RESUMO

Ovarian pregnancy is a rare form of extrauterine ectopic pregnancy. Risk factors such as reproductive treatments and infertility have been identified in recent studies. In this article, we present a case of ovarian ectopic pregnancy occurring following in vitro fertilization treatment and a fresh embryo transfer. The diagnosis of ovarian pregnancy was made during transvaginal sonography performed due to suspected ectopic pregnancy. Ovarian ectopic pregnancy is a rare clinical phenomenon. Late diagnosis and lack of appropriate intervention may have catastrophic results. Several mechanisms and risk factors are proposed, and their acknowledgment may improve early diagnosis and prevention of complications.

@#Primary ovarian pregnancy accounts for less than 1% of all cases of ectopic pregnancies. Its diagnosis is most commonly made intra-operatively and the approach in its management has been geared towards conservative measures such as oophorectomy and resection. Use of intrauterine device still remains the most established risk factor for the development of ovarian pregnancy. The diagnosis is established following the criteria first described by Spiegelberg in 1878. Several cases of ovarian gestation have been described in literature, although very few cases with live term fetus have been reported. Majority of the cases were diagnosed intra-operatively, and the management was tailored depending on the complexity of each of the different cases. This paper reports a case of primary ovarian pregnancy with a live term fetus, which was only diagnosed intra-operatively. Total hysterectomy with right salpingo-oophorectomy was performed due to difficulties encountered brought about by dense adhesions.

RESUMO

Primary ovarian ectopic is a rare variant of ectopic pregnancies. It is commonly confused with tubal pregnancy aborted over ovary, hemorrhagic ovarian cyst, ruptured corpus luteal cyst. The women with ovarian ectopic generally presents early because of early onset hemorrhage in ovary. This is a rare case where woman with ectopic pregnancy presents at 13 weeks. The clinical picture is also highly unusual with just spotting and fainting attacks at the end of first trimester to make a diagnosis of ectopic pregnancy. The laparotomy done showed an unruptured ovarian ectopic pregnancy and with 350 cc hemoperitoneum. Salpingoopherectomy was done and the ectopic mass was removed as hardly any ovarian tissue was left to conserve the ovary. Hence, clinicians should be cautious enough to keep a differential diagnosis of ectopic even at advanced gestation.

El embarazo ectópico es en la actualidad un problema de salud a nivel mundial atribuido al aumento de su incidencia y la morbimortalidad que crea. La localización ovárica del embarazo ectópico es un evento raro pero posible y también en ascenso. Presentamos el caso de una paciente que acude al cuerpo de guardia con escasa sintomatología. Se le realizó el diagnóstico ultrasonográfico de la entidad y apoyado en una prueba de embarazo positiva. Luego le practicaron laparotomía de urgencia en la que se comprobó el embarazo ovárico derecho no complicado. También se efectuó una salpingooforectomía y la paciente tiene una evolución satisfactoria trans-posoperatoria(AU)


Ectopic pregnancy is currently a global health problem due to the increase in its incidence and morbidity and mortality. The ovarian location of ectopic pregnancy is a rare but possible event and on the rise. We present the case of a patient who comes in with little symptoms. The ultrasound diagnosis of the entity was made and supported by a positive pregnancy test. Then emergency laparotomy was performed, which proved that the right ovarian pregnancy was not complicated. Salpingo-oophorectomy was also performed and the patient has a satisfactory trans and postoperative evolution(AU)

