RESUMO

We reported a case of a patient dignosed as tuberculosis and severe malnutrition with coronavirus disease 2019 (critical type) treated with a combination of Chinese medicine and Western medicine. Through the retrospective analysis of the diagnosis and treatment process of this patient, on the basis of Chinese medicine's understanding of the etiology and pathogenesis of “old state” and “deficient state”, the critical coronavirus pneumonia combined with pulmonary tuberculosis and severe malnutrition was mostly due to the physical condition and the invasion of epidemic toxin, resulting in dysfunctions of the internal organs such as the lungs, spleens, kidneys and other organs. Based on the understanding of the cause and mechanism of the coronavirus disease, the treatment combined Chinese and Western medical therapies was given. The western medicine was used with the main treatments of oxygen therapy, anti-viral, intestinal nutritional support, and anti-coagulation, while the Chinese medicine was used by tonifying qi, blood, yin, and yang, warming yang and dissipating cold, and clearing heat and dampness, then tonifying qi, nourishing yin and eliminating heat, in which tonifying middle and replenishing qi ran through the whole process. The integrated treatment eventually improved the patient's symptoms and accelerated the negative conversion of nucleic acid of the coronavirus.

ObjectiveTo observe the clinical efficacy of Danzhi Jiangtang capsules with the functions of replenishing Qi, nourishing Yin, and dredging collaterals on patients with type 2 diabetes mellitus (T2DM) combined with lower-extremity macroangiopathy and serum levels of homocysteine (Hcy) and cystatin C (Cys C). MethodA total of 80 eligible patients who were treated in the department of endocrinology of the First Affiliated Hospital of Anhui University of Chinese Medicine from May 2017 to December 2019 were randomized into the treatment group (40 cases) and control group (40 cases). Both groups received the basic therapies for diabetes and Danzhi Jiangtang capsules (oral) was added to the treatment group. The levels of glycosylated hemoglobin A1c (HbA1c), fasting plasma glucose (FPG), 2-hour postprandial glucose (2 hPG), fasting C-peptide (C-P), and 2-hour postprandial C-peptide (2 hC-P), triglycerides (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C) and serum levels of Hcy and Cys C were measured and the traditional Chinese medicine(TCM) syndrome scores were calculated before and after treatment in the two groups. ResultAfter 12 weeks of treatment, levels of HbA1c, FPG, and 2 hPG were lower than those before treatment in the two groups (P<0.05). Levels of C-P (P<0.05) and 2 hC-P were higher than those before treatment in the two groups. After treatment, levels of HbA1c, FPG, and 2 hPG in the treatment group were lower than those in the control group (P<0.05), while levels of C-P and 2 hC-P showed no significant difference between two groups. After treatment, the levels of TG, TC, and LDL-C were lower than those before treatment (P<0.05) and HDL-C level was higher than that before treatment (P<0.05) in both groups. After treatment, levels of TG and LDL-C in the treatment group were lower than those in the control group (P<0.05), and levels of TC and HDL-C demonstrated no significant difference between two groups. After the treatment, the TCM syndrome score was lower than that before the treatment in both groups (P<0.05) and lower in the treatment group than in the control group (P<0.05). The overall effective rate of the treatment group was higher than that of the control group (χ2=7.585, P<0.05). The levels of Cys C and Hcy were lower than those before treatment in the two groups (P<0.05) and lower in the treatment group than in the control group (P<0.05). Doppler echocardiography of the lower limbs showed no obvious improvement in the control group after treatment. However, for the treatment group, slight decrease in intima-media thickness of the lower limb arteries and a slight reduction in the plaque area were observed, but the difference was not statistically significant. ConclusionDanzhi Jiangtang capsules has definite therapeutic effect on T2DM combined with lower-extremity macroangiopathy, which can improve glucolipid metabolism and reduce serum levels of Hcy and Cys C. This study can serve a reference for the prevention and treatment of T2DM combined with macroangiopathy.

