RESUMO

A tuberculose é uma das doenças mais antigas do mundo. Os fatores de vulnerabilidade social, individual, programática, permitem que a tuberculose fique em latência por vários anos no indivíduo. Objetivo: Analisar, por meio de dados secundários, incidência e protocolo de infecção latente de tuberculose no município de São Paulo. Método: Trata-se de revisão sistemática da literatura. Em 29 de agosto de 2023, com a busca no Descritores em ciências da saúde: Tuberculose AND Infecção Latente, foram encontrados 4.527 artigos, após aplicação dos filtros: Base de dados LILACS e MEDLINE, texto completo, em Português, 2018 a 2023, restaram 38 documentos que foram submetidos ao checklist PRISMA. Resultados: Restaram 16 artigos que embasaram os resultados e a discussão. Conclusão: Iniciativas, como a busca ativa de comunicantes de tuberculose, devem ser construídas a partir de estratégias propostas em conjunto com pesquisadores e gestores alinhados ao Ministério da Saúde.(AU)


Tuberculosis is one of the oldest diseases in the world. Social, individual and programmatic vulnerability factors allow tuberculosis to remain latent for several years in the individual. Objective: To analyze, using secondary data, the incidence and protocol of latent tuberculosis infection in the city of São Paulo. Method: This is a systematic literature review. On August 29, 2023, 4,527 articles were found after applying the filters in the Health Sciences Descriptors: Tuberculosis AND Latent Infection: LILACS and MEDLINE database, full text, in Portuguese, 2018 to 2023, 38 documents remained that were submitted to the PRISMA checklist. Results: 16 articles were found to support the results and discussion. Conclusion: Initiatives, such as the active search for tuberculosis communicants, should be built on strategies proposed jointly with researchers and managers in line with the Ministry of Health.(AU)


La tuberculosis es una de las enfermedades más antiguas del mundo. Factores sociales, individuales y programáticos de vulnerabilidad permiten que la tuberculosis permanezca latente por varios años en el individuo. Objetivo: Analizar, a partir de datos secundarios, la incidencia y el protocolo de infección tuberculosa latente en el municipio de São Paulo. Método: Se trata de una revisión sistemática de la literatura. El 29 de agosto de 2023, fueron encontrados 4.527 artículos después de la aplicación de los filtros en los Descriptores de Ciencias de la Salud: Tuberculosis E Infección Latente: Base de datos LILACS y MEDLINE, texto completo, en portugués, 2018 a 2023, permanecieron 38 documentos que fueron sometidos a la lista de verificación PRISMA. Resultados: Quedaron 16 artículos, que constituyeron la base de los resultados y la discusión. Conclusión: Iniciativas como la búsqueda activa de comunicantes de tuberculosis deben construirse a partir de estrategias propuestas conjuntamente por investigadores y gestores en consonancia con el Ministerio de Salud.(AU)

As one of the pathogenic mechanisms contained in The Inner Canon of Yellow Emperor (《黄帝内经》), “disease with latent pathogen induced by a new pathogen” means that the induced new pathogen resulted to a combination of the latent previous pathogen and the new pathogen, which caused the disease. Based on this, it is believed that the change of “nodule-cancer transformation” of pulmonary nodules is actually based on the deficiency of original qi, and the new pathogen induces the latent pathogens like phlegm coagulation, qi stagnation, blood stasis, toxicity, so healthy qi can not drive the pathogens out, and the long-time detention generated into cancerous turbidity, and deve-loped into cancerous tumour at the end. Therefore, based on the three-stage treatment of unformed cancer, dense cancerous toxin, and developed cancer, the clinical practice applied six methods of clearing, expelling, dissipating, tonifying, harmonizing, and transforming, taking into account both the manifestation and root cause, moving the treatment window of pulmonary nodules forward, attacking the pathogens when the toxin was not yet overbearing, supporting the healthy qi before declining, delaying the process of nodules-cancer transformation, and providing ideas for the prevention and treatment of pulmonary nodules “nodule-cancer transformation” in traditional Chinese medicine.

@#School is a densely populated place, which can easily lead to tuberculosis clusters, then affect the physical and mental health of students and the normal teaching order of school. Tuberculosis latent infection (LTBI) screening for new students and close contacts of tuberculosis patients has become important parts of school tuberculosis prevention and control strategies. Previous studies have shown that the LTBI rate of Chinese in-school students is about 5.74% to 11.67%, and there are differences in gender, studying phase and urban-rural distributions. Preventive treatment is an effective measure to prevent LTBI from developing into active tuberculosis, but the proportion of LTBI preventive treatment for students in most areas is low. The difficulties in implementing preventive treatment may be attributed to concerns about adverse reactions to medication and sense of shame towards illness among students, and lack of awareness about preventive treatment among medical staff. This review searches the research literature published from 2016 to 2023, and summarizes the prevalence of LTBI in Chinese students and progress on preventive treatment, so as to provide insights into prevention and control of tuberculosis among students.

