RESUMO

Objective:To compare the chemical composition of decoction and granules of Sangju Decoction; To provide a method for quality evaluation of Sangju Decoction.Methods:HPLC was used to establish fingerprints, and a comprehensive comparative study was conducted on the traditional decoction and formula granules of Sangju Decoction from four aspects: chemical composition type, fingerprint similarity, chemical pattern recognition analysis, and representative index component content.Results:The fingerprint similarity of the 10 batches of traditional decoction was >0.988. 35 peaks were identified and 12 peaks were identified as common peaks (neochlorogenic acid for peak 7, chlorogenic acid for peak 10, cryptochlorogenic acid for peak 11, 1,3-dicaffeoylquinic acid for peak 13, rutin for peak 17, lenoside A for peak 19, lignan for peak 20, isochlorogenic acid B for peak 24, ammonium glycyrrhizate for peak 25). The fingerprint similarity of the formulation pellets was >0.983, and 29 characteristic peaks were identified. Compared with the traditional decoction, some batches of the granules lacked peaks 14, 26, 27, 30, 32 and 34, and clustering analysis (CA), principal component analysis (PCA), and orthogonal partial least squares discriminant analysis (OPLS-DA) could distinguish between the two. The contents of the 10 index components neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, 1,3-dicaffeoylquinic acid, forsythia ester glycoside A, grass glycosides, isochlorogenic acid B, 3,5-O-dicaffeoylquinic acid, forsythia glycosides, monkshood glycosides in the traditional soup were higher than that in the granules, and the contents of rutin and ammonium glycyrrhizate in the granules were higher than that in traditional decoction.Conclusions:The content and composition of traditional decoction and formula granules of Sangju Decoction are significantly different. The combination of fingerprinting and chemical pattern identification effectively can effectively evaluate the difference between traditional decoction and formula granules of Sangju Decoction, which can lay a foundation for the quality control and rational clinical application of formula granules of Sangju Decoction.

RESUMO

Xiexin decoctions (XXDs) display beneficial anti-inflammatory and anti-diabetic effects, which raises interests on this group of formulae for broad clinical applications. However, there was no report about systematic analysis of XXDs to elucidate the constitution of chemical components, which hampers further investigations on the therapeutic values of XXDs. In this work, crude herbs were extracted and prepared to obtain the XXDs for systemic analysis on their chemical compositions, according to the information described in the ancient Zhang Zhongjing's herbal formulae. LC-MS analysis of five XXDs was carried out to facilitate recognition of the source herbs for compounds in the mixture. A total number of 93 compounds were identified through our methods and their chemical classes encompassed five major groups, including protoberberine alkaloids, flavonoids, stilbenes, anthraquinones and saponins. Our current work provided important information about material basis for pharmacological studies on XXDs and would help shed light on relationships between chemical compositions and therapeutic effects.

Objective To evaluate the efficacy of Bupi-Qingchang recipe combined with traditional Chinese medicine enema on the patients with radiation proctitis.Methods Eighty patients with radioactive proctitis who met the inclusion criteria were randomly divided into two groups,40 in each group.The control group was treated with western medicine combined with enema,and the observation group was treated with oral spleen and clearing spleen combined with traditional Chinese medicine enema.Symptoms were scored before and after treatment from abnormal sensation of bowel movement,blood in stool,stool traits and frequency,abdominal pain;TNF-α,IL-8,IL-6,IL-10 levels were detected by ELISA.The intestinal mucosal healing and recurrence were observed by microscopy.Results After treatment,the serum TNF-α (1.18 ± 0.61 ng/ml vs.1.78 ± 0.66 ng/ml,t=4.860),IL-6 (2.52 ± 0.75 ng/ml vs.3.02 ± 0.81 ng/ml,t=3.297),IL-8 (2.51 ± 0.68 ng/ml vs.3.01 ± 0.82 ng/ml,t=3.417),IL-10 (0.56 ± 0.25 ng/ml vs.0.89 ± 0.29 ng/ml,t=6.275) in the observation group were significantly lower than those in the control group (P<0.01).The total scores of abdominal pain,blood in the stool,stool characteristics and frequency,abnormal feeling of defecation and symptoms were significantly lower in the observation group than in the control group (t values were 8.108,2.085,4.965,7.865,9.586,P<0.01 or P<0.05).The total effective rate was 92.5% (37/40) in the observation group and 67.5% (27/40) in the control group.The difference between the two groups was statistically significant (x2=7.813,P<0.01).The intestinal mucosa of the observation group was treated.The healing rate was significantly better than that of the control group (x2=15.535,P<0.01),and the recurrence rate was significantly lower than that of the control group (x2=10.912,P<0.01).Conclusions The Bupi-Qingchang recipe combined with traditional Chinese medicine enema can improve the clinical symptoms of patients with radiation proctitis,reduce inflammation,reduce recurrence and improve curative effect.

