RESUMO

Retinitis pigmentosa (RP) is a group of progressive and blinding hereditary fundus diseases characterized by damaged retinal photoreceptor cells and retinal pigment epithelium. With the clinical manifestations of night blindness and progressive visual field defect, RP has a high possibility of developing into blindness, which seriously affects the quality of life of the patients. The recent years have witnessed increasing studies about the pathogenesis and treatment of RP. By reviewing the relevant articles, we conclude that the pathogenesis of RP is mainly related to genes, and retinal blood perfusion, oxidative stress injury, and inflammatory cascade all affect the progression of this disease. The traditional Chinese medicine (TCM) therapies for RP mainly include TCM compound prescriptions, Chinese medicine extract, acupuncture combined with medicine, and comprehensive TCM treatment. The Western medicine therapies include gene therapy, stem cell therapy, optogenetic therapy, retinal prosthesis, drugs, treatment of complications and other therapies. The intervention mechanisms of traditional Chinese and Western medicine often involve gene modification, alternative therapy, improvement of retinal blood perfusion, antioxidant damage, and nutritional support. By summarizing the specific methods and effects of traditional Chinese and Western medicine in treating RP, we hope to provide a reference for the management and treatment of RP.

ObjectiveTo investigate the predictive indicators of early efficacy of Bushen Shengxue prescription combined with western medicine in the treatment of aplastic anemia, and provide prognosis indicators for the treatment of aplastic anemia (AA) with kidney-tonifying therapy in traditional Chinese medicine (TCM) combined with western medicine. MethodA total of 126 patients treated by Bushen Shengxue prescription combined with western medicine in 19 hospitals including Xiyuan Hospital of the China Academy of Chinese Medical Sciences from September 2018 to March 2021 were selected for a retrospective study. The therapy was proven to be effective after six months of treatment. According to the efficacy after 4 months of treatment, the patients were assigned into a 4-month effective group and a 4-month ineffective group. The age, sex, disease severity (including severe aplastic anemia and non-severe aplastic anemia), course of disease, degree of bone marrow nucleated cell proliferation, baseline hemogram levels [including white blood cell count (WBC), absolute neutrophil count (ANC), hemoglobin (HGB), platelets (PLT), and reticulocytes (RET)], T lymphocytes subsets, and the expression levels of T-box transcription factor (T-bet) and GATA-binding protein-3 (GATA-3) were compared between the two groups before treatment. ResultThe proportions of patients within the age ranges of [20, 40) and [60, 80) were higher in the 4-month effective group (P<0.05). The sex, disease severity, course of disease, and comorbidities had no significant differences between the two groups. The 4-month effective group had higher baseline levels of HGB, WBC, ANC, and PLT than the 4-month ineffective group (P<0.05), and there was no significant difference in the RET level between the two groups before treatment. Binary Logistic regression analysis showed that the PLT level before treatment was an independent factor affecting the onset time, while other indicators did not affect the onset time. The receiver operating characteristic (ROC) curve was established to analyze the value of PLT level before treatment for predicting the onset time, and the area under the curve was 0.691. With the critical value of 40.5×109/L, the sensitivity and specificity of the prediction that the therapy will take effect within 4 months were 0.569 and 0.893, respectively. The two groups of patients were graded according to age {(14, 20), [20, 40), [40, 60), and [60, 80)} and PLT level before treatment (PLT<40×109/L, PLT≥40×109/L). The proportion of the patients with PLT≥40×109/L before treatment in the 4-month effective group was significantly higher than that in the 4-month ineffective group (P<0.05). The degree of bone marrow nucleated cell proliferation before treatment had no significant difference between the two groups. The level of total T lymphocytes in the 4-month effective patients was lower than that in the 4-month ineffective patients before treatment (P<0.05). The levels of Th1 cells, Th2 cells, CD4+ T cells, and CD8+ T cells showed no significant differences between the two groups before treatment. The T-bet expression level in the 4-month effective group was higher than that in the 4-month ineffective group before treatment (P<0.05), while the expression level of GATA-3 showed no significant difference between the two groups before treatment. ConclusionBushen Shengxue prescription combined with western medicine will achieve faster effect for the patients within the age ranges of [20, 40) or [40, 60), with higher levels of HGB, WBC, ANC, and PLT (especially those with PLT≥40×109/L), lower level of total T lymphocytes, or higher T-bet expression level before treatment.