ObjectiveTo explore the common syndrome elements of cerebral ischemic stroke (CIS) complicated with obstructive sleep apnea-hypopnea syndrome (OSAHS), reveal the characteristics of traditional Chinese medicine (TCM) syndromes of the disease, clarify the syndrome differentiation and syndrome types, provide evidence for clinical syndrome differentiation, and provide reference for establishing the TCM syndrome type standards of CIS complicated with OSAHS. MethodThe clinical information form of CIS complicated with OSAHS formulated by the research group was used for syndrome survey, and the four-examination information of 300 patients with CIS complicated with OSAHS was collected. The four-examination information of patients was analyzed by latent structure method and comprehensive cluster analysis, and the common syndrome elements of CIS complicated with OSAHS were extracted by combining the TCM basic theory and clinical experience. On this basis, the characteristics of TCM syndromes and the syndrome types in line with reality were summarized. ResultThe top five syndrome elements in patients with CIS and OSAHS are sleep snoring, open mouth breathing, physical obesity, night awakening and dizziness. The top five tongue and pulse manifestations are enlarged tongue, slippery pulse, slippery coating, thick and white coating and purple tongue. The disease locations are the lung, spleen, stomach, kidney, liver and brain. The nature of disease includes deficiency, depression, blood stasis, phlegm, dampness and fire. The clinical syndrome types include the syndrome of stagnation of phlegm and dampness, syndrome of phlegm-dampness blocking the mind, syndrome of spleen deficiency with dampness, syndrome of Yin deficiency leading to fire hyperactivity, syndrome of Qi depression blocking collaterals, syndrome of liver depression and blood stasis, syndrome of Qi deficiency with dampness, and syndrome of Yang deficiency induced water retention. ConclusionIn addition to the common phlegm-, dampness- and blood stasis-related syndromes in patients with CIS and OSAHS, there are also depression- and deficiency-related syndromes. The main etiology and pathogenesis is the disturbance of Qi movement. In clinical practice, attention should be paid to the specific situation of individual patients to differentiate between deficiency and excess, and the treatment should be performed by the method of soothing and reinforcing, or unblocking and clearing, or both.

Abstract Background: The Brief Measure of Preoperative Emotional Stress (B-MEPS) is a suitable screening tool for Preoperative Emotional Stress (PES). However, personalized decision-making demands practical interpretation of the refined version of B-MEPS. Thus, we propose and validate cut-off points on the B-MEPS to classify PES. Also, we assessed if the cut-off points screened preoperative maladaptive psychological features and predicted postoperative opioid use. Methods: This observational study comprises samples of two other primary studies, with 1009 and 233 individuals, respectively. The latent class analysis derived emotional stress subgroups using B-MEPS items. We compared membership with the B-MEPS score through the Youden index. Concurrent criterion validity of the cut-off points was performed with the severity of preoperative depressive symptoms, pain catastrophizing, central sensitization, and sleep quality. Predictive criterion validity was performed with opioid use after surgery. Results: We chose a model with three classes labeled mild, moderate, and severe. The Youden index points −0.1663 and 0.7614 of the B-MEPS score classify individuals, in the severe class, with a sensitivity of 85.7% (80.1%-90.3%) and specificity of 93.5% (91.5-95.1%). The cut-off points of the B-MEPS score have satisfactory concurrent and predictive criterion validity. Conclusions: These findings showed that the preoperative emotional stress index on the B-MEPS offers suitable sensitivity and specificity for discriminating the severity of preoperative psychological stress. They provide a simple tool to identify patients prone to severe PES related to maladaptive psychological features, which might influence the perception of pain and analgesic opioid use in the postoperative period.

Resumo Pouco mais de uma década separa este estudo da primeira proposição acadêmica do entrincheiramento organizacional, e, até o momento, pesquisas longitudinais não foram realizadas, apesar da importância reconhecida desse método para enriquecer os estudos no campo comportamental. Este artigo objetivou identificar características do entrincheiramento organizacional entre servidores de instituições federais de ensino no Brasil, com base na avaliação longitudinal dos perfis latentes. Realizou-se uma pesquisa quantitativa e longitudinal com 1060 participantes na primeira coleta. Foram feitas análises descritivas, de comparação de médias e análise de transição latente. Os servidores da amostra apresentaram baixos níveis de entrincheiramento. Entre os principais achados, está a estabilidade dos perfis formados pelos servidores, tendo as médias mais altas sido encontradas nas dimensões "ajustamentos à posição social" e "arranjos burocráticos impessoais", o que reforça alguns achados teóricos transversais. Os resultados desta pesquisa contribuem para orientar gestores sobre a importância dos fatores internos para o entrincheiramento dos servidores e sobre como a atenção aos itens da dimensão "ajustamentos à posição social" pode favorecer o trabalho com esse vínculo. O presente estudo encontrou que, ao longo do tempo, o entrincheiramento é um vínculo estável.


Resumen Ha transcurrido poco más de una década entre la primera propuesta académica de atrincheramiento organizacional y el presente estudio y, hasta la fecha, no se ha llevado a cabo ninguna investigación longitudinal, a pesar de la reconocida importancia de este método para enriquecer los estudios en el campo del comportamiento. Esta investigación tuvo como objetivo identificar las características del atrincheramiento organizacional entre funcionarios de instituciones federales de enseñanza en Brasil, a partir de una evaluación longitudinal de perfiles latentes. Se realizó una encuesta cuantitativa y longitudinal con 1060 participantes. Se realizaron análisis descriptivos, comparación de medias y análisis de transición latente. Los funcionarios de la muestra presentaron bajos niveles de atrincheramiento. Entre los principales hallazgos están la estabilidad de los perfiles formados por los funcionarios, con las medias más altas encontradas en las dimensiones ajustes a la posición social y arreglos burocráticos impersonales, lo que refuerza algunos hallazgos teóricos transversales. Los resultados de esta investigación ayudan a orientar a los gestores sobre la importancia de los factores internos para el atrincheramiento de los funcionarios y sobre cómo la atención a los ítems de la dimensión ajustes a la posición social puede favorecer el trabajo con ese vínculo. Esta investigación constató que el atrincheramiento es un vínculo estable a lo largo del tiempo.


