OBJECTIVE@#To investigate the efficacy and clinical results of total internal protection technique in anterior cruciate ligament reconstruction.@*METHODS@#A total of 56 patients undergoing anterior cruciate ligament reconstruction treated from January 2018 to December 2019 were selected. According to the different surgical methods, they were divided into total internal reconstruction group and standard bone tunnel group. There were 21 patients in the total internal reconstruction group, including 15 males and 6 females, aged from 20 to 48 with an average of (35.6±6.7) years old, and 35 patients in the standard tibial tunnel group, including 26 males and 9 females, aged 22 to 51 years old with an average of (33.7±9.6) years old. Preoperative examination of Lachman test was positive, magnetic resonance indicated anterior cruciate ligament rupture. There were no significant differences between the two groups in age, sex, body mass index, time from injury to ACL reconstruction, combined meniscus injury and operation method, operation time, ligament diameter, ligament length and other general information. Postoperative evaluation included operation duration, length and diameter of transplanted tendon after braid. International Knee Documentation Committee (IKDC) score, Lysholm score, Tegner score and perioperative complications 2 years after surgery.@*RESULTS@#Both groups were followed up, ranging from 24 to 30 months with an average of (26.9±3.4) months. Postoperative incision healing was good, and no failure or joint infection occurred at the last follow-up. There was no statistically significant difference between the two groups in IKDC score, Lysholm score and Tegner score before, 1 year and 2 years after surgery. However, IKDC score, Lysholm score and Tegner score at 1 year and 2 years after surgery.@*CONCLUSION@#The same postoperative function and stability of knee joint can be obtained by both the residual whole technique and the standardized reconstruction technique. In the residual whole group, only the semitendinosus muscle is taken, and the femoral thin muscle is retained, with greater tibial bone mass preserved, which is safe and effective in clinical practice.

OBJECTIVE@#To investigate short-term effectiveness of arthroscopic repair via modified subacromial viewing portal (hereinafter referred to as modified viewing portal) in treatment of LafosseⅠsubscapularis tendon tears.@*METHODS@#A clinical data of 52 patients with LafosseⅠsubscapularis tendon tears, who underwent the arthroscopic repair via modified viewing portal between October 2020 and November 2022 and met the selective criteria, was retrospectively analyzed. There were 15 males and 37 females with an average age of 63.4 years (range, 41-76 years). Twelve patients had trauma history and the other 40 patients had no obvious inducement. The main clinical symptom was shoulder pain and the hug resistance tests were positive in all patients. The interval between symptom onset and admission ranged from 3 to 26 months (mean, 7.2 months). The shoulder pain and function were evaluated by visual analogue scale (VAS) score, American Shoulder and Elbow Surgeons (ASES) score, and University of California Los Angeles (UCLA) score before operation and at 12 months after operation. The shoulder range of motion (ROM) of forward flexion, abduction, and external rotation and the internal rotation strength were measured before operation and at 3 and 12 months after operation. MRI was performed at 3-6 months after operation to assess the tendon healing and the structural integrity and tension of reattached tendon. Patient's satisfactions were calculated at last follow-up.@*RESULTS@#All incisions healed by first intention, no complication such as incision infection or nerve injury occurred. All patients were followed up 12-37 months (mean, 18.5 months). The VAS, UCLA, and ASES scores at 12 months after operation significantly improved when compared with those before operation ( P<0.05). The ROMs of abduction and forward flexion and the internal rotation strength at 3 and 12 months significantly improved when compared with those before operation ( P<0.05); and the ROMs at 12 months significantly improved compared to that at 3 months ( P<0.05). However, there was no significant difference ( P>0.05) in the ROM of external rotation at 3 months compared to that before operation; but the ROM at 12 months significantly improved compared to that before operation and at 3 months after operation ( P<0.05). Thirty-one patients underwent MRI at 3-6 months, of which 28 patients possessed intact structural integrity, good tendon tension and tendon healing; 3 patients underwent tendon re-tear. At last follow-up, 41 patients (78.8%) were very satisfied with the effectiveness, 7 were satisfied (13.5%), and 4 were dissatisfied (7.7%).@*CONCLUSION@#Arthroscopic repair via modified viewing portal for Lafosse Ⅰsubscapularis tendon tears, which can achieve the satisfactory visualization and working space, can obtain good short-term effectiveness with low overall re-tear risk.

RESUMO

Abstract Mucoid degeneration of the anterior cruciate ligament (ACL) is an uncommon cause of pain in the posterior region of the knee, of unknown pathophysiology and underdiagnosed. The best treatment modality is still under discussion. Resection of the lesion with partial ACL debridement has shown good results without the occurrence of instability. The authors present a case of mucoid degeneration of the ACL treated with resection of the mucoid degeneration and partial debridement of the ACL by arthroscopy.


Resumo A degeneração mucoide do ligamento cruzado anterior (LCA) é uma causa pouco frequente de dor na região posterior do joelho, de patofisiologia desconhecida e subdiagnosticada. A melhor modalidade de tratamento ainda está em discussão. A ressecção da lesão com desbridamento parcial do LCA tem apresentado bons resultados, sem a ocorrência de instabilidade. Os autores apresentam um caso de degeneração mucoide do LCA tratado com ressecção da degeneração mucoide e desbridamento parcial do LCA por artroscopia.

Abstract Objective To validate the low-cost model for arthroscopy training and analyze the acceptance and usefulness of the developed simulator in medical teaching and training. Method Ten medical students, ten third-year orthopedic residents, and ten shoulder surgeons performed predetermined tasks on a shoulder simulator twice. The parameters used were time to complete the tasks, number of looks at the hands, GOALS score (Global Operative Assessment of Laparoscopic Skills) and comparison between groups and within groups. An adapted Likert scale was applied addressing the individuals' impressions about the simulator and its applicability. Results In the intergroup comparison, the shoulder surgeons had better scores and times than the other groups. When the tasks were repeated, the group of surgeons had a 59% improvement in time (p < 0.05), as did the group of medical students. In the GOALS score, shoulder surgeons had consistently better scores than the other groups. And when we evaluated the evolution from the first to the second test, the group of surgeons and the group of academics had a statistically significant improvement (p < 0.05). In terms of lookdowns, there was a decrease in all groups. There was consensus that the simulator is useful in training. Conclusion The simulator developed allowed the differentiation between individuals with different levels of training in arthroscopic surgery. It was accepted by 100% of the participants as a useful tool in arthroscopic shoulder surgical training.


