RESUMO

This study was conducted to evaluate the effect of argan oil with the exposure of low frequency electromagnetic field (EMF)on open wound healing in mice. Eighteen male mice (20-40 g) were divided into three groups: phosphate buffer saline(PBS) as negative control, solcoseryl gel as positive control, and argan oil with the exposure of low frequency EMF, 1.2mT (treatment group). Full thickness wounds (4 mm diameter) were induced on the shaved dorsal of the mouse. All micewere sacrificed on day 12 after the final treatment. Macroscopic observation, wound contraction rate, histopathologyanalysis and total protein content were examined in this study. Results showed that wounds treated with argan oil andexposed to low frequency EMF has a significant increase in wound contraction rate (p < 0.05) and total protein content(p < 0.05). Moreover, histopathological analysis on the wound tissues displayed complete re-epithelization with thick anddense collagen fibers in the argan oil with low frequency EMF exposure treated group. In conclusion, topical treatment ofargan oil with low frequency EMF exposure yield a better healing progress and showed the ability to accelerate woundhealing.

RESUMO

Objective To investigate the clinical effect of Kangfuxin liquid topical treatment for open wound of perianal abscess.MethodsEighty patients with open wound were treated with perianal abscess from June 2015 to June 2016.The patients were divided into two groups according to their treatment group.Patients were treated with conventional regimen, while 40 patients in the observation group were treated with routine regimen+rehabilitation solution;the experimental data were compared between the two groups.The two groups were observed and compared the wound healing time (d), granulation growth cycle (d) and healing rate (%);the two groups were observed and compared the incision pain, edema, anal bulge and other symptoms disappeared time;compared two groups of clinical curative effect and complication rate.ResultsThe clinical treatment effect of the observation group was better than that of the control group, the difference was statistically significant (P<0.05).ConclusionThe clinical effect of the treatment of open wound in patients with perianal abscess with conventional therapy+rehabilitation therapy is significant, and the wound healing time is short, which is widely used in the treatment of open wound of perianal abscess.

Objective To explore the effect of kangfuxin liquid topical treatment on open wound of postoperative perianal abscess.Methods200 cases of patients with open wound abscess surgery in our hospital from February 2014 to September 2016 were randomly divided into control group and experimental group,with 100 cases in each group.The control group was given conventional treatment, the experimental group were treated by kangfuxin liquid.The relevant therapeutic indicators and clinical healing time between two groups were compared.ResultsThe effective rate of the treatment group was 93.0%, which was significantly higher than that of the control group (85.0%), the difference was statistically significant(P<0.05).The granulation growth period of the experimental group was (5.14±1.41) days, and the average healing time was (15.21±2.49) days.Which were significantly shorter than those of the control group[(7.45±1.32) days, (21.16±2.69) days], and the differences were Statistical differences (P<0.05).ConclusionTo improve the treatment effect to a great extent open wound infection of kangfuxin liquid in treatment of perianal abscess after surgery, shorten the healing time, improve the patient's symptoms, with further clinical promotion and application significance.

RESUMO

Soft tissue defect combined with an open fracture is a very challenging problem to the orthopaedic surgeon. Many complicated open fractures remain with soft tissue defect, chronic osteomyelitis, and sometimes terminate with major limb amputation. Soft tissue defect should be reconstructed as soon as possible, particularly when the bone, tendon, or neurovascular structures are exposed. Exposure for longer than a week significantly increases the risk of secondary infection and tissue necrosis. For the simple soft tissue defect, negative pressure wound closure technology has been introduced and many superficial wounds have been treated successfully using this method. For the more complicated wounds, many kinds of local flaps, pedicled flaps, muscle and fascisocutaneous flaps can be indicated according to the characteristics of the wounds. The free flaps including free vascularized bone graft can be considered as a final choice for the most difficult wound problems. In this article, various reconstruction strategies for soft tissue defect after traumatic open fracture are reviewed.

Wound management is a major concern in open fracture cases. Negative Pressure Wound Therapy (NPWT) is an advanced method for managing open wounds. It is a topical treatment using sub-atmospheric pressure to increase blood flow, remove bacteria and increase growth of granulation tissue in the wound. The study was performed to evaluate the results of NPWT in patients with open fracture in lower extremity. Using Aquarium pump as an NPWT device, 16 patients were prospectly treated for open fractures in their inferior extremity. Mean patients' age range was 21 to 60 yrs. The patients under study either had suffered from trauma, fall or had post operative wound infection. Many of them had wounds with underlying tendon or bone exposure. Necrotic tissues were debrided before applying NPWT. Dressings were changed every 3rd or 4th day and treatments were continued for 07 to 28 days. Exposed tendons and bones were successfully covered with healthy granulation tissue in all cases, depth of the wounds reduced as well as surface areas. In 12 cases coverage of granulation tissue were achieved and further managed by skin grafting, 4 cases with wound infections were closed with secondary suture. No significant complications were noted regarding the treatment. NPWT was found to facilitate the rapid formation of healthy granulation tissue on open wounds in lower extremity and thus to shorten healing time and minimize secondary soft tissue defect coverage procedures.