Abstract Little more than a decade separates this research from the first academic proposition of organizational entrenchment. To date, no longitudinal research has been carried out despite the recognized importance of this method for enriching studies in the behavioral field. This research aimed to identify characteristics of organizational entrenchment among civil servants at federal educational institutions in Brazil based on a longitudinal assessment of latent profiles. A quantitative and longitudinal survey was carried out with 1060 participants in the first collection. Descriptive analysis, comparison of means, and latent transition analysis were carried out. The civil servants in the sample showed low levels of entrenchment. Among the main findings are the stability of the profiles formed by the civil servants and the higher averages found in the dimensions of adjustments to social position and impersonal bureaucratic arrangements, reinforcing some cross-sectional theoretical findings. The results help to guide managers on the importance of internal factors for the entrenchment of civil servants and how attention to the items in the dimension of adjustments to social position can favor working with this bond. This research found that, over time, entrenchment is a stable bond.

ABSTRACT Background: The treatment strategy for latent tuberculosis infection is to reduce the number of tuberculosis cases and consequently reduce the transmission of pathogenic bacteria. This study aimed to determine the safety, effectiveness, and adherence of isoniazid use for latent tuberculosis infection treatment. Methods: To identify studies on isoniazid use for latent tuberculosis infection, five electronic databases were searched. The methods and results are presented in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Results: Most studies (53) used isoniazid for 9 months. The prevalence of use and adherence to treatment varied considerably (18% to 100%), and were evaluated by participant completion of isoniazid treatment for latent tuberculosis infection. The adverse events most frequently reported were hepatotoxicity, gastric intolerance, and neuropathy; the rates of occurrence ranged from < 1% to 48%. In the studies that evaluated the effectiveness of isoniazid for latent tuberculosis infection, the rate varied from 0 to 19.7% for patients who did not have active tuberculosis after the follow-up period. Conclusions: The importance of maintaining follow up for patients using isoniazid should be emphasized due to the risk of developing adverse events. Despite the treatment challenges, the rates of patients who used isoniazid and developed active tuberculosis during the follow-up period were low. We believe that isoniazid continues to contribute to tuberculosis control worldwide, and better care strategies are required.

ABSTRACT Objective: The aim of this study was to synthesize the evidence on the prevalence of latent Mycobacterium tuberculosis infection (LTBI) among undergraduate health care students. Methods: A systematic review of prevalence with meta-analysis was conducted. Prospective and retrospective cohorts and cross-sectional studies involving probable exposure to M. tuberculosis during undergraduate education, along with the tuberculin skin test (TST) or interferon-γ release assay (IGRA) for investigation of latent tuberculosis were searched. Searches were conducted in MEDLINE, CINAHL, EMBASE, LILACS, Scopus, and Web of Science databases. Independent reviewers were responsible for the selection and inclusion of studies. Data were extracted, critically appraised, and synthesized using the JBI approach. PRISMA was used to report the study. Results: Twenty-two studies were analyzed. The overall prevalence in healthcare undergraduate students was 12.53%. Conclusion: The prevalence of LTBI in undergraduate health students was high for such a highly educated population. Screening with TST and/or IGRA and chemoprophylaxis, when necessary, should be provided to undergraduate health students when in contact with respiratory symptomatic patients.


RESUMO Objetivo: O objetivo deste estudo foi sintetizar as evidências sobre a prevalência de infecção de Mycobacterium tuberculosis (ILTB) entre estudantes de graduação da área da saúde. Método: Foi realizada uma revisão sistemática de prevalência com metanálise. Coortes prospectivas e retrospectivas e estudos transversais envolvendo provável exposição a M. tuberculosis durante a graduação, juntamente com o teste tuberculínico (TT) ou ensaio de liberação de interferon-γ (IGRA) para investigação de tuberculose latente foram pesquisados. As buscas foram realizadas nas bases de dados MEDLINE, CINAHL, EMBASE, LILACS, Scopus e Web of Science. Revisores independentes foram responsáveis pela seleção e inclusão dos estudos. Os dados foram extraídos, avaliados criticamente e sintetizados utilizando a abordagem JBI. PRISMA foi usado para relatar o estudo. Resultados: Vinte e dois estudos foram analisados. A prevalência geral em estudantes de graduação da área da saúde foi de 12,53%. Conclusão: A prevalência de ILTB em estudantes de graduação em saúde foi alta para uma população com alto nível de escolaridade. Triagem com TT e/ou IGRA e quimioprofilaxia, quando necessária, deve ser fornecida aos estudantes de graduação da área da saúde quando em contato com pacientes sintomáticos respiratórios.

Las búsquedas se realizaron en las bases de datos MEDLINE, CINAHL, EMBASE, LILACS, Scopus y Web of Science. Revisores independientes fueron responsables de la selección e inclusión de los estudios. Los datos se extrajeron, se evaluaron críticamente y se sintetizaron utilizando el enfoque JBI. Se utilizó PRISMA para informar el estudio. Resultados: Se analizaron veintidós estudios. La prevalencia global en estudiantes universitarios en salud fue del 12,53%. Conclusión: La prevalencia de LTBI en estudiantes universitarios de salud fue alta para una población con un nivel educativo tan alto. Se debe proporcionar tamizaje con TST y/o IGRA y quimioprofilaxis, cuando sea necesario, a los estudiantes universitarios en salud cuando estén en contacto con pacientes sintomáticos respiratorios.

