RESUMO

Background: Ulcer healing in the diabetic patients is challenging due to a prolonged inflammatory response, extracellular matrix degradation irregularities, and increased bacteria presence. Recombinant human epidermal growth factor (REGEN-D 150), which was cloned and over expressed in E. coli, has shown enhanced healing of chronic diabetic foot ulcers (DFU) by significantly reducing the duration of healing in addition to providing excellent quality of wound healing and reepithelization. Aim: To compare the efficacy and safety of collagen granule dressings and conventional dressing in deep wounds in terms of reduced healing time, number of dressing, healing quality and complications. Materials and methods: Thirty patients with foot ulcer were evaluated. A recombinant human Epidermal growth factor gel or conventional dressings were applied, and the patients were followed as per standard post-application treatment protocol. Patients underwent dressing changes every day until wound healing or for maximum period of 12 weeks. Changes in wound size were recorded when the dressing was removed; and at 4 and 12 weeks. Results: Common age groups were 41-50 and 51-60 years of age. Complete healing was present in 12 cases. Conclusion: There was statistically significant difference between the results of Recombinant human epidermal growth factor gel and saline dressings as collagen dressings had better healing response rate as compared to placebo when given along with standard treatment of diabetic foot ulcer.

RESUMO

Objective Few studies are reported on the prevention and management of oral mucositis(OM) induced by post-operative radiotherapy in patients with oral cavity cancer. This study aimed to investigate the therateutic effect of the recombinant human epidermal growth factor(rhEGF) gel on radiation-induced OM. Methods Sixty-eight cases of radiation-induced OM after surgery for oral cavity cancer were randomly divided into an experimental and a control group of equal number, the former treated with rhEGF gel while the latter with borax mouthwash. Comparisons were made between the two groups of patients in the severity of oral cavity mucous membrane injury, the intensity of radiation-induced oral pain, the in-cidence of oral cavity infection, and the duration of radiotherapy. Results At 3 weeks of radiotherapy, the patients of the experimental group showed mainly mild and moderate OM while the controls chiefly moderate and severe OM,and at 6 weeks, the former exhibited mainly moderate while the latter chiefly severe OM, with statistically significant differences in the incidence rate between the two groups (P<0.01). The experimental group, compared with the controls, had a significantly higher incidence rate of grade-Ⅰ radiation-induced pain (47.1% vs 20.6%,P<0.01),but lower rates of grade-Ⅱ(17.6% vs 32.4%,P<0.01) and grade-Ⅲpain(2.9% vs 20.6%,P<0.01).The rates of oral infection and antibiotics medication were remarkably lower in the experimental group than in the controls(23.5% vs 38.2% and 11.8% vs 26.5%,P<0.05),and the duration of radiotherapy was markedly shorter in the former than in the latter ([42.37±3.14] d vs [48.47±4.39] d, P<0.05). Conclusion The rhEGF gel can significantly reduce the severity of radiation-induced oral mucositis, improve its symptoms, and shorten the time of postoperative radiotherapy for patients with oral cavity cancer.

RESUMO

PURPOSE: These days, mesenchymal stem cells (MSCs) have received worldwide attention because of their potentiality in tissue engineering for implant dentistry. The purpose of this study was to evaluate various growth inducing factors in media for improvement of acquisition of bone marrow mesenchymal stem cells (BMMSCs) and colony forming unit-fibroblast (CFU-F). MATERIALS AND METHODS: The mouse BMMSCs were freshly obtained from female C3H mouse femur and tibia. The cells seeded at the density of 106/dish in media supplemented with different density of fetal bovine serum (FBS), 1alpha, 25-dihydroxyvitamin (VD3) and recombinant human epidermal growth factor (rhEGF). After 14 days, CFU-F assay was conducted to analyze the cell attachment and proliferation, and moreover for VD3, the 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium (MTS) assay was additionally conducted. RESULTS: The cell proliferation was increased with the increase of FBS concentration (P.05). CONCLUSION: FBS played the main role in cell attachment and growth, and the growth factor like rhEGF played the additional effect. However, VD3 did not have much efficacy compare with the other two factors. Improvement of the conditions could be adopted to acquire more functional MSCs to apply into bony defect around implants easily.