RESUMO

The first case of primary ovarian ectopic pregnancy was reported by St. Maurice in 1689. Primary ovarian ectopic pregnancy is rare entity, with incidence of around 3% of all ectopic pregnancies. The aim of present case report is to study the role of different modalities like clinical findings, biochemistry, sonography, surgery and histopathology in diagnosis of primary ovarian ectopic pregnancy. Authors present a case of a 24 years old female with history of 2 months amenorrhoea, per vaginal spotting and mild intermittent abdominal pain in RIF. Examination was unremarkable and serum βHCG was 2007.5IU/ml. An ultrasound scan showed an ectopic mass in the right adnexa suggestive of a tubal ectopic pregnancy and she underwent surgical management at our institute.  At laparoscopy, both fallopian tubes were noted to be normal with an ectopic mass attached to the right ovary with a pedicle. With these unusual laparoscopic findings, possibilities of tubal miscarriage, ovarian pregnancy, or abdominal pregnancy were suspected. Ectopic mass was coagulated and cut through the pedicle with bipolar cautery and specimen was sent for HPE. Our patient made an uneventful recovery and no further medical management was indicated. The diagnosis of right ovarian ectopic pregnancy was finally confirmed by histopathological evidence of the presence of chorionic villi in a background of ovarian stromal tissue, in consideration with Spigelberg’s criteria. Present case highlights the importance of considering non-tubal ectopic pregnancies when making a diagnosis based on ultrasound scan. Pre-operative diagnosis of ovarian ectopic pregnancy still remains a challenge in spite of current medical advances. Despite the benefits and reliability of ultrasound scanning, there will still be situations where the definitive diagnosis can only be made at surgery. However, histopathological examination is confirmatory and always mandatory.

@#<p>Primary ovarian ectopic pregnancy, is one of the rarest forms of ectopic pregnancy. Ovarian ectopic pregnancy may occur with or without the essence of any of the classical risk factors or signs and symptoms. The diagnosis is made often at surgery and required histologic confirmation. This is a case of a primary ovarian pregnancy in a 36-year old elderly primigravida who presented with abdominal pain and subsequently underwent ovarian wedge resection. Diagnosis of ovarian pregnancy was later confirmed using the Spiegelberg criteria. This paper presents a review of literature regarding ovarian pregnancy's clinical presentation, risk factors, and the possibility of a conservative management.</p>

El embarazo ectópico es en la actualidad un problema de salud a nivel mundial, atribuido el aumento de su incidencia, a las inflamaciones pélvicas provocadas por enfermedades de transmisión sexual, el aborto, uso de dispositivos intrauterinos, la aplicación de técnicas de reproducción asistida, uso de inductores de la ovulación, entre otros. Se presentó un caso de embarazo ectópico ovárico en una mujer de la raza negra de 38 años de edad, que acudió con cuadro clínico de vómitos, escalofríos y dolor abdominal al Cuerpo de Guardia del Margret Marquart Catholic Hospital en Kpando, región Volta, Ghana. Al examen físico se palpó una tumoración que ocupaba gran parte del hipogastrio, fondo de saco de Douglas abombado y doloroso. Con antecedentes patológicos personales de anemia crónica y portadora de virus de inmunodeficiencia humana (VIH). Se ingresó, se realizó laparotomía y se egresó luego de evolución satisfactoria.


Ectopic pregnancy is nowadays a worldwide health problem due to the incidence of pelvic inflammatory diseases caused by abortion, sexually transmitted diseases, the use of intrauterine devices and assisted reproductive techniques among others. A 38- year- old black woman came to the emergency room at the Margret Marquart Catholic Hospital of Kpando, Volta Region in Ghana with a history of vomiting, chills and abdominal pain. On physical examination, a tumor mass was found, that occupied a large part of the epigastrium, Douglas sack fundus, which was convex and painful. She has a personal pathological history of chronic anaemia and she was a carrier of HIV. She was admitted in the hospital and a laparotomy was performed. She was discharged after her satisfactory evolution.

Ovarian pregnancy is a rare ectopic pregnancy.In recent years,the development and popularization of assisted reproductive technology increase the incidence of ovarian pregnancy.This article focused on the ultrasound features of ovarian pregnancy following in vitro fertilization and embryo transfer (IVF-ET) by comparing those of pregnant corpus luteum and tubal pregnancy,and analyzed the advantages of transabdominal ultrasonography (TAS) and transvaginal ultrasonography (TVS) in the diagnosis.