Objetivo: Objetivo: analisar na literatura científica os fatores associados à infecção latente pelo Mycobacterium Tuberculosis em profissionais da Atenção Primária à Saúde. Método: revisão integrativa da literatura, com coleta realizada entre novembro e dezembro de 2021. Considerou-se como critérios de inclusão: Estudos primários relacionados à temática e que incluam os seguintes profissionais: enfermeiros, médicos, dentistas, técnicos de enfermagem e técnicos de saúde bucal, sem delimitação temporal e de idiomas. Foram critérios de exclusão: estudos oriundos de opiniões de especialistas, editoriais, trabalhos de conclusão de curso, monografias, dissertações e teses. As bases de dados Biblioteca Virtual de Saúde, Medline, Scopus; Web of Science e Embase foram utilizadas. Resultados: foram selecionados nove estudos, que apresentaram os fatores associados idade, sexo e presença de doenças crônicas, estilo de vida e condições de trabalho. Conclusão: foi possível analisar os fatores associados à ILTB, o que poderá subsidiar a realização de políticas públicas mais assertivas e contribuir para o controle deste agravo(AU)


Objective: to analyze in the scientific literature the factors associated with latent infection by Mycobacterium Tuberculosis in Primary Health Care professionals. Method: integrative review of the literature, with collection carried out between November and December 2021. The inclusion criteria were considered: Primary studies related to the theme and which include the following professionals: nurses, doctors, dentists, nursing technicians and oral health technicians, without time or language limitations. Exclusion criteria were: studies originating from expert opinions, editorials, course completion works, monographs, dissertations and theses. The Virtual Health Library, Medline, Scopus databases; Web of Science and Embase were used. Results: nine studies were selected, which presented factors associated with age, sex and presence of chronic diseases, lifestyle and working conditions. Conclusion: it was possible to analyze the factors associated with LTBI, which could support the implementation of more assertive public policies and contribute to the control of this problem(AU)


Objetivo: analizar en la literatura científica los factores asociados a la infección latente por Mycobacterium Tuberculosis en profesionales de la Atención Primaria de Salud. Método: revisión integradora de la literatura, con recolección realizada entre noviembre y diciembre de 2021. Se consideraron como criterios de inclusión los estudios primarios relacionados con el tema y que incluyan a los siguientes profesionales: enfermeros, médicos, odontólogos, técnicos en enfermería y técnicos en salud bucal, sin limitaciones de tiempo ni de idioma. Los criterios de exclusión fueron: estudios provenientes de opiniones de expertos, editoriales, trabajos de finalización de cursos, monografías, disertaciones y tesis. Se utilizaron para las investigaciones: Biblioteca Virtual de Salud, Medline, Scopus; Web of Science y Embase. Resultados: se seleccionaron nueve estudios que presentaron factores asociados a la edad, sexo y presencia de enfermedades crónicas, estilo de vida y condiciones de trabajo. Conclusión: fue posible analizar los factores asociados a la ITBL, que podrían apoyar la implementación de políticas públicas más asertivas y contribuir al control de este problema(AU)

Objetivo: compreender os fatores que interferem no diagnóstico da sífilis em homens à luz da Teoria da Diversidade e Universalidade do Cuidado Cultural. Método: estudo qualitativo, aprovado por Comitê de Ética em Pesquisa, realizado em Hospital Universitário no Rio de Janeiro, entre setembro de 2017 e março de 2018. Foram entrevistados 32 homens diagnosticados com sífilis adquirida, e a análise temática foi utilizada para o tratamento dos dados. Resultados: a maioria dos homens tinha história de contágio por outra Infecção Sexualmente Transmissível e foi diagnosticada no ambulatório de imunologia. O momento diagnóstico é encarado com surpresa, tem repercussões psicossociais e é influenciado por fatores culturais e sociais. Considerações finais: alguns fatores interferem positivamente e outros negativamente no diagnóstico da sífilis na população masculina. Para detectar essa infecção nos homens, deve-se conhecer o contexto sociocultural em que estão inseridos para, assim, implementar estratégias tanto diagnósticas quanto preventivas mais eficazes(AU)


Objective: to understand the factors that interfere with the diagnosis of syphilis in men in the light of the Theory of Diversity and Universality of Cultural Care. Method: qualitative study, approved by the Ethics and Research Committee, conducted at a University Hospital in Rio de Janeiro, during september 2017 to march 2018. Thirty-two men diagnosed with acquired syphilis were interviewed, and thematic analysis was used for data treatment. Results: most men had a history of contagion by another STI and were diagnosed at the immunology outpatient clinic. The moment of diagnosis is faced with surprise, has psychosocial repercussions and is influenced by cultural and social factors. Final considerations: some factors interfere positively and others negatively in the diagnosis of syphilis in the male population. To detect this infection in men, it is necessary to know the sociocultural context in which they live, in order to implement more effective diagnostic and preventive strategies(AU)


Objetivo: comprender los factores que interfieren en el diagnóstico de la sífilis en los hombres a la luz de la Teoría de la diversidad y universalidad del cuidado cultural. Método: estudio cualitativo, aprobado por el Comité de Ética en Investigación, realizado en un Hospital Universitario de Río de Janeiro, de septiembre de 2017 a marzo de 2018. Se entrevistó a 32 hombres diagnosticados de sífilis adquirida y se utilizó el análisis temático para el tratamiento de los datos. Resultados: la mayoría de los hombres tenía antecedentes de contagio por otra Infección Sexualmente Transmisible y el diagnóstico se hizo en el ambulatorio de inmunología. El momento del diagnóstico se afronta con sorpresa, tiene repercusiones psicosociales y está influenciado por factores culturales y sociales. Consideraciones finales: algunos factores interfieren positivamente y otros negativamente en el diagnóstico de la sífilis en la población masculina. Para detectar esta infección en los hombres, se debe conocer el contexto sociocultural en el que se insertan, para entonces poner en marcha estrategias de diagnóstico y de prevención más eficaces(AU)

