RESUMO

Objective:To explore the methods of improving the clinical thinking ability of clinical medical interns in neurology department. Methods:Clinical medical interns from 2017 to 2018 were selected as the research subjects. The clinical medical interns in 2018 and in 2017 were set as the experimental group (98 people) and the control group (95 people). In the clinical practice of the neurology department, the experimental group was trained in clinical thinking ability, and the control group was trained by the traditional methods. Before the end of the internship, the self-made Clinical Medical Interns Clinical Thinking Ability Questionnaire was used to evaluate the teaching effect. SPSS 14.0 was used for t-test and Chi-square test. Results:The experimental group and the control group had the problems of one-sidedness, appearance, fixedness, passivity, laziness, simplification, confusion of clinical thinking. The clinical thinking problems in the experimental group were significantly improved than those in the control group ( P<0.001). There were significant differences in clinical thinking between the experimental group and the control group in terms of one-sidedness, appearance, fixedness, passivity and laziness ( P<0.05). In addition to language communication and expression ability, the self-assessment score of clinical thinking ability in the experimental group was statistically different from that in the control group ( P<0.05). Compared with the control group, there were also significant differences in the examination results of the experimental group ( P< 0.05). Conclusion:The implementation of clinical thinking training in neurology practice is conducive to cultivating students' clinical thinking ability and improving the quality of personnel training.

RESUMO

Topical steroids are the most commonly prescribed drugs in dermatological setting, and they are also one of the most abused ones, as these drugs are very cheap, easily available over the counter, and provide quick relief in symptoms. This study aims to find the level of awareness and attitude regarding topical steroids among medical interns. MethodsThis was a cross sectional questionnaire-based study conducted among medical interns of 2014 batch of Rohilkhand Medical College in the Department of Dermatology, Venereology and Leprosy. The study was conducted with the help of a questionnaire that was modified from the study, Verma P, et al. This was followed by a presentation on topical steroids use, misuse and what steps can be taken to prevent their rampant misuse. Finally, a feedback was taken on importance of the study. ResultsTotal 69 medical interns were included in the study. Fifty-two interns had previously prescribed or used topical corticosteroids. Steroids (clobetasol, betamethasone, mometasone) in combination with either tretinoin & hydroquinone or antimicrobials were commonly abused. The commonest indication for which they were used were infections of which tinea was the most common followed by dark spots or as a fairness cream, acne, eczema, and other conditions such as pruritus, insect bite, burn, etc. ConclusionsThis study shows significant knowledge gap among medical interns regarding topical steroids. Thus, knowledge regarding topical corticosteroids is very essential and should be emphasized more during the MBBS period. More studies are required to study regarding their awareness and misuse.

RESUMO

Background: Diabetes has become a major health problem in India. Inadequate knowledge and training of frontline medical care givers like nursing staff, intern doctors and resident doctors may result in increase in both acute and chronic complications amongst diabetes patients. So, it results to high economic burden on family especially in rural parts of India. So, objective of this study was to assess knowledge about diabetes and its management amongst medical interns and nursing staff in tertiary care teaching hospital in Marathwada region of Maharashtra. Methods: This was a cross-sectional study conducted at IIMSR Medical College, Badnapur, Jalna, Maharashtra amongst 70 nursing staff and 70 medical interns by convenient sampling method during the period of June to August 2018. Knowledge on diabetes was assessed through 30 questions. Results: Knowledge related to diabetes and its management is assessed in 70 nursing staff and 70 medical interns using 50 item questionnaires. Out of 70 nursing staff mean knowledge score was 38.13 and out of 70 medical interns mean knowledge score was 40.17. Those nursing staff that had experience more than 5 years and positive family history of DM had significant greater knowledge than counterpart. There was no significant difference in knowledge level in gender, age, degree or diploma and whether in-service education opted or not amongst nursing staff. Amongst intern knowledge was more in those who had positive family history of DM. There was no significant difference in knowledge level in male and female interns. Conclusions: This study concluded that there was knowledge gap about diabetes and its management amongst frontline care givers like nursing staff as compared to medical interns suggesting requirement of additional training and educations amongst frontline health care providers.

RESUMO

Objective To study the intervention effectiveness of intensive training in medical interns’occupational exposure protection. Methods Medical interns in seven colleges and universities in Wuhan between 2011 and 2015 were chosen and divided into control group (675 medical interns in 2011 - 2013,comprehensive training about healthcare-associated infection control was conducted for once before internship )and intervention group (682 medical interns in 2014 - 2015,hospital-department-tutor three steps of intensive training was conducted),the occurrence of occupational exposure and exposure knowledge awareness between two groups were compared. Results The awareness rate of knowledge about occupational exposure protection and the implementation rate of protective measures in intervention group were both higher than control group (all P <0.05).Incidence of occupa-tional exposure in intervention group was lower than control group (21 .70%[n=148]vs 65.33%[n=441 ],χ2 =262.91 ,P <0.01);percentage of interns who conducted active serological virus detection and intensified vaccination of hepatitis B virus were both higher than control group (14.66%[n=100]vs 2.96%[n=20];11 .73%[n=80]vs 2.67%[n=18],respectively).Occupational exposure before and after intervention occurred most frequently when interns were preparing medicine, occupational exposure reporting rate in intervention group was higher than control group (72.97%[108/148]vs 50.11 % [221/441 ],χ2 =52.78,P <0.01 ).Conclusion The intensive training of occupational exposure protection among medical interns can improve the awareness and skills of occupational protec-tion,and reduce the occurrence of occupational exposure.

RESUMO

Clinical tutors' personality manifesting during the process of clinical practice and daily communication with medical interns has a series of influences on medical ethics formation of medical interns, including influences of direction, demonstration, contraction, and halo effect. Thus, clinical tutors should pay attention to cultivating a decent moral personality, and hospitals should value the proper combination of moral evaluation and supervision for medical staff in order to achieve the unity of heteronomous morality and self discipline, and effectively promote the formation of decent morality and personality of clinical tutors.
