RESUMO

Objective To approach new and simple method in treatment of ear cerumen. Methods Retrospective analysis of 110 cases of cerumen embolization from March 2015 to March 2017. Results The outer ear canal cerumen was removed in one time without perforation of the eardrum. 12 cases of external auditory canal injury. Conclusion Cerumen under the hard ear endoscope is a safe, simple and effective method.

RESUMO

Objective To investigate the application of ear endoscopic in artificial stapes surgery of otosclerosis.Methods Artificial stapes surgery was performed by two amplification device, microscope or ear endoscope, in 64 cases of patients with otosclerosi. Then compare the operation time and analyze the effect of surgery.Results Patients postoperative hearing of the two groups were signiifcantly improved, the difference of air bone conduction was statistically signiifcant (P 0.05).Conclusion Compared with microscopic surgery, ear endoscope artificial stapes surgery has its’ unique advantages with clear image and small trauma.

RESUMO

Objective To investigate the application of ear endoscope in the treatment of secretory otitis media in children with ineffective drug therapy.Methods 80 secretory otitis media children with ineffective drug therapy were selected as the research subjects.According to the random number table method,all patients were divided into treatment group and control group,40 cases in each group.The observation group received otoendoscopic tympanic membrane tube surgery treatment,the control group received auripucture with otic endoscope surgery treatment.The complications of the two groups were observed,and the clinical curative effect and postoperative recurrence were compared.Results The recurrence rate of the observation group was 5 .00%,which was significantly lower than 20.00% of the control group (χ2 =4.114,P=0.042).The middle ear effusion time of the observation group was (8.53 ±1.25)d,which was significantly shorter than (10.46 ±1.44)d of the control group (t=5.074,P=0.000). In the observation group,37 patients after treatment had change,the total effective rate was 92.50%.In the control group,28 patients after treatment had change,the total effective rate was 70.00%,the total effective rate of the observation group was significantly higher than the control group,the difference was statistically significant (χ2 =6.646,P=0.009).The incidence rate of complication in the observation group was 7.50%,which was significantly lower than 32.50% in the control group (χ2 =7.812,P=0.005 ).Conclusion Tympanostomy tube under ear endoscope surgery for the treatment of drug refractory secretory otitis media in children has good effect,the recurrence rate is relatively lower,has less complications,high safety,it is worthy of clinical application.

RESUMO

Objective To investigate the clinical efficacy of repairment using ear endoscope for small perfo-ration of the tympanic membrane after traditional tympanoplasty.Methods A retrospective analysis of 42 cases of repairment using ear endoscope for small perforation of the tympanic membrane after traditional tympanoplasty was investigated.Adipose tablet of earlobe to repair small perforation or perichondrium of hircus for medium perforation was used.Results The success rate after one repairment of small perforation was 82.76%(24 /29).The success rate for medium perforation was 61.54%(8 /13).9 cases received the repairment for 2 times or even more to heal the tym-panic membrane.The mean improvement of air conduct at language frequency was 20.03dB by 6 months after the successful operations (t =1.98,P <0.05)and air bone guide.The mean difference between the air conduct and bone conduct decreased as much as 19.82dB (t =1.86,P <0.05)with statistical significance.Conclusion The success rate was high to repair the perforation using ear endoscope after traditional tympanoplasty.The hearing was improved obviously and the patients were very compliant.

RESUMO

Objective To study the effects of the integrity of the tensor tympani muscle on the postoperative hearing recovery and the morphology of tympanic membrane,to provide the reference for the handling of the tensor tympani muscle during the operation.Methods A total of 145 cases of chronic tympanitis were grouped by the integ-rity of the tensor tympani muscle and the implanted types of biological auditory ossicles,the clinical data were ana-lyzed retrospectively.There were 80 cases in the tensor tympani muscle intact group,including 45 cases of PORP group and 35 TORP group.The tensor tympani muscle rupture group of 65 cases consisted of 34 cases of PORP group,and 31 cases of TORP group.The postoperative recovery conditions of tympanic membrane morphology, hearing threshold Ac and air-bone gap(ABG)between the tensor tympani muscle intact group and the tensor tym-pani muscle rupture group were compared 1 month or 3 months,and 6 months after the operation.ResuIts One month after the operation,there was no significant difference in tympanic membrane morphology between the two groups.Three months after the operation,the tensor tympani muscle intact group had a higher ratio about the loca-tion and shape of tympanic membrane ,closer to the normal than the tensor tympani muscle rupture group in which the ratio in the intact group was 81.25% (65/80),while in the rupture group it was 52.31% (34/65 ).After 6 months,with the operation of PORP,the tensor tympani muscle intact group of AC value was 27.48±10.02 dB HL, and ABG value was 13.57±6.36 dB,while the rupture group of AC value was 32.36±9.34 dB HL,and ABG value was 25.73±7.44 dB.With the operation of TORP,the tensor tympani muscle intact group of AC value was 28.76± 7.14 dB HL,and ABG value was 21.02±5.48 dB,while the rupture group of AC value was 39.93 ±5.12 dB HL, and ABG value was 31.41±6.25 dB.The postoperative recovery condition of the tensor tympani muscle intact group was better than those of in the rupture group.ConcIusion The tensor tympani muscle can maintain the morphology and location of the postoperative tympanic membrane.The integrity of the tensor tympani muscle may has a positive effect on the improvement of the postoperative hearing.

RESUMO

Objective To evaluate the effect and safety ofotoscopic myringoplasty and microscopic myringoplasty.Methods PubMed,CNKI,VIP database and Wanfang database were searched to identify citations mentioned otoscopic myringoplasty or microscopic myringoplasty.In addition,manual collection of clinical study of related reviews and its references were also conducted.Inclusion criteria of the ordinance made Meta analysis.Results A total of 15 clinical studies with 915 patients (otoscopic group was in 691patients and microscope group was in 224 patients) was identified.According to Q test,there was no heterogeneity between 2 groups (I2 =0),all computed by fixed effects model.Meta analysis demonstrated that there was no significant difference between 2 groups on the type of perforation (OR =1.00,95% CI:0.70-1.42,P =0.13).Both curable rate and enhance of air conduction had no significant difference between 2groups (OR =0.97,95% CI:0.72-1.29,P=0.97;OR =1.14,95% CI:0.68-1.91,P=0.96).The curable rate in otoscopic group was 0.92 (95% CI:0.90-0.94),and the complication rate was 0.030 (95% CI:0.009-0.051).Conclusions The available clinical evidence shows that there is no significant difference between otoscopic myringoplasty and microscopic myringoplasty.However,otoscopic myringoplasty should be recommended to perform in clinical practice because of the feasible and convenient procedure.

RESUMO

Objective To evaluate clinical effects of endoscopic transcanal myringoplasty. Methods Endoscopic transcanal myringoplasty was conducted in 26 cases (27 ears) from August 2001 to May 2004. After an endoscope was introduced into the external auditory canal, an examination was made regarding the size, location and margin of the tympanic membrane perforation, as well as mucosa of the middle ear and the ossicula chain. Results Myringoplasty was completed successfully under endoscope without additional incision in all the cases, including overlay grafting in 10 ears and underlay grafting in 7 ears. The operation time was 20~50 min (30?5 min). Postoperative hospital stay was 1~3 days. Follow-up for 1~2 months in all the 26 cases showed living grafts in 25 ears and dead grafts in 2 ears, the success rate of myringoplasty being 92 6%(25/27). Conclusions Endoscopic transcanal myringoplasty gives advantages of simple performance, minimal invasion, quick recovery, and excellent curative effects.