Introducción. Debido a la poca evidencia sobre el modelamiento de los patrones de alimentación y actividad física (AF), basado en variables latentes, el presente estudio de revisión pretende describir las técnicas estadísticas aplicadas para modelar estos patrones en niños y adolescentes y valorar su calidad metodológica. Materiales y métodos. La búsqueda se realizó en bases de datos electrónicas (Science Direct, PubMed, SCOPUS, Web of science y Cochrane) con las palabras "diet", 'physical activity', children y 'latent variable'. Se incluyeron artículos que utilizaron modelos estadísticos basados en variables latentes para analizar patrones de alimentación y AF en niños y adolescentes sanos, publicados entre 2014­2019, en inglés o español. Resultados. Entre los 27 artículos seleccionados, el Modelo de Ecuaciones Estructurales (MEE) fue el más utilizado (77,78%); seguido del Modelo de Perfil Latente (7,41%), mientras, el restante, 14,81% aplican el Modelo del Factor Común, Modelo Ecológico y el Modelo de Regresión Logística Multinivel. El MEE fue aplicado a 12 de los 16 artículos con enfoque de AF, y en 7 de los 9 artículos con enfoque de Alimentación. El 48,15% de los estudios sí justificaba el uso del modelo, y el 37,04% poseen una calidad "Excelente" (cumplen el 85% o más de los ítems de STROBE). Conclusiones. El MEE fue el más utilizado para abstraer los patrones de AF y alimentación en niños y adolescentes, sin embargo, solo la mitad de los artículos justifica su pertinencia. Las guías de reporte de estudios deberían evaluar la calidad metodológica de los modelos estadísticos aplicados(AU)


Introduction. Due to the limited evidence on the modeling of eating and physical activity (PA) patterns based on latent variables, the present review study aims to describe the statistical techniques applied to model these patterns in children and adolescents and to assess their methodological quality. Materials and methods. The search was performed in electronic databases (Science Direct, PubMed, SCOPUS, Web of science and Cochrane) with the words 'diet', 'physical activity', children and 'latent variable'. We included articles that used statistical models based on latent variables to analyze diet and PA patterns in healthy children and adolescents, published between 2014-2019, in English or Spanish. Results. Among the 27 selected articles, the Structural Equation Model (SEM) was the most used (77.78%); followed by the Latent Profile Model (7.41%), while, the remaining 14.81% applied the Common Factor Model, Ecological Model and Multilevel Logistic Regression Model. The SEM was applied to 12 of the 16 articles with PA approach, and in 7 of the 9 articles with eating approach. The 48.15% of studies did justify the use of the model, and 37.04% were classified as "Excellent" quality (meet 85% or more of the STROBE items). Conclusions. The SEM was the most commonly used to model the PA and eating patterns in children and adolescents, however, only half of the articles justify their relevance. Study reporting guidelines should evaluate the methodological quality of the statistical models applied(AU)

El uso preventivo de antimicrobianos es de larga data y no se restringe a antibacterianos. Lo más consensuado y estructurado es la profilaxis antimicrobiana perioperatoria y ante procedimientos invasivos. Fuera de este contexto hay gran cantidad de situaciones, menos caracterizadas, con riesgo de infecciones en que se usan ampliamente, muchas veces con menor sistematización. Esta presentación presenta las bases conceptuales y operativas de este segundo tipo de profilaxis. Conceptualmente la profilaxis primaria pretende evitar la infección por agente único conocido o variados, por exposición ambiental o susceptibilidad específica de ese hospedero y es implementable antes o después de la exposición. Producida esta infección la meta de la profilaxis secundaria intenta evitar la enfermedad y puede tomar dos modalidades, en infecciones sin evidencias de enfermedad clínica o daños, la profilaxis corresponde a "tratamiento de infección latente" y, si aún en ausencia de manifestaciones clínicas, hay elementos de laboratorio precoces premonitorios de progresión, la profilaxis se denomina "tratamiento anticipatorio". Se presentan operacionalmente y resumidas las situaciones en contexto médico no invasivo con uso potencial preventivo de antimicrobianos en base a agentes posibles, situaciones ambientales de riesgo, vulnerabilidad del hospedero, medicamentos a usar, su duración y efectividad con enfoque mayoritario en medicina de adultos.


Antimicrobial use with preventive purpose probably began shortly after its therapeutic use, especially antibiotics. More consensus and sistematization exist with perioperative and invasive procedures prophylaxis. However, beyond that context, there is great number of non invasive medical situations with high risk of secondary infections either by acquisition of pathogens or activation of latent ones, in which antimicrobials are routinely used with preventive purpose, albeit with less sistematization and consensus. This presentation aims to lay down the conceptual and operational basis for antimicrobial prophylaxis in these settings, whose objective is preventing an infection (primary prophylaxis) by a known or a variety of pathogens, either by person to person transmission, enviromental exposure or particular susceptibility of the host, and can be implemented before or after exposure. If already infected, the antimicrobial prophylaxis goal is to avoid progression to disease (secondary prevention) and may take two conceptual approaches; first, without clinical disease but significant risk of progression, the modality can be called "treatment of latent infection". In the second, also clinically asymptomatic, but with premonitory laboratoy signs of impending progression present, early use of antimicrobial is called "preemptive treatment". This presentation will describe the most frequent medical situations where preventive use of antimicrobials is employed, together with the medications most consensually used, according to the host, the agent(s) and medical situation, with emphasis in adults.

