity,whole blood high－viscosity,whole blood low－cut viscosity,fibrinogen level in the two groups were lower than those before treatment,which in the study group were lower than those in the control group(all P<0.05).The incidence rate of adverse reactions in the study group was 15.22% ,which in the control group was 10.87% ,there was no statistically significant difference between the two groups (χ2 =0.383,P>0.05).Conclusion The combination of Wenxin granules, bisoprolol fumarate and aerobic exercise for patients with unstable angina pectoris not only can reduce the frequency of angina pectoris,MMP－1 and NT－proBNP levels,but also can improve the treatment of disease.Besides,it does not increase the risk of adverse reactions.

Effect of Wenxin granule combined with routine therapy on cardiac function, ventricular remodeling and local inflammatory response in patients with paroxysmal atrial fibrillation / 中国基层医药

Objective To explore the effects of Wenxin granule combined with conventional therapy on cardiac function,ventricular remodeling and local inflammatory response in patients with paroxysmal atrial fibrillation (PAF).Methods From February 2016 to February 2017,116 patients with PAF in the Integrated Traditional Chinese and Western Medicine Hospital of Taizhou were randomly divided into control group(55 cases) and combined group(61 cases) according to the digital table.The control group was treated with routine amiodarone hydrochloride tablets and enalapril maleate tablets,and the combined group was treated with Wenxin granule on the basis of the control group.The clinical efficacy of the two groups was compared,and the cardiac function,the level of inflammatory reaction and the index of ventricular remodeling before and after treatment were compared and analyzed in the two groups.The adverse reactions were observed and analyzed in the two groups.Results The total effective rate in the combined group was 93.44％,which was significantly higher than 76.36％ in the control group (x2 =6.745,P ＜ 0.05).After treatment,the left atrial diameter,the FS and CI levels in the combined group were significantly higher than those in the control group and before treatment (all P ＜ 0.05).The level of Pd in the combined group after treatment was (47.16 ± 0.80) ms,,which was significantly lower than that in the control group [(50.25 ± 0.75) ms] and that before treatment (t =21.395,48.318,all P ＜ 0.05).The levels of hs-CRP and BNP in the combined group were (7.12 ± 2.71) mg/L,(118.43 ± 11.16) ng/L,respectively,which were significantly lower than those in the control group [(11.21 ± 2.89) mg/L,(123.6 ± 11.27) ng/L] and before treatment (t =7.865,2.504,8.585,8.752,all P ＜0.05).After treatment,the LVEF level in the combined group was higher than that in the control group and before treatment (t =4.785,10.573,all P ＜ 0.05).The LVEDD and LVSV levels in the combined group were significantly lower than those in the control group and before treatment (t =9.089,2.313,10.890,4.299,all P ＜ 0.05).The total incidence rate of adverse reactions in the combined group was 6.56％,which was slightly lower than 7.28％ in the control group,but there was no statistically significant difference between the two groups(x2 =0.023,P＞0.05).Conclusion Wenxin granule combined with routine treatment in the treatment of PAF can significantly improve the clinical efficacy,improve the cardiac function obviously,delaye the ventricular remodeling,decrease the level of local inflammatory reaction significantly and the safety is high.

Curative effect of Wenxin Granule and antiarrhythmic drugs in the treatment of atrial fibrillation:a Meta-analysis / 中国中药杂志

To evaluate the efficiency and safety between Wenxin Granule and antiarrhythmic drugs in the treatment of atrial fibrillation(AF). A total of 8 major electronic databases(CNKI, WanFang, VIP, CBM, Cochrane Library, Web of Science, PubMed, EMbase) were retrieved since the establishment of the database to January 10, 2019. Two reviewers extracted data, and assessed the methodological quality of the included studies. The Meta-analysis was made by RevMan 5.3 software. Finally, 42 studies involving 4 657 patients were included. The results of Meta-analysis showed that compared with antiarrhythmic drug, the combined administration with Wenxin Granule and antiarrhythmic drug had a better clinical efficiency(OR=3.37, 95%CI[2.69,4.22],I~2=0%,P<0.000 01)and efficacy on cardioversion(OR=2.32,95%CI[1.67,3.22],I~2=0%,P<0.000 01), with reduction in P_d(MD=-5.48,95%CI [-7.32,-3.64],I~2=0%,P<0.000 01)and P_(max)(MD=-9.91,95%CI[-12.86,-6.95],I~2=0%,P<0.000 01). The comparison between the combined application with Wenxin Granule and the single application of amiodarone showed a clinical efficiency(OR=2.89,95%CI[1.96,4.26],I~2=44%,P<0.000 01),and efficacy on sinus rhythm maintenance(OR=2.58,95%CI[1.82,3.66],I~2=3%,P<0.000 01). The comparison between the combined application with Wenxin Granule and the single application of amiodarone showed a clinical efficiency(OR=0.88,95%CI[0.53,1.46],I~2=0%,P=0.63). The combined treatment with Wenxin Granule has a better clinical efficiency in AF better than amiodarone, with no evidence for more benefits from the single administration with Wenxin Granules.

