RESUMO

Hyperkeratosis of the nipple and areola is a rare condition first described by Tauber in 1923. Less than 100 cases have been reported in the literature. Hyperkeratosis of the nipple and areola presents as hyperkeratotic, hyperpigmented plaques on the nipple and areola. It is more common in females. An 18-year-old female patient presented with hyperkeratotic, plaque-like, hard crusts on both nipples and areolas. The examining physician could successfully remove this crust using his finger. The crust had accumulated as a result of the patient's reluctance to touch or clean the breast area due to psychological issues. A crusted nipple and areola may occur as a secondary condition due to a patient's reluctance to touch or clean their breasts.

La hiperqueratosis nevoide de areola y pezón es una entidad patológica de la dermatología escasamente presentada en la literatura. Descrita por primera vez en 1938 por Levy – Frankel,2 quien además establece una clasificación etiológica de la misma. En este trabajo se presenta un caso de hiperqueratosis nevoide bilateral de areola y pezón, y su tratamiento con galvanocauterio que da muy buenos resultados no solo estéticos sino primordialmente por la nula recidiva.


Hyperkeratosis of the nipple and the areola is a pathological entity of dermatology sparsely reported, first described in 1938 by Levy - Frankel, who established an etiological classification of that entity. In this paper a case of bilateral hyperkeratosis of the nipple and the areola and its successful treatment with galvanocautery surgery with no long-term recurrence is reported.

RESUMO

A synthetic analogue of calcitriol (1,25-dihydroxymitamin D3), has shown immunomodulatory effects as well as induction of cell differentiation and inhibition of cell proliferation. This agent has been widely used for psoriasis. We report a case of seborrheic keratosis of the left areola in a 33-year-old female who was treated with calcipotriol ointment. Because of the lesion's location and shape, it was difficult to try ablative methods. However, complete disappearance of the lesion was observed after 6 weeks of topical calcipotriol application. This case suggests that the calcipotriol ointment is effective in the treatment of seborrheic keratosis.

RESUMO

Hyperkeratosis of the nipple and areola (HNA) is a rare benign condition. Clinically the typical lesions are characterized by diffuse verrucous thickening and brownish discoloration of the nipple or areola (or both). Histopathologic examination shows an acanthotic and papillomatous epidermis with hyperkeratosis. Levy-Franckel classified this condition into three categories: HNA as an extension of an epidermal nevus, HNA associated with ichthyosis, and an isolated nevoid form. The third type is usually bilateral and predominant among women in the second and third decade of life. We report an unusual clinical manifestation of nevoid HNA, on the right nipple and areola of a 33-year-old woman, in which her skin lesion was unilateral and confined to the superomedial aspect of right nipple and areola.

RESUMO

We report a case of hyperkeratosis of the nipple and areola occuing in 33 year old woman. She did not have a history of endocriruipathy, horrnonal therapy and other ceatoses, so this case seems to be the nevoid form by Levy-Franckel classification. Interestingly, however the lesion which had developed before marriage suddenly aggr avated during pregnancy. This may be a unique feature which has not been reported elsewhere. Histapathologic findings are compatible with breviously reported eases. There was no response to topical steroid and retinoic acid treatrnent.

RESUMO

Hyperkeratosis of the nipple and areola is a rare skin disordr characterized clinically by thickening and discoloration of the skin of the areola and/or nipple and pathoiog cally by papillomaosis or acanthosis-nigricans like changes. We report a case of hyperkeratosis of the nipple and areola occuring in a woman with no underlying endocrinopathy and no other additional hyperkeratotic conditions. Ttie iresent case seems to be the nevoid form by the Levy-Franckel classification. Treatment with topical medications including steroids and keratolytic agents produced no improvement. Therefore we recommeneded surgical excision because the paient was greatly disturbed by the appearance of the lesions. Surgical excision and full thickness skin graft were performed sucessfully.

We report two cases of hyperkeratosis of the nipple and areola which occurred in 22-year-old woman and 13-year-old girl. The former was suffered from mild pruritic, verrucous thickening and brownish discoloration of both nipples and areolae for 5 years. The latter had asymptomatic, verrucous thickening and dark brownish hyper- pigmentation of the right nipple and areola for 3 years. Both showed mild to marked hyperkeratosis and papillomatosis with increased pigmentation in the basal cell layer histopathologically. In both cases, the lesions were not responsive to the topical medications including steroids and keratolytic agents. Therefore we recommended surgical excision because the patients are greatly disturbed by the appearance of the lesions.