Abstract HIV risk perception and Intimate partner violence (IPV) are related to HIV infection in women. However, there is a lack of information regarding on the association between risk perception and IPV in the Mexican population. This study aimed to identify and characterize subgroups based on their risk perception through the analysis of latent classes and to evaluate the variables associated with HIV risk perception. An online survey was conducted to assess sexual and preventive behavior, HIV risk perception, and intimate partner violence. Three hundred thirty-two women participated; the sampling was for convenience. Two latent classes were identified: a group with risk perception (85%) and one without risk perception (14.6%). The variables associated with the risk perception were a sexual attraction to men, not having a stable partner, perceiving oneself as at risk of acquiring HIV due to sexual behavior, and the presence of IPV. HIV prevention programs for women must address the stigmas related to HIV, such as the idea that the risk only occurs when they have multiple sexual partners, promote accurate HIV risk perception and prevent IPV in all its expressions and contexts.


Resumen En mujeres, la violencia de pareja está relacionada a una mayor probabilidad de adquirir la infección por VIH. De igual forma, una inadecuada percepción de riesgo de adquirir el virus coloca a las mujeres en una situación vulnerable. Existe una falta de información ente la asociación de la VPI y la percepción de riesgo es mujeres mexicanas. Este estudio persiguió dos objetivos: el primero fue identificar y caracterizar subgrupos en función de su percepción de riesgo a través del análisis de clases latentes; el segundo objetivo consistió en evaluar las variables asociadas con la percepción de riesgo de VIH. Se realizó una encuesta en línea para evaluar el comportamiento sexual y preventivo, la percepción del riesgo de VIH y la VP. Participaron 332 mujeres; el muestreo fue por conveniencia. Se identificaron dos clases latentes: un grupo con percepción de riesgo (85%) y otro sin percepción de riesgo (14,6%). Las variables asociadas a la percepción de riesgo fueron atracción sexual por hombres, no tener pareja estable, percibirse en riesgo de adquirir el VIH por la conducta sexual y haber sufrido de violencia de pareja en los últimos seis meses. Se concluye que los programas de prevención del VIH para mujeres deben abordar los estigmas relacionados con el VIH, como la idea de que el riesgo solo ocurre cuando tienen múltiples parejas sexuales, promover una percepción precisa del riesgo del VIH y principalmente, prevenir la VP en todas sus expresiones y contextos; así como también dar atención personalizada a mujeres que ya sufren de violencia de pareja.

Introducción. La tuberculosis continúa siendo un problema frecuente en contextos de vulnerabilidad socioeconómica. El objetivo principal fue establecer la prevalencia de infección latente y viraje tuberculínico en contactos escolares de casos de tuberculosis. Población y métodos. En un área programática del sur de la ciudad, se evaluó la prevalencia de infección y viraje tuberculínico de 691 niñas, niños y adolescentes utilizando la prueba cutánea de tuberculina. Se investigó la asociación entre pérdida de seguimiento por parte del equipo de salud y características demográficas, escolares y asistencia inicial, y se describió el grado de adherencia cuando la quimioprofilaxis con isoniacida fue indicada. Resultados. Según las definiciones consideradas, la prevalencia de infección latente fue entre el 3,4 % (IC95 %: 2,3-5,2) y el 11,6 % (IC95 %: 9,3-14,4) de los 610 contactos con al menos una prueba cutánea aplicada. La incidencia de viraje tuberculínico se encontró entre el 0,3 % y el 6,8 % de los 294 evaluados. La edad mayor de 18 años, la mayor prevalencia de necesidades básicas insatisfechas en la comuna escolar, la pertenencia al turno escolar vespertino, la negatividad en la baciloscopia del caso índice y la ausencia de aplicación de la prueba cutánea inicial se asociaron con pérdida de seguimiento del contacto. Conclusiones. La incidencia de viraje tuberculínico en contactos escolares fue baja. La adherencia a isoniacida continúa siendo limitada. Se identificaron factores asociados con la pérdida de seguimiento de contactos que podrían orientar estrategias necesarias para mejorar este proceso.


Introduction. Tuberculosis continues to be a common problem in settings of socioeconomic vulnerability. Our primary objective was to establish the prevalence of latent infection and tuberculin conversion among school contacts of tuberculosis cases. Population and methods. In a programmatic area in the south of the City of Buenos Aires, the prevalence of latent infection and tuberculin conversion was assessed in 691 children and adolescents using the tuberculin skin test. The association between loss to follow-up by the health care team and the demographic, school, and baseline care characteristics was studied, and the level of adherence when isoniazid chemoprophylaxis was indicated was described. Results. According to established definitions, the prevalence of latent infection was between 3.4% (95% confidence interval [CI]: 2.3­5.2) and 11.6% (95% CI: 9.3­14.4) in the 610 contacts with at least one skin test. The incidence of tuberculin conversion was between 0.3% and 6.8% in the 294 assessed participants. Age older than 18 years, a higher prevalence of unmet basic needs in the school district, attending the afternoon school shift, negative sputum smear results in the index case, and absence of baseline skin test were associated with contact lost to follow-up. Conclusions. The incidence of tuberculin conversion among school contacts was low. Adherence to isoniazid treatment remains limited. Factors associated with loss of contact tracing were identified, which may guide strategies necessary to improve this process.

Abstract The purpose of the cross-sectional study was to identify patterns of modifiable lifestyle behaviors and examine the relationship between sociodemographic characteristics and distinct lifestyle behaviors. The data were gathered from the National Health Survey 2019, a study that included adults with diabetes. Four domains of lifestyle behaviors were used to define these behaviors: smoking, alcohol consumption, physical activity, and diet. The association between patterns of lifestyle behaviors and variables of interest was assessed using multinomial regression analysis. The three lifestyle patterns identified were: Class 1, referred to as "unhealthy diet," comprised 17.0% of the sample and was characterized by unhealthy eating habits; Class 2 (less active and insufficient fruit and vegetable intake) represented 71.2% of the sample; Class 3 referred to as "low risk" (11.8%) is characterized by a lower probability of engaging in most risky behaviors. A person over 45 years of age with little or no education and no health care coverage was less likely to be a member of Class 1. Male individuals who do not attend a doctor regularly exhibited more chances of belonging to Class 2. Mixed-race individuals aged 45 years or more with a low level of education have a lower chance of belonging to this class.