Influence of Wenxin granule combined with trimetazidine on inflammatory factors and cardiac function in patients with chronic heart failure / 中国基层医药

Objective To investigate the influence of Wenxin granule combined with trimetazidine on inflam-matory factors and cardiac function in patients with chronic heart failure(CHF).Methods From January 2015 to November 2016,100 patients with CHF in Tengnan Hospital of Zaozhuang Mining Industry Group were chosen,and were divided into control group and research group.The control group was treated with trimetazidine,the research group was treated with Wenxin granule on the basis of the control group.After treatment for 6 months,the clinical effects,cardiac function,inflammatory factors and adverse reactions between the two groups were compared.Results The total effective rate of the research group was higher than that of the control group,the difference was statistically significant (90.00% vs.72.00%,χ2＝5.263,P＜0.05).Before treatment,the cardiac function between the two groups had no statistically significant differences(t＝0.062,0.185,0.511,all P＞0.05).After treatment,the LVEF of the research group was higher than that of the control group,the LEVSD and LVEDd of the research group were lower than those of the control group,the differences were statistically significant (t＝12.754,7.936,7.367,all P＜0.05).Before treat-ment,the inflammatory factors between the two groups had no statistically significant differences(t＝0.145,0.156,all P＞0.05).After treatment,the levels of IL－6 and TNF－αof the research group were lower than those of the control group,the differences were statistically significant (t ＝6.366,10.473,all P ＜0.05 ).During the treatment,all patients had no adverse drug reactions such as vomiting,dizziness,irritability,nausea and so on.There was no signifi-cant change in liver and kidney function and hematuria.Conclusion Wenxin granule combined with trimetazidine can furtherly improve the inflammatory reactions and cardiac function of patients with CHF,it is conducive to the improvement of clinical efficacy.

Influence of Wenxin granule combined propranolol on related hormone levels and cardiac function in patients with hyperthyroidism and atrial fibrillation / 心血管康复医学杂志

Objective : To observe influence of Wenxin granule combined propranolol on related hormone levels and cardiac function in patients with hyperthyroidism and atrial fibrillation (AF).Methods :A total of 136 AF patients with hyperthyroidism diagnosed in our hospital were randomly and equally divided into propranolol group and com-bined treatment group (received propranolol combined Wenxin granule ).Both groups received routine treatment for eight weeks also .Serum levels of free triiodothyronine (FT3) ,free thyroxine (FT4) and thyrotropic-stimulating hormone (TSH) ,blood pressure :SBP 、 DBP ,HR and LVEF before and after treatment were compared between two groups.Results :Compared with before treatment ,there were significant reductions in serum levels of FT 3 and FT4 ,SBP ,DBP and HR ,and significant rise in serum TSH level and LVEF after treatment ,in two groups , P＜0.05 or ＜0.01. Compared with propranolol group after treatment ,there were significant reductions in serum levels of FT3 [ (6.68 ± 1.94) pmol/L vs.(4.39 ± 1.34) pmol/L] and FT4 [ (28.67 ± 5.17) pmol/L vs .(22.67 ± 4.86) pmol/L] ,SBP [ (127.79 ± 10.86) mmHg vs.(121.76 ± 9.43) mmHg] ,DBP [ (81.65 ± 8.41) mmHg vs.(75.12 ±8.16) mmHg] and HR [ (85.67 ± 7.64) beats/min vs .(80.79 ± 6.47) beats/min] ,and significant rise in serum TSH level [ (1.28 ± 0.26) mU/L vs.(1.81 ± 0.34) mU/L] and LVEF [ (56.28 ± 5.12)% vs.(59.76 ± 4.25)%] in combined treatment group ,P＜0.05 or ＜0.01. Total effective rate of combined treatment group was significant-ly higher than that of propranolol group (95.59% vs.82.35%,P＝0.014).There was no significant difference in incidence of adverse reactions between two groups , P＝1.000 .Conclusion :Wenxin granule combined propranolol can significantly improve thyroid hormone levels and cardiac function in patients with hyperthyroidism ,which is worth extending .