Resumo Objetivo Validar o modelo de baixo custo para treinamento em artroscopia e analisar a aceitação e utilidade do simulador desenvolvido no ensino e treinamento médico. Método Dez acadêmicos do curso de medicina, dez residentes do terceiro ano em ortopedia e dez cirurgiões de ombro realizaram tarefas pré determinadas em um simulador de ombro duas vezes. Os parâmetros utilizados foram o tempo para completar as tarefas, quantidade de olhares para as mãos, escore de GOALS (Global Operative Assessment of Laparoscopic Skills) e comparados entre os grupos e intragrupos. Uma escala de Likert adaptada foi aplicada abordando as impressões dos indivíduos acerca do simulador e de sua aplicabilidade. Resultados Na comparação intergrupos, os cirurgiões de ombro tiveram melhores escores e tempos que os demais grupos. Quando as tarefas foram repetidas, o grupo de cirurgiões, teve uma melhora de 59% no tempo (p < 0,05), assim como no grupo de acadêmicos. No escore de GOALS os cirurgiões de ombro apresentaram escores consistentemente melhores que os demais grupos. E quando avaliamos a evolução do primeiro para o segundo teste, o grupo de cirurgiões e o grupo de acadêmicos tiveram melhora estatisticamente significante (p < 0,05). No quesito de lookdowns houve diminuição em todos os grupos. Houve consenso em que o simulador é útil no treinamento. Conclusão O simulador desenvolvido permitiu a diferenciação entre indivíduos com diferentes níveis de treinamento em cirurgia artroscópica. Foi aceito por 100% dos participantes como uma ferramenta útil no treinamento cirúrgico artroscópico do ombro.

Introducción La aplicación de la recuperación acelerada en cirugía de hombro no ha tenido una aceptación tan favorable. Por ello, el objetivo de este estudio fue presentar y describir el uso de bloqueo interescalénico para favorecer la recuperación acelerada en una serie de pacientes sometidos a cirugía artroscopica de hombro. Métodos Se incluyeron 35 pacientes sometidos a cirugía artroscópica de hombro, en quienes se administró bloqueo interescalénico y sedación. Posteriormente se evaluó la intensidad del dolor, náuseas, vómito, disnea, presencia de síndrome de Horner, visión borrosa, ronquera, tiempo transcurrido hasta el alta, reingresos no planeados, satisfacción del paciente y cumplimiento de los criterios de alta hospitalaria en las primeras 12 horas siguiendo los criterios de una recuperación acelerada. Resultados En total, 27 pacientes (77,1%) tuvieron clasificación de la (ASA) I y 8 pacientes (22,9%) ASA II. Además, 97,1% fueron reparaciones de manguito rotador. Previo al alta, dos pacientes (5,7%) presentaron náuseas. Al momento del alta ningún paciente presentó disnea o visión borrosa, dos pacientes (5,7%) presentaron ronquera y la mediana de intensidad del dolor fue de 1,0 (0,0 a 7,0). Entre las 24 y 48 horas solo un (2,8%) paciente presentó náuseas y la mediana de intensidad del dolor fue de 1,0 (0,0 a 8,0). Todos los pacientes se mostraron satisfechos con disposición a repetir la experiencia. El 100% de pacientes cumplió los criterios médicos de alta a las 12 horas y 30 pacientes (85,7%) se dieron de alta el mismo día. La estancia fue de 12 (11,5 a 12,5) horas y ningún paciente reingresó. Conclusión En pacientes seleccionados, con un equipo quirúrgico-anestésico comprometido, capacitado y con experiencia, hay una alta posibilidad de que el bloqueo interescalénico favorezca la realización de programas de recuperación acelerada en cirugía artroscópica de hombro.


Introduction The application of enhanced recovery in shoulder surgery has not had such a favorable acceptance, therefore, the objective of this study was to present and describe the use of interscalene block to promote enhanced recovery in a series of patients undergoing shoulder arthroscopic surgery. Methods Thirty-five patients undergoing arthroscopic shoulder surgery were included, in whom interscalene blockade and sedation were administered. Subsequently, pain intensity, nausea, vomiting, dyspnea, presence of Horner's syndrome, blurred vision, hoarseness, time elapsed to discharge, unplanned readmissions, patient satisfaction, and compliance with hospital discharge criteria in the first 12 weeks were evaluated, hours following the criteria of an enhanced recovery. Results 27 patients (77,1%) had ASA I and 8 patients (22,8%) ASA II, 97,1% were rotator cuff repairs. Before discharge, two patients (5.7%) had nausea. At discharge, no patient had dyspnea or blurred vision, two patients (5.7%) developed hoarseness, and the median pain intensity was 1.0 (0.0-7.0). Between 24 and 48 hours only one patient (2.8%) presented nausea and the median pain intensity was 1.0 (0.0-8.0). All the patients were satisfied with their willingness to repeat the experience, 100% of the patients met the criteria for medical discharge after 12 hours, 30 patients (85.7%) were discharged the same day, the stay was 12 (11.5 to 12.5) hours, and no patient was readmitted. Conclusion In selected patients with a committed, trained and experienced surgical-anesthetic team, there is a high possibility that the interscalene block will favor the performance of enhanced recovery programs in shoulder arthroscopic surgery.

ABSTRACT Introduction: Total internal arthroscopic anterior cruciate ligament reconstruction is one of the new technologies in recent years. The main advantage is the need for only one tendon for the surgical procedure. Objective: Compare the clinical effects of total internal and traditional anterior cruciate ligament reconstruction techniques. Methods: From January 2019 to January 2022, the clinical data of 45 patients with anterior cruciate ligament reconstruction were retrospectively analyzed, including 32 males and 13 females aged 18-33 years, mean of 24.2 ± 3.3 years. Total internal reconstruction was performed in 22 cases (total internal group) and traditional reconstruction in 23 cases (traditional group). The two groups recorded and compared the time of injury, duration of surgical procedure, postoperative VAS score, and recovery of knee function. The International Knee Literature Committee (IKDC) and the Lysholm scoring system were used to evaluate clinical efficacy. Results: 45 patients were followed for 14 to 18 months, mean (15.4 ± 1.3) months. There were no significant differences between the two groups in time between operation and injury, duration of operation, IKDC, and Lysholm score of the affected knee at the last follow-up. However, there were significant differences in the VAS score on day one, day three, day seven, two weeks, and one month after the operation (P < 0.05), with no significant difference at three months, six months, and one year after the operation. Conclusion: The effect of total internal reconstruction of the anterior cruciate ligament is equivalent to that of traditional methods, with less postoperative pain, making it the ideal choice for this treatment. RESUMO Introdução: A reconstrução artroscópica interna total do ligamento cruzado anterior é uma das novas tecnologias dos últimos anos. A principal vantagem é a necessidade de apenas um tendão para o procedimento cirúrgico. Objetivo: Comparar os efeitos clínicos da técnica interna total e da técnica tradicional na reconstrução do ligamento cruzado anterior. Métodos: Entre janeiro de 2019 a janeiro de 2022, os dados clínicos de 45 pacientes com reconstrução do ligamento cruzado anterior foram analisados retrospectivamente, incluindo 32 homens e 13 mulheres, com idades entre 18-33 anos, média de 24,2 ± 3,3 anos. A reconstrução interna total foi realizada em 22 casos (grupo interno total) e a reconstrução tradicional em 23 casos (grupo tradicional). O tempo da lesão, duração do procedimento cirúrgico, escore de VAS pós-operatório e recuperação da função do joelho dos dois grupos foram registrados e comparados. O Comitê Internacional de Literatura de Joelho (IKDC) e o sistema de pontuação de Lysholm foram utilizados para avaliar a eficácia clínica. Resultados: 45 pacientes foram acompanhados durante 14 a 18 meses, média de (15,4 ± 1,3) meses. Não houve diferença significativa entre os dois grupos no tempo entre a operação e a lesão, a duração da operação, a pontuação do IKDC e Lysholm do joelho afetado no último acompanhamento. Porém, houveram diferenças significativas na pontuação do VAS no primeiro dia, terceiro dia, sétimo dia, duas semanas e um mês após a operação (P < 0,05), sem diferença significativa em três meses, seis meses e um ano após a operação. Conclusão: O efeito da reconstrução interna total do ligamento cruzado anterior é equivalente ao dos métodos tradicionais, com menor dor pós-operatória, sendo a escolha ideal para esse tratamento. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción: La reconstrucción artroscópica interna total del ligamento cruzado anterior es una de las nuevas tecnologías de los últimos años. La principal ventaja es la necesidad de un solo tendón para el procedimiento quirúrgico. Objetivo: Comparar los efectos clínicos de la técnica interna total y la técnica tradicional en la reconstrucción del ligamento cruzado anterior. Métodos: Desde enero de 2019 hasta enero de 2022, se analizaron retrospectivamente los datos clínicos de 45 pacientes con reconstrucción del ligamento cruzado anterior, incluidos 32 hombres y 13 mujeres, de entre 18 y 33 años, con una media de 24,2 ± 3,3 años. Se realizó una reconstrucción interna total en 22 casos (grupo interno total) y una reconstrucción tradicional en 23 casos (grupo tradicional). Se registraron y compararon el momento de la lesión, la duración del procedimiento quirúrgico, la puntuación postoperatoria de la VAS y la recuperación de la función de la rodilla de los dos grupos. Se utilizaron el Comité Internacional de Literatura sobre la Rodilla (IKDC) y el sistema de puntuación de Lysholm para evaluar la eficacia clínica. Resultados: 45 pacientes fueron seguidos durante 14 a 18 meses, con una media de (15,4 ± 1,3) meses. No hubo diferencias significativas entre los dos grupos en cuanto al tiempo transcurrido entre la operación y la lesión, la duración de la operación, la puntuación IKDC y Lysholm de la rodilla afectada en el último seguimiento. Sin embargo, hubo diferencias significativas en la puntuación de la VAS al primer día, al tercer día, al séptimo día, a las dos semanas y al mes después de la operación (p < 0,05), sin que hubiera diferencias significativas a los tres meses, a los seis meses y al año después de la operación. Conclusión: El efecto de la reconstrucción interna total del ligamento cruzado anterior es equivalente al de los métodos tradicionales, con menos dolor postoperatorio, lo que la convierte en la opción ideal para este tratamiento. Nivel de evidencia II; Estudios terapéuticos - investigación de los resultados del tratamiento.

