Advantages and disadvantages of forearm skin flap and lateral femoral skin flap in repair of oral and maxillofa-cial defects / 局解手术学杂志

Objective To compare and analyze the advantages and disadvantages of the forearm skin flap and the lateral femoral skin flap in the repair of oral and maxillofacial defects.Methods Of the 34 patients of oral and maxillofacial tumor resection received postopera-tive reconstruction of maxillofacial defects treatment,18 patients repaired by forearm flap( forearm flap group) ,16 patients repaired by antero-lateral thigh flap ( anterolateral thigh flap group) .The clinical effect and the restoration of two groups after surgery were compared.Results The survival rate of disposable surgical flap in forearm flap group was 94.44%,the survival rate of the anterolateral thigh flap group was 87.50%.The difference between two groups was not statistically significant (P>0.05).The difference of swallowing function in 3 months after surgery between two groups was not statistically significant (P>0.05).The difference of survival quality score between the two groups was not statistically significant (P >0.05) in 6 months after surgery.The scar proliferation rate (27.78%), pigment deposition rate (88.89%), temporary sexual dysfunction rate(66.67%),a permanent disability rate (27.78%) of forearm flap group were significantly higher than those of the femoral anterolateral flap group patients (P<0.05).Conclusion The forearm skin flap and the lateral femoral skin flap have a good clincal effect on the repair of oral and maxillofacial defects,and the incidence of complications of the femoral anterior lateral femoral flap is lower.

Free forearm flap and free iliac bone reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period / 中华显微外科杂志

ObjectiveTo probe into the clinical efficiency of reconstruction to person who had composite tissue defect of the floor of mouth and the lower part of face with the free forearm flap and free iliac bone in the same term.MethodsBefore the surgery, the surgeon should carefully evaluate the scope and shape of defects in 11 cases who had the composite tissue defects of the floor of mouth and the lower part of face, to design the individual forearm flap, so that it matches with the defect region. During surgery, the first resumption of defects using titanium plates forming the basic shape of mandible and the occlusal relationship,then used to re-sawing to take modeling of the iliac bone,transplanted free iliac bone and fixed after the inside of the titanium plate, thereby restoring the continuity of mandible missing.ResultsFree forearm flap and free iliac bone all survived in 11 cases, the success rate of 100％. The patients were followed up for 6 to 12 months, although the shape of restoration areas were different levels of fat, but eating, swallowing and other oral function had been well improved, 7/11 could enter normal diet, 4/11 to enter liquid diet, At the same time, the patients in social activities can communicate in a language daily, sensory function of skin flap and facial appearance has been satisfactory recovery.ConclusionConformal free forearm flap and free iliac bone is an ideal way to reconstruct the composite tissue defects of the floor of mouth and the lower part of face in the same period, not only safe, practical prognostic effect, and also can significantly improve the patients quality of life, which is worthy of reference and use.

Forearm free skin flap to repair the whole defect of the lip / 临床外科杂志

Objective To sum up transplantation of forearm skin flap to repair the whole defect of the lip.Method Forearm free skin flap was used to repair the whole defect of the lip in 5 cases. Results The treatment was completed in one stage and the results were satisfactory in 5 cases.And the reconstructed lip showed good shape.Conclusions Forearm skin flap has many strong points:the skin flap has big area and wide blood vessel diameter.It is successed easy to be identical,operation can be completed once, and don't suffered harm to face or neighbour supply area.The skin flap has safe grain,more thinner, good springy and similar colour.It plays an important role in the repairs of the whole defect of the lip with nearby skin soft tissue defect.

Reconstruction of nasal tip and columella using dorsal forefinger skin flap based on proper palmar digital artery and reverse forearm skin flap on tiny cutaneous branches of radial artery / 中华医学美学美容杂志

Objective To seek new skin flap donor sites beyond face to repair deformity of nose. Methods Seven patients underwent reconstruction of nasal tips and columellae with dorsal forefinger sk in flap based on proper palmar digital artery and reverse forearm skin flap base d on tiny cutaneous branches of radial artery. Results Al l flaps survived. Similar external appearance in color and texture compared with adjacent normal skin was achieved for 1～4 years follow-up. Conclus i on Remarkable advantage without second deformity in face is archeive d due to contribution of skin flap. These two methods of correction can be more acceptible by patients than usual ones.
