Background and Objectives: the second greatest cause of human poisoning in Brazil is caused by venomous animals. Thus, this study aimed to analyze clinical and sociodemographic data, in order to outline the epidemiological profile of accidents involving venomous animals in Maranhão. Methods: this is an ecological study with a quantitative approach, carried out from data collection by the Notifiable Diseases Information System (SINAN) of notifications of accidents by venomous animals that occurred in the state of Maranhão from 2012 to 2021. Results: of the 34,808 cases reported, it was found that the highest incidence occurred in 2019 and, in general, January is the month in which most accidents are recorded. When analyzing the sociodemographic profile, it is noted that the majority of victims are represented by male individuals, between 20-39 years old, with an unidentified level of education. In the case of clinical and epidemiological criteria, it was evident that snakes of the genus Bothrops are responsible for the highest incidence, with most cases involving venomous animals occurring within 1-3 hours after the bite. Among the cases identified, 63% were described as mild and 82% progressed to cure. Conclusion: the epidemiological profile described in the study can be used by health agents to plan preventive measures in Primary Health Care, and knowing the profile of victims is essential to prevent and promote quality care.(AU)
Justificativa e Objetivos: a segunda maior causa de envenenamento humano no Brasil é ocasionada por animais peçonhentos. Dessa forma, este estudo visou analisar dados clínicos e sociodemográficos, a fim de traçar o perfil epidemiológico dos acidentes envolvendo animais peçonhentos no Maranhão. Métodos: trata-se de estudo ecológico de abordagem quantitativa, realizado a partir da coleta de dados pelo Sistema de Informações de Agravos de Notificação (SINAN) de acidentes por animais peçonhentos ocorridos no estado do Maranhão no período de 2012 a 2021. Resultados: dos 34.808 casos notificados, constatou-se que a maior incidência ocorreu no ano de 2019 e, em geral, janeiro é o mês em que mais se registram acidentes. Ao analisar o perfil sociodemográfico, nota-se que a maior parte das vítimas é representada por indivíduos do sexo masculino, entre 20 e 39 anos, com grau de escolaridade não identificado. Tratando-se de critérios clínicos e epidemiológicos, evidenciou-se que as serpentes do gênero Bothrops são as responsáveis pela maior incidência, sendo que a maior parte dos atendimentos envolvendo animais peçonhentos ocorreu no intervalo de 1-3 horas após a picada. Entre os casos identificados, 63% foram descritos como leve e 82% evoluíram para a cura. Conclusão: o perfil epidemiológico descrito no estudo pode ser utilizado por agentes de saúde para o planejamento de medidas preventivas na Atenção Primária à Saúde, e conhecer o perfil das vítimas é essencial para prevenir e promover uma assistência de qualidade.(AU)
Justificación y Objetivos: la segunda causa de envenenamiento humano en Brasil es causada por animales venenosos. Así, este estudio tuvo como objetivo analizar datos clínicos y sociodemográficos, con el fin de delinear el perfil epidemiológico de los accidentes con animales venenosos en Maranhão. Métodos: se trata de un estudio ecológico con abordaje cuantitativo, realizado a partir de la recolección de datos por el Sistema de Información de Enfermedades de Declaración Obligatoria (SINAN) de relatos de accidentes por animales ponzoñosos ocurridos en el estado de Maranhão entre 2012 y 2021. Resultados: de los 34.808 casos reportados, se encontró que la mayor incidencia ocurrió en 2019 y, en general, enero es el mes en el que se registran más accidentes. Al analizar el perfil sociodemográfico, se observa que la mayoría de las víctimas están representadas por personas del sexo masculino, entre 20-39 años, y con nivel de escolaridad no identificado. En el caso de los criterios clínicos y epidemiológicos, se evidenció que las serpientes del género Bothrops son las responsables de la mayor incidencia, y la mayoría de los casos que involucran animales venenosos ocurren entre 1 y 3 horas después de la mordedura. Entre los casos identificados, el 63% fueron descritos como leves y el 82% progresó hacia la curación. Conclusión: el perfil epidemiológico descrito en el estudio puede ser utilizado por los agentes de salud para planificar medidas preventivas en la atención básica de salud, y conocer el perfil de las víctimas es fundamental para prevenir y promover una atención de calidad.(AU)

