RESUMO

The camps have been organized for rural cervical cancer screening in the villages of Lucknow which offer valuable platform for the detection of infertility in young women and their cytological examination. The cervical cytology has been studied in the cases of infertility to find out if there is any increase in abnormal cytology and compare them with those obtained in fertile women (controls).Since its inception in May 2013, the camps organized were 186 in number and 5682 women attended these camps. Primary infertility was found in 182 and the secondary infertility in 31 women. The incidence of Squamous intraepithelial lesions of cervix (SIL) was found to be 14.8% in women suffering from primary infertility and 16.1% in the secondary infertility cases and was comparable with SIL rate of 17.4% in the controls. Persistent vaginal infections caused by the poor personal genital hygiene practiced by the rural women may be the reason for a high SIL rate in the fertile women. The SIL rate was seen high in the younger women upto 30 years with pain in lower abdomen and erosion cervix in the primary infertility cases while the high SIL rate was seen in 21- 30 years age groups with parity 2 in secondary infertility cases with no SIL seen with symptoms and clinical lesions. Mostly young women upto 30 years of age complained of infertility and showed high SIL rate. These women were referred to the Hospital of Era Medical College, Lucknow for further investigation and treatment.

Background: Syndromic management has been the centre for treating STIs in a resource constraint facility since its introduction by WHO and CDC more than a decade back. Recently there has been lots of debate on its empirical use in an era of antibiotics resistance and ever-changing varieties of microorganisms causing these infections. Present study evaluated the efficacy of pharmacotherapy of syndromic management measured by symptomatic improvement in females presenting with STIs.Methods: It was a two year cross-sectional prospective study including all the females of STI syndromes, attending Gynecology OPD of Sanjay Gandhi Memorial Hospital and results were assessed in pharmacology department, Gajra Raja medical college Gwalior from Jan 2015 to December 2016.Results: During the study period total of 24,556 patients attended the Gynecology OPD out of which 8562 patients presented with different STI syndromes. Lower abdomen pain with vaginal discharge was the most common symptom while Ulcerative lesions were the least common complaint. Maximum recurrence was seen in patients of vaginal discharge and itching followed by PID syndrome which responded to second line of drugs.Conclusions: Syndromic Management of STIs is an old approach to deal with a very common gynecological problem and needs to be reviewed in an era of wide spread antibiotics resistance.

RESUMO

Objective] To explore the theoretical origin, clinical application method, effect and practical significance of the treatment of ectopic pregnancy with blood stasis. [Method] From blood stasis on the treatment by promoting blood circulation to remove blood stasis, Yiqi Yangxue Huayu Shapei, expelling blood circulation, method to eliminate the disease, stage of treatment, a reasonable choice of promoting blood circulation and removing blood stasis drugs in the treatment of ectopic pregnancy. [Results] From blood stasis on the treatment of ectopic pregnancy, it can accelerate the beta HCG negative, and speed up the ectopic pregnancy mass absorption, clinical curative effect significantly, the greatest degree of protection of fallopian tube integrity, improve blood circulation, and tubal function recovery, increase after treatment the chances of conception. [Conclusion] Qi stagnation, blood stasis and Qi deficiency and blood stasis are ectopic pregnancy basic pathogenesis, less empirical abdominal blood stasis is the essence of its pathogenesis. The existence of blood stasis in ectopic pregnancy of each type and disease development of the whole process, in the occurrence of the disease, and occupies an important position in the development, promoting blood circulation and removing blood stasis is the basic treatment, should be in clinical dialectical treatment of the whole process of fusion.

<p>In Kampo, respiratory symptoms are treated with prescriptions related not only to “lung”, but also other parenchymatous viscera. We report 2 patients whose stress-induced chronic cough was ameliorated by <i>hachimijiogan</i>. Case 1 was a 25-year-old female who was working under stressful circumstances at her company and reported an oppressive feeling in the chest. She was initially prescribed <i>hangekobokuto </i>because of a feeling that something was stuck in the pit of her stomach on abdominal examination, but her cough did not get better. As she also noted a dull feeling in her back ; she was switched to <i>hachimigan</i>, and her cough disappeared. Case 2 was 42-year-old female who suffered from depression, sore throat and an obstructive feeling in the throat ; she had been receiving infertility treatment for several years. <i>Hangekobokuto </i>and <i>bakumondoto </i>showed insufficient effect on her persistent cough. Her coughing stopped when she was prescribed <i>hachimigan </i>for back pain. Neither of these patients showed lack of resistance of the lower abdomen on abdominal examination.<br>Chronic <i>ki </i>(qi) stagnation under stressful conditions may cause <i>ki </i>deficiency, especially kidney deficiency. The symptom of stress-induced cough in our cases was considered to be due to kidney deficiency, and therefore <i>hachimijiogan</i>, but not <i>hangekobokuto</i>, was effective. The short duration of the complaints and relatively young age (20-40's) of the patients may account for the absence of the typical abdominal sign of kidney deficiency. Back stiffness and pain may also be important signs for cough due to kidney deficiency.</p>

RESUMO

A 29 year-old female patient with excessive cold sensitivity (“hiesho”) was regularly treated with acupuncture therapy. She complained of symptoms from autumn to winter, and her symptoms were also susceptible to changes in temperature. From the oriental medical diagnosis, we found that she exhibited “oketsu” and “suitai”. The effects of acupuncture therapy were comprehensively assessed by the Oketsu score, Visual Analogue Scale (VAS) thermograms, etc. We also examined the influence of temperature and menstrual periods on cold sensitivity.<BR>After five months of this therapy (eight times), the Oketsu score and VAS showed a decrease. In addition, this therapy held to elevate the skin temperatures of extremities. It was noted that the temperarure, rather than the menstrual periods, had a stronger influence on VAS. These results were that the acupuncture therapy was effective against the cold sensitivity.