OBJECTIVE To provide reference for the clinical application and further research and development of Shenling baizhu powder. METHODS Using the methods of textual research and bibliometrics, the key information such as the composition, dose, processing and usage of Shenling baizhu powder in ancient and modern literature were systematically summarized and sorted out, and the number of publications, keywords, and other contents of modern literature were visually analyzed. RESULTS & CONCLUSIONS Shenling baizhu powder was first collected in Taiping Huimin Hejiju Fang (abbreviated “Jufang”), the composition of core drugs and most widely used dose ratio in previous generations were the same as those in Jufang, basically focusing on powder making and being taken after mixed with liquid; the dosage was concentrated on 2-3 qian; the function and application of it had been expanded to a certain extent compared with Jufang; its clinical application had gradually extended from internal medicine to surgery, gynecology, pediatrics, ophthalmology, dermatology, etc., covering all aspects of the disease caused by spleen deficiency in general; the composition mechanism revolved around “strengthening the spleen, replenishing qi and removing dampness”; the recommended prescription drugs were all selected from the corresponding varieties in the pharmacopoeia. Modern research mainly focuses on clinical practice, with traditional Chinese medicine or traditional Chinese and Western medicine combined with this formula as the main modified treatment for various intestinal diseases, diarrhea diseases, and lung diseases. The efficacy and application are the frontier hot spots today, and the mechanism of action has begun to be explored. The usage in the literature and the inclusion in the pharmacopoeia are inconsistent, and further research is needed.

Heart failure with high prevalence is the endpoint of many cardiovascular diseases. Once diagnosed, patients usually need lifelong medication, which seriously affects their quality of life. The drugs commonly used to treat heart failure include angiotensin-converting enzyme inhibitors, beta-blockers, aldosterone receptor antagonists, and diuretics. However, the long-term use of those drugs can lead to side effects such as hypotension, depletion of body fluid, and electrolyte imbalance and even increase mortality. According to the theory of traditional Chinese medicine (TCM), Qi deficiency and blood stagnation is the major cause of heart failure and when Qi is not moving, blood is not flowing. Therefore, the TCM clinical treatment of heart failure uses the Chinese medicinal materials which replenish Qi, activate blood, and dispell stasis to treat both internal cause and external symptoms. Recent studies have demonstrated that Chinese herbal medicines such as Astragali Radix, Ginseng Radix et Rhizoma, Notoginseng Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma, Angelicae Sinensis Radix, as well as the compound formulas such as Buyang Huanwutang, Simiao Yongantang, Qili Qiangxin capsules, and Qishen Yiqi drops, play a significant role in the prevention and treatment of heart failure via replenishing Qi, activating blood, and dispelling stasis. Inhibition of oxidative stress and inflammatory responses, mitigation of myocardial fibrosis, improvement of calcium cycling, and protection of mitochondrial function represent the key mechanisms for the treatment of heart failure with Chinese medicinal materials. Focusing on the pathogenic mechanisms and signaling pathways of heart failure, this paper systematically describes the pharmacological effects, molecular mechanisms, and research progress in the clinical application of Chinese medicinal herbs with effects of replenishing Qi, activating blood, and dispelling stasis and their compound formulas in the prevention and treatment of heart failure, aiming to provide scientific evidence for the development and clinical use of anti-heart failure Chinese medicinal materials.

Heart failure is a complex clinical syndrome,which is the final result of compensatory failure of heart injury caused by various reasons. Long-term persistent cardiac stress leads to mitochondrial dysfunction,which in turn further damages cardiomyocytes and leads to disease progression. Timely removal of damaged mitochondria in cardiomyocytes and maintaining a good living environment of viable mitochondria is not only an effective means to protect cardiomyocytes,but also a new way to prevent and treat heart failure and ventricular remodeling. Mitochondrial quality control is a series of cellular activities for mitochondria to maintain their structural and functional stability,including oxidative stress response,regulation of mitochondrial dynamics,mitochondrial autophagy,intracellular calcium regulation and so on. Traditional Chinese medicine(TCM) mostly uses drugs of replenishing Qi and activating blood circulation in the treatment of chronic heart failure,and Qi and mitochondria are similar in function. According to TCM,the performance of the body as "static,descending and inhibitory" in the case of Qi deficiency can also be compared with the energy defect of mitochondria. The classical method of tonifying qi and activating blood circulation in TCM can be applied here. In recent years,TCM takes mitochondria as the target and carries out many related experimental studies from the point of view of myocardial energy supply. It is found that Chinese herbs for replenishing Qi and activating blood circulation can participate in regulating the quality control mechanism of intracellular mitochondria with multiple targets and links. It is proved by experiments that Chinese herbs for replenishing Qi and activating blood circulation can exert myocardial protective effect through this mechanism.