Background: In Colombia, several species of Buthidae scorpions belonging to the genera Centruroides and Tityus coexist, and their stings are considered life-threatening to humans because of their venom neurotoxins. Despite previous studies focusing on neurotoxins from these scorpion genera, little is known about the enzymes present in their venoms and their relationship with whole venom toxicity. Methods: Here, using proteomic and biochemical protocols the enzymatic activities of the venoms of three Colombian scorpion species, C. margaritatus, T. pachyurus, and T. n. sp. aff. metuendus, were compared to establish the presence and absence of enzymes such as phospholipases, hyaluronidases, and proteases that could be related to venom toxicity. Results: C. margaritatus was positive for hyaluronidases, T. n. sp. aff. metuendus for proteases, and T. pachyurus exhibited activity for all three mentioned enzymes. Conclusion: This information provides valuable insights into the specific enzyme diversity of each species' venom and their potential role in venom toxicity, which could contribute to the development of better treatments and prevention strategies for scorpion envenomation.

Resumen Objetivo: Evaluar la implementación de una intervención educativa con elementos de Educación Popular en Salud sobre el empoderamiento y los cambios en salud para la prevención de la picadura de alacrán, en mujeres del municipio de Tetecala, Morelos. Material y métodos: La intervención educativa se realizó en mujeres adscritas al programa PROSPERA, con 16 participantes, así, consistió en cinco sesiones de manera semanal, con tiempo de dos horas, aproximadamente, cada una. Se realizó un triple diagnóstico sobre los conocimientos y las causas de la picadura de alacrán, con esa información, se diseñan los principales componentes de la estrategia educativa (1. Conocimiento del alacrán; 2. Riesgos para la picadura de alacrán; 3. Prevención de la picadura de alacrán), lo que se lleva a cabo en los meses de febrero y marzo del 2019. Se evalúan dos categorías, empoderamiento y cambios en salud, ahondando en elementos descritos por Noelle Wiggins, mediante metodología mixta. Las mediciones se realizan al mismo grupo previo y posterior a la intervención educativa; la evaluación cualitativa se realiza mediante el análisis de registro etnográfico, asimismo, la parte cuantitativa se aplica cuestionario sobre conocimientos y prácticas. Resultados: Se identifican cambios en elementos de empoderamiento, así como cambios en salud. Conclusiones: Emplear elementos de Educación Popular en Salud tiene efectos similares a estrategias donde se emplea la metodología pura, así, en la prevención de la picadura de alacrán, muestra resultados similares con otros problemas en salud.
Abstract Objective: To evaluate implementation of an educational initiative with elements of Popular Health Education on empowerment and changes in health for prevention of scorpion stings, in women from municipality of Tetecala, Morelos. Material and methods: Educational initiative was carried out in women assigned to PROSPERA program, with 16 participants. It consisted of 5 weekly sessions, with a time of approximately 2 hours each. A diagnosis was made on the knowledge and causes of the scorpion sting, with this information the main components of educational strategy were designed (1. Knowledge of scorpion; 2. Risks for scorpion sting; 3. Prevention of scorpion sting). It was carried out in the months of February and March 2019. Two categories, empowerment and changes in health, were evaluated, delving into elements described by Noelle Wiggins, using a mixed methodology. The measurements were made to the same group before and after educational initiative. Qualitative evaluation was carried out by analyzing ethnographic record. Quantitative part was applied questionnaire on knowledge and practices. Results: Changes in elements of empowerment were identified, as well as changes in health. Conclusions: Using elements of Popular Health Education has similar effects than strategies where pure methodology is used. In the prevention of Scorpion stings, it showed similar results to other health problems.
RESUMO
Evolution and natural selection have endowed animal venoms, including scorpion venoms, with a wide range of pharmacological properties. Consequently, scorpions, their venoms, and/or their body parts have been used since time immemorial in traditional medicines, especially in Africa and Asia. With respect to their pharmacological potential, bioactive peptides from scorpion venoms have become an important source of scientific research. With the rapid increase in the characterization of various components from scorpion venoms, a large number of peptides are identified with an aim of combating a myriad of emerging global health problems. Moreover, some scorpion venom-derived peptides have been established as potential scaffolds helpful for drug development. In this review, we summarize the promising scorpion venoms-derived peptides as drug candidates. Accordingly, we highlight the data and knowledge needed for continuous characterization and development of additional natural peptides from scorpion venoms, as potential drugs that can treat related diseases.