The treatment by Kampo decoction is partly covered by National Health Insurance in Japan. However, this system is facing bankruptcy crisis because of rising prices of crude drugs in China, their main producer. The board of crude drug materials of the Japan Society for Oriental Medicine (JSOM) distributed questionnaires to 7416 JSOM member doctors, and performed statistical analysis (with JSOM approval) of 1877 answers to visualize the data. Twenty-six percent of respondents said that they had prescribed a decoction, and 29% of respondents said they had not, but wanted to prescribe a crude drug. Eighty-eight percent of doctors who prescribe decoctions offered medical treatment primarily to insured patients. Nine percent offered medical treatment at patients' own expense. The latter group prescribed decoctions more frequently. Many doctors were aware of the financial risk of prescribing crude drugs imposed by the drug price standard and rising crude drug import prices. Four hundred and fifty­-five doctors explained when they were most inclined to prescribe a decoction. Thirty-five percent of these said they used decoctions when they couldn't treat patients with extracts. This result implies a necessity for decoctions. Many doctors said they prescribed decoctions for autoimmune or allergic diseases. Financial constraints were the most frequently cited barrier to treatment with decoctions. This survey made clear the financial difficulties clinics are facing. We therefore calculated the amount of crude drugs used for decoctions to validate the possibility of their domestic production.

Objective To study the clinical effect of Yiqi-Huoxue decoctions combined with exercise rehabilitation training for patients with chronic heart failure and its effect on serum MMP-1, TIMP-1. Methods A total of 120 patients with chronic heart failure in our hospital from February 2016 to January 2017 were enrolled in this study. The subjects were randomly divided into the control group (n=60) and the treatment group (n=60). The control group were treated with conventional treatment, and the treatment group were treated with Yiqi-Huoxue decoctions combined with exercise training. The two groups were treated for 30 days. The clinical effects of the two groups after treatment were compared. The SBP, DBP, LVESD, LVEDD and LVEF of the two groups before and after treatment were compared. The serum MMP-1, TIMP-1 of the two groups before and after treatment were compared. The adverse reaction rates of the two groups during treatment were compared. Results The total effect rate of the treatment group was 93.3% (56/60), significantly higher than 73.3% (44/60)of the control group (χ2=8.640, P=0.003). After treatment, the SBP (125.17 ± 13.51 mmHg vs. 140.82 ± 14.63 mmHg, t=6.087), DBP (74.36 ± 10.31 mmHg vs. 86.29 ± 11.17 mmHg, t=6.079), LVESD (41.11 ± 3.23 mm vs. 49.69 ± 4.99 mm, t=11.181) and LVEDD (57.36 ± 3.28 mm vs. 64.16 ± 4.05 mm, t=10.107) of the treatment group were significantly lower than those of the control group (P<0.05). The LVEF (69.82% ± 5.05% vs. 51.40% ± 4.11%, t=21.913) of the treatment group was significantly higher than the control group (P<0.05). After treatment, the serum MMP-1 (141.52 ± 15.22 μg/L vs. 164.10 ± 16.18 μg/L, t=7.874) of the treatment group were significantly lower than those of the control group (P<0.05), and the serum TIMP-1 (3.98 ± 0.22 μg/L vs. 3.51 ± 0.16 μg/L, t=13.383) of the treatment group were significantly higher than those of the control group (P<0.05). There was no significant differences of the adverse reaction rates of the two groups during treatment (χ2=0.152, P=0.697). Conclusions The Yiqi-Huoxue decoctions combined with exercise rehabilitation training for patients with chronic heart failure showed good efficacy and low incidence of adverse reactions, can significantly improve cardiac function and the cardiac remodeling.

Among the medical texts excavated in 2012-2013 from the Han Dynasty tomb at Tianhui township in Sichuan province, we found a collection on the treatment of 60 ailments. Under each ailment, we found one or more formulas, for a total of 106 formulas. The authors of this paper compiled and analyzed these texts based on the original bamboo slips and named this collection with the title Methods for Blending Decoctions to Treat 60 Ailments because the content was focused on blending and making medicinal formulas, and also due to the historical connections with the texts the Record of the Court Scribe, "Biography of Bian Que and Cang Gong". For these reasons, this title was determined, where "blending decoctions" mean "blending and harmonizing medicines optimally in preparation for decocting". The Tianhui texts preserve ancient forms of medicinal formulas, including some once believed to be lost, such as "grain decoctions", "fermented alcohol decoctions", and "fiery decoctions". Based on the historical evidence, we can now ascertain that this text is the "Blending Formulas and Making Decoctions" mentioned in the Record of the Court Scribe written by Cang Gong. Moreover, the medical texts, Canon Formulas in Decoction Form (from the book of Han Dynasty) and the Imperial Pharmacy Formulas to Benefit the People in Song Dynasty are both of this genre of medical literature. The Tianhui text is therefore a representative of this genre of literature in the Western and eastern Han, acting as a key link between early medical formula books and later formula books.