OBJECTIVE: To analyze the preoperative diagnostic clues to ovarian pregnancy (OP). METHODS: This study conducted a retrospective chart review of 23 patients with OP and 46 patients with tubal pregnancy (TP) from October 1, 2003 to September 31, 2016 in Hanyang University Hospital. RESULTS: There were no significant differences in demographic and clinical characteristics between the two groups. The presence of an ectopic gestational sac and hemoperitoneum was significantly higher in the TP group (13.0% vs. 95.7%, P=0.000; 13.0% vs. 54.3%, P=0.001, respectively) in preoperative ultrasonogram. The OP group had more ruptured ectopic gestational sacs than the TP group (73.9% vs. 45.7%, P=0.039) in surgical findings. CONCLUSION: For the patients in whom a gestational sac is not detected in the uterus or the fallopian tubes, it is important to be aware of the possibility of OP and rupture of an ovarian gestational sac to promote early diagnosis and surgical intervention.

Primary ovarian pregnancy is a rare form of ectopic pregnancy due to the implantation of the gestational sac in the ovary. It is reported to occur in 1 in 25000-40000 pregnancies following natural conceptions and accounts for 0.3- 3% of all ectopic pregnancies. The preoperative diagnosis is difficult and is mostly diagnosed following surgery or histo pathological examination. Here we present a case of ovarian pregnancy which was managed surgically and its review of the literature is discussed.

Ectopic pregnancy is defined as an extrauterine pregnancy. We report three cases where the ectopic pregnancies were implanted in different sites. The first case was a 28-year-old in her second pregnancy at 9 weeks gestation. She presented with painless vaginal bleeding. Ultrasound showed unruptured cornual pregnancy with hCG level of 7456mIU/ml. A single dose of 75mg IM methorexate was given and she responded well with significant reduction of hCG level. The second case, a 26-year-old gravida 5 para 2+2, with 2 previous ectopic pregnancies and bilateral salpingectomy, conceived via in-vitro fertilization (IVF). She presented with acute abdomen and one episode of syncope at 8 weeks 4 days gestation. Laparotomy showed ruptured ectopic pregnancy at the left tubal stump requiring a left salpingectomy. The third case was a 26-year-old, gravida 5 para 2+2, with two previous vaginal deliveries and two previous first trimester miscarriages. Her menses was irregular since she took injectable progestin. She presented to the emergency department with sudden onset of lower abdomen pain. Urine pregnancy test was positive. Ultrasound showed empty uterus, no adnexal mass but there was significant free fluid in the cul-de-sac. During laparoscopy, a ruptured ovarian pregnancy was diagnosed and salpingo-oophorectomy performed. There was no significant risk factor contributing to ectopic pregnancy identified in the first and third case. In the second case, despite previous bilateral salpingectomy, the patient still had ectopic pregnancy in the left fallopian tube remnant.

Reportar un caso de embarazo ectópico ovárico y realizar una revisión de la literatura acerca del diagnóstico y su tratamiento.Materiales y métodos: se reporta el caso de un embarazo ectópico ovárico en una paciente usuaria de dispositivo intrauterino (DIU) de cobre. Fue tratado por laparoscopia, en una institución general privada de tercer nivel de complejidad. Se realiza búsqueda de la información objeto de la revisión en las bases de datos PubMed, Ovid, ScienceDirect y Lilacs, mediante terminología MeSH: "pregnancy, ectopic"; "pregnancy, ovarian"; "pregnancy complications". Se buscaron reportes y series de caso y artículos de revisión.Resultados: se encontraron 23 referencias bibliográficas de las cuales 1 es un capítulo de libro, 9 son revisiones de tema y 13 son reportes o series de casos. Para el diagnóstico se requiere un alto grado de sospecha clínica, medición de la subunidad beta de la hormona gonadotropina coriónica humana(β-HCG) y ecografía obstétrica transvaginal realizada por un operador experimentado. El tratamiento generalmente es quirúrgico, con enfoque por vía laparoscópica o laparotomía exploratoria de acuerdo con la experiencia y el entrenamiento del cirujano tratante. No hay evidencia que respalde el uso de metrotexate en esta condición.Conclusión: el embarazo ectópico ovárico es un cuadro clínico de presentación muy poco frecuente, el ultrasonido tiene utilidad limitada y el tratamiento quirúrgico con resección en cuña o la ooforectomía es el más utilizado...