Introducción: Las lesiones de la sindesmosis tibioperonea son frecuentes tanto de forma aislada o asociadas a fracturas del maléolo posterior. El diagnóstico clínico es corroborado mediante estudios imagenológicos. Los métodos de fijación pueden ser rígidos o dinámicos. Objetivo: Actualizar aspectos sobre las lesiones de la sindesmosis tibioperonea en cuanto al diagnóstico, métodos de fijación y el empleo de la vía artroscópica. Método: La búsqueda y análisis de la información se realizó en un periodo de 61 días (1 de octubre al 30 de noviembre de 2022) y se emplearon las siguientes palabras: syndesmosis injury, ankle syndesmosis instability, syndesmosis instability AND tight rope ankle instability, posterior maleolar fracture. A partir de la información obtenida se realizó una revisión bibliográfica de un total de 258 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote. Desarrollo: Se hizo referencia a las maniobras clínicas para el diagnóstico y las clasificaciones tomográficas en caso de fractura asociada del maléolo posterior. Se mencionaron los métodos de fijación rígidos y dinámicos, donde se describieron las características de cada uno basado en varios factores. Se expusieron las ventajas de la vía artroscópica tanto para el diagnóstico como para el tratamiento. Conclusiones: Las lesiones de la sindesmosis tibioperonea distal son frecuentes. Los estudios tomográficos ofrecen ventajas al determinar la extensión de la fractura del maléolo posterior. La vía artroscópica ofrece múltiples ventajas diagnósticas y terapéuticas.


Introduction: Tibioperoneal syndesmosis injuries are frequent both in isolation or associated with posterior malleolus fractures. Clinical diagnosis is confirmed on imaging studies. Fixation methods might be rigid or dynamic as well. Objective: Provide new aspects related to tibioperoneal syndesmosis injuries in terms of diagnosis, fixation methods and the use of arthroscopic procedure. Method: Search and analysis of information needed were carried out in a period of 61 days (from October 1 to November 30, 2022) and the following words were used: syndesmosis injury, ankle syndesmosis instability, syndesmosis instability AND tight rope ankle instability, posterior maleolar fracture. Based on the information obtained, a bibliographic review of the total of 258 articles published in the PubMed, Hinari, SciELO and Medline databases was carried out using the search engine and reference manager EndNote. Development: It was expressed in the study aspects related to clinical management used in diagnostic and classification on the types of CT scan for fracture associated with posterior malleolar fracture. The rigid and dynamic fixation methods and its characteristics were mentioned as well. The arthroscopic approaches and its advantages for both diagnosis and treatment were presented. Conclusions: Distal tibioperoneal syndesmosis injuries are frequent ankle injury. Tomographic studies offer some advantages for determining how large the posterior malleolus fracture is. The arthroscopic approach offers multiple diagnostic and therapeutic advantages too.


Introdução: As lesões da sindesmose tibiofibular são frequentes isoladamente ou associadas a fraturas do maléolo posterior. O diagnóstico clínico é confirmado por exames de imagem. Os métodos de fixação podem ser rígidos ou dinâmicos. Objetivo: Atualizar aspectos das lesões da sindesmose tibiofibular quanto ao diagnóstico, métodos de fixação e uso da abordagem artroscópica. Método: A busca e análise das informações foi realizada em um período de 61 dias (1º de outubro a 30 de novembro de 2022) e foram utilizadas as seguintes palavras: syndesmosis injury, ankle syndesmosis instability, syndesmosis instability AND tight rope ankle instability, posterior maleolar fracture. Com base nas informações obtidas, foi realizada uma revisão bibliográfica de um total de 258 artigos publicados nas bases de dados PubMed, Hinari, SciELO e Medline, utilizando o gerenciador de busca e Gerenciador de referências do EndNote. Desenvolvimento: Foi feita referência a manobras clínicas para diagnóstico e classificações tomográficas em caso de fractura associada do maléolo posterior. Foram citados métodos de fixação rígidos e dinâmicos, onde foram descritas as características de cada um com base em vários fatores. As vantagens da abordagem artroscópica tanto para o diagnóstico quanto para o tratamento foram discutidas. Conclusões: As lesões da sindesmose tibiofibular distal são frequentes. Os estudos tomográficos oferecem vantagens na determinação da extensão da fratura do maléolo posterior. A abordagem artroscópica oferece múltiplas vantagens diagnósticas e terapêuticas.