Objective:To establish an optimal extraction technology for gastrodine in Tianma Gouteng decoctions. Methods: An orthogonal design with extract yield and gastrodine concentration as the indices was carried out to observe the influence of the extraction time, water amount and extract times on the extraction result. Results:The extract times showed significant influence on the extraction percentage of gastrodine. The best extraction parameters were as follows:adding 12-fold water and decocting 3 times with 1. 5h for each time. Conclusion:The established extraction process is feasible, which can be used in the preparation of the effective part for Tianma Gouteng decoctions with gastrodine concentration as the index.

Objective:To investigate the influence of different decoction containers on the chemical components in Fufang Baidu decoctions to provide scientific evidence for the establishment of rational decoction method for traditional Chinese medicine decoctions. Methods:The method of UPLC was adopted to study the influence of different decoction containers on the content of astilbin, forsytho-side A and forsythoside in Fufang Baidu decoctions. Results:With the different containers, the content change regularity was as fol-lows:porcelain pot >stainless steel pot> decocting machine of Chinese herbs> earthen pot> glass pot. Conclusion:Decocting ma-chine for Chinese herbs can be an substitute for traditional boiling utensils.

This study was aimed to compare different preparation methods of Wei-Chang Fu-Yuan (WCFY) De-coction, which is a self-made traditional Chinese medicine (TCM) decoction of our hospital, through the investiga-tion of difference in recovery time of gastrointestinal function after gynecological abdominal surgery. A total of 120 cases after gynecological abdominal surgery in our hospital between 2011 and 2012 were randomly divided into the treatment group and the control group, with 60 cases in each group. The hospital self-made WCFY Decoction was given to patients 8 hours after surgery in the treatment group and control group. Decoctions given to two groups were prepared with different methods. Decoction given to the treatment group was by the decoction ma-chine of multiple decoctions preparation. Decoction given to the control group was prepared as the traditional sin-gle dose decoction. The first anus flatus time was observed in both groups. The results showed that the flatus time of treatment group was 16 h and that of the control group was 13 h. The flatus time of patients from two groups was in skewed distribution, so the distance between the median and quartile was used to describe. The non-para-metric tests (Kolmogorov-Smirnov method) were used in the comparison of flatus time of two groups. There was statistical significance between two groups (P < 0.05). It was concluded that different preparation methods affect the clinical efficacy of WCFY Decoction. Compared with the traditional single dose decoction, the fist flatus time was postponed 3 hours by the decoction machine of multiple decoctions preparation. Therefore, with the widely using of decoction machine of multiple decoctions preparation today, the function of decoction machine should be improved and the program should be perfected in order to increase the clinical efficacy of this preparation method.

Objective To observe the effect of jiaweibugan decoction on the expression of p-JNK mitogen-activated protein kinase in sciatic nerve of experimental diabetic rats and explore the preventive and therapeutic effects of Jiaweibugan decoction on peripheral neuropa-thy in experimental diabetic rats. Methods The diabetic rat model was established by streptozotocin (STZ). The rats were killed on the 4th or 8th week from the beginning of treatment, and the expression of p-JNK mitogen-activated protein kinase in sciatic nerve was detected by immunohistochemistry. Serum SOD was determined by xanthine oxidasemethod. Results The immunohistochemistry results showed that the expression of p-iNK in the model group on the 4th or 8th week was higher than that in normal control group (P<0.05) , while p-JNK in jiaweibugan decoction group was markedly lower than that in model rats(P<0.05). The results showed that SOD in the model group on the 4th or 8th week was lower than that in the normal control group (P<0.05) , while SOD in jiaweibugan decoction group was markedly high-er than that in model rats(P<0.05). Conclusion Jiaweibugan decoction has a preventive and therapeutic effect on peripheral neuropathy in experimental diabetic rats.