To report one case of ectopic ovarian pregnancy, and to review the literature on the diagnosis and treatment of this condition.Materials and methods: An ectopic ovarian pregnancy in a patient using a copper intrauterine device (IUD) is reported. Laparoscopic treatment was provided in a private, level III general care institution. A search was conducted on the subject of the review in the PubMed, Ovid, ScienceDirect and Lilacs databases using the MeSH terms "pregnancy, ectopic"; "pregnancy, ovarian"; "pregnancy complications". The search included reports, case series and review articles.Results: Overall, 23 references were found in the literature, including one book chapter, 9 topic reviews, and 13 reports or case series.Diagnosis requires a high degree of clinical suspicion, measurement of the beta subunit of the human chorionic gonadotropin hormone (β-HCG), and transvaginal obstetric ultrasound performed by an experienced operator. Treatment is usually surgical through a laparoscopic approach or exploratory laparotomy, depending on the experience and training of the treating surgeon. There is no evidence supporting the use of methotrexate for this condition.Conclusion: Ectopic ovarian pregnancies are infrequent clinical occurrences, ultrasound is of limited usefulness, and surgical treatment with wedge resection or oophorectomy is the most frequent form of management...

Intrafollicular ovarian pregnancy is a rare event of ectopic pregnancy with a reported incidence of 1/7000-1/40000 pregnancies. Only 15% of cases of ovarian pregnancy are intrafollicular in origin. In spite of advances in clinical sciences diagnosis of ovarian pregnancy is diffi cult pre-operatively as it mimics hemorrhagic cyst, luteal cyst, and adnexal mass. Th e diagnostic criteria for ovarian pregnancy were described by Spiegelberg in 1878. Diagnosis of ovarian pregnancy should be suspected when the hemorrhagic mass is identifi ed near the ovary with normal fallopian tube during surgery of ectopic ovarian pregnancy. Th e classical management of ruptured ovarian pregnancy is surgical like any other ruptured ectopic pregnancy. Th e extent of surgery varies according to the amount of tissue destruction. Recent advances in the management of ovarian pregnancy are laparoscopic laser ablation and methotrexate for unruptured ovarian pregnancy.

Ovarian pregnancy is a rare type of ectopic pregnancy its incidence is 1-3% of all ectopic pregnancies. Clinical presentation of ovarian pregnancy is similar to that of tubal ectopic pregnancy. Most of the cases diagnosis is made at laparotomy and is confirmed by histological examination. Here we are reporting a case of un-ruptured ovarian pregnancy at 12.1 weeks of gestational age.

Primary ovarian pregnancy is a rare type of extra-uterine pregnancy, and occurs in 1:7000 to 1: 40,000 deliveries. This is a case that occurred in a 26 years old multiparous woman. She had presented with pain abdomen and bleeding per vaginum and was hemodynamically stable, negative pregnancy test, but vaginal examination showed painful cervical movements, slight bleeding through external os, uterus just bulky, tender mass felt in right adnexa. Ultrasonography revealed—Right tubo-ovarian mass with fluid in peritoneal cavity. Laparotomy showed Right Ovarian Pregnancy. Managed by Right Ovariectomy.

Asymptomatic unilateral absence of a portion of a fallopian tube with or without adjacent ovarian agenesis is a very rare condition. The true incidence is unknown. Two etiologic causes are possible. Asymptomatic segmental torsion of the uterine tube and ovarian pedicle may occur for certain reasons during adulthood, in childhood, or even during the fetal stages. Consequently, torsion may give rise to necrosis and autoamputation. Alternatively, the absence of these organs may be congenital. Ovarian pregnancy is a rare form of ectopic pregnancy occurring in 0.5~1% of all ectopic pregnancies. Moreover, ovarian pregnancy in patient with congenital unilateral tubal and ovarian agenesis has not been reported. But, the presenting symptom is very similar and difficult to distinguish with other forms of ectopic pregnancy. Early detection is important role in preserving fertility to avoid the ablation of functional ovarian tissue. We report a case of congenital unilateral tubal and ovarian agenesis diagnosed during laparoscopic treatment of ovarian pregnancy.