OBJECTIVE@#To explore the effectiveness and advantages of using Fastpass Scorpion suture passer to stitch the inferior capsulolabral complex in arthroscopic Bankart repair compared with traditional arthroscopic suture shuttle.@*METHODS@#The clinical data of 41 patients with Bankart lesion, who met the selection criteria and were admitted between August 2019 and October 2021, was retrospectively analyzed. Under arthroscopy, the inferior capsulolabral complex was stitched with Fastpass Scorpion suture passer in 27 patients (FS group) and with arthroscopic suture shuttle in 14 patients (ASS group). There was no significant difference between the two groups ( P>0.05) in gender, age, injured side, frequency of shoulder dislocation, time from first dislocation to operation, and preoperative Rowe score of shoulder. Taking successful suture and pull-tightening as the criteria for completion of repair, the number of patients that were repaired at 5∶00 to 6∶00 (<6:00) and 6∶00 to 7∶00 positions of the glenoid in the two groups was compared. The operation time, and the difference of Rowe shoulder score betwee pre- and post-operation, the occurrence of shoulder joint dislocation, the results of apprehension test, and the constituent ratio of recovery to the pre-injury movement level between the two groups at 1 year after operation.@*RESULTS@#Both groups completed the repair at 5∶00 to 6∶00 (<6∶00), and the constituent ratio of patients completed at 6∶00 to 7∶00 was significantly greater in the FS group than in the ASS group ( P<0.05). The operation time was significantly shorter in the FS group than in the ASS group ( P<0.05). All incisions in the two groups healed by first intention. All patients were followed up 12-36 months (mean, 19.1 months). No anchor displacement or neurovascular injury occurred during follow-up. Rowe score of shoulder in the two groups significantly improved at 1 year after operation than preoperative scores ( P<0.05), and there was no significant difference in the difference of Rowe shoulder score between pre- and post-operation between the two groups ( P>0.05). At 1 year after operation, no re-dislocation occurred, and there was no significant difference in the apprehension test and the constituent ratio of recovery to the pre-injury movement level between the two groups ( P>0.05).@*CONCLUSION@#Compared with the arthroscopic suture shuttle, using Fastpass Scorpion suture passer to stitch the inferior capsulolabral complex in arthroscopic Bankart repair is more convenient, saves operation time, and has good effectiveness.

ABSTRACT We report the first pediatric case of a Jaguajir agamemnon scorpion sting. This case occurred in the state of Paraná and is the first record of this species in southern Brazil. The patient was admitted within 15 min, with pain, local edema, erythema, whole-body pruritus, and tongue paresthesia within the first two hours, which disappeared thereafter. The patient's condition was considered mild, with a positive outcome, and she recovered completely.