Postmenopausal osteoporosis (PMOP) is a systemic disease characterized by increased bone fragility caused by insufficient estrogen secretion in women after menopause,resulting in decreased bone mass and damage to the microstructure of bone tissues. The main clinical manifestations are low back pain,osteoporotic fractures,spinal deformities,and multiple organ dysfunction. PMOP directly leads to high morbidity, high mortality, and a decline in the quality of life. In addition to miss diagnosis, it is often not treated in time. In recent years, significant progress has been made in the research on factors related to the pathogenesis of PMOP. Based on the previous findings in recent years,this article described three major pathogenesis of PMOP, including intestinal flora imbalance,oxidative stress,and abnormal differentiation of bone marrow mesenchymal stem cells (BMMSCs), and analyzed the current status of PMOP treatment, such as syndrome differentiation and treatment,acupuncture and moxibustion,exercise therapy, and external treatment in traditional Chinese medicine (TCM), and basic measures,drug intervention,and physical therapy in western medicine. Among them,drug intervention in western medicine treatment is generally divided into bone resorption inhibitors,bone formation promoters,and other mechanism drugs according to the mechanism of action. This article summarized the specific methods and effects or mechanisms of TCM and western medicine in the clinical treatment of PMOP,which is expected to provide a reference for formulating reasonable health management models and drug treatments in the future.

@#AIM:To evaluate the clinical efficacy of different interventions of traditional Chinese medicine in the treatment of traumatic optic neuropathy by means of mesh Meta-analysis.<p>METHODS:The Computer searched of Chinese databases(CNKI, VIP, WanFang, SinoMed)and English databases(PubMed, Embase, Cochrane Library). Search time was limited from their inception to March 2020. Randomized controlled clinical trials of traditional Chinese medicine for treatment of traumatic optic neuropathy were subjected to Meta-analysis. Cochrane evaluation tool was used to evaluate the quality of the included studies and Stata 14.2 software was used to conduct network Meta-analysis.<p>RESULTS: Twenty-one RCT studies involving 1 297 patients were included and 7 interventions were involved. Among them, Chinese medicine, Chinese medicine+hormone drugs, acupuncture+Chinese medicine+neurotrophic drugs were better than those with neurotrophic drugs alone. Chinese medicine+hormone drugs, acupuncture+Chinese medicine+neurotrophic drugs, acupuncture+Chinese medicine+hormone drugs were better than that of hormone drugs alone(<i>P</i><0.05), and the intervention measures ranked first in terms of effective treatment rate is acupuncture+Chinese medicine+neurotrophic drugs.<p>CONCLUSION: Chinese medicine has advantages in the treatment of traumatic optic neuropathy. Among them, acupuncture+ Chinese medicine+neurotrophic drugs is most likely to be the best choice.

RESUMO

Cytokine storm is an excessive immune process when the body is stimulated. The disease progresses rapidly with a high mortality and complicated pathogenesis. In the outbreak of coronavirus disease 2019 (COVID-19), the rapid deterioration of the condition of patient is closely related to the outbreak of cytokine storm in the body. The sixth edition of Diagnosis and Treatment of Novel Coronavirus Pneumonia also provides a traditional Chinese medicine treatment plan. This paper reviewed the pharmacological effects of traditional Chinese medicine in antigen clearance, immune modulation and tissue protection based on the source, process and harm of cytokine storm. Furthermore, traditional Chinese medicine is contributive in prevention and treatment of cytokine storm, which can provide valuable medication guidance for the treatment combining traditional Chinese and Western medicine in clinic.

