RESUMO

Urothelial carcinoma (UC) is the second most common genitourinary tumor, and upper tract urothelial carcinoma (UTUC) accounts for 5% to 10% of UC. Intravesical recurrence (IVR) occurs in 22% to 47% of patients after radical resection. This article reviews the mechanism, characteristics, related risk factors, regular bladder monitoring, diagnosis and treatment, and preventive measures of IVR, hoping to provide reference for clinical work.

RESUMO

Objective To evaluate the treatment effect of bladder instillation chemotherapy on glandular cystitis. Methods The clinical data of glandular cystitis of 54 cases were retrospectively analyzed, all cases were treated individually and the applications of chemotherapeutic drugs bladder instillation were not adopted. Results Follow-up period ranged from 2.5 to 9.0 years, and the average was 4.6 years. Lower urinary tract symptoms score before treatment, 3 month , 6 month, 2 years , 4 years after treatment was (8.5 ± 3.7), (5.7 ± 2.3), (3.9 ± 1.3), (4.0 ± 1.9), (4.2 ± 1.9) scores, and the scores after treatment were improved significantly compared with that before treatment (P < 0.05). Recurrence rate 3 months , 6 months, 2 years and 4 years after treatment was 3.7%(2/54), 13.0%(7/54), 5.6%(3/54) and 1.9%(1/54). Conclusions Eliminating the inducements and improvement of symptoms provides a significantly curative effect in glandular cystitis. Postoperative bladder instillation chemotherapy is not recommended.

Objective To discuss the clinical effect of early bladder instillation of pirarubicin after transurethral resection of bladder tumor and the psychological intervention measures. Methods 86 patients from August 2015 to December 2016 for transurethral resection of bladder tumor treated with pirarubicin(PHT) early intravesical therapy were divided randomly into experimental group 43 cases and control group 43 cases, control group in transurethral resection of bladder tumor after transurethral resection take the routine drug regimen, early implementation of pirarubicin therapy after surgery in the experimental group, to compare and analyze the recurrence of bladder cancer after operation in the two groups, and two groups of patients were given psychological nursing, to explore the implementation and effect of psychological nursing measures. Results The patients in the experimental group the recurrence rate was 9.3%, significantly higher than the control group 18.6%, the differences between groups was statistically significant (P<0.05); two groups of patients were given psychological nursing was found to improve patient psychological treatment, increase treatment compliance. Conclusion THP in transurethral resection of bladder tumor after star early bladder irrigation can effectively reduce the recurrence rate of bladder cancer after operation, psychological nursing in the treatment process, can enhance the tolerance in the treatment of patients, improve the treatment compliance, is worthy of clinical popularization and application.

RESUMO

Objective To investigate the effects of intravesical therapy with elemene in preventing postoperative recurrence of superficial bladder cancer and its toxic and side effects. Methods This series included 123 patients with superficial bladder cancer (T 1),consisting of transitional cell carcinoma GⅠ in 37 cases,GⅡ in 73 and GⅢ in 13.They all underwent surgical treatment. Postoperatively, they were randomly assigned to 2 groups:63 patients in elemene group received instillation of elemene (400 mg,once a week) 2 weeks after operation and 60 patients in mitomycin C (MMC) group received instillation of MMC(40 mg,once a week) 2 weeks after operation. The instillations were repeated for 6 weeks and thereafter monthly for 1 year.The recurrence rates,side effects,and NK cell activity before and after treatment were evaluated. Results The recurrence rate of elemene group (mean follow-up of 19.7 months) was 7.9% (5 cases),which was significantly lower than that (25.0%,15 cases) of MMC group (mean follow-up of 19.4 months;P

RESUMO

Bacillus Calmette-Guerin(BCG) has been widely used for the prophylaxis of superficial bladder tumor recurrence and for the treatment of bladder carcinoma in situ. More than 95% of patients who receive BCG instillation tolerate the treatment well and side reactions have been reported in less than 5% of patients. Most side effects are minor and self-limiting. However, a rear occurrence of severe systemic reactions have been reported. Among the severe systemic reactions, hypersensitivity pneumonitis should be considered in patients with pneumonic complications after BCG instillation in cases where the culture for mycobacteria is negative in the sputum, brochoalveolar lavage and blood specimen. In addition, a fiberoptic bronchoscopy with transbronchial lung biopsy demonstrates a fibrosis of the alveolar septums, where there is and an increased lymphocyte count without tuberculous inflammatory changes, the and CD4:CD8 ratio is increased and no symptomatic response to antituberculosis chemotherapy is observed. Here we report a 68 years old man with interstitial pneumonitis following intravesical BCG instillation.