Objective: to describe the completeness of notifications of accidents involving venomous animals held on the Notifiable Health Conditions Information System (SINAN), in Brazil and its macro-regions, from 2007 to 2019. Methods: we analyzed essential and non-mandatory fields for snakebite, spider bite and scorpion sting notifications, considering the following completeness categories: Excellent (≤ 5.0% incompleteness), Good (5.0% to 10.0%), Regular (10.0% to 20.0%), Poor (20.0% ≤ 50.0%) and Very Poor (> 50.0%). Proportional change in completeness between 2007 and 2019 was estimated. Results: 1,871,462 notifications were investigated. The "localized manifestations", "systemic manifestations", "case classification", "case progression" and "zone of occurrence" fields had excellent or good completeness. Completeness was regular or poor for the "schooling" and "race/color" fields. The "occupation" field was predominantly poorly or very poorly filled in. There was a proportional worsening in completeness ((VP < 0) in most regions for the "zone of occurrence", "case progression" and "schooling" fields. Conclusion: completeness of most fields improved, although socioeconomic and occupational fields require more attention.
Objetivo: describir la completitud de las notificaciones de envenenamiento por serpientes, arañas y escorpiones en el Sistema de Información de Enfermedades de Declaración - Sinan, en Brasil y regiones, de 2007 a 2019. Métodos: se analizaron campos esenciales y no obligatorios para las notificaciones de ofidismo, aracneismo y escorpionismo, considerando las categorías de completitud: Excelente (≤ 5,0% de incompletitud), Buena (5,0% a 10,0%), Regular (10,0% a 20,0%), Mala (20,0% ≤ 50,0%) y Muy mala (> 50,0%). Se estimó el cambio proporcional en la completitud entre 2007-2019. Resultados: se investigaron 1.871.462 notificaciones. Los campos "manifestaciones locales y sistémicas", "zona de ocurrencia", "clasificación y evolución de casos" presentaron excelente o buena completitud. "Escolaridad" y "raza/color de piel" regular o mala completitud. "Ocupación" fue predominantemente mala/muy mala completitud. "Zona","evolución" y "escolaridad" mostraron empeoramiento proporcional en la completitud en la mayoría de las regiones ((VP < 0). Conclusión: hay mejoría y completitud en la mayoría de los campos, aunque los campos socioeconómico y ocupacional demandan atención.

Resultados: foram investigadas 1.871.462 notificações; os campos "manifestações locais", "sistêmicas", "classificação do caso", "evolução do caso" e "zona de ocorrência" apresentaram completude excelente ou boa; "escolaridade" e "raça/cor da pele", completude regular ou ruim; ocupação, completude ruim ou muito ruim; "zona", "evolução do caso" e "escolaridade" apresentaram piora proporcional na completude, na maioria das regiões ((VP < 0). Conclusão: melhorou a completude na maioria dos campos, exceto socioeconômicos e ocupacionais, que demandam maior atenção.

This synoptic review aims to bring some general information on fossil scorpions, namely those trapped in amber - fossilized resin - ranging from Lower Cretaceous through the Palaeocene and up to the Miocene. The question to be addressed is how the study of these fossils can be connected with possible present scorpionism problems. A precise knowledge of these ancient lineages provides information about the evolution of extant lineages, including the buthoids, which contain most known noxious species. Among the Arthropods found trapped in amber, scorpions are considered rare. A limited number of elements have been described from the Late Tertiary Dominican and Mexican amber, while the most ancient Tertiary amber from the Baltic region produced more consistent results in the last 30 years, primarily focusing on a single limited lineage. Contrarily, the Cretaceous amber from Myanmar, also called Burmite, has yielded and continues to yield a significant number of results represented by several distinct lineages, which attest to the considerable degree of diversity that existed in the Burmese amber-producing forests. As in my previous similar contributions to this journal, the content of this note is primarily addressed to non-specialists whose research embraces scorpions in various fields such as venom toxins and public health. An overview knowledge of at least some fossil lineages can eventually help to clarify why some extant elements associated with the buthoids represent dangerous species while others are not noxious.(AU)