RESUMO

Objective To study the role of recombinant human epidermal growth factor (rhEGF) in the prognosis of multiple organ dysfunction syndrome (MODS) in mice. Methods One hundred and twenty clean male Kunming mice were randomly ( random number) divided into normal saline control group (n =15),MODS model control group (n =15) and MODS + rhEGF treatment group (n =90).The MODS models were made by using Caballero ME method with thioacetamide (TAA) 2000 mg/kg injected intraperitoneally to establish monophasic rapid onset pattern of MODS model in mice.MODS + rhEGF treatment group was further randomly divided into two subgroups, namely intraperitoneal injection group (n =45 ) and subcutaneous injection group (n =45 ).Each subgroup was divided again into three small subgroups (n =15) as per different doses of rhEGF used, namely 10 μg/kg,30 μg/kg and 50 μg/kg.Within 24 hours after modeling, the respiration, body weight, food eaten and general physical changes were observed.Mortality was calculated 24 hours after modeling. After the animals sacrificed, the tissues of viscus including liver, kidney, heart, brain, lung, spleen, pancreas, intestine and stomach were collected immediately. The histological changes of visceral tissues were studied by using hematoxylin -eosin staining under the light microscope. All the experimental data were presented in,and body weight changes were compared using t-test,and after different routes of administration with different doses of rhEGF used in MODS,the mice body weight changes were analysed by using the Dunnett method, and the mortalities of mice were compared by using Fisher exact test, and P < 0.05 was considered statistically significant difference. Results There was no significant difference in mortality betweeu mice in rhEGF subcutaneous administration group and MODS model control group (P > 0.05 ),but the total mortality of hrEGF MODS intraperitoneal administration group (6.7% in dose of 50 μg/kg and 20% in dose of 30 μg/kg) was significantly lower than that of MODS model control group (73.3%) ( P < 0.05 ) and the mortality of mice treated with intraperitoneal 50μg/kg rhEGF (6.7% ) was lower than that treated with 10μg/kg rhEGF (P=0.014).The mortality of mice in rhEGF MODS (50 μg/kg ) intraperitoneal administration group was significantly lower than that in subcutaneous administration group (40%) (P =0.031 ), The histopathological changes in rhEGF MODS treatment group were not as remarkable as seen in mice of control group. The histopathological changes were dose - dependent. The higher doses of rhEGF,the lesser hepatic congestion, liver cell apoptosis, hepatic cell cloudy swelling and cell vacuolization.Similarly,as RhEGF dosage increased, pulmonary interstitial congestion, inflammatory cells and apoptotic bodies reduced, and bronchial ciliated columnar epithelium less shed. Conclusions RhEGF plays a positive role in repairement of tissue damage in TAA - induced MODS murine model. The rhEGF given by intraperitoneal route of administration is more effective to reduce the 24 h mortality of MODS mice than that by subcutaneous route.

RESUMO

PURPOSE: Recent studies have already demonstrated experimentally that the topical application of epidermal growth factor (EGF) accelerates the healing of partial-thickness wounds. Although previous studies have proved the effect of recombinant human EGF (rhEGF), the clinical use of rhEGF in case of patients have generated some controversy in advantage. In this study, twelve patients had partial thickness wounds because of burn and skin graft donor site were treated with rhEGF and we investigated the clinical effect of rhEGF on partial thickness wounds of patients. METHODS: We evaluated two groups of patients had partial-thickness wounds. One group (n=5) is consisted of the patients have second degree burn wounds, and the other group (n=7) is consisted of the patients have donor site wounds after skin graft. Each wound was divided two area, one area was applied twice daily with rhEGF and the other area was treated without rhEGF. The extent of reepithelized area was recorded by percentage ratio. And the statistical analysis was conducted about the difference of the number of days between two area until total wound was reepithelized. RESULTS: In the group treated with rhEGF, the mean of time to healing was about 9.75 days, whereas, in the group without rhEGF, it was about 11.08 days. In the number of days until total wound was reepithelized, there were statistically significant difference between two groups (P<0.05). CONCLUSION: It was concluded that the topical application of rhEGF accelerated the healing of partial-thickness wound clinically.

For experimental group, we cleaned the wound with normal saline, applied 0.5 ml of EasyEF liquid(R) (rhEGF 0.5 mg/10 ml) and covered the wound with the humidified gauze 2 times a day. For control group, we cleaned with normal saline and covered with the medifoam for 2 times a day. We estimated the change of the size and the stage of the ulcer weekly for 4 weeks. The longest region of the wound was measured by the width, and the longest line perpendicular to the width was measured by the length. RESULTS: The width and length of the stage 3 ulcer of experimental group significantly decreased, while control group showed a slightly decreased. The experimental group showed significant improvement compared to the control group (p<0.05) in stage 3 ulcer. In stage 4 ulcer treatment, there were no significant differences between the two groups. In experimental group, 5 regions of the six stage 3 ulcer improved into the stage 2 ulcer during the study, even though only 1 region improved into the stage 2 ulcer in control group. CONCLUSION: We confirmed that rhEGF was effective in the stage 3 ulcer. rhEGF treatment may be useful for healing of the stage 3 ulcers.