Introducción: Existe escasa información en la literatura nacional sobre los cuerpos libres articulares en la articulación de la rodilla. Estos ocurren como consecuencia de lesiones traumáticas, degenerativas, inflamatorias e isquémicas. Objetivo: Actualizar los conocimientos en los aspectos más generales de los cuerpos libres articulares en la rodilla y de su tratamiento mediante la vía artroscópica. Método: La búsqueda y análisis de la información se realizó en un periodo de 59 días (1 de enero al 28 de febrero de 2023) y se emplearon las siguientes palabras: foreing body AND knee, articular loose body AND knee, free body AND knee, locking knee arthroscopy AND locking. A partir de la información obtenida se realizó una revisión bibliográfica de un total de 211 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline. Se empleó el gestor de búsqueda y administrador de referencias EndNote. Del total se utilizaron 33 contribuciones seleccionadas para realizar la revisión, 32 fueron de los últimos cinco años. Desarrollo: Se hace referencia al diagnóstico positivo basado en los antecedentes, cuadro clínico e imagenología. En relación al diagnóstico diferencial de esta entidad se hace especial énfasis con todas las afecciones que producen bloqueo articular. En específico, se revisan las lesiones de menisco, así como las clasificaciones más empleadas según tamaño, origen, cantidad y movilidad. En relación al tratamiento artroscópico se describen las cuatro etapas que consisten en: identificación, atrapamiento, extracción y revisión. Consideraciones finales: La vía artroscópica por sus múltiples ventajas representa la modalidad quirúrgica más efectiva para el diagnóstico y tratamiento de pacientes con cuerpos libres articulares de la rodilla.


Introduction: Currently, there is a lack of information in the national literature concerning joint loose bodies in the knee joint. These occur as a consequence of traumatic, degenerative, inflammatory and ischemic injuries. Objective: To update knowledge on the most general aspects concerning joint loose bodies in the knee and the use of the arthroscopy procedure on its treatment. Method: Search and analysis of the information was performed on 59 days (January 1 to February 28, 2023) and the following keywords were used: foreing body AND knee, articular loose body AND knee, free body AND knee, locking knee arthroscopy AND locking. Based on the information obtained, a bibliographic review was made of a total of 211 articles published in the PubMed, Hinari, SciELO and Medline databases. The EndNote search manager and reference manager was used. Of the total of articles, 33 contributions selected for the review were used, 32 were published the last five years. Development: It was refered in the study on the positive diagnosis based on the history, clinical picture and imaging. In relation to the differential diagnosis of this entity, special emphasis is made up with all the conditions that produce joint blockage. Specifically, meniscal lesions are reviewed, as well as the most commonly used classifications according to size, origin, quantity and mobility. In relation to the arthroscopic treatment, the following four stages were described: identification, trapping, extraction and revision. Final considerations: The arthroscopic approach, due to its multiple advantages, represents the most effective surgical modality for the diagnosis and treatment of patients with joint loose bodies in the knee.


Introdução: Há poucas informações na literatura nacional sobre corpos articulares livres na articulação do joelho. Ocorrem como consequência de lesões traumáticas, degenerativas, inflamatórias e isquêmicas. Objetivo: Atualizar o conhecimento nos aspectos mais gerais dos corpos livres articulares no joelho e seu tratamento por via artroscópica. Método: A busca e análise das informações foi realizada em um período de 59 dias (1º de janeiro a 28 de fevereiro de 2023) e foram utilizadas as seguintes palavras: foreing body AND knee, articular loose body AND knee, free body AND knee, locking knee arthroscopy AND locking. Com base nas informações obtidas, foi realizada revisão bibliográfica de um total de 211 artigos publicados nas bases de dados PubMed, Hinari, SciELO e Medline. O mecanismo de busca EndNote e o gerenciador de referências foram usados. Do total, 33 contribuições selecionadas foram utilizadas para realizar a revisão, sendo 32 dos últimos cinco anos. Desenvolvimento: Refere-se ao diagnóstico positivo baseado na história, quadro clínico e imagiologia. Em relação ao diagnóstico diferencial desta entidade, é dada especial ênfase a todas as condições que causam bloqueio articular. Especificamente, são revisadas as lesões meniscais, assim como as classificações mais utilizadas quanto ao tamanho, origem, quantidade e mobilidade. Em relação ao tratamento artroscópico, são descritas as quatro etapas, que consistem em: identificação, encarceramento, extração e revisão. Considerações finais: A abordagem artroscópica, por suas múltiplas vantagens, representa a modalidade cirúrgica mais eficaz para o diagnóstico e tratamento de pacientes com corpos livres articulares do joelho.

Introducción: A pesar de su baja incidencia, las infecciones osteoarticulares pueden generar complicaciones y secuelas devastadoras para el niño en desarrollo con su esqueleto en crecimiento. El manejo general de la patología debe ser multidisciplinario, asociando la terapia antibiótica y un tratamiento quirúrgico adecuado si se requiere. El objetivo de nuestro trabajo es analizar los resultados sobre las diferentes opciones terapéuticas quirúrgicas (punción-aspiración, lavado artroscópico o lavado abierto) en el tratamiento de la artritis séptica en la edad, mediante la realización de revisión sistematizada de la bibliografía. Metodología: Realizamos una búsqueda bibliográfica online en los buscador PubMed y en la Biblioteca Virtual en Salud (BVS) utilizando los siguientes términos MESH: ("Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh)), utilizando filtros de búsqueda y aplicando criterios de inclusión y exclusión según nuestro objetivo. De los artículos incluidos se obtuvieron entre otros los siguiente datos: país, año, nivel de evidencia, número de pacientes con artritis séptica, articulación afectada, edad media, seguimiento promedio, tipo de tratamiento quirúrgico realizado de inicio y sus resultados según cada autor : complicaciones (de la enfermedad y del procedimiento ) y/o secuelas (en base a evolución clínica y radiológica), necesidad de un tratamiento quirúrgico adicional ante la falla del método terapéutico inicial y además de conclusiones relevantes de cada autor. Resultados: Se incluyeron un total de 22 artículos: 8 para el lavado por artroscopia, 6 para el lavado por artrocentesis, 4 para el lavado por artrotomía, ademas de otros 4 artículos que realizaba un análisis comparativo: dos de ellos entre lavado artroscópico y lavado por artrotomía y los otros dos entre lavado por artrocentesis y lavado abierto. El lavado artroscópico se encuentra en lo más alto de nuestra revisión ya que presenta los mejores resultados con solo un 6% complicaciones y/o secuelas (en solo 4 de los 8 artículos incluidos), con un total de 13 pacientes (8,7% ) que requirieron un tratamiento adicional (5 artrotomías y 8 un nueva lavado artroscópico ) y se destacan ademas 3 artículos con un 100% de buenos resultados, en los que no se presentaron complicaciones y/o secuelas, ni se requirió un segundo lavado, mientras que solo en 3 trabajos, de los 8, se necesitó de la artrotomía adicional para la resolución final. Para el lavado por artrocentesis se obtuvo el menor porcentaje de complicaciones y/o secuelas con un 2%, (reportado en tres de los 6 trabajos), y solo el 9,7% necesitaron de un segundo método terapéutico diferente a la aspiración articular (23 artrotomías y 2 artroscopias), destacando ademas la repetición de la punción (hasta 4 veces) como dentro de la directiva terapéutica del autor en dos artículos, y que para éste método se encontró el mayor número de pacientes. El lavado por artrotomía como método inicial obtuvo los peores resultados, con un 12% de complicaciones y/o secuelas y un 22,6% de lavados adicionales. Conclusiones: A pesar de que el lavado abierto por artrotomía se toma como el ¨ gold-standard ¨, teniendo indicaciones absolutas en el tratamiento quirúrgico de la artritis séptica en niños (especialmente en cadera), de nuestro análisis se desprende que ésta opción arroja los peores porcentajes de resultados, duplicando el porcentaje de necesidad de lavado adicional y el de complicaciones, en comparación con el lavado artroscópico. Existen otros métodos terapéuticos que se presentan como procedimientos menos invasivos, seguros y eficaces, respaldados por buenos resultados en la biografiaría, como son: el lavado por artroscopia (que presentó el porcentaje más bajo de pacientes con necesidad de lavado adicional, con el mayor número de artículos con buenos resultados en general, con un porcentaje de complicaciones por debajo del lavado por artrotomía) y el lavado por artrocentesis (que presentó el porcentaje de compilaciones más bajo de las 3 opciones analizadas.