Objective: To evaluate the efficacy and safety of Qishen Yiqi dropping pills in the treatment of chronic heart failure. Method: Eight databases were searched at home and abroad, and the randomized controlled trials of Qishen Yiqi dropping pills in the treatment of chronic heart failure were screened. Literature quality of the included studies was evaluated by the bias risk assessment tool of Cochrane collaboration network, and the Meta-analysis was performed by RevMan 5.3 software. Result: A total of 606 related literature were retrieved, and 11 studies were eventually included, involving 931 patients. The quality of the included literature was generally low. Meta-analysis results showed that efficacy of Qishen Yiqi dropping pills combined with conventional western medicine was better than conventional western medicine alone in improving B-type natriuretic peptide[weighted mean difference (WMD)=-29.47, 95% confidence interval (CI) (-37.29,-21.64), PPPP=0.005], 6-minute walk test[WMD=50.13, 95%CI (22.32, 77.93), P=0.000 4]. In terms of safety indicators, one study intended to observe safety indicators, but it was not reported in the actual literature, and the remaining studies did not find adverse reactions of Qishen Yiqi dropping pills combined with conventional western medicine. Conclusion: The results of this study support the clinical use of Qishen Yiqi dropping pills combined with conventional western medicine in the treatment of chronic heart failure, but in view of the limitations in the quantity and quality of clinical studies, the positive results are only used as a reminder and reference for clinical diagnosis and treatment. In the future, more high-quality researches are needed to further confirm its efficacy.

Objective To explore the therapeutic effect of epinephrine combined with acupuncture on anaphylactic shock and its mechanism. Methods Sixty male Kunming mice were randomly divided into normal saline (NS) group, anaphylactic shock model group, and integrated traditional Chinese and Western medicine treatment group with 20 mice in each group. The anaphylactic shock model was reproduced by egg albumin infusion: intraperitoneal injection of 0.25 mL egg albumin (0.01 mmol/L), repeated injection 1 week later, and intravenous injection of 0.5 mL egg albumin through caudal vein on the 3rd week to induce anaphylactic shock. The mice in the NS group were injected with NS. The mice in the treatment group were immediately subcutaneously injected with 0.2 μg of 0.1% epinephrine, and intraperitoneally injected with aminophylline 0.2 mg, combined with acupuncture at Shuigou, Neiguan and Hegu points. Number of died mice in each group were observed at 1, 6, and 12 hours after model reproduction. The mice were sacrificed at 12 hours, the blood was harvested, and the serum tryptase, immunoglobulin E (IgE), tumor necrosis factor-α (TNF-α), interleukins (IL-1 and IL-6) were determined by enzyme linked immunosorbent assay (ELISA). The lung tissues were harvested, and the protein expressions of p65, phosphorylation of p65 (p-p65), and phosphorylation of nuclear factor-κB inhibitor α (p-IκBα) were determined by Western Blot. Results No mice died in the NS control group at 12 hours. In the treatment group, the mortality at 12 hours was significantly lower than that in the model group (10% vs. 80%, P < 0.01). The levels of tryptase and IgE in the model group were significantly higher than those in the NS control group [tryptase (μg/L): 1.53±0.28 vs. 0.91±0.23, IgE (μg/L): 33.3±3.1 vs. 21.3±1.9, both P < 0.01], both levels in the treatment group were significantly lower than those in the model group [tryptase (μg/L): 1.31±0.26 vs. 1.53±0.28, IgE (μg/L): 25.6±2.2 vs. 33.3±3.1, both P < 0.05]. The levels of TNF-α, IL-1 and IL-6 in the model group were significantly higher than those in the NS control group [TNF-α (ng/L): 35.3±4.7 vs. 16.4±3.5, IL-1 (ng/L): 13.8±3.3 vs. 4.2±1.8, IL-6 (ng/L): 15.3±4.8 vs. 5.5±2.1, all P < 0.01]. The serum inflammatory factors of the treatment group were significantly higher than those of the model group [TNF-α (ng/L): 26.1±4.3 vs. 35.3±4.7, IL-1 (ng/L): 7.2±2.7 vs. 13.8±3.3, IL-6 (ng/L): 8.8±3.8 vs. 15.3±4.8, all P < 0.05]. It was shown by Western Blot results that there was no significant difference in p65 protein expression of the lung tissue among the three groups. In the NS control group, the expression of p-p65 protein in the nucleus of the lung tissue was extremely low but was significantly increased in the model group, and p-p65 protein in the treatment group was significantly decreased as compared with that in the model group. The expression tendency of p-IκBα protein was consistent with that of p-p65. Conclusion Epinephrine combined with acupuncture plays a therapeutic role in mice with anaphylactic shock by inhibiting the activation of NF-κB signaling pathway.