Shengxiantang, one of the classical traditional Chinese medicine (TCM) prescriptions, was developed by ZHANG Xi-chun, a well-known doctor in the Republic of China, to treat the syndrome of pectoral Qi sinking. Due to the unique herbal composition, this prescription has yielded remarkable outcomes, and it is still widely used for dealing with diseases of various systems. This paper reviewed the clinical research concerning Shengxiantang in order to provide reference for its clinical application. Following the retrieval of related articles published in the past ten years from China National Knowledge Infrastructure (CNKI), Wanfang Data Knowledge Service Platform, Chongqing Weipu Database for Chinese Technical Periodicals (VIP), and Chinese Biomedical Literature Service System (SinoMed) with the Chinese key words of “Shengxiantang (升陷汤)”, “sinking of pectoral Qi (大气下陷)”, “replenishing Qi and raising the sinking (益气升陷)”, and“pectoral Qi theory (大气理论)”, this paper collated the clinical application experience of Shengxiantang. To be specific, the corresponding etiology and pathogenesis, interpretations of Shengxiantang, as well as its indications, application experience and precautions were summarized to provide a basis for its further research and application. Based on the collected more than 50 representative articles, it was found that Shengxiantang has been extensively utilized for treating complex and critical diseases of various systems, especially the circulatory and respiratory diseases and organ prolapse. The research on the clinical application of Shengxiantang has deepened, exhibiting a promising development trend, which will provide new ideas for its clinical application.

ShenqiJiangtang Granule is composed of Panax ginseng, Astragali Radix and other valuable Chinese herbal medicines and extract, which has the effect of replenishing qi and nourishing yin, nourishing spleen and kidney, and it is also widely used in the treatment of type 2 diabetes and complications. After more than 20 years of clinical application, abundant research data have been accumulated. In order to accurately locate the clinical position and promote the rational use of drugs, a project was established by the Chinese Society of Traditional Chinese Medicine and experts organized by the Diabetes Branch to compile this consensus. This consensus defines the application of ShenqiJiangtang Granule in the clinical treatment of prediabetes, type 2 diabetes and its complications. It is suitable for clinicians at all levels of hospitals and grass-roots medical and health institutions across the country (traditional Chinese medicine, Western medicine, integrated traditional Chinese and Western medicine).

Leukopenia is a common clinical disease with diverse pathogenesis. Leukopenia is mostly caused by drugs, especially anti-tumor drugs. For the treatment of leukopenia, Chinese materia medica can play an ability of comprehensive adjusting control from multi-angle, multi-dimension, and multi-target. As a good Chinese medicine for tonifying blood and replenishing qi, Lvjiao Buxue Granules has the advantages of high curative effect, convenient to use, and easy to be accepted by patients. It has been reported in clinical application for elevating leukocytes. This present review will focus on the research progress on the elevating leukocytes effects and mechanisms of Lvjiao Buxue Granules based on complicated Chinese herbal formula, herb-pair, and single herb, so as to provide a reference for expanding the clinical indications and rational use of the drug.

Objective:To investigate the effect of modified Sijunzi Tang on protein and mRNA expressions of Occludin, Claudin-1 and zonula occludens-1(ZO-1) in cerebral ischemia rats. Method:Totally 60 male Sprague-Dawley rats were randomly divided into sham operation group, model group, edaravone group, small-dose modified Sijunzi Tang group, middle-dose modified Sijunzi Tang group and high-dose modified Sijunzi Tang group. The middle cerebral artery occlusion (MCAO) model was prepared by suture method. After 7 days of treatment, the modeling group was put to death. Western blot was used to detect the contents of Occludin, Claudin-1 and ZO-1 in the ischemic cerebral cortex of rats. Detection of Occludin, ZO-1, Claudin-1 mRNA expression in the ischemic cortex of rats by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). Result:Compared with the sham operation group, protein expressions of Occludin, Claudin-1, and ZO-1 in the model group were significantly down-regulated (PPPPConclusion:Modified Sijunzi Tang may protect the blood-brain barrier and reduce brain edema in ischemic stroke rats by increasing the expression of tight junction proteins Occludin, ZO-1 and Claudin-1.