Introduction: Despite its low incidence, osteoarticular infections can generate devastating complications and sequelae for the developing child with his growing skeleton. The general management of the pathology must be multidisciplinary, associating antibiotic therapy and adequate surgical treatment if required. The objective of our work is to analyze the results on the different surgical therapeutic options (puncture-aspiration, arthroscopic lavage or open lavage) in the treatment of septic arthritis in the pediatric age, by carrying out a systematic review of the bibliography. Methodology: We conducted an online bibliographic search in the PubMed search engine and in the Virtual Health Library (VHL) using the following MESH terms: ("Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh)), using search filters and applying inclusion and exclusion criteria according to our objective. The following data were obtained from the included articles, among others: country, year, level of evidence, number of patients with septic arthritis, affected joint, mean age, average follow-up, type of initial surgical treatment and its results according to each author: complications (of the disease and of the procedure) and/or sequelae (based on clinical and radiological evolution), the need for additional surgical treatment in the event of failure of the initial therapeutic method, and in addition to relevant conclusions of each author. Results: A total of 22 articles were included: 8 for arthroscopic lavage, 6 for arthrocentesis lavage, 4 for arthrotomy lavage, in addition to another 4 articles that performed a comparative analysis: two of them between arthroscopic lavage and lavage. by arthrotomy and the other two between lavage by arthrocentesis and open lavage. Arthroscopic lavage is at the top of our review as it presents the best results with only 6% complications and/or sequelae (in only 4 of the 8 articles included), with a total of 13 patients (8.7% ) that required additional treatment (5 arthrotomies and 8 a new arthroscopic lavage) and also highlight 3 articles with 100% good results, in which there were no complications and/or sequelae, nor did a second lavage be required, while that only 3 of the 8 studies required an additional arthrotomy for the final resolution. For lavage by arthrocentesis, the lowest percentage of complications and/or sequelae was obtained with 2% (reported in three of the 6 works), and only 9.7% required a second therapeutic method other than joint aspiration (23 arthrotomies and 2 arthroscopies), also highlighting the repetition of the puncture (up to 4 times) as within the author's therapeutic directive in two articles, and that the largest number of patients was found for this method. Arthrotomy lavage as the initial method obtained the worst results, with 12% complications and/or sequelae and 22.6% additional lavages. Conclusions: Even though open lavage by arthrotomy is taken as the ¨ gold-standard ¨, having absolute indications in the surgical treatment of septic arthritis in children (especially in the hip), from our analysis it can be deduced that this option yields the worse percentages of results, doubling the percentage of need for additional lavage and the percentage of complications, compared to arthroscopic lavage. There are other therapeutic methods that are presented as less invasive, safe and effective procedures, supported by good results in the biography, such as: arthroscopic lavage (which presented the lowest percentage of patients requiring additional lavage, with the highest number of articles with good results in general, with a percentage of complications below lavage by arthrotomy) and lavage by arthrocentesis (which presented the lowest percentage of compilations of the 3 options analyzed).

O manejo geral da patologia deve ser multidisciplinar, associando antibioticoterapia e tratamento cirúrgico adequado, se necessário. O objetivo do nosso trabalho é analisar os resultados das diferentes opções terapêuticas cirúrgicas (punção-aspiração, lavagem artroscópica ou lavagem aberta) no tratamento da artrite séptica em idade pediátrica, através da realização de uma revisão sistemática da bibliografia. Metodologia: Foi realizada uma busca bibliográfica online no buscador PubMed e na Biblioteca Virtual em Saúde (BVS) utilizando os seguintes termos MESH: ( "Arthritis, Infectious/surgery"(Mesh) OR "Arthritis, Infectious/therapy"(Mesh) ), utilizando filtros de busca e aplicando critérios de inclusão e exclusão de acordo com nosso objetivo. Os seguintes dados foram obtidos dos artigos incluídos, entre outros: país, ano, nível de evidência, número de pacientes com artrite séptica, articulação afetada, idade média, seguimento médio, tipo de tratamento cirúrgico inicial e seus resultados de acordo com cada autor: complicações (da doença e do procedimento) e/ou sequelas (com base na evolução clínica e radiológica), necessidade de tratamento cirúrgico adicional em caso de falha do método terapêutico inicial e, além das conclusões relevantes de cada autor. Resultados: Foram incluídos 22 artigos: 8 para lavagem artroscópica, 6 para lavagem artrocentese, 4 para lavagem artrotômica, além de outros 4 artigos que realizaram uma análise comparativa: dois deles entre lavagem artroscópica e lavagem por artrotomia e a outros dois entre lavagem por artrocentese e lavagem aberta. A lavagem artroscópica está no topo da nossa revisão por apresentar os melhores resultados com apenas 6% de complicações e/ou sequelas (em apenas 4 dos 8 artigos incluídos), com um total de 13 doentes (8,7%) que necessitaram de tratamento adicional (5 artrotomias e 8 uma nova lavagem artroscópica) e também destacam 3 artigos com 100% de bons resultados, nos quais não houve complicações e/ou sequelas, nem foi necessária uma segunda lavagem, enquanto que apenas 3 dos 8 estudos necessitaram de uma artrotomia para a resolução final. Para a lavagem por artrocentese, o menor percentual de complicações e/ou sequelas foi obtido com 2% (relatado em três dos 6 trabalhos), e apenas 9,7% necessitaram de um segundo método terapêutico diferente da aspiração articular (23 artrotomias e 2 artroscopias), destacando também a repetição da punção (até 4 vezes) conforme diretriz terapêutica do autor em dois artigos, e que o maior número de pacientes foi encontrado para este método. A lavagem por artrotomia como método inicial obteve os piores resultados, com 12% de complicações e/ou sequelas e 22,6% de lavagens adicionais. Conclusões: Apesar de a lavagem aberta por artrotomia ser tida como ¨padrão-ouro¨, tendo indicações absolutas no tratamento cirúrgico da artrite séptica em crianças (especialmente no quadril), da nossa análise pode-se deduzir que esta opção rende os piores percentuais de resultados, dobrando o percentual de necessidade de lavagem adicional e o percentual de complicações, em comparação com a lavagem artroscópica. Existem outros métodos terapêuticos que se apresentam como procedimentos menos invasivos, seguros e eficazes, corroborados por bons resultados na biografia, tais como: a lavagem artroscópica (que apresentou a menor percentagem de doentes que necessitaram de lavagem adicional, com o maior número de artigos com boa resultados em geral, com percentual de complicações abaixo da lavagem por artrotomia) e lavagem por artrocentese (que apresentou o menor percentual de compilações das 3 opções analisadas).