ABSTRACT Objective: To characterize scorpion accidents at the Information and Toxicological Assistance Center (CIATox) in Campinas, to analyze risk factors related to the moderate and severe classification, and to determine the age group at greatest risk for this classification. Method: Cross-sectional and retrospective study, with patients assisted in person at CIATox, who had a scorpion accident, from January 2015 to December 2019. Descriptive and inferential analysis was conducted. For the age variable, a ROC curve was constructed to determine cutoff points in relation to the severity classification. Poisson regression models were adjusted considering severity classification as the dependent variable. Results: A total of 754 cases with a mean age of 36.05 years, mostly female and non-occupational accidents that occurred in the urban area, was analyzed. The most frequent scorpion was the Tityus serrulatus. The risk factors found for greater severity were age group up to 22 years and previous care in other health services. Conclusion: The age range up to 22 years old should be used as a predictive factor of severity in the clinical evaluation of patients stung by scorpions to carry out adequate management of cases.
RESUMEN Objetivo: Caracterizar los accidentes de escorpiones en el Centro de Información y Asistencia Toxicológica (CIATox) de Campinas, analizar los factores de riesgo relacionados con la clasificación moderada y severa y determinar el grupo etario de mayor riesgo para esta clasificación. Método: Estudio transversal y retrospectivo, con pacientes atendidos presencialmente por el CIATox, que sufrieron un accidente de escorpión, desde enero de 2015 a diciembre de 2019. Se realizó un análisis descriptivo e inferencial. Para la variable edad se construyó una curva ROC con el fin de determinar puntos de corte en relación a la clasificación de severidad. Los modelos de regresión de Poisson se ajustaron considerando la clasificación de severidad como variable dependiente. Resultados: Se analizaron 754 casos con edad media de 36,05 años, en su mayoría mujeres y accidentes no laborales ocurridos en la zona urbana. El escorpión más frecuente fue el Tityus serrulatus. Los factores de riesgo encontrados para mayor gravedad fueron el grupo etario hasta 22 años y la atención previa en otros servicios de salud. Conclusión: El grupo etario hasta los 22 años debe ser utilizado como factor predictivo de severidad en la evaluación clínica de los pacientes picados por escorpiones para realizar un manejo adecuado de los casos.

Método: Estudo transversal e retrospectivo, com pacientes atendidos presencialmente pelo CIATox, que sofreram acidente escorpiônico, de janeiro de 2015 a dezembro de 2019. Foi realizada análise descritiva e inferencial. Para a variável idade foi construída uma curva ROC com o intuito de determinar pontos de corte em relação à classificação de gravidade. Foram ajustados modelos de regressão Poisson considerando a classificação de gravidade como variável dependente. Resultados: Foram analisados 754 casos com idade média de 36,05 anos, maioria do sexo feminino e acidentes não ocupacionais ocorridos na zona urbana. O escorpião mais frequente foi o Tityus serrulatus. Os fatores de risco encontrados para maior gravidade foram faixa etária até 22 anos e atendimento prévio em outros serviços de saúde. Conclusão: A faixa etária até 22 anos deve ser usada como fator preditivo de gravidade na avaliação clínica dos pacientes picados por escorpião para realização de manejo adequado dos casos.