New evidence supporting “intermittent pulse and severe palpitation” theory for prevention and treatment of complications of atrial fibrillation and heart failure / 中草药

The latest international clinical results show that heart failure and atrial fibrillation are two common heart diseases with extremely high coincidence. However, the existing therapeutic drugs and methods are all aimed at a single target of their respective diseases, ignoring the complex factors of complications. It is hopeful to find effective therapeutic drugs for arrhythmia and complications of heart failure from combination drugs, especially natural Chinese herbal medicines, based on the perspective of Chinese medicine. This article started with the existing clinical medicine and treatment methods for heart failure and arrhythmia, combed the traditional Chinese medicine prescriptions and single herb medicines for treating these two diseases, in which prepared licorice decoction and its derived modern Chinese medicines Wenxin Granule were systematically explored. The common targets of heart failure and arrhythmia and the prediction of the target of Wenxin Granule in the previous period were analyzed by network pharmacology, it was suggested that “intermittent pulse and severe palpitation” may be the comprehensive clinical manifestations of arrhythmia and heart failure. This article traced the origin and interpretation of the scientific connotation of “intermittent pulse and severe palpitation”.

Curative effect of bisoprolol fumarate combined with wenxin granule for patients with heart failure complicated with premature ventricular / 药物评价研究

Objective To explore the curative effect of bisoprolol fumarate combined with wenxin granule for patients with heart failure complicated with premature ventricular.Methods 132 patients with heart failure and premature ventricular complexes were enrolled in our hospital from January 2013 to January 2016,and which patients divided into two groups,bisoprolol fumarate group (Group BF,n =66) accepted bisoprolol fumarate treatment (5 mg/time,1 times/day),and bisoprolol fumarate combined with wenxin granule group (Group BW n =66) adopted wenxin granule (9 g/time,3 times/day) based on the patients in Group BF.After 4 weeks treatment,the curative effect of all patients were evaluated with electrocardiogram monitor and cardiac ultrasound examination,and the plasma NT-proBNP concentration and hemorheology indexes were analyzed compared with pre-treatment.Results The recovery of ventricular function in Group BW was better significantly than that patients in Group BF from the cardiac ultrasound examination (P ＜ 0.05);The total relieved effective of clinical symptom,premature ventricular complexes and heart failure in Group BW were more remarkable than those in Group BF,and which difference from them was significance (P ＜ 0.05);The blood viscosity,plasma viscosity,plasma fibrinogen and plasma NT-proBNP concentration in patients with Group BW were significantly lower than those Group BF (P ＜ 0.05).Conclusions The clinical curative effect of bisoprolol fumarate combined with Wenxin Granule for heart failure complicated with premature ventricular was more remarkable compare with bisoprolol fumarate single treatment,and which possessed the relative safety.

Curative Efficacy of Wenxin Granule Combined with Propafenone in Treatment of Arrhythmia and Serum Levels of hs-CRP, TNF-α and IL-6 and Heart Function / 现代生物医学进展