Introducción: Las fracturas osteocondrales que afectan la articulación de la rodilla se presenta de forma aislada o asociada a otras afecciones traumáticas. El diagnóstico inicial es en ocasiones difícil de realizar por la convergencia de signos clínicos e imagenológicos. Objetivo: Brindar información actualizada sobre los elementos más importantes de esta enfermedad traumática. Métodos: La búsqueda y el análisis de la información se realizó en un periodo de 31 días (del primero al 31 de agosto de 2021) y se emplearon las siguientes palabras del idioma ingles: osteochondral lesions, osteochondral fractures, osteochondral injuries a partir de la información obtenida se realizó una revisión bibliográfica de un total de 315 artículos publicados en las bases de datos PubMed, Hinari, SciELO y Medline mediante el gestor de búsqueda y administrador de referencias EndNote. Resultados: Se hace referencia al mecanismo de producción, aspecto artroscópico, localización, características en cuanto a extensión, profundidad, clasificación y desplazamiento. Se describen las modalidades imagenológicas empleadas en el diagnóstico y su utilidad. Con relación al diagnóstico diferencial se describen un grupo de enfermedades con características similares. Se mencionan las modalidades de tratamiento basadas en el tamaño y tiempo de evolución. Conclusiones: Las fracturas osteocondrales son lesiones frecuentes, para su diagnóstico se necesita de un alto índice de sospecha, ya que el diagnóstico clínico e imagenológico inicial no son concluyentes en la mayoría de los pacientes por estar combinada con otras afecciones.

The initial diagnosis is sometimes difficult to make due to the convergence of clinical and imaging signs. Objective: To provide updated information on the most important elements of this traumatic entity. Methods: The search and analysis of the information was carried out in a period of 31 days (from August the first to the 31st, 2021) and the following words of the English language were used: osteochondral lesions, osteochondral fractures, osteochondral injuries from the Information obtained, a bibliographic review of a total of 315 articles published in the PubMed, Hinari, SciELO and Medline databases was carried out using the EndNote search manager and reference manager. Results: Reference is made to the mechanism of production, arthroscopic appearance, location, characteristics in terms of extension, depth, classification and displacement. The imaging modalities used in diagnosis and their usefulness are described. Regarding the differential diagnosis, a group of entities with similar characteristics are described. Treatment modalities based on size and time of evolution are mentioned. Conclusions: Osteochondral fractures are frequent injuries, for their diagnosis a high index of suspicion is needed, since the initial clinical and imaging diagnosis are not conclusive in most patients because they are combined with other conditions.

ABSTRACT Objective: To describe a protocol of obtention of mesenchymal stem cells and to report their use as a biological adjuvant in three patients undergoing arthroscopic rotator cuff repair. Methods: Case series of patients who underwent arthroscopic repair of isolated full-thickness supraspinatus tear using mesenchymal stem cells obtained from the bone marrow as a biological adjuvant. All patients were operated on at the same institution, by a surgeon with 13 years of experience. The cells were applied at the end of the procedure, at the tendon-bone interface, at an approximate concentration of 2,000,000 mesenchymal cells/mm3 and a total volume of 5 ml. Results: All patients improved with the procedure, with one excellent and two good results. All cases overcame the minimally important clinical difference. All cases reached tendon healing, without partial or complete re-tears. We observed no complications. Conclusion: Arthroscopic rotator cuff repair with added mesenchymal cells obtained from bone marrow and submitted to a cell expansion process led to good functional results and healing in all cases in the sample, with no complications. RESUMO Objetivo: Descrever o protocolo de obtenção de células mesenquimais e relatar seu uso como adjuvante biológico em três pacientes submetidos ao reparo artroscópico do manguito rotador. Métodos: Série de casos de pacientes submetidos ao reparo artroscópico de rotura transfixante do músculo supraespinal utilizando como adjuvante biológico células mesenquimais obtidas da medula óssea. Todos ospacientes foram operados na mesma instituição por um cirurgião com 13 anos de experiência. As células foram aplicadas ao final do procedimento, na interface do tendão com o osso, na concentração aproximada de 2 milhões de células mesenquimais/mm3 e volume total de 5 ml. Resultados: Todos os pacientes melhoraram após o procedimento, havendo um resultado excelente e dois bons. Todos superaram a diferença clínica minimamente importante. Em todos os casos ocorreu cicatrização tendínea, sem a presença de rerroturas parciais ou completas. Não observamos complicações. Conclusão: O reparo do manguito rotador artroscópico com adição de células mesenquimais obtidas da medula óssea e submetidas a processo de expansão celular levou a bons resultados funcionais e cicatrização, sem complicações, em todos os casos da amostra. Objective:To compare the clinical efficacy of arthroscopic all-inside technique versus full-tibial tunnel technique for anterior cruciate ligament (ACL) reconstruction.Methods:Literature on all-inside versus full-tibial tunnel for ACL reconstruction was retrieved from databases including China National Knowledge Infrastructure (CNKI), Wanfang Data, Full-text Database of Chinese Medical Journals, PubMed, Web of Science, and Cochrane Library from their establishment to February 2023. Meta-analysis was performed on the included studies. Random effects model was used if heterogeneity was large, while fixed effects model was used if heterogeneity was small.Results:A total of 13 studies with 511 cases in the all-inside group and 465 cases in the full-tibial tunnel group were included in the meta-analysis, with a mean follow-up of 6-25.8 months. Meta-analysis showed that the graft diameter [ MD=0.42, 95% CI (0.11, 0.73), P=0.007] and International Knee Documentation Committee (IKDC) subjective score [ MD=1.11, 95% CI (0.36, 1.86), P=0.004] of the all-inside group was larger than full-tibial tunnel group, and the tibial tunnel widening of the all-inside group was less than full-tibial tunnel group [ MD=-1.70, 95% CI (-2.38, -1.01), P<0.001]. There were no significant differences in IKDC objective score [ OR=1.09, 95% CI (0.67, 1.78), P=0.730], Lysholm score [ MD=0.99, 95% CI (-0.08, 2.05), P=0.070], Tegner activity score [ MD=-0.01, 95% CI (-0.48, 0.45), P=0.950], bilateral knee anterior laxity difference [ MD=0.32, 95% CI (-0.22, 0.86), P=0.250], negative rate of pivot shift test [ OR=0.80, 95% CI (0.33, 1.91), P=0.610], graft re-rupture rate [ OR=0.76, 95% CI (0.34, 1.74), P=0.524] and return to sport rate [ OR=1.56, 95% CI (0.85, 2.86), P=0.150]. Conclusion:All-inside ACL reconstruction provides satisfactory efficacy. Compared with full-tibial tunnel technique, it has the advantages of larger graft diameter and less tibial tunnel widening, with no difference in joint function within 2 years postoperatively.