In clinical work, many chronic prostatitis whose main clinical manifestations of sexual neurasthenia are misdiagnosed due to lack of typical lower urinary tract symptoms and pelvic floor pain. Therefore, attention is paid to prostate digital rectal examination and routine examination of prostatic fluid, and neuromodulators or psychotropic medicine should not be applied in the performance of sexual neurasthenia alone. Instead, integrated traditional Chinese and Western medicine treatment measures should be actively adopted to achieve satisfactory clinical results.

Objective To study the curative effects of strengthening spleen and tonifying lung Chinese medicine combined with western medicine treatment on 60 cases of acute stage of chronic obstructive pulmonary disease (AECOPD).Methods 60 cases of AECOPD were randomly divided into the study group(30 cases) and the control group(30 cases) by the method of random number table.The study group received strengthening spleen and tonifying lung Chinese medicine combined with western medicine treatment,and the control group only received western medicine treatment.The therapeutic effect,pulmonary function,SGRQ scores,6 MVD results and the clinical syndromes score were compared between the two groups.Results The cure relative indicators of the two groups showed no statistical difference before treatment (all P>0.05),and the effective rate in the study group (93.3%) was significantly higher than 63.3% in the control group (χ2=7.954,P=0.005).The results of 6 MVD of the two groups were significantly increased after treatment (all P<0.001),while the value of 6 MVD in the study group[(287±41)m]was significantly higher than (239±37)m in the control group after treatment (t=4.76,P<0.001).The SGRQ[(38.79±14.78) points]and the syndrome integral[(6.3±1.4) points]in the study group were lower than those in the control group (t=-2.439,-7.596,all P<0.05),while the syndrome integral of the two groups after treatment were significantly lower than before treatment (all P<0.001).The result of FVC of the two groups showed no statistical difference after treatment (t=1.169,P=0.247),while the levels of the FEV1(t=2.083,P=0.042) and FEV1 % (t=3.487,P=0.001) had statistically significant differences.Meanwhile,the FVC[(2.88±0.72)L]and FEV1[(1.91±0.5)L]in the study group were ascending after treatment (P<0.05),and the FEV1 % of the two groups were significantly increased after treatment (P<0.001).Conclusion Application of strengthening spleen and tonifying lung Chinese medicine combined with western medicine in the treatment of patients with AECOPD can significantly improve the lung function and reduce syndrome,which can improve the quality of life,with significant curative effects,so it is worthy of further clinical application and promotion.