Objective:To study the curative efficacy of wenxin granule combined with propafenone in the treatment of arrhythmia and its effects on the serum levels ofhs-CRP,TNF-α,IL-6 and heart function.Methods:96 patients of arrhythmia who were treated from July 2014 to October 2016 in our hospital were selected and randomly divided into the observation group (n=48) and the control group (n=48).The control group was treated with propafenone,while the observation group was treated with wenxin granuleon.Then the serum levels of hs-CRP,TNF-α and IL-6,heart function,clinical efficacy and incidence of adverse reactions were observed and compared between the two groups before and after the treatment.Results:After treatment,the total effective rate of observation group was significantly higher than that of the control group (P＜0.05).The serum levels of hs-CRP,TNF-α and IL-6 of observation group were significantly lower than those of the control group (P＜0.05);the left ventricular end diastolic dimension (LVEDD) and left ventricular end systolic diameter (LVESD) of observation group were significantly lower than those of the control group,and the left ventricular ejection fraction (LVEF) was significantly higher than that of control group(P＜0.05);the incidence of adverse reactions in the observation group was significantly lower than that of the control group (P＜0.05).Conclusion:Wenxin granule combined with propafenone was effective for arrhythmia,it could effectively improve the heart function and high safety,which might be related to the decrease of serum hs-CRP,TNF-and IL-6 levels.

Efficacy and safety of Wenxin granule combined with metoprolol in the treatment of arrhythmia in elderly patients with coronary heart disease / 中国生化药物杂志

Objective To investigate the efficacy and safety of Wenxin Granule Combined with metoprolol in the treatment of arrhythmia in elderly patients with coronary heart disease.Methods84 cases of elderly patients with coronary heart disease arrhythmia treated in our hospital from April 2014 to May 2016, were randomly divided into observation group and control group, 42 cases in each group.The observation group was given the Wenxin Granule Combined with metoprolol treatment, the control group were treated with metoprolol,The changes of systolic status, the ischemic condition, hemodynamic changes and plasma viscosity were compared before and after treatment between the two groups, and the clinical effect after treatment were compared.ResultsAfter treatment, two groups of patients with atrial premature contraction, ventricular premature contractions and junctional premature contractions were decreased, the observation group was lower than the control group, the difference between the two groups was statistically significant (P<0.05);After treatment, two groups of patients with ischemic frequency were reduced, the observation group (5.45±1.12)times was lower than the control group (8.12±2.70)times, the difference between the two groups was statistically significant (P<0.05);After treatment, the two groups of patients with total ischemic time is shortened, the observation group (16.45±3.87)min shorter than the control group (22.45±4.89)min, the difference between the two groups was statistically significant (P<0.05);The total effective rate of the observation group was 92.85% higher than the control group (69.20%), the difference was statistically significant(P<0.05).ConclusionElderly patients with coronary heart disease arrhythmia patients with Wenxin granule combined with metoprolol treatment, can effectively improve the clinical symptoms of patients with significant effect.

Efficacy and safety analysis of combination use of Amiodarone and Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia / 中国生化药物杂志

Objective To investigate the efficacy of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia. Methods A total of 80 patients with unstable angina pectoris complicated with ventricular arrhythmias from February 2014 to May 2016 were randomly divided into the observation group and the control group, with 40 patients in each group. Two groups were treated with conventional dilatation vascular anticoagulation and antiplatelet treatment, on the basis of routine treatment, the observation group was given Amiodarone combined with Wenxin granule, and the control group was given Amiodarone plus conventional treatment, the effects of the treatment in two groups were compared. Results After a course of treatment, the average times within 24 hours of myocardial ischemia and angina occurred of patients in the control group were less than the control group, the cumulative dosage of nitroglycerin is less than the control group (P<0.05), at the same time, the control group during the follow-up period, the incidence of ventricular fibrillation and ventricular rate was lower than the control group. The incidence of adverse reactions was also lower than that of the control group. Conclusion Compared with the control group and the control group, the efficacy and safety of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmias are better than Amiodarone alone. After combination treatment, the times of partial ventricular arrhythmias and angina pectoris in patients were reduced with less adverse reactions and better safety, compared with single use of Amiodarone.

