RESUMO

Objective. To validate the content of the indicators proposed from the Nursing Outcome Classification in a care plan for delirium management in older adults. Methods. Content validity study, conducted under the expert judgment technique. The procedure was developed in five moments: organization of indicators that respond to the nursing outcome classification for delirium management, support with literature of the indicators that responds to the result, selection of experts, establishment of agreements, and discussion. Quality criteria evaluated: pertinence and relevance, the Content Validity Coefficient and average scores assigned by the experts were calculated. Results. The study had the participation of 14 experts. The indicators, according to criteria of pertinence and relevance evaluated by experts showed a global average content index value of 0.93; 97.05% (66) of the indicators had Content Validity Coefficient > 0.75. Conclusion. The quantitative findings of the indicator validation process showed high relevance and pertinence index, which favors their being applied to measure care changes in patients with delirium.


Objetivo. Validar el contenido de los indicadores propuestos a partir de Nursing Outcome Classification en un plan de cuidados para el manejo de delirium en los adultos mayores. Métodos. Estudio de validación de contenido realizado bajo la técnica de juicio de expertos. El procedimiento se desarrolló en cinco momentos: organización de los indicadores que responden a la clasificación de resultados de enfermería para manejo de delirium, soporte con literatura de los indicadores que responde al resultado, selección de expertos, establecimiento de acuerdos y discusión. Criterios de calidad evaluados: pertinencia y relevancia. Se calculó el Coeficiente de Validez de Contenido, así como los promedios de los puntajes asignados por los expertos. Resultados. Se contó con la participación de 14 expertos. Los indicadores según los criterios de pertinencia y relevancia evaluados por expertos mostraron un valor global promedio de índice de contenido de 0.93. El 97.05% (66) de los indicadores presentaron Coeficiente de Validez de Contenido mayor a 0.75. Conclusión. Los hallazgos cuantitativos del proceso de validación de los indicadores mostraron alto índice de relevancia y pertinencia lo que favorece que puedan ser aplicados para medir cambios de cuidado en los pacientes con delirium.

Estudo de validação de conteúdo, realizado através da técnica de julgamento de especialistas. O procedimento foi desenvolvido em 5 momentos: organização dos indicadores que respondem à classificação dos resultados de enfermagem para manejo do delirium, suporte com literatura dos indicadores que respondem ao resultado, seleção de especialistas, estabelecimento de acordos e discussão. Foram calculados os critérios de qualidade avaliados: relevância e pertinência, o Coeficiente de Validade de Conteúdo e as médias das notas atribuídas pelos especialistas. Resultados. Participaram 14 especialistas. Os indicadores segundo os critérios de relevância e pertinência avaliados por especialistas apresentaram valor médio do índice de conteúdo global de 0.93. O 97.05% (66) dos indicadores apresentaram Coeficiente de Validade de Conteúdo superior a 0.75. Conclusão. Os achados quantitativos do processo de validação dos indicadores apresentaram alto índice de relevância e pertinência, o que favorece sua aplicação para mensurar mudanças no cuidado de pacientes com delirium.

@#Introduction: Organizational commitment is manifested in the forms of taking pride in organization, enhanced motivation for engaging in organizational duties, staying at organization for a longer time and knowing organization as a valuable organization. So decreased organizational commitment among medical staff led researchers to investigate the organizational commitment of CCUs in Mazandaran Heart Center. Methods: This is a descriptive-analytical study carried out in teaching hospitals of Mazandaran University of Medical Sciences, Heart Center. Allen and Meyer's organizational commitment questionnaire was used to collect data from 90 nurses selected by census from CCU nurses. Data was analyzed using SPSS 21. Results: Of studied cases, 19 (18.9%) were male and 73 (81.1%) were female with a mean age of 7.25±34.4. Among age, job experience and work hour variables, only age and job experience were considered to be significant. Conclusions: Supervisors and managers should promote nurses' organizational commitment through giving facilities and rewards.

OBJECTIVE: To analyze the current status of core emergency response capability of nurses for systematically managing coronavirus disease(COVID-19) in intensive care units and to explore the related influencing factors. METHODS: A total of 100 nurses who systematically manage COVID-19 in the intensive care units from a class 3 grade A hospital in Wuhan City were selected as study subjects by convenience sampling method. The self-designed COVID-19 Core Emergency Response Capability Questionnaire for Nurses was used to investigate the status of COVID-19 core emergency response capability. RESULTS: The total score of the core emergency capability of the nurses averaged 133.1±18.1, and the scoring rate was 80.7%, which was at a high level. The scoring rates of each dimension from high to low were prevention ability, rescue ability and preparation ability, with the score rate of 85.2%, 81.7% and 71.0% respectively. The results of generalized linear regression analysis showed that the level of core emergency response capability of nurses was higher in high-risk departments such as respiratory department, infectious department, intensive care unit, outpatient department and emergency department than that of low-risk departments [partial regression coefficient(β) =11.18, P<0.01]. The levels of core emergency response capability of nurses who were married, once worked in the fever clinic, and were members of the National Health Emergency Rescue Team was higher(β were 8.28, 24.34 and 10.89, respectively, P<0.05). CONCLUSION: The core emergency response capability of nurses who systematically manage COVID-19 in the critical care units in Wuhan City is at a high level. Their core emergency response capability is affected by factors such as personal work experience, marital status, emergency work experience and trainings.

Resumen La insuficiencia renal aguda se define como la pérdida de función del riñón ocasionada por diversas causas, desde la infección hasta la ingesta de fármacos, esta entidad sigue teniendo alta morbilidad y mortalidad en las unidades de cuidados críticos, el tratamiento de esta enfermedad va desde la propia protección renal hasta la sustitución artificial de las funciones del riñón lesionado; en la actualidad la terapia de reemplazo renal continua se ha utilizado como soporte renal, ofreciendo mayor estabilidad clínica a los pacientes más inestables, en esta revisión se comentan conceptos e indicaciones, así como el método de programación.


Abstract Acute renal failure, defined as the loss of kidney function caused by various causes, from infection to the intake of drugs, has still a high mortality in critical care units, the therapy of this disease is from its own protection to the artificial kidney replacement of injured kidney functions, currently the continuous renal replacement therapy has been used as a way of renal support, providing greater clinical stability in the most unstable patients, this review discusses concepts and indications, as well as the programming method.
