RESUMO

The construction of hospital administration talent echelon has become a " bottleneck" problem in the core competence construction of county-level public hospitals in ethnic regions. West China-Mabian Medical Alliance has made a preliminary exploration on the cultivation of hospital management talents in county-level public hospitals. The hospital carried out the working principle of " setting up a talent pool by post" , and gradually established a reserve talent pool with suitable scale and dynamic adjustment by providing part-time project management positions for young employees in the hospital. There were three kinds of part-time project management positions: part-time assistant to president, part-time project supervisor and part-time department management assistant. In addition, the hospital strengthened the ideological and political education, medical management theory training and practical training of the reserve talents in a planned way. The practical experience of hospital reserve management personnel training based on West China-Mabian Medical Alliance can be used for reference by other county-level hospitals.

Objective @#To learn the awareness of chronic diseases prevention and control and its influencing factors among high school students in regions inhabited by ethnic groups,so as to strengthen the prevention of chronic diseases among adolescents in these regions. @*Methods@#Totally 1 050 high school students in Tuquan County,Hinggan League,Inner Mongolia were selected by cluster sampling method. The general information,family factors,attitude to learning,psychological factors,behavioral styles and awareness of chronic diseases prevention and control were investigated,and the influencing factors for the awareness were analyzed by multivariate logistic regression model.@*Results@#There were 1 001 valid questionnaires recovered,with a response rate of 95.33%. There were 685 Han students,accounting for 68.43%,241 Mongol students,accounting for 24.08%,and 75 students with other ethnic minorities,accounting for 7.49%. The average awareness rate of chronic diseases prevention and control was 53.25%;the awareness rates of hypertension prevention measures,smoking and drinking hazards,food susceptible to aflatoxin pollution and high cholesterol food were high,all more than 70%;while the awareness rates of long-term high lipid hazards,high blood pressure diagnosis standard and diabetes symptoms was low,all less than 40%. There was no significant difference in the awareness rate of chronic diseases prevention and control among different nationalities(P>0.05). The results of multivariate logistic regression analysis showed that the students who were female(OR=1.309,95%CI:1.008-1.700),had high achievement(OR=1.509,95%CI:1.055-2.159),and thought exams were important(OR=1.570,95%CI:1.204-2.046)were more likely to have high awareness,while the students who spent five to less than eight hours a day on screens(OR=0.514,95%CI:0.335-0.789)were less likely to have high awareness. @*Conclusions@#The awareness rate of chronic diseases prevention and control of high school students in Tuquan County is 53.25%,and there is no significant difference between Mongol and Han students. Gender,academic performance,attitude to examination and time spent on screens can affect the awareness rate.

@#AIM: To compare and study the differences of eyeball biometric measurements among Han, Hani and Yi nationality in Honghe Hani and Yi Autonomous Prefecture(Honghe Prefecture), and prediction accuracy of the intraocular lens(IOL)degree by SRK-T and Haigis formulas in the different eye axes, to provide further objective clinical evidence for the majority of basic-level hospitals and the blind prevention and treatment projects in minority areas.<p>METHODS: Selected 186 cases(200 eyes)cataract patients in our department, divided them into three groups according to different nations, get their eyeball biometric measurements(ocular axial length, anterior chamber depth and corneal curvature)by A-ultrasound combined with corneal curvature meter and corneal topography, and then compared the differences. According to the different eye axes, they were divided into three groups and then randomly divided into two groups. SRK-T and Haigis formulas were used to predict the IOL degree, and collected postoperative optometry results, calculated the absolute prediction error, then conducted statistical analysis. <p>RESULTS: There were no difference in the mean axial length, mean anterior chamber depth and mean corneal curvature(measured respectively by corneal topography and corneal keratometer)in the different ethnic groups(<i>P</i>>0.05), and the mean corneal curvature measured by the two methods had no difference(<i>P</i>>0.05). There were no difference of the mean corneal curvature measured by the two methods in the three axial eye groups(<i>P</i>>0.05)and in the same axial eye group(<i>P</i>>0.05). There were no difference in the absolute error of the two IOL measurement formulas in the three eye axis groups(<i>P</i>>0.05). The absolute error calculated by SRK-T formula for the short and the middle eye axis groups were smaller, while it calculated by Haigis formula for long eye axis group was smaller.<p>CONCLUSION: In our department, there are no statistical difference in the eye biometrics of Han, Yi and Hani nationality. Corneal curvature measured by corneal topography and corneal keratometer have no significant difference. SRK-T and Haigis formula both have high predictive value for IOL degree, SRK-T formula has smaller predictive error for patients which with short and middle eye axis, and Haigis formula has better predictive value for the long ones.

AlM:To determine the group distribution characteristics of lachrymal duct obstruction diseases in major Li Miao minority areas of Hainan province. METHODS: Totally 5 353 residents were selected and researched by randomized cluster sampling in the major Li Miao minority areas of Hainan province. Ocular examination and lachrymal duct flushing were carried out, and questionnaire survey on lachrymal duct obstruction was conducted. The ratio of lachrymal duct obstruction diseases and group distribution characteristics were analyzed based on above research. RESULTS: The prevalence ratios of lachrymal duct obstruction was 4. 47% in major Li Miao minority areas of Hainan province, with 2. 62% in urban area, and 5. 93% in rural area respectively. Prevalence ratios of men and women group were 1. 69% and 6. 39% correspondingly. Difference between the two groups was statistically significant (χ2 = 67. 2821, P = 0. 0000 ). The highest prevalence ratio was 40-69 year-old group, second one was 70-79 year-old group, especially for women in these groups. The prevalence ratios of Ledong, Lingshui, Baisha and Changjiang county were higher than those of Baoting, Qiongzhong county and Wuzhishan city. No significant difference was found between both eyes. CONCLUSlON: ln major Li Miao minority areas of Hainan province, lachrymal duct obstruction mainly occurs in 40-79 year-old patients, with specially higher ratio of women. Statistically, significant difference of the prevalence ratio between urban and rural areas exists. The higher prevalence ratio is attributed to age, gender, geographical location, climate condition, health environment and so on. The prevalence ratio is higher in the dry and windy areas than in the humid and less windy areas.

ObjectiveTo probe into the maternal care utilization by minority women, for the purpose of policy recommendations on better maternal care in minority areas. MethodsA combination of stratified random sampling and typical sampling was made on 445 married women of reproductive age in six counties in Yunnan, Guizhou, Qinghai and Tibet provinces, a field survey on their utilization of maternal care services. ResultsTheir average prenatal detection rate is 78.24%, a level lower than the national rural average of 93.7% and grade-4 rural average of 81.2% in 2008; their post partook rate is 30.7%, lower than the national rural average of 54.3% and grade-4 rural average of 58.9% in the same period; their average coverage rate is 52.18%, a level lower than the national rural average of 87.1%and grade-4 rural average of 64.3% in 2008. ConclusionThe maternal care utilization is found to be low for women in minority areas. Effective solutions are expected for payment of indirect expenditure of hospital delivery; better health education for enhancing health knowledge and health awareness of minority women; effective incentive mechanism for village doctors, consolidating the base of the three-level healthcare network.