Objective To explore the effect of warming yang for diuresis method combined with blood purification on the clinical therapeutic effect of patients with acute renocardiac syndrome (ARCS).Methods Sixty ARCS patients admitted to the Department of Intensive Care Unit (ICU) of Affiliated Hospital of Tianjin Institute of Chinese Medicine from May 2013 to October 2016 were enrolled, they were divided into an observation group and a control group by random number table, 30 cases in each group, one case died of multiple organ failure due to aggravation of infection, thus the actual observation number was 29 patients in the control group. The continuous vein-vein hemofiltration (CVVH) was applied in the two groups, 6-13 hours each time, and according to the disease situation, the continuous ultrafiltration time was determined, generally about 3-7 days; the observation group was additionally treated with warming yang for diuresis oral herbal decoction, 1 dose daily, 2 times a day, once 100 mL orally taken. The changes of cardiac and renal function indexes, traditional Chinese medicine (TCM) syndrome score and clinical efficacy were observed before and after treatment in the two groups.Results After treatment, the heart and renal function indexes were significantly improved, serum creatinine (SCr), urea nitrogen (BUN), N-terminal pro-brain nitric peptide (NT-proBNP) were all significantly lower than those before treatment in both groups, glomerular filtration rate (GFR), urine output, left ventricular ejection fraction (LVEF), cardiac output index (CI) were all significantly higher than those before treatment in both group; 7 days after treatment, BUN was significantly lower in observation group than that in control group (mmol/L:10.38±1.02 vs. 13.68±2.67), GFR, urine output, CI were significantly higher in observation group than those in control group [GFR (mL/min): 62.02±4.47 vs. 52.95±1. 92, urine output (mL/24 h): 875.41±44.26 vs. 537.82±79.65, CI (L·min-1·m-2): 3.12±0.51 vs. 3.07±0.17, allP < 0.05]; the effective rate of TCM syndrome score and clinical efficacy were significantly higher in observation group than those in control group [total effective rate of TCM syndrome score: 80.0% (24/30) vs. 55.17% (16/29), total effective rate: 93.33% (28/30) vs. 72.41% (21/29), both P < 0.05].Conclusions The warming yang for diuresis TCM decoction intake combined with CVVH can improve the cardiac and renal functions and clinical symptoms, and increase urine output for patients with ARCS.

Objective To observe the effective treatment of severe in peri-menopausal syndrome,and improve the quality of life of women in perimenopausall and postmenopausal women.Methods We review the different treatment methods in the 216 patients diagnosed with moderate to severe menopausal syndrome.Group A:simple hormone replacement therapy (HT) of 60 cases.Group B:simple treatment of traditional Chinese medicine of 90 cases.Group C:hormone replacement and comprehensive therapy of Chinese medicine of 66 cases.Follow up the improvement of symptoms and adverse reactions after the treatment one and three month respectively.Results The three groups after treatment in patients with clinical symptoms were improved after three month.Efficient comparisons among third group,the differences were statistically significant (90% (54/60),82% (76/90),100% (66/66);x2 =13.160,P < 0.01).Adverse reactions of 3 treatment group comparison difference had statistical significance (45% (27/60,1% (1/90),6% (4/66);x2 =60.720,P <0.01).Compared with pure hormone treatment group,treatment group of Chinese medicine,comprehensive treatment group adverse reactions happen was less,the difference was statistically significant(P<0.01).Chinese medicine treatment group and combined treatment group,there was no statistically significant difference (P >0.05).Conclusion Severe menopausal syndrome with hormone replacement therapy indications without contraindications suggestions take comprehensive treatment of traditional Chinese and Western medicine,The hormone supplement contraindications or unwilling to hormone replacement therapy,also has good effect of Chinese medicine treating.

Objective To observe the effect of Chinese medicine to wash, acupressure and Avene cream to moistur-ize for infantile eczema.Methods 100 babies who were diagnosed infantile eczema were chosen and separated to interven-tion group and control group randomly.The control group was treated by Chinese medicine to wash, acupressure and cream to moisturize, and the intervention group was treated by hormone medicine and Avene cream to moisturize.Observe the treat-ment effect after 7 days, 14days and 1 week after cured.Results The severity,effective rate and cure rate of two groups after 7days haven′t the statistical difference(p>0.05), and the severity of intervention group after 14days was lower than that of control group (p<0.05),the effective rate of intervention group after 14days was higher than and that of control group (p<0.05), and the recurrence rate of intervention group was lower than that of control group (p<0.05).Conclusion Combi-ning traditional Chinese and western medicine treatment can promote the treatment effect of infantile eczema , and reduce the recurrence, should be generalization.

