RESUMO

Objective To investigate the clinical efficacy of entecavir in the treatment of hepatitis B cirrhosis associated with hepatic diabetes. Methods 78 cases of hepatitis B Hepatic Cirrhosis with diabetes were selectedfrom June 2015 to December 2016, and randomly divided into study group (n=39) and control group (n=39). The study group were treated with entecavir, the control group were treated with adefovir dipivoxil. Two groups of patients with hepatitis B cirrhosis combined with hepatic diabetes were treated continuously for 48 weeks, the changes of FPG, HOMA-β, beta and HOMA-IR were recorded. Results There was no significant difference between the two groups in the treatment of FPG, HOMA-β, and HOMA-IR. After treatment, the two groups of FPG and HOMA-IR decreased significantly compared with before treatment, while HOMA-β was significantly higher than before treatment, but the changes of the above indexes in the study group were better than those in the control group (P<0.05). Conclusion Entecavir combined with hypoglycemic agents can achieve more ideal blood glucose control effect in patients with hepatitis B cirrhosis combined with hepatic diabetes..

RESUMO

Objective To observe the correlation between vitamin D and liver cirrhosis classification in hepatic diabetes.Methods Thirty-eight patients with hepatic diabetes,48 patients with type 2 diabetes and 30 cases of healthy controls were collected.The level of serum 25-(OH)D3 was determined,and the Child-Pugh score was calculated.The correlation between pathogenesis and pathogenetic condition of hepatic diabetes was analyzed.Results The levels of serum 25-(OH)D3 in hepatic diabetes patients,type 2 diabetes patients and healthy controls were (38.23 ± 12.47),(63.33 ± 13.58) and (86.14 ± 16.25) μ mol/L,and there was statistical difference (P < 0.05).There was negative correlation in hepatic diabetes between serum 25-(OH)D3 level and liver cirrhosis Child-Pugh score (r =-0.363,P < 0.05).Conclusion Low level of vitamin D may be one of the pathogenesis of hepatic diabetes.

RESUMO

Objective To study the relationship between serum adiponection and liver function and insulin level in patients with hepatogenous diabetes.Methods The research subjects included:28 patients with hepatog-enous diabetes who were admitted at Liaocheng People's Hospital (hepatogenous diabetes group);27 patients with simple type 2 diabetes without any complication (simple type 2 diabetes group),and 27 normal controb (control group).Blood adiponectin,fasting blood glucose (FBG),fasting insulins (FINS),alanine aminotransferase (ALT),aspartate transferase (AST),total bilirubin (TBIL) and albumin (ALB) were measured,and according-ly the insulin sensitivity index (ISI) was calculated.Results Compared with the control group.serum adiponec-tin levels and ISI levels were significantly lower in the other two groups (P<0.01),and the adiponectin levels were significantly higher in hepatogenous diabetes group than in simple type 2 diabetes group (P<0.05).The FBG and FINS levels in the hepatogenous diabetes group and the FBG levels in the simple type 2 diabetes group were significantly higher than those in control group (P<0.01).The FINS levels were significantly higher in the hepatogenous diabetes group than in simple diabetes group (P<0.05),while the levels of FBG and ISI were lower than that of simple type 2 diabetes group (P<0.05).Adiponectin levels were negatively correlated with ISI (r= 0.5265,P=0.000),and positively correlated with FBG (r=-0.6040,P=0.000),and were not correlated with liver functions Conclusion Insulin resistance and abnormal metabolism of adipenection exist in patients with hepatogenous diabetes and may together affect the occurrence and development of hepatogenous diabetes.
