RESUMO

We report a case of somnambulism successfully treated with sansoninto. A 55-year-old woman had an initial episode of somnambulism in childhood, which recurred in her early 20s along with development of psychotic manifestations. The patient was hospitalized in a psychiatric hospital with a diagnosis of schizoaffective disorder and coexisting Crohn's disease. At the age of 55, she began to show nocturnal wandering tendencies and we prescribed yokukansan at 2.5 g/day, though the symptoms only temporarily improved. Following a switch to sansoninto at 7.5 g/day, the symptoms disappeared. Based on our experience with this case, we speculate that the Heart component of the five viscera theory in traditional Chinese medicine plays a key role in somnambulism.

We present seven cases of insomnia successfully treated with hochuekkito. Two patients showed improvement of their insomnia after taking hochuekkito before going to bed, and three patients showed improvement after taking hochuekkito twice per day. The other two patients could sleep better after adding hochuekkito to other Kampo formulations. All these patients were light sleepers, and became easily tired, excessive sleepy after meals, and had daytime sleepiness. However, they had no gastrointestinal symptoms, such as appetite loss. Five of the seven patients reported waking up feeling better after taking hochuekkito. Two other Kampo formulations, sansoninto and kihito, were also given to patients with deficient constitution, who complained of insomnia. Sansoninto and kihito are formulae that compensate for qui and blood deficiency. Kihito contains more herbs with beneficial effects on “spleen and stomach”, and “heart” functions more than sansoninto, and therefore, kihito may be preferred for patients with a more deficient constitution. The reason why our patients were able to sleep more deeply and wake up smoothly with hochuekkito may be that they exhibited remarkable qui deficiency, showing general fatigue, excessive sleepiness after meals, and daytime sleepiness, but without the symptoms of blood deficiency, such as palpitations or uneasiness, being easily frightened or forgetful, or showing anemia or bleeding.

Sleep disturbance is often observed among patients with diabetes and poor sleep is associated with higher HbA 1 c levels. Therefore, treatment of sleep disturbance is important. We experienced two cases in which sansoninto was useful in the treatment of insomnia with type 2 diabetes mellitus. A 58-year-old man had been treated with insulin for 10 years and a 79-yerar-old man had been treated oral hypoglycemic agents for 15 years. We administered sansoninto in each patient, after which sleep disturbance disappeared gradually. These results suggest that sansoninto is useful for sleep disturbance in patients with diabetes.

RESUMO

A 7 year-old male patient who suffer from trigeminal trophic syndrome following herpes zoster was treated with a combination of rokumigan and sansoninto. We conclude that the pathogeneses in this case were excessive rises in liver yang and liver wind caused by yin deficiencies of the kidneys and liver, and that these were ameliorated by the effect of the rokumigan and sansoninto combination in nourishing kidney yin, clearing away heat-evil, and eliminating restlessness.

Chronic fatigue is a common symptom in primary health care and caused by various organic and functional diseases. Although improvement is attained by controlling underlying organic disease, it is sometimes difficult to relieve fatigue in patients with chronic fatigue syndrome or dysthymia. We have encountered patients with idiopathic chronic fatigue (ICF) successfully treated with sansonintokagen. A 62-year-old female complained of chronic fatigue several years after menopause, and had normal status in routine blood and image analyses at a nearby hospital. Her condition was diagnosed as ICF with sleeplessness. Treatment with sansonintoaogibakumondo for 8 weeks improved her chronic fatigue and sleeplessness, and enabled her to continue housekeeping. This observation suggests that this kampo formula may be a useful agent for ICF.

We report two elderly cases in the extended care unit who were successfully treated for emitting a strange squealing sound with Sansonin-to. Case 1 was a 97-year-old female who was hospitalized with a strange squeal after repeated aspiration pneumonia. She made this strange sound especially during the night. This sound disappeared with the administration of Sansonin-to (TJ-103: 7.5g/day) except at the times of position change and diaper change, and she also became able to eat after practicing swallowing. Case 2 was an 80-year-old female who was hospitalized after a cerebral hemorrhage. She also made a strange squealing sound during the night. After administration of Sansonin-to, this sound was decreased, and she became able to sleep well at night and live actively with rehabilitation training in the daytime. With an aging society, troublesome behaviors due to dementia will be an increasing problem. These cases suggest that there may be certain conditions of elderly nighttime delirium that are treatable with Sansonin-to.