Objective To observe the effect of Jiaweibugan decoction on c-jun mRNA expression of sciatic nerve in experimental di-abetic rat and explore the preventive and therapeutic effects of Jiaweibugan decoction on peripheral neuropathy in experimental diabetic rats. The results showed that SOD of the model group on the 4th or 8th week was lower than that of the normal control group (P < 0.05) , while those in model rats treated by Jiaweibugan decoction were markedly higher than those in non-treated model rats(P < 0.05). Conclusion Jiaweibugan decoction has a preventive and therapeutic effect on peripheral neuropathy in experimental diabetic rats.

RESUMO

Objective To study the anti-depressant effect of Xiaoyaofang on mice with behavioral despair. Method The mice were given the forced swimming test and tail suspension test to observe the anti-depressant effect of Xiaoyaofang. Result The high dosage of Xi-aoyaofang significantly shortened fast time in the tail suspension test of mice(P < 0.05). And Xiaoyaofang showed the trend of shortening the needed time in the forced swimming test. Conclusion Xiaoyaofang have an anti-depressant effect on mice with behavioral despair.

Objective To explore mRNA and protein expression of PAl-1 in kidney tissue of mice with IgA nephropathy (IgAN) and the effects of YiShen Decoction. Methods The IgAN model was built by the method of oral intake of bovine serum albumin(BSA)together with the injection of staphylococcus enterotoxin B(SEB)through caudal vein. The mRNA expression of PAI-1 was measured by fluorescent quantitative polymerase chain reaction (FQ-PER)and the expression of PAI-1 protein was measured by immunohistochemistry. Results No significant difference of mRNA and protein expression of PAI-1 was found between the low concentration and high concentration Yishen Decoction group. But the secretion and mRNA expression of PAI-1 in the low and hiigh concentration Yishen Decoction group was decreased more than that in the IgAN model group. Conclusion The abnormal expression of PAI-1 mRNA and protein played an important role in the onset and development of IgAN. The TCM Yishen Decoction could reduce the abnormal expression of the mRNA and protein of PAI-1 in kidney tissue of mice with IgA nephropathy.

Objective To observe the efficacy of Buyanghuanwu herbal decoctions on microalbuminur in noninsulin-dependent diabetes mellitus. Methods Sixty-six patients with noninsulin-dependent diabetes mellitus patients with 30~300 mg/24 h microalbuminur were randomly divided into the treated group (treated with Buyanghuanwu herbal decoctions) and the control group (treated with injection of Huangqi). To explore indications regularity of analogous decoctions of Erchen Decoction.323 analogous decoctions of Erchen Decoction were found in Concise Thesaurus of TCM Formula.Diseases and symptoms of TCM were analyzed.22 diseases and 118 symptoms of TCM were found.The main indication of analogous decoctions of Erchen Decoction included spleen-stomach and lung system,other symptoms were broadly involved.Erchen Decoction was the most appropriate for phlegm.

RESUMO

A taste survey was conducted on decocted Kampo medicines, and the strength and weakness of the taste on each decoction expressed numerically. This data was used to construct a decocted Kampo medicine taste rank table and a table summarizing the tastes for the different Kampo decoctions.<br>The taste rank table and the summary table can be used to know the taste of the Kampo formula being administered to the Patient, and are therefore thought to be useful in Kampo treatment and as administration guidelines.

AIM:To analyze the changes in the active components alignment of Puerariae Decoctions between its two different compatibility proportion. METHODS: Reversed phase high-performance liquid chromatography (RP-HPLC) was applied. RESULTS: When the compatibility ratio of Radix Puerariae Lobatea, Radix Scutellariae, Rhizoma Coptidis and Radix et Rhizoma Glycyrrhizae Praeparata cum melle was 5∶3∶3∶2, the contents of the eight drug active components alignment were 2.48% puerarin, 0.101% daidzin, 0.614% baicalin, 0.155% berberine, 0.048% palmatine, 0.094% jateorhizine, 0.154% glycyrrhizic acid, 0.283% liquiritin, and the proportional relationship of compatibility was 51.7∶2.1∶12.8∶3.2∶1∶2∶3.2∶5.9. When the compatibility ratio of Radix puerariae lobatae, Radix scutellariae, Radix Coptidis and Radix et Rhizoma Glycyrrhizae Praeparata cum melle was 8∶3∶3∶2, the proportion of contents of the eight active components alignment were 1.67%, 0.143%, 0.36%, 0.085%, 0.031%, 0.048%, 0.126%, 0.174%. And the proportional relationship of compatibility was 54.4∶4.7∶11.7∶2.6∶1∶1.5∶4.1∶5.7. CONCLUSION: When the compatibility percentage consists of 8∶3∶3∶2, only the puerarin content relatively increases. The active components alignment of the rest have no significant difference. This illustrates the contents of the active components alignment of puerariae decoction is related to the compatibility percentage and herb solubility.