Resumo Neste estudo de caráter transversal objetivou-se identificar os padrões de comportamento de estilo de vida e sua associação com características sociodemográficas. Utilizou-se como base de dados a Pesquisa Nacional de Saúde de 2019, com adultos (≥ 18 anos) diabéticos. Os padrões de saúde foram definidos pela Análise de Classes Latentes em quatro domínios: tabagismo, consumo de bebidas alcoólicas, atividade física e alimentação. Foi aplicada análise de regressão multinominal para identificar a associação entre os padrões de comportamento e as variáveis sociodemográficas de interesse. Identificou-se três padrões de comportamento: a Classe 1 corresponde a 17% da amostra e compreende indivíduos com maior probabilidade de comportamentos de risco ligados à alimentação; a Classe 2 (baixos nível de atividade física e consumo de frutas e hortaliças) compreende 71,2% da amostra; e a Classe 3 (11,8% da população) reúne os indivíduos com menor chance de desenvolver comportamentos de risco. Indivíduos com 45 anos ou mais, com baixa escolaridade e sem plano de saúde têm menos chances de pertencer à Classe 1. Homens, que não fazem visitas regulares ao médico têm maiores chances de pertencer à Classe 2, bem como aqueles com 45 anos ou mais, com baixa escolaridade.

Abstract Since the onset of the COVID-19 outbreak, numerous articles have highlighted a possible link between COVID-19 vaccination or infection and Herpesviridae co-infection or reactivation. The authors conducted an exhaustive literature review on this topic, the results of which are presented individually for each member of the Herpesviridae family: Herpes Simplex Virus (HSV) types-1 (HSV-1) and 2 (HSV-2); Varicella-Zoster Virus (VZV); Epstein-Barr Virus (EBV); Cytomegalovirus (CMV); HHV-6; HHV-7; and HHV-8. These human herpesviruses can serve as prognostic markers for the COVID-19 infection and may even underlie some of the clinical manifestations initially attributed to SARS-CoV-2. In addition to SARS-CoV-2 infection, all corresponding vaccines approved to date in Europe appear capable of inducing herpesvirus reactivation. It is important to consider all viruses of the Herpesviridae family when managing patients infected with or recently vaccinated against COVID-19.

Resumen Estudios sugieren la presencia de patrones de violencia en las relaciones de noviazgo durante la adolescencia. En la población adolescente mexicana se conoce poco sobre los patrones de abuso cara a cara y digital en el noviazgo y cómo pueden ser explicados por el consumo de alcohol. El objetivo de esta investigación fue identificar los patrones de victimización por abuso en las relaciones de noviazgo y determinar si el consumo de alcohol predice los patrones encontrados. Fue un estudio transversal con alcance explicativo. Participaron 398 adolescentes estudiantes (62.8% mujeres) de 15 a 18 años de edad (M = 16.1 años; DT = 1). Se usó un análisis de clases latentes y se encontraron tres clases: 1) violencia generalizada baja (45%), 2) violencia psicológica moderada y control digital alto (38%) y 3) violencia generalizada alta (17%). Se encontró que el consumo de alcohol se asoció con la pertenencia a la clase de violencia psicológica moderada y control digital alto (β = 0.48, p = .022) y con la pertenencia a la clase de violencia generalizada alta (β = 0.66, p = .004). Es importante considerar, en la generación de intervenciones, la existencia de patrones de violencia en el noviazgo en adolescentes y la influencia que tiene el consumo de alcohol sobre estos.


Abstract Studies suggest the existence of patterns of dating violence during adolescence. In the adolescent Mexican population, little is known about the patterns of face-to-face and cyber dating abuse and to what extent they can be explained by alcohol consumption. The aim of this research was to identify patterns of dating abuse victimization and to determine whether alcohol use predicts the patterns found. It was a cross-sectional study with an explanatory scope. A total of 398 adolescent students (62.8% women) from 15 to 18 years of age (M = 16.1 years; SD = 1) participated in the study. Latent class analysis was used, and three classes were found: 1) low generalized violence (45%); 2) moderate psychological violence and high digital control (38%); and 3) high generalized violence (17%). Alcohol consumption was found to be associated with the membership in the moderate psychological violence and high digital control (β = 0.48, p = .022) and were included in the high generalized violence class (β = 0.66, p = .004). It is important to consider, in the generation of interventions, the existence of patterns of violence in dating relationships among adolescents and the influence that alcohol consumption has on them.

Abstract Background: This study aimed to describe the clinical and demographic characteristics of children with confirmed tuberculosis disease and identify associated factors. Methods: We conducted a retrospective and observational study at the Hospital Civil de Guadalajara Dr. Juan I. Menchaca. Inpatient and outpatient children under 18 years of age who were reported to the National Epidemiological Surveillance System (SINAVE, for its Spanish acronym) for suspected tuberculosis and who had molecular or microbiological tests for mycobacteria were included in the study. Multivariate analysis with logistic regression was used to analyze associated factors. Results: One hundred and nine patients under 18 years of age with suspected tuberculosis were included in the study. About 50.5% (55/109) were male, and the median age was 11 years. Tuberculosis was confirmed in 55% (n = 60): 15% (9/60) had a pulmonary infection, and the rest (51/60) had an extrapulmonary infection. The diagnostic tests used were histopathological study (n = 26), expectoration or gastric aspirate stains (n = 17), polymerase chain reaction (n = 12), and cultures (n = 5). Positive purified protein derivative (PPD) or interferon-gamma release assay (IGRA) tests were found in 33.9%. Malnutrition (odds ratio [OR] 15.9, 95% confidence interval [CI]: 2.3-109), and consumption of unpasteurized products (OR 7.45, 95% CI: 1.02-54.3) were associated with tuberculosis disease in children. Conclusions: Malnutrition and consumption of unpasteurized dairy products are associated with tuberculosis.