Efficacy and safety of amiodarone combined with Wenxin Granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia / 中国生化药物杂志

Objective To study the efficacy and safety of amiodarone combined with Wenxin Granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia. Methods 80 cases of unstable angina pectoris and ventricular arrhythmia in our hospital were selected, were randomly divided into early death and experimental group, 40 patients in each group. The two groups were treated with antiplatelet therapy, anticoagulant therapy and other symptomatic treatment. The control group was treated with amiodarone, and the experimental group was treated with amiodarone and Wenxin granule. The safety and clinical efficacy of two groups of angina attack times were compared and analyzed. Results The experimental group had ventricular fibrillation and ventricular premature beat probability were 2.50% and 5.00% lower than the control group, the control group had adverse reaction of nausea and vomiting was 11 cases significantly more than the control group, with statistical difference (P<0.05). The experimental group 24 h in myocardial ischemia times (11.34±3.45) times the number was significantly less than the control group (20.80±4.87), the control group during the course of angina frequency (24.50±5.67) was significantly higher than the experimental group, there was significant difference (P<0.05). Conclusion The combination of amiodarone and Wenxin granule is effective and safe in the treatment of unstable angina pectoris complicated with ventricular arrhythmia.

Efficacy and safety analysis of combination use of Amiodarone and Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia / 中国生化药物杂志

Objective To investigate the efficacy of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia. Methods A total of 80 patients with unstable angina pectoris complicated with ventricular arrhythmias from February 2014 to May 2016 were randomly divided into the observation group and the control group, with 40 patients in each group. Two groups were treated with conventional dilatation vascular anticoagulation and antiplatelet treatment, on the basis of routine treatment, the observation group was given Amiodarone combined with Wenxin granule, and the control group was given Amiodarone plus conventional treatment, the effects of the treatment in two groups were compared. Results After a course of treatment, the average times within 24 hours of myocardial ischemia and angina occurred of patients in the control group were less than the control group, the cumulative dosage of nitroglycerin is less than the control group (P<0.05), at the same time, the control group during the follow-up period, the incidence of ventricular fibrillation and ventricular rate was lower than the control group. The incidence of adverse reactions was also lower than that of the control group. Conclusion Compared with the control group and the control group, the efficacy and safety of Amiodarone combined with Wenxin granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmias are better than Amiodarone alone. After combination treatment, the times of partial ventricular arrhythmias and angina pectoris in patients were reduced with less adverse reactions and better safety, compared with single use of Amiodarone.

Objective To study the efficacy and safety of amiodarone combined with Wenxin Granule in the treatment of unstable angina pectoris complicated with ventricular arrhythmia. Methods 80 cases of unstable angina pectoris and ventricular arrhythmia in our hospital were selected, were randomly divided into early death and experimental group, 40 patients in each group. The two groups were treated with antiplatelet therapy, anticoagulant therapy and other symptomatic treatment. The control group was treated with amiodarone, and the experimental group was treated with amiodarone and Wenxin granule. The safety and clinical efficacy of two groups of angina attack times were compared and analyzed. Results The experimental group had ventricular fibrillation and ventricular premature beat probability were 2.50% and 5.00% lower than the control group, the control group had adverse reaction of nausea and vomiting was 11 cases significantly more than the control group, with statistical difference (P<0.05). Effect of Wenxin Granule on the Related Indexes of Senile Patients with Primary Hypertension and Acute Left Cardiac Failure / 中国药房

OBJECTIVE:To investigate the effects of Wenxin granule on the related indexes of senile patients with primary hy-pertension and acute left cardiac failure. METHODS:160 senile patients with primary hypertension and acute left cardiac failure were randomly divided into control group(80 cases)and observation group(80 cases). Control group received 12.5 mg captopril, orally,twice to 3 times a day (if patients had taken plenty of diuretic drugs,in low sodium and/or low blood volume,patients with normal or low blood pressure,the initial dose was 6.25 mg,3 times a day). Observation group additionally given 9 g of Wenxin granule,3 times a day. The treatment course for both groups was 4 weeks. SBP,DBP,PP,SDANN,SDNN-Index,RMS-SD,PNN50,improvement of cardiac functions before and after treatment,and incidence of adverse reactions in 2 groups were ob-served. RESULTS:After treatment,the SBP,DBP and PP in 2 groups were significantly lower than before,and observation group was lower than control group,SDNN,SDANN,SDNN-Index、RMSSD and PNN 50 were significantly higher than before,and ob-servation group was higher than control group,the differences were statistically significant(P0.05). CONCLUSIONS:Based on conventional treatment,wenxin granule can significantly reduce the bloop pressure of senile patients with primary hypertension and acute left cardiac failure,improve cardiac functions and inhibit autonomic disorders,with good safety.