Objective: To explore the mechanism of extract of Ginseng Radix et Rhizoma, Notoginseng Radix et Rhizoma and Chuanxiong Rhizoma in delaying the senescence of vascular endothelial cells induced by high glucose and high fat. Method: The 40 mmol·L-1 glucose and 100 μmol·L-1 palmitate were used to induce endothelial cell senescence. The experiment was divided into control group, model group and low, medium and high-dose traditional Chinese medicine groups (50,100,200 mg·L-1). The intervention lasted for 48 h. Cell proliferation was detected by cell counting kit-8(CCK-8); cell senescence was detected by senescence β-galactosidase staining; p16 and p21 protein expression levels were detected by Western blot; p-H2A. X(Ser139) expression, mitochondria ROS(mtROS) production and changes in mitochondrial membrane potential(MMP) were detected by immunofluorescence. Result: Compared with the control group, in model group, the cell proliferation ability and the number of SA-β-gal blue-stained cells decreased(PPPPβ-gal blue-stained cells, the mtROS production, and expression levels of p16, p21 and p-H2A. X(Ser139)(PPConclusion: The above results suggest that extract of Ginseng Radix et Rhizoma, Notoginseng Radix et Rhizoma and Chuanxiong Rhizoma delay of endothelial cellular senescence induced by high glucose and high fat, and its mechanism may be related to increasing mitochondrial membrane potential and reducing DNA damage accumulation caused by ROS production.

Astragali Radix is a classic Chinese materia medica with multiple effects including replenishing qi, consolidating exterior, promoting pus and discharge tissue regeneration, inducing diuresis to alleviate edema and other effects. Modern scientific researches have confirmed that different forms of Astragali Radix (single drug and compound) exhibit various pharmacodynamic effects on different pathological diseases, and their material basis are not the same. This review summarized the researches on the material basis of multi-effect Astragali Radix and provided new ideas for the related studies of material basis of Astragali Radix and its formula.

[Objective] To explore and analyze Professor ZHANG Yonghong's clinical experience in treating cerebral tumor.[Methods]Through the clinical study with the teacher to collect and collate the relevant information and medical records, from the etiology and pathogenesis of cerebral tumor, governance and other aspects of the specific analysis and discussion on Professor ZHANG Yonghong's treatment of this disease, academic point of view and clinical experience,it summed up the law of condemnation, and to the case of evidence,through regular follow-up, observing the treatment effect. [Results]Professor ZHANG Yonghong views that the pathogenesis of cerebral tumor can be summarized as Qi deficiency, phlegm and blood stasis, internal movement of the liver, and confusion of the body. Its pathologic factors are related to wind, fire, phlegm, blood stasis and poison. In the treatment of cerebral tumors, it is advocated to improve Qi, nourish Yin, promote positive Qi, and break loose to remove cancer toxins. On the basis of the selection of invigorating the spleen and protecting the stomach, the rational use of invigorating the Qi and nourishing the Yin and combining with the drugs of eliminating evils and removing toxin, in order to achieve the goal of strengthening the anti -cancer. The two cases both achieved good results by strengthening the body resistance, eliminating evils and removing toxin.[Conclusion]Professor ZHANG Yonghong stressed that strengthening of the body resistance is a primary treatment when the treatment for cerebral tumor. We should pay attention to replenishing Qi and nourishing Yin, eliminating evils and removing toxin are adjuvant therapy. The spleen and stomach should be protected throughout the treatment, and patient's mood are also important. It is of great significance to instruct TCM to treat cerebral tumor.

In this study,we collected the prescriptions of Professor Liu Fengbin used for the out-patients with irritable bowel syndrome(IBS),counted the usage frequency of the herbs and core-combination herbs in the prescriptions,analyzed the medication principles of Professor Liu Fengbin,and mined the new recipes for IBS by the methods of association rule mining and complex system entropy clustering presented in the Traditional Chinese Medicine Inheritance Support System.The results showed that Professor Liu is experienced in treating IBS based on liver-spleen differentiation,mainly applying the therapies of invigorating spleen and replenishing qi,soothing liver and regulating qi,also using the methods of resolving dampness,alleviating depression,relieving pain and activating blood,and prescribing the basic recipe of Chai Shao Sijunzi Decoction.The medication principles of Professor Liu Fengbin for the treatment of IBS will be beneficial to the further exploration of the syndrome pattern distribution of IBS and new recipes for IBS.

Objective]To sum up director He Ruoping's feature in recognizing ovarian cancer causa morbi and mechanism and clinical treatment rules; To explore the clinical experience of director He Ruoping treatment with ovarian cancer.[Method]By learning from director doctor He Ruoping for three years and combining my own clinical experience, from the etiology and pat hogenesis, it sums up the teacher He' s unique feature of by stages treatment of ovarian cancer, and with one case for detailed explanation.[Result]Teacher He advocates the combination of traditional Chinese medicine and western medicine, puts forward the principle of ovarian cancer.By the way of strengthening body resistance and eliminating evil, Teacher He adopts the help of invigorating the spleen and tonifying the kidney,supplementing qi and nourishing yin,soothing the liver and regulating qi,clearing away heat and toxic material,promoting blood circulation to remove blood stasis, softening firm scattered nodes,and water permeability wet for the main therapy in treatment of ovarian cancer and the curative effect is remarkable.[Conclusion] To take Teacher He' s experience for treating ovarian canceR from essence,replenishing qi and blood,removing turbidity can significantly relieve patients' clinical symptoms and improve patients' life quality.