Rupture of anterior cruciate ligament (ACL) is one of the most common sports injuries of the knee, which may cause limitation of motor function and degeneration of the knee, seriously affecting patients' living quality. Currently, arthroscopic ACL reconstruction (ACLR) has been identified as the gold standard for ACL rupture. In an effort to further increase surgical accuracy and decrease surgical trauma so as to promotes quick recovery, all-inside ACLR has gained increasing attention in recent years. All-inside ACLR is as effective as traditional reconstruction, but it has benefits such as the preservation of tendons, preservation of bone mass, reduction of postoperative pain, prevention of synovial fluid leakage, expansion of the tendon-bone healing area, avoidance of complications related to extrusion screws, and suitability for children and adolescents. However, applying of numerous novel techniques for all-inside ACLR increases the learning curve, operating time, and cost. Besides, improper bone tunnel fabrication and graft fixation may lead to the widening of the bone tunnel, affecting the surgical effect. Therefore, in order to provide a theoretical foundation for further promoting the clinical application of this technique, this article reviews the development, evolution, and clinical outcomes of this technique as well as its advantages and disadvantages.

RESUMO

This study shows a case of a patient with synovial chondromatosis of the hip misdiagnosed as rice body bursitis. The patient complained of pain and limited activity in his left hip. He was diagnosed with synovial chondromatosis of the hip by medical history, physical examination, imaging examination and postoperative pathology. Based on literature review, the characteristics and differential diagnosis of the disease in epidemiology, imaging and pathology were discussed in detail, so as to improve the understanding of the disease and avoid misdiagnosis. He was treated with hip arthroscopy and obtained satisfactory therapeutic effect. The patient was followed up for 1 year without recurrence.

Objective:To investigate the efficacy of arthroscopic reduction and double-tunnel button fixation in the treatment of fractures of the tibial intercondylar eminence in children.Methods:From January 2019 to June 2022, 8 children with fracture of the tibial intercondylar eminence were treated at Orthopedics Department, Shenzhen Children's Hospital with arthroscopic reduction and double-tunnel button fixation. They were 6 boys and 2 girls, with an age of (8.4±1.9) years. Injured sites: 4 left knees and 4 right knees. Their operation time, intraoperative blood loss, knee flexion and extension, and complications were recorded. Their Lysholm scores were compared between preoperation and the last follow-up.Results:The operation time was (161.9±57.9)min. All patients were followed up for (12.1±5.7) months. The intraoperative blood loss was 3.5 (1.0,4.3)mL. Their knee flexion was 145.0°±5.0° and extension 0.9°±1.2° after operation. All their incisions healed at one stage, with no postoperative infection or other complications. Their preoperative Lysholm score [(20.3±15.8) points (from 2 to 54 points)] increased significantly to (99.3±1.0) points (from 98 to 100 points) after operation ( P<0.05). Conclusions:Arthroscopic reduction and double-tunnel button fixation is effective for fractures of the tibial intercondylar eminence in children, leading to limited complications, limited bleeding and a low infection rate.

Objective:To evaluate the clinical efficacy of arthroscopic side-to-side suture with remnants preserved in repair of transtendinous rotator cuff tears.Methods:A retrospective study was conducted to analyze the data of 17 patients who had been treated by arthroscopic side-to-side suture with remnants preserved for transtendinous rotator cuff tear caused by trauma at Sports Medicine Center, The Second Hospital Affiliated to Inner Mongolia Medical University from January 2017 to January 2020. There were 11 males and 6 females with an age of (47.9±8.3) years and a duration from injury to surgery of (50.4±21.3) d. Recorded were range of motion and muscle strength of the shoulder, University of California at Los Angeles (UCLA) shoulder function score, Constant-Murley shoulder function score, visual analogue scale (VAS) pain score, re-tears and complications before operation and at the last follow-up.Results:The 17 patients were followed up for (16.5±3.5) months after operation. Retear of the rotator cuff occurred in 2 patients after operation while MRI showed good healing of the rotator cuff in the other patients with no such postoperative complications as infection or wound dehiscence. At preoperation and the last follow-up, respectively, the range of shoulder flexion was 152.9°±8.5° and 172.4°±5.6°, the abductor muscle strength 3.5 (2.6, 4.1) kg and 6.9 (6.3, 8.3) kg, the external rotator muscle strength (3.8±1.0) kg and (5.9±1.6) kg, the internal rotator muscle strength 3.9 (3.4,4.7) kg and 5.2 (4.5,5.9) kg, the UCLA score (13.2±1.9) points and (30.9±2.4) points, the Constant score (40.1±2.8) points and (86.1±4.6) points, and the VAS score (6.7±0.8) points and (0.9±0.6) points, all showing a significant difference between preoperation and the last follow-up ( P<0.05). Conclusion:In repair of transtendinous rotator cuff tears, arthroscopic side-to-side suture with remnants preserved can lead to significantly improved clinical outcomes in range of motion, muscle strength, functional recovery and pain relief.

Objective:To compare the medium-term clinical effects of arthroscopic double row repair between traumatic and degenerative medium supraspinatus tear.Methods:A retrospective study was conducted to analyze the clinical data of 23 patients who had been treated for traumatic or degenerative medium supraspinatus tear by the same arthroscopic double row repair and postoperative rehabilitation at Sports Medicine Center, The First Hospital Affiliated to Army Medical University between January 2015 and August 2020. They were assigned into 2 groups according to different tears. In the traumatic group of 8 cases of traumatic medium supraspinatus tear, there were 5 males and 3 females with an age of (46.1±4.3) years and a tear size of (1.3±1.0) cm 2. In the degenerative group of 15 cases of degenerative medium supraspinatus tear, there were 4 males and 11 females with an age of (59.9±8.1) years and a tear size of (4.1±1.1) cm 2. At preoperation and the last follow-up, the shoulder pain was evaluated by visual analogue scale (VAS), and the shoulder function by American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley score and Simple Shoulder Test (SST); the improvements in active range of motion (ROM) of the shoulder were recorded at the last follow-up. Results:The 2 groups were comparable because there was no significant difference between them in the general clinical data ( P>0.05). The traumatic and degenerative groups were followed up for (40.3±11.2) and (36.4±12.4) months, respectively. At the last follow-up, the improvements in range of anterior flexion and internal rotation vertebral rank in the degenerative group [55.3°±33.6° and (4.1±1.3) ranks] were significantly greater than those in the traumatic group [27.5°±22.5° and (2.3±1.9) ranks] ( P<0.05). At the last follow-up, the VAS, ASES, Constant-Murley, and SST scores in the degenerative group were improved respectively by (3.7±0.8), (40.9±14.0), (38.4±9.4), and (6.5±1.4) points compared with their preoperative values, significantly greater than those in the traumatic group [(2.3±0.7), (19.6±14.6), (19.2±7.9), and (3.8±0.7) points] ( P<0.05). Conclusion:Arthroscopic double row repair can achieve significant medium-term improvements in shoulder function for both traumatic and degenerative medium supraspinatus tears, but the improvements may be grater for the degenerative ones.