OBJECTIVE: The purpose of this study was to evaluate the clinical characteristics and the management of the unusual ectopic pregnancies. METHODS: Sixty one patients who were diagnosed as unusual ectopic pregnancies in the Department of Obstetrics and Gynecology, Inje University Ilsan Paik Hospital from December 1999 to June 2006, were retrospectively analyzed. The chief complaints, past histories, time of diagnosis, serum beta-hCG concentrations and method of the management were reviewed. RESULTS: During the designated study period, the incidence rate of the ectopic pregnancy was 9.5% of the all deliveries and that of the unusual ectopic pregnancy was 18.8% of the total ectopic pregnancies. In twenty two cases, implantation sites were not found out exactly by sonography at initial diagnosis. Theses ectopic pregnancies were cornual pregnancy (3 cases), ovarian pregnancy (6 cases), abdominal pregnancy (3 cases), and unclassified unusual ectopic pregnancy (10 cases). Unusual ectopic pregnancies which were diagnosed before six weeks of gestation were eleven cases. These unusual ectopic pregnancies were 6 cases in cervical pregnancy, 1 case in abdominal pregnancy, and 4 cases in unclassified unusual ectopic pregnancy. Initial mean serum beta-hCG concentration was the highest in cornual pregnancies and the lowest in abdominal pregnancies. MTX (methotrexate) was administered in seven cases, operation was done in forty six cases, and expectant management was done in nine cases. All initial treatment was successful except one case with cornual pregnancy, which was MTX administered with alternative regimen. CONCLUSIONS: In our study the diagnostic accuracy of the unusual ectopic pregnancy was 63.9% and the successful treatment was done in 98.3% of them. There were significant statistical differences between the serum beta-hCG concentrations and the kinds of unusual ectopic pregnancies (p<0.001).

OBJECTIVE: To evaluate the early diagnostic key of ovarian pregnancy. METHODS: A Retrospective case study of 31 cases of ovarian pregnancies treated between 1990 and 2007 at Kangnam St. Mary's hospital Catholic University of Korea was performed. The maternal age, obstetric history, clinical symptoms, sonographic findings, preceding factors, diagnostic modalities, and surgical treatment methods were investigated. RESULT: 31 ovarian pregnancies, diagnosed between 1990 and 2007, comprised (31/1176) 2.6% of all ectopic pregnancies. The most common sonographic finding was fluid collection surrounding the ovary and an ipsilateral enlarged ovary (83.9%). Presenting symptoms were similar to those of tubal pregnancies. Before 1997, wedge resection was done mostly by laparotomy (15/16 cases, 93.8 %) and from then on by laparoscopy (8/14 cases, 57.1%). CONCLUSION: Ovarian pregnancy should be suspected in young woman with symptom of lower abdominal pain, history of pelvic adhesion and sonographic finding of fluid collection around ovary or enlarged ovary for an early diagnosis. Early diagnosis and management can preserve fertility and ovarian function.

We report a case of ovarian pregnancy mimicking cornual pregnancy managed with laparoscopy. Early detection is important role in preserving fertility to avoid the ablation of functional ovarian tissue. The diagnostic criteria, etiology, and management are discussed.

Primary ovarian pregnancy is rare form of ectopic pregnancy. It can be diagnosed if the only implantation occurs in the ovary. The clinical diagnosis of ovarian pregnancy is difficult. Owing to the availability of sensitive assays for human chorionic gonadotropin and the development of transvaginal ultrasonography, early diagnosis of an ectopic pregnancy has been made more feasible. But clinical and even intraoperative diagnosis of ovarian pregnancy is still difficult and only microscopic examination of tissue specimen can confirm the diagnosis. Misdiagnosis is common because it can be confused with a ruptured corpus luteum. Laparoscopic wedge resection is treatment of choice. The authors present a case of ovarian pregnancy with a brief review of the literatures.