PURPOSE: The purpose of this study is to evaluate the functional and aesthetic outcomes of the reconstruction of complicated open wounds with exposure of the bone, periosteum, or tendon using AlloDerm(R) and skin graft in hand and foot. MATERIALS AND METHODS: Seven cases of hand and 4 cases of foot were treated with combined AlloDerm(R) graft and split thickness skin graft in a single stage for managing the soft tissue defect of extremities where bone, periosteum, or tendon were exposed but flap operation was not feasible. The mean postoperative follow-up period was 30 months (range: 14-48 month). Postoperatively, we assessed graft taking rate, wound healing process, and the recovery of motion of hand and foot. RESULTS: AlloDerm(R) and skin graft were readily assimilated into the wound in all cases. The mean postoperative range of motions of the 5 fingers with exposed digital tendons were 89.2% of the normal side. Reconstructions of wound in the foot were successfully settled without unstable scar or ulcer in all cases. CONCLUSION: Composite grafting with AlloDerm(R) and autologous skin graft for reconstruction of complex open wound in hand and foot where deep structures are exposed provides stable reconstruction of the wound with high taking rate, successful recovery of tendon gliding motion by preventing adhesion.

OBJECTIVE:To study the bacteria strains and susceptible drugs in open wound in patients with earthquake injury. METHODS:Bacteriological test and the drug susceptibility test were conducted for earthquake injury patients with open wound and the results of bacterial culture and the drug susceptibility test were analyzed using Excel 2003. RESULTS:Of the 1 172 earthquake injury cases,the patients with trauma / wounds represented 93.9%,the wound had obvious infection in 132 cases; of the infected cases,the proportions of patients with open fractures,soft tissue injuries and traumatic brain injury accounted for 63%,28% and 9%,respectively. The positive rate of the bacterial culture was 69%,and gram-negative of bacteria which accounted for 85.6% took the lead,chiefly Acinetobacter baumanii,Enterobacter cloacae and Escherichia coli. The results showed bacterial drug resistance was on the high side. CONCLUSION:The open wounds in patients with earthquake injury are likely to be infected with gram negative organisms. Debridement and correct choice of anti-infective drugs are needed to reduce the risk of wound infection,

Objective To explore the clinical features and urgent treatment strategies of open stabbing and penetrating wound of the heart. Methods In recent five years, 22 cases of cardiac trauma patients were sent to our emergency department, among them 19 sustained penetrating wounds produced by sharp instruments, 3 with perforating wounds, produced by firearm in 2 and sharp instrument in one. Emergent thoracotomy was performed in all the patients. The injury involved right auricle in 4, left auricle in 1, right ventricle in 11, left ventricle in 3, and perforating wounds of both right and left ventricles in 3 patients. Results Among them 19 survived, and their cardiac and pulmonary recovered 2 weeks after the operation. Three patients with injury of the great vessel died, one of them died 8 hour after the operation. Conclusion When the patients who presented open wounds on the anterior or posterior chests, showing a low or undetectable blood pressure, which could not raised even with fluid resuscitation with two venous lines or in some of them displaying signs of Beck's triad, open cardiac injury should be seriously suspected, and emergent thoracotomy should be taken in order to rescue the patients.

RESUMO

The extensive traumatic wound has posed a serious problem to the surgeon. Various drugs and dressing methods have been used for this kind of wound but there has been no ideal method. The pigskin xenograft was used initially for the treatment of burn wound. The salutary effect of porcine xenograft are the stimulation of growth of healthy granulation tissue, as well as epithelial borders, inhibition of bacterial growth and pain relief. We have performed porcine xenograft in 20 cases having extensive traumatic wound with skin defect from January 1981 to February 1982. The results obtained are as follows: 1. Protect large open wound until autograft are available. 2. Alleviate pain in the wound. 3. Promote granulation tissue growth which is needed for early autograft. 4. Protect the exposed wound areas (bone and tendon) from infection. 5. The healing time of the wound depended upon the severity and the state of underlying tissue in the wound rather than wound size.
