RESUMO

TetR family transcriptional regulators (TFRs) are widely distributed in bacteria and archaea, and the first discovered TFR was confirmed to control the expression of tetracycline efflux pump in Escherichia coli. TFRs can bind DNAs and ligands. Small molecule ligands can induce conformational changes of TFRs, inhibiting or promoting TFRs to control target gene expression. Currently, TFRs have a wide variety of ligands, including carbohydrates, proteins, fatty acids and their derivatives, metal ions, and so on. Due to the diversity of ligands, TFRs regulate a wide range of physiological processes, from basic carbon metabolism and nitrogen metabolism to quorum sensing and antibiotic biosynthesis. On the basis of the recent studies in our laboratory and the literature, we review here the regulatory mechanism mediated by ligands of TFRs in primary and secondary metabolism, as well as the application of ligands for TFRs in the development of gene route and the activation of antibiotic biosynthesis.

RESUMO

Objective To investigate the effect of feed forward control on hypoglycemia in patients with diabetes mellitus. Methods from March 2014 to February 2016,60 cases of diabetic patients in our hospital were studied. All patients were treated with routine hypoglycemic therapy. The patients were randomly divided into two groups. Control group 30 cases, patients with routine hypoglycemia management, test group 30 cases, patients with feed-forward control for hypoglycemia management. The fasting blood glucose levels, glycosylated hemoglobin levels and incidence of hypoglycemia in the two groups were observed. Results During the period of in-hospital hypoglycemic treatment and blood glucose control, fasting blood glucose test group (5.8 ± 0.6) mmol/L patients during hospital self management, glycosylated hemoglobin test group (6.6 ± 0.5)%, two indexes were better than the control group (6.7 ± 0.8) mmol/L, (7.8 ± 0.7)%, t=5.216, 4.931, P < 0.05. During the period of hospital sugar control,1 cases of hypoglycemia occurred in the experimental group,8 cases of hypoglycemia occurred in the control group,and the incidence of hypoglycemia in the experimental group was significantly less than that in the control group. Conclusions The application of feed forward control in the hospital management of hypoglycemia in diabetic patients can significantly reduce the risk of hypoglycemia and severity,but also plays a positive role in improving the self management of blood glucose level in patients,is worthy to be popularized.

Objective The objective of this study was to explore the construction of a comprehensive and systematic quality management system for pharmacy intravenous admixture service (PIVAS) and form quality management method and model reflecting distinct gradations,specific responsibility and all staff participation,so that we can provide safe and effective intravenous solutions for the patients and guarantee the quality and safety of intravenous infusion and improve management quality.Methods According to Quality Management Standards of PIVAS published by Ministry of Public Health and JCI Standard,with the theory and methods of feed forward control and delicacy management,by establishing and completing three-grade-quality management organization,defining the quality control indicators,selecting and demonstrating with the application of expert meeting method and Delphi method,a scientific quality control system was formed.Statistics on unreasonable prescriptions,adverse drug reaction of intravenous infusion,defect of pharmacy intravenous admixture and satisfaction of medical staff and patients were compiled and evaluated throughout the implementation of the practice changes in 38 wards.Results Significant differences were observed in various indicators,comparing statistics collected during 6 months of the same season throughout the implementation of the practice changes.x2 value was 160.54,21.39 and 10.29 by comparison of unreasonable prescriptions,adverse drug reaction and number of drug preparation errors.The satisfaction degree of medical staff and patients improved evidently,x2 value was 14.34 and 26.76.Conclusions The construction and implementation of three-grade-quality management system adapt to high-risk,high strength,high sense of responsibility,process-oriented and intensive management of PIVAS.The impact on the continuous improvement PIVAS quality and assurance in safety of intravenous infusion is remarkable.
