RESUMO

Objective:To investigate the role of pro-inflammatory cytokine IL-17 involved in the pathogenesis of cytomegalovirus hepatitis in vivo.Methods:First of all,disseminated infection model was established.Then,mice were randomly divided into 4 groups:normal control group,MCMV-infected control group,IL-17 blockade group,and isotype control group.Mice were sacrificed on day 7 after infection.The levels of IL-17 protein were detected by Western blot.Hematoxylin eosin (HE) staining was performed to evaluate the pathologic change of the liver.Serum ALT levels were detected by a Roche DPPI biochemical analyzer.The level of serum IL-17 was measured by double antibody sandwich ELISA.The expressions of mRNA of IL-17R,IFN-γand IL-10 in liver were detected by RT-PCR.Results:Compared with MCMV-infected mice and isotype control,the blockade of IL-17 inhibited the expression of IL-17 protein in liver (P<0.05).The degree of liver damage reduced obviously.The serum ALT was significantly lower [(146±15)vs (102±11)vs (37±12),P<0.05].The level of serum IL-17 was relatively reduced[(719.76±6.06)vs (722.1±4.62) vs (707.53 ±8.58),P<0.05].The expression of IFN-γmRNA [(0.56± 0.06)vs (0.55±0.13)vs (0.96±0.2),P<0.05] and IL-10 mRNA[(0.55±0.073) vs (0.51 ±0.07) vs (0.903 ±0.18),P<0.05] increased significantly,while that of IL-17R did not change apparently[(0.81±0.16)vs (0.89±0.38) vs (0.87±0.23),P>0.05].Conclusion:The increased expression of pro-inflammatory cytokine IL-17 is involved in the pathogenesis of immune injury in cytomegalovirus hepatitis.The blockade of IL-17 is helpful to relieve the liver damage and improve the liver function.

RESUMO

Objective To explore the clinical value of combined detection of human leucocyte antigen G (HLA-G) and interleu-kin10(IL-10) in children patients with cytomegalovirus(CMV) hepatitis .Methods 122 children patients with CMV hepatitis in our hospital from January 2013 to January 2017 were selected as the children patients group and contemporaneous 116 healthy children were selected as the control group .The levels of plasma HLA-G and IL-10 were detected by adopting ELISA .Then their correlation and diagnostic value were analyzed .Results The levels of plasma HLA-G and IL-10 in the children patients group were significant-ly higher than those in the control group ,and the difference was statistically significant (P<0 .05) .In grouping according to the age of the month ,the levels of plasma HLA-G and IL-10 in the 1-6 months old patients group were highest .HLA-G and IL-10 had a positive correlation(r=0 .445) .Moreover ,the receiver operating characteristic curve analysis results showed that the area under the curve of combined detection of plasma HLA-G and IL-10 was maximal and its diagnostic efficiency was highest .Conclusion The combined detection of plasma HLA-G and IL-10 can increase the accuracy of CMV infection diagnosis and has an important clinical significance .

Obgective To investigate the clinical effect of Adenine arabinoside monophosphate (Ara-A) on the treatment of infant cytomegalovirus hepatitis.Methods One hundred cases of infants with cytomegalovirus hepatitis in the Third Affiliated Hospital of Zhengzhou University from January 2012 to October 2013 were included and divided into 2 groups:Am-A group treated with Ara-A [a course of treatment lasting for 2 months included 10 mg/(kg · d) for first 2 weeks followed by 2 weeks' interval,and then resumed],and then control group was given ganciclovir [10 mg/(kg · d) for 14 days and 5 mg/(kg · d) for 1 week after 1 week's interval,for a total treatment period of 1.5 to 2.0 months].Both groups were given conventional therapy.Both before and after treatment,liver function,time of jaundice and transaminase back to normal,quantification of viral DNA returns to negative,side effects,hospitalization time and cost were also compared.Results After 2 weeks,alanine aminotramferase(ALT) in Ara-A group was significantly lower than that of the control group,and there was significant difference (P <0.05).After 2 months,ALT,aspartate transaminase in Ara-A group were significantly lower than those in the control group (all P < 0.05).Time of transaminase back to normal [(38.5 ± 16.7) d] was significantly reduced compared with the control group [(44.3 ±22.9) d] (F =3.845,P < 0.05).Time of jaundice back to normal [(27.1 ± 10.5) d],quantification of viral DNA back to negative [(39.5 ±24.0) d],hospitalization time [(22.6 ±5.8) d] and costs [(10 521.9 ±2 662.3) yuan] in Ara-A group had no significant difference compared with those of the control group (F =1.111,2.837,0.840,2.223,all P > 0.05).The negative rate of viral DNA quantification in Ara-A group (80.9%) was higher than that of the control group (62.1%),and the liver injury rate (7.1%) was lower than that of the control group (15.5%),and the difference was statistically significant (x2 =9.137,11.514,all P < 0.05).Condusion Ara-A is safe and effective for infant cytomegalovirus hepatitis and it is suitable for the clinical practice.