Resumen Introducción: El objetivo de este estudio fue describir las características clínicas y demográficas de niños con enfermedad tuberculosa confirmada e identificar los factores asociados. Métodos: Se realizó un estudio observacional retrolectivo en el Hospital Civil de Guadalajara Dr. Juan I. Menchaca. Se incluyeron menores de 18 años hospitalizados y ambulatorios que se notificaron al Sistema Nacional de Vigilancia Epidemiológica (SINAVE) por sospecha de tuberculosis y que contaron con pruebas moleculares o microbiológicas para micobacterias. El estudio de los factores asociados se realizó mediante análisis multivariado con regresión logística. Resultados: Se incluyeron en el estudio 109 menores de 18 años con sospecha de tuberculosis. El 50.5% (55/109) fueron de sexo masculino y la mediana de edad fue de 11 años. Se confirmó enfermedad tuberculosa en el 55% (n = 60) de los casos: el 15% (9/60) presentaron infección pulmonar y el resto extrapulmonar. Las pruebas diagnósticas utilizadas fueron el estudio histopatológico (n = 26), tinciones de expectoración o aspirado gástrico (n = 17), reacción en cadena de la polimerasa (n = 12) y cultivos (n= 5). 33.9% de los pacientes presentaron prueba de derivado proteico purificado (PPD) o ensayo de liberación de interferón gamma (IGRA) positiva. Se observó que la desnutrición (razón de momios (RM) 15.9, intervalo de confianza (IC) 95% 2.3 - 109) y el consumo de productos no pasteurizados (RM 7.45, IC 95% 1.02 - 54.3) se asociaron con enfermedad tuberculosa en niños. Conclusiones: La desnutrición y el consumo de lácteos no pasteurizados se asocian con la enfermedad tuberculosa.

Introducción: la infección tuberculosa latente es un estado de respuesta inmune tipo hipersensibilidad celular retardada del organismo a la estimulación por antígenos de Mycobacterium tuberculosis sin evidencia clínica y radiológica de tuberculosis. Puede ser diagnosticada mediante la prueba de tuberculina o derivado proteico purificado de tuberculina. Objetivos: determinar la frecuencia de infección tuberculosa latente en estudiantes de Medicina y las variables asociadas a una reacción positiva en la prueba de tuberculina. Metodología: se realizó un estudio descriptivo de corte transversal, en el cual se procedió a realizar la prueba de tuberculina por el método de Mantoux utilizando 0,1 ml de derivado proteico purificado de tuberculina Tubersol, en estudiantes de la carrera de Medicina de una universidad. Resultados: participaron 290 estudiantes. La frecuencia de infección tuberculosa latente fue 19,3%. Al relacionar las variables demográficas con la reacción positiva en la prueba de tuberculina, los hombres presentaron 1,895 veces mayor probabilidad de dar positivo a la prueba de tuberculina que las mujeres. Conclusiones: la frecuencia de infección tuberculosa latente en estudiantes de la carrera de Medicina fue 19,3% y la variable que presentó asociación estadística a la prueba de tuberculina positiva fue el sexo masculino.


Introduction: Latent tuberculosis infection is a state of delayed cellular hypersensitivity immune response of the organism to stimulation by Mycobacterium tuberculosis antigens without clinical and radiological evidence of tuberculosis. It can be diagnosed by tuberculin test or tuberculin purified protein derivative. Objectives: To determine the frequency of latent tuberculosis infection in medical students and the variables associated with a positive reaction in the tuberculin test. Methodology: A descriptive cross-sectional study was carried out, in which the tuberculin test was made by the Mantoux method using 0.1 ml of purified protein derivative of Tubersol tuberculin in students of the medicine career of a university. Results: Two hundred ninety students participated. The frequency of latent tuberculosis infection was 19.3%. When relating the demographic variables to the positive reaction in the tuberculin test, men were 1,895 times more likely to test positive for the tuberculin test than women. Conclusions: The frequency of latent tuberculosis infection in medical students was 19.3% and the variable that presented a statistical association with the positive tuberculin test was male sex.

Purpose: To assess the perception of college?going girls toward corneal donation in Northern India. Methods: An online survey with a pre?structured, pre?validated questionnaire was conducted on 1721 college?going girls in Northern India. The knowledge and attitude scores were regressed, and latent class analysis was carried out. Results: The average of scores for all participants was computed individually for the knowledge questions and the attitude questions, and based on this score, total participants were divided into two groups: Better corneal donation behaviors (BCDB) and poor corneal donation behaviors. The binomial logistic regression model of knowledge domain for predicting BCDB, age of the participant, their awareness about corneal donation, and willingness to discuss eye donation among family members were found significant. Similarly, for the attitude domain, awareness about corneal donation, knowledge about hours within which ideal eye donation needs to be undertaken, and knowledge about eye donation during coronavirus disease 2019 (COVID?19) pandemic were found to be significant. Latent class analysis identified one subset of participants having poorer knowledge and attitude scores and that they were more from a rural background, were having more than first order as birth order, were belonging to SC/ST classes, had illiterate or secondary education of father and mother, and were living in rented houses. Conclusion: The findings of the study significantly contribute to devising a mechanism to improve knowledge and influencing the attitude about eye donation among the youth, especially young women, who can act as counselors and motivators for the masses as well as their own families, in the generations to come.