Objective:To compare the clinical effectiveness between arthroscopic hollow screws combined with a suture anchor, hollow screws and proximal humerus internal locking system (PHILOS) in the treatment of split-type fractures of humeral greater tuberosity.Methods:A retrospective study was conducted to analyze the 54 patients with split-type fracture of humeral greater tuberosity who had been admitted to Department of Joint Surgery, Hospital of Traditional Chinese Medicine, Affiliated to Southwest Medical University from May 2015 to August 2020. There were 17 males and 37 females with an age of (58.4±12.1) years. According to different treatment methods, they were divided into 3 groups. Group A of 18 cases was treated with arthroscopic hollow screws combined with a suture anchor, group B of 18 cases with hollow screws, and group C of 18 cases with PHILOS. The length of surgical incision, and range of shoulder motion, visual analogue scale (VAS), and American Shoulder and Elbow Surgeons (ASES) score at the last follow-up were recorded and compared between the 3 groups.Results:There was no statistically significant difference in the preoperative general information between the 3 groups, indicating the 3 groups were comparable ( P>0.05). The surgical incision in group A [(0.7±0.1) cm] was the shortest, followed by (5.0±1.4) cm in group B, and (12.8±2.1) cm in group C, showing statistically significant differences in pairwise comparison ( P<0.05). In the 3 groups at the last follow-up, respectively, the shoulder forward flexion was 159.7°±13.4°, 154.9°±16.2°, and 160.5°±12.9°, and the shoulder abduction 149.6°±11.3°, 142.4°±12.0°, and 145.1°±10.4°, showing no statistically significant difference among the 3 groups ( P>0.05); the external rotation was 41.1°±8.1°, 38.1°±7.8° and 43.7°±6.2°, showing a statistically significant difference between groups B and C ( P<0.05); the dorsal extension was T 12 (L 5 to T 6), T 12 (L 5 to T 7), and T 12 (L 3 to T 6), showing no statistically significant difference among the 3 groups ( P>0.05). There was no statistically significant difference among the 3 groups in the VAS score or ASES score at the last follow-up ( P>0.05). Respectively, there were 2, 6, and 4 patients in groups A, B and C who developed complications, showing statistically significant differences between the 3 groups ( P<0.05). Conclusions:In the treatment of split fractures of humeral greater tuberosity, arthroscopic hollow screws combined with a suture anchor, hollow screws and PHILOS can all relieve pain and restore joint function of the shoulder. However, arthroscopic hollow screws combined with a suture anchor are the most recommendable due to their advantages in minimally invasiveness and reduction in complications.

Objective:To compare the clinical efficacy between reservation and sacrifice of remnants in the footprint area in arthroscopic repair of rotator cuff tear.Methods:A retrospective study was conducted to analyze the clinical data of 32 patients with rotator cuff tear plus remnants in the footprint area (2 cm < tear size <5 cm) who had been admitted to Department of Sports Medicine, The People's Hospital of Northern Jiangsu from May 2020 to July 2021. The patients were divided into 2 groups according to reservation or sacrifice of remnants in the footprint area in arthroscopic repair of rotator cuff tear. In the remnant-reservation group (16 cases): 5 males and 11 females with an age of (61.8±9.9) years, 9 left and 7 right shoulders affected, and (3.7±1.1) cm in size of rotator cuff tear; in the remnant-sacrifice group (16 cases): 4 males and 12 females with an age of (61.3±8.8) years, 8 left and 8 right shoulders affected, and (3.9±0.9) cm in size of rotator cuff tear. The 2 groups were compared in terms of visual analogue scale (VAS), American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley shoulder function score (Constant score), and range of motion of the affected shoulder before surgery, 3 months after surgery and at the last follow-up. The ratio of bilateral abductor muscle strengths (affected side/healthy side) was analyzed and compared between the 2 groups, and the healing of the rotator cuff was evaluated by MRI at the last follow-up.Results:The 2 groups were comparable because there were no significant differences in all their preoperative demographic data ( P>0.05). The 32 patients were followed up for (14.3±3.5) months after surgery. At 3 months after surgery, the VAS score in the remnant-reservation group [1.0 (0.0,1.0) point] was significantly lower than that in the remnant-sacrifice group [1.0 (1.0,1.0) point] ( P<0.05), but there was no significant difference between the 2 groups in ASES score, Constant score or range of motion of the affected shoulder ( P>0.05). At the last follow-up, the ASES score, forward flexion, abduction and ratio of bilateral abductor muscle strengths (affected side/healthy side) in the remnant-reservation group [(96.1±4.8) points, 170.0 (170.0,170.0)°, 160.0 (160.0,170.0)°, and 85.5%±13.8%]were significantly better than those in the remnant-sacrifice group [(91.4±5.9) points, 160.0 (160.0,170.0)°, 150.0 (140.0,155.0)°, and 72.6%±16.9%] ( P < 0.05), but there were no statistically significant differences between the 2 groups in VAS score, Constant score, neutral external rotation angle, or body-side internal rotation ( P>0.05). The Sugaya grading for MRI rotator cuff healing was significantly different between the 2 groups at the last follow-up ( P<0.05). Conclusion:In arthroscopic repair of rotator cuff tear, reservation of remnants in the footprint area can significantly relieve postoperative shoulder pain, and has obvious advantages in restoration of shoulder forward flexion, abduction and abductor muscle strength, leading to better healing of the rotator cuff and the large nodule than the remnant-sacrifice technique.

Objective:To evaluate the shoulder arthroscopic treatment of avulsion fracture of humeral greater tuberosity with double-row suture bridge.Methods:Retrospectively analyzed were the 13 avulsion fractures of humeral greater tuberosity which had been treated by shoulder arthroscopic double-row suture bridge at Bone and Joint Department, Guangzhou Liwan District Orthopedic Hospital from March, 2018 to March, 2020. There were 6 males and 7 females, with an average ages of 52.3 years (from 35 to 69 years). According to the Mutch classification, all the fractures of humeral greater tuberosity were attributed to the avulsion type. Of them, 3 were simple avulsion fractures of humeral greater tuberosity and 10 were complicated with shoulder anterior dislocation which was reduced manually before operation. There were 3 obsolete avulsion fractures of humeral greater tuberosity. Regular clinic and X-ray follow-ups were carried out. At the final follow-up, the pain, range of motion (ROM) and stability of the shoulder joint were assessed using visual analogue scale (VAS), American shoulder elbow scores (ASES), and Korean shoulder scores (KSS).Results:All the patients were followed up for an average of 12.3 months (from 10 to 16 months) postoperatively. No infection or shoulder instability was observed. At the final follow-up, the abduction and elevation averaged 164.6° (from 135° to 180°), the lateral external rotation 62.7° (from 40° to 80°), the internal rotation touch back test T10 level (from L2 to T6), the VAS 0.65 (from 0 to 2.5), the ASES 90.5 (from 78 to 100), and the KSS 91.5 (from 84 to 100).Conclusion:Shoulder arthroscopic treatment of avulsion fracture of humeral greater tuberosity with double-row suture bridge is minimally invasive and allows for repair of combined injury, leading to quick postoperative recovery.