Objective To study the efficacy and side effects of Ganciclovir combined with L-ornithine-L-aspartate on infant cytomegalovirus(CMV) hepatitis.Methods Sixty infants with CMV hepatitis hospitalized in our hospital from Dec.2009 to Dec.2010 were treated with ganciclovir combined with L-ornithine-L- aspartate.The parameters observed in the study included the pre-and post-treatment data on total Bilirubin (TBIL),alanine aminotransferase (ALT),alkaline Phosphatase (AKP)and the retraction of liver and spleen,as well as the adverse reactions of the treatment.Results The treatment significantly decreased serum TBIL (t =5.74,P < 0.05 ),ALT( t =2.92,P < 0.05 ) and liver( t =8.27 P < 0.05 ) and spleen volume ( t =5.03,P <0.05).However,side effects such as liver damage and rash occurred occasionally during the ganciclovir treatment.Intravenous infusion of L-omithine-L-aspartate caused side effects such as vomiting and other mild gastrointestinal reactions.Conclusion The treatment of Ganciclovir combined with L-ornithine-L-aspartate on infant cytomegalovirus hepatitis created good efficacy and can be considered as the first treatment choice.Though it is relatively safe,adverse reactions should be monitored during the treatment.

Objective To observe the clinical effects of short-term intermittent ganeiclovir treatment on infants with cytomegalovirus (CMV) hepatitis. Methods The clinical data of infants with CMV hepatitis were analyzed retrospectively. The liver functions including total bilirubin (TBil), direct bilirubin (DBil), alanine aminotransferase (ALT), aspartate aminotransferase (AST) and γ-glutamyltransferase (γ-GT) of the patients in treatment group (85 cases) and control group (37 cases) were collected before and after treatment. Meanwhile, the side effects of ganciclovir during treatment were observed. The measurement data were compared by analysis of variance and numeration data were compared by chi-square test. Results After short-term intermittent ganciclovir treatment in treatment group, TBil level was decreased from (109.1±677.8)μmol/L to (62.9±68.1)μmol/L (F=15.34,P<0.01); ALT level was decreased from (160.2±395.3) U/L to (68.1±56.0) U/L (F=4.73, P<0.05). In control group, TBil level was decreased from (94.9±647.4)μmol/L to (49.2±631.5) μmol/L (F=14.80, P<0.01) ; while ALT level was decreased from (131.6±206.2) U/L to (55.3±631.2) U/L (F=3.50, P=0.067). The readmission rate in control group was significantly higher than that in treatment group (21.6% vs 10.6%). Only one case (0.8%) received three times of intermittent ganciclovir treatment. The longest hospitalization time was six weeks. Conclusions Short-term intermittent ganciclovir treatment may be more suitable for infants with CMV hepatitis. There is no obvious side effect observed during the treatment and the hospitalization time can be shortened.

Objective To explore clinical value of serum total bile acid(TBA) and cholinesterase(CHE) in infantile hepatopathy.Methods Sixty infants diagnosed respectively with breast milk jaundice,cytomegalovirus hepatitis and congenital bile duct teratism were included.Twenty healthy infants were as control.The infants of cytomegalovirus hepatitis were self-compared in acute stage and convalescence stage.Serum TBA and CHE in every group were measured and compared.Results Serum TBA in control group,breast milk jaundice group,cytomegalovirus hepatitis group and congenital bile duct teratism group were(6.47?3.25),(8.71?1.79),(45.86?40.85),(119.50?31.73) ?mol/L,respectively;and CHE were(11295.50?1731.91),(10762.89?1237.30),(9771.32?860.27),(7967.304?31.58) U/L,respectively.Compared with other groups,the level of serum TBA and CHE in cytomegalovirus hepatitis group and the congenital bile duct teratism group were significantly different(all P

Objective To explore the relationship between tumor necrosis factor(TNF)-? promoter G238A,G308A and interferon(IFN)-?+874A/T polymorphism and susceptibility to cytomegalovirus(CMV) hepatitis.Methods A TaqMan fluorescence polymerase chain reaction(PCR) in the IFN-?+874A/T and TNF-? gene promoter region single nucleotide polymorphism was tested in the subjects,including 87 infants with CMV hepatitis and 89 children without CMV hepatitis.Results No evident difference of TNF-? +238G/A and+308G/A allele frequency was found between infants with CMV hepatitis and controls.The cases of IFN-?+874 AA,AT and TT genotype were 64,20,3 cases in the infant CMV hepatitis group,and 45,26 and 18 cases in the control group,respectively.There was significant difference in IFN-?+874 allele genotype and frequency between infants with CMV hepatitis and the control group(P=0.001,P