Meta-analysis on efficacy and safety of Wenxin Granule combined with metoprolol for treatment of ventricular premature / 中草药

To evaluate the clinical efficacy and safety of Wenxin Granule combined with metoprolol for treatment of ventricular premature. We searched CNKI, Wanfang Database, VIP, Jiangsu Engineering Technology Literature Information Center, CCRCT, Medline, and Sciencedirect from 1995 to 2014. Meta analysis was performed using Rev Man 5.2 software. Twenty trials were involved, including 1 568 cases. The results showed that there was significant difference in the clinical symptom effectiveness of Wenxin Granule combined with metoprolol and metoprolol alone in the treatment of ventricular premature [RR=1.26, 95% CI (1.17, 1.36), P<0.00001], as with the clinical effectiveness [RR=1.22, 95% CI (1.16, 1.29), P<0.000 01] and the incidence of adverse drug reactions [RR=0.70, 95% CI (0.50, 0.98), P=0.04]. A subgroup analysis was done in the view of the severe adverse reaction. The results showed that there was significant difference in the incidence of atrioventricular block between the two groups [RR=0.20, 95% CI (0.04, 0.89), P=0.04], while no significant difference was found in the incidence of bradycardia [RR=0.54, 95% CI (0.26, 1.09), P=0.08]. The results of sensitivity analysis of final indicators were consistent with the fundamental analysis except the incidence of adverse drug reactions. Comparing with metoprolol alone, Wenxin Granule combined with metoprolol for treatment of ventricular premature can effectively improve the symptoms of patients, and significantly diminish the ventricular premature, while there is no significant difference in the incidence of total adverse drug reactions and bradycardia between the two groups. Also the difference in the incidence of atrioventricular block is with marginal significance. So the safety between the two groups may need further study after more samples be tested.

Effect of wenxin granule combined with metoprolol on blood rheology and clinlical efficacy of patients with hypertension and ventricular premature beat / 中国生化药物杂志

Objective To investigate effect of wenxin granule combined with metoprolol on blood rheology and clinlical efficacy of patients with premature hypertension heart disease.Methods 71 cases of hypertensive patients with hypertension and ventricular premature beat were selected, and randomly divided into a control group of 34 cases and observation group of 37 cases.The control group received metoprolol, the observation group on the base of the control group with wenxin granule.Blood rheology level, clinical efficacy and adverse reaction rate were compared before and after the treatment.Results Before treatment, high blood viscosity, whole blood low shear viscosity, plasma viscosity and fibrinogen levels had no significant difference in two groups of patients.After treatment, these indexs reduced compared with the same group pre-treatment(P<0.05);compared with the control group, the observation group had lower blood rheology(P<0.05), the total effective rate was higher(χ2 =8.60,P<0.05), and the incidence rate of adverse reactions was no difference.Conclusion Wenxin Granule combined with metoprolol in treatment of hypertension and ventricular premature beat patients, can improve effectively blood rheology of patients, and has low adverse reactions.

The clinical observation on Wenxin granule combined with paroxetine in the treatment of the elderly patients with depression after fracture / 国际中医中药杂志

Objective To observe the effect of Wenxin granule combined with Paroxetine in the treatment of the elderly patients with depression after fracture.Methods 80 patients with depression after fracture were randomly divided into a control group and a study group(n=40 cases,per group),and both groups were treated with the conventional treatment. On this basis, the control group was treated with Paroxetine,while the study group was treated with Wenxin granule combined with Paroxetine. The HAMD score and HAMA score were detected in the patients before the treatment and 4 and 8 weeks after the treatment separately.Results Before the treatment,there was no statistical significance in HAMD and HAMA score(t=0.512, 0.520,P>0.05). After treated for 4 and 8 weeks,compare with the control group,both HAMD score(t=1.828, 2.259,P<0.05) and HAMA score(t=1.819, 2.261,P<0.05) were decreased in the study group. After treatment for 4 and 8 weeks, compare with the control group, the total efficiency showed in the HAMD reduction rate of the study group was better(χ2=3.203, 4.370,P<0.05).Conclusions Wenxin granule combined with Paroxetine can significantly decrease HAMD and HAMA score,obviously improve the depression symptoms in the elderly patients with depression after fracture.