Replenishing qi, nourishing yin and activating blood circulation therapy is an effective treatment for diabetic nephropathy (DN) in traditional Chinese medicine (TCM). In this article, mechanisms of this therapy in DN were summarized, which included improvement of glucose and lipid metabolism, reducing inflammation and relieving renal fibrosis. And it provided theoretical supports for DN treatment with TCM.

Objective To investigate the clinical efficacy of integrated Chinese and western medicine for the treatment of anovulatory dysfunctional uterine bleeding. Methods A total of 108 anovulatory dysfunctional uterine bleeding patients were evenly randomized into treatment group and control group. Both groups were given basic conventional treatment, estrogen-progestogen sequential therapy for 3 menstrual cycles for dysfunctional uterine bleeding patients at puberty and child-bearing period, and Marvelon for patients at menopause transition period after excluding uterine tumor by diagnostic curettage. Additionally, the treatment group was given Chinese herbal decoction of Radix Rehmanniae Preparata, Semen Cuscutae, Fructus Corni, Placenta Hominis, Herba Ecliptae, Fructus Schisandrae Chinensis, Rhizoma Alismatis, raw Rhizoma Dioscoreae, Rhizoma Atractylodis Macrocephalae, Rhizoma Cyperi, Radix Codonopsis, Radix Astragali, Radix Angelicae Sinensis, Radix Paeoniae Alba, Pericarpium Citri Reticulatae, and charred Herba Schizonepetae. Therapeutic effect, bleeding-arresting time, and recurrence rate during 3-month follow-up were observed in the two groups. Results ( 1) The treatment group had a total effective rate of 100.0%, higher than 94.4%in the control group, the difference being significant (P<0.05). (2) Bleeding-arresting time was (45.50 ± 13.0) hours in the treatment group and was (58.0 ± 15.0) hours in the control group, and the difference was statistically significant (P<0.05). (3) During the 3-month follow-up, the treatment group had the recurrence rate of 16.7% ( 9 cases) , lower than 29.6% ( 16 cases) in the control group, and the difference was statistically significant ( P<0.05). Conclusion Integrated Chinese and western medicine has certain effect for the treatment of anovulatory dysfunctional uterine bleeding, showing good prospects for extensive application in gynecological clinic.

Objective To provide solid data for TCM treatment and research of triple negative breast cancer (TNBC) by investigating its syndromes distribution.Methods 180 patients diagnosed of TNBC in Department of Oncology,Guang'anmen Hospital from Janurary 2007 to November 2012 participated in this clinical trial.Eight principals syndrome differentiation,and Qi,Blood and viscal syndrome differentiation were used.According to the syndromes diagnosis standard,the differentiation of one single syndrome firstly and then multi-syndrome were conducted and related variables were analyzed finally Results 404 symptoms presented in all 180 patients.Qi deficiency syndrome (35.89%) and blood stasis syndrome (32.18%) showed statistical difference when compared with blood deficiency syndrome (3.22%),yin deficiency syndrome (4.95%),yang deficiency syndrome (3.96%),spleen-dampness syndrome (3.47%),qi stagnation syndrome (7.43%),toxic heat syndrome (4.95%) and dampness-heat syndrome (1.49%) (P<0.01,x2=124.709,116.007,127.813,126.913,105.411,116.007,140.015; 118.273,105.447,117.630,120.445,97.411,105.447,110.273respectively).92.22% syndroms were multi-syndrome including 23.89% bi-syndrome,32.22% tri-syndrome and 25.56% qua-syndrome.Conclusion The syndromes of TNBC were complicated but with predominant Qi dificency and Blood stasis; single syndrome identification should be put in the first place and then combining them together may be practical approaches; Replenishing Qi and activating blood circulation might be breaking through in TNBC treatment.

Objective To observe the effects of complex prescription with the effects of replenishing Qi, activating blood and resolving phlegm on the levels of LN and PC-Ⅲ in BALF and the changes of lung tissue and figure in rat with pulmonary fibrosis. Method The rat model of pulmonary fibrosis was established by injecting pinyamycin, then the changes of the levels of LN and PC-Ⅲ in every group (TCM group, model group and control group) in 7, 28 days respectively were observed.
