RESUMO

Objective:To evaluate the effect of occlusal adjustment on the restoration of the idiopathic cervical lesions.Methods:295 teeth with dental idiopathic cervical lesions in 83 patients were divided into 2 groups,142 teeth with occlusal interference were included in group 1 and the 153 without occlusal interference in group 2.The deffects of all the teeth were filled with Z350 nano-resin and followed up for 2 years.Then the teeth with treatment failure were divided into the experimental group (A,30 from group 1 and 14 from group 2) and the control group(B,30 from group 1 and 15 from group 2).The idiopathic cervical lesions of all teeth were filled with Z350 nano-resin,the occlusion of the corresponding teeth in group A was adjusted at the same time,that of group B was not treated.The cases were followed up for 1 year.Results:The success rate of group 1 and group 2 was 57.75% and 81.05% repectively (P < 0.05).A subsequently treatment and 1 year follow-up showed that the success rate of gorup A and B for the cases with treatment failure of group 1 was 83.33% and 56.67% (P < 0.05),for group 2 was 85.72% and 40% (P < 0.05) respectively.Conclusion:The occlusal adjustment is benefit to the clinical effect of the treatment outcome of idiopathic cervical lesions.

La pérdida de dientes temporales anteriores debido a las caries, patología pulpar y fracturas coronarias ha sido reconocida como uno de los factores etiológicos más severos de los problemas dentales en el campo de la odontopediatría. Uno de los mayores avances en la odontología en los últimos años implica la adhesión al esmalte de los dientes primarios, lo que ha llevado a desarrollar muchas técnicas para restaurar estéticamente al diente. Los obstáculos en este tema son la presencia de la capa aprismática en la superficie de los dientes temporales, cuyo espesor llega a ser de 25 μ, y la cantidad reducida del esmalte. Sin embargo, los dentistas pueden superar estos factores y producir restauraciones hermosas. Las coronas fabricadas con composite para técnicas indirectas, utilizadas en el laboratorio, optimizan las características químicas, físicas y por lo general se polimerizan mediante el uso de medios físicos (luz, calor, presión, etc.), por lo que presentan superficies homogéneas y libres de poros, lo que le da una buena estética, mayor resistencia a la abrasión y una adaptación adecuada. El objetivo del siguiente trabajo es presentar un caso clínico de rehabilitación con restauraciones de composites indirectas (Signum-ceramis-Heraeus), mostrando una técnica para la rehabilitación de los dientes anteriores superiores temporales, sin la colocación de provisionales. La rehabilitación de los dientes temporales con coronas de nano-resina híbrida prefabricadas en laboratorio son una buena opción en odontopediatría.


Within the field of dentistry, loss of primary teeth due to caries, pulp conditions and crown fractures has been recognized as one of the most severe etiological factors of dental problems in the pediatric dentistry field. One of the greatest headways in dentistry in the last years encompasses the adhesion to enamel of primary teeth, this has led to the development of many techniques to restore the tooth. Obstacles that might be encountered are the presence of an aprismatic layer on the surface of primary teeth. This layer might reach 25 μ as well as the reduced amount of enamel present. Nevertheless, dentists might overcome these factors and manufacture esthetically-pleasing restorations. Crowns manufactured with composite resins and used in indirect techniques in the laboratory optimize physical and chemical characteristics and in general, polymerize with the use of physical means (light, heat, pressure, etc.). Therefore, they present smooth, pore-free surfaces, which allow for suitable esthetics, greater resistance to abrasion and proper adaptation. The aim of the present work was to present a clinical case of rehabilitation with indirect composite resin restoration (Signum-ceramis-Heraeus) and to show a technique for the rehabilitation of primary, upper anterior teeth which did not entail placement of provisional teeth. Rehabilitation of primary temporary teeth with nano-resin-hybrid crowns prefabricated in the lab represents a suitable option in the field of pediatric dentistry.

BACKGROUND:Compared with the conventional composite resin, the 3M Z350 nano-resin has good wear resistance, physical mechanical properties, and polishing, and exerts a lower irritation to the dental pulp. Besides fil ing materials, a reliable tooth-prosthesis bonding interface is necessary for resin bonded repairs. OBJECTIVE:To compare the clinical effects of self-etch bonding Adper Easy One and total-etch bonding Single Bond 2 on nano-resin bonding restoration of the anterior teeth. METHODS:120 anterior teeth with vital pulp, which had defects at the incisal ends and were to be restored with nano resins, were divided into two groups randomly. Two kinds of adhesives, self-etch adhesive and total-etch adhesive, combined with nano-resin were used to restore the teeth. The patients were re-examined immediately, 6 months, 1 year and 2 years after the treatment. The fil ings, teeth and pulps of patients were examined, including whether the prosthesis and tooth color were coordinated, whether the gap between the prosthesis and the teeth were sealed, whether the surface of the prosthesis was intact with no loose, whether the prosthesis and teeth had no staining and secondary caries, whether the condition of the tooth pulp had hot or cold stimulation-induced pain. RESULTS AND CONCLUSION:No significant difference in the fil ing effects was found between the two groups when the patients were re-examined immediately, 6 months and 1 year after the treatment (P>0.05). The pulp lesions of the self-etching group were fewer than those of the total-etch group 2 years after the treatment (Pincomplete restoration and secondary caries, respectively. No statistical y significant differences were found in these four aspects between the two groups (P>0.05). The 2-year fol ow up showed a low incidence of pulp lesions and satisfactory clinical performance after 3M Z350 nano-resin working with self-etching bonding system in the nano-resin fil ing of anterior teeth with vital pulp.