Objective To study the therapeutic effect of Danshen injection combined with conventional therapy on children with recurrent asthmatic bronchitis. Methods A prospective study was conducted. Fifty-five children with asthmatic bronchitis admitted in People's Hospital of Henan Province from January 2012 to July 2014 were enrolled and randomly divided into a integrative Chinese and western medicine group(30 cases)and a conventional therapy group(25 cases);in the same period,18 healthy children having passed physical examinations were chosen as the healthy control group. In the conventional therapy group,rest,oxygen inspiration,anti-infection, fluid supplement and aerosol inhalation,etc. were given,while in the integrative Chinese and western medicine group,based on the similar conventional therapies,additionally Danshen injection 0.4 mL · kg-1 · d-1 was given,once per day for a total of 14 days. The wheezing remission time and the pulmonary rale extinction time were observed in the two patient groups,and the fractional exhaled nitric oxide(FeNO)content was investigated in the three groups. Results The wheezing remission time and the pulmonary rale extinction time of the integrative Chinese and western medicine group were obviously shortened compared with those in the conventional therapy group〔the wheezing remission time(days):2.3±0.8 vs. 5.2±1.1,the pulmonary rale extinction time(days):6.8±1.5 vs. 8.4±1.1,both P<0.01〕. The contents of FeNO in the integrative Chinese and western medicine group and the conventional therapy group before treatment were significantly higher than those in the healthy control group(×10-9:30.3±9.5,30.4±8.2 vs. 13.6±3.4,both P<0.01),but after treatment the FeNO levels of both patient groups were markedly lowered,and the degree of change in integrative Chinese and western medicine group was more significant(×10-9:18.6±4.7 vs. 24.0±6.8,P<0.01). Conclusion Danshen injection combined with conventional therapy can obviously down-regulate the FeNO content,alleviate the airway inflammation,effectively relieve the children recurrence of asthmatic bronchitis.

Objective To explore the clinical effect of traditional Chinese medicine and western medicine in treatment of patients with angina pectoris. Methods 120 cases with angina pectoris admitted in the Fourth Hospital of Baotou City from May 2012 to August 2013 were randomly divided into treatment group (n=60)and control group (n=60). All patients were given secondary prevention of coronary heart disease,and besides this, treatment group were added nitrates medications and traditional Chinese medicine treatment,while control group were only given nitrates medications.The treatment course was 14 days. The efficacy of angina pectoris,nitroglycerin loss rate and electrocardiogram efficacy were compared and the safety were evaluated between two groups. Results The efficacy of angina pectoris in treatment group was 90.00%,while control group was 71.67%,the difference was statistically significant(P<0.05 ). ECG efficiency in treatment group was 68.33%,while control group was 48.33%,the difference was statistically significant(P<0.05 ). The reduction of nitroglycerin was 86.67%,while control group was 61.67%,the difference was statistically significant(P<0.05). In addition,there were no adverse reaction in two groups. Conclusion Compared with nitrates medications,combined Chinese and Western medicine in treatment of patients with angina pectoris on basis of secondary prevention of coronary heart disease has better therapeutically effective rate.

@#Objective To observe the effect of dredging acupoints combined with Vojta training on spastic type cerebral palsy (CP).Methods220 children with spastic type CP were randomly divided into the treatment group and control group with 110 cases in each group. The children in the treatment group were treated with traditional Chinese medicine acupuncture and Vojta training, and those in the control group were treated with drug through intravenous dropping. The gross motor function of all children in two groups was assessed with Gross Motor Function Measure (GMFM) before and after treatment.ResultsThe GMFM scores of children in the treatment group were significantly higher than that of the cases in the control group ( P<0.001).ConclusionThe dredging acupoints combined with Vojta training can improve motor function of children with spastic type CP obviously.