Introducción. El emponzoñamiento por Tityus carrilloi n. sp. representa una amenaza para la vida. Según las manifestaciones clínicas, se clasifica en leve, moderado y grave. Objetivo. Comparar las características epidemiológicas y bioquímicas en niños con escorpionismo leve, moderado y grave. Población y métodos. Estudio descriptivo, transversal y retrospectivo. Se analizaron las consultas de menores de 15 años picados por Tityus carrilloi n. sp. entre enero de 2017 y diciembre de 2018 en un hospital pediátrico de tercer nivel en Santa Fe (Argentina). Resultados. Se incluyeron 524 niños, el 81 % (421) con dolor local y el 19 % (103) con manifestaciones sistémicas. Los niños con síntomas sistémicos de escorpionismo fueron más pequeños en edad que los que presentaron manifestaciones locales (p <0,001). En el invierno los niños desarrollaron 8 veces más manifestaciones sistémicas de escorpionismo y durante la primavera, 2,4 veces más que durante el verano. De los 103 niños internados, 80 fueron casos moderados y 23, graves. No hubo diferencias entre grupos en edad (p = 0,29) ni en la demora en recibir suero antiescorpiónico (p = 0,81). El tiempo de internación fue mayor en los graves (p <0,001). Los valores de glóbulos blancos o glucemia mayores a 30 000 cel/ml y 300 mg/dl respectivamente estuvieron presentes casi exclusivamente en escorpionismos graves. Conclusión. En niños picados por el escorpión Tityus carrilloi n. sp., el riesgo de desarrollar manifestaciones sistémicas fue mayor cuanto menor fue la edad y durante el invierno y la primavera. Los valores de glóbulos blancos y de glucemia fueron mayores en niños con escorpionismo grave.
Introduction. Scorpion envenomation by Tityus carrilloi n. sp. represents a threat to life. Depending on its clinical manifestations, it is classified as mild, moderate or severe. Objective. To compare the epidemiological and biochemical characteristics among children with mild, moderate, and severe scorpionism. Population and methods. Descriptive, crosssectional, and retrospective study. The consultations at a tertiary care children's hospital in Santa Fe (Argentina) of children under 15 years of age stung by Tityus carrilloi n. sp. between January 2017 and December 2018 were analyzed. Results. In total, 524 children were included, 81% (421) with local pain and 19% (103) with systemic manifestations. Children with systemic symptoms of scorpionism were younger in age than those with local manifestations (p < 0.001). In the winter, children developed 8 times more systemic manifestations of scorpionism; during the spring, 2.4 times more than during the summer. Out of the 103 hospitalized children, 80 were moderate cases and 23 severe cases. There were no differences between age groups (p = 0.29) or in the delay in receiving the anti-scorpion serum (p = 0.81). The length of hospital stay was longer among severe cases (p < 0.001). WBC and blood glucose levels higher than 30 000 cell/mL and 300 mg/dL, respectively, were present almost exclusively in severe scorpionism cases. Conclusion. In children stung by the scorpion Tityus carrilloi n. sp., the younger the age and during winter and spring, the higher the risk for systemic manifestations. WBC and blood glucose levels were higher in children with severe scorpionism.

Resumen Hombre de 44 años que acude al servicio de urgencias posterior a picadura de escorpión. Presentaba parestesias, dolor toracoabdominal y náuseas. Desde el momento del ingreso presentó dolor torácico atípico, dolor abdominal epigástrico de intensidad leve, asociado a náuseas. Se realizó electrocardiograma que no evidenció alteraciones. La troponina fue elevada, por lo que se llevó a cuidado intensivo y se inició manejo antiisquémico, corroborando posteriormente en la angiografía una disección coronaria tipo 1 en el ramus intermedio como causante de su evento coronario. El paciente recibió un vial de suero antiescorpiónico en primer nivel de atención y posteriormente se indicó manejo con betabloqueante, antiagregación con ácido acetilsalicílico y clopidogrel, con disminución progresiva de sus síntomas. Es egresado asintomático. Se reporta un caso de infarto agudo de miocardio tipo 2 por disección coronaria posterior a una picadura de escorpión, del cual no hay casos reportados previamente en el mundo.
Abstract A 44-year-old man was admitted to the emergency department after a scorpion sting. He presented paresthesia, thoracoabdominal pain and nausea. From the moment of admission, he presented atypical chest pain, epigastric abdominal pain of mild intensity, associated with nausea. An ECG was taken that did not show alterations, troponin was elevated, he was admitted to intensive care unit, management was initiated, subsequently corroborating in the angiography, a type 1 coronary artery dissection in the intermediate ramus as the cause of coronary event. The patient received a vial of antiscorpionic serum in the first level of care, later treatment with beta-blocker, antiaggregation with acetylsalicylic acid and clopidogrel was indicated, with progressive reduction of his symptoms. He is an asymptomatic graduate. A case of type 2 acute myocardial infarction due to coronary dissection after a scorpion sting is reported, there are no previously reported cases in the world.

Introducción: En el Ecuador se desconoce la frecuencia, los factores de riesgo, el efecto preciso de las picaduras de escorpión, sus toxinas, la fisiopatología e interacción con la población. Objetivo: Registrar la incidencia de las picaduras de escorpión, el cuadro clínico, su manejo y las limitaciones para la obtención del antídoto en Ecuador. Métodos: Se realizó un estudio de cohorte retrospectivo, en el cual se recolectaron datos como edad, sexo, residencia, sitio corporal de picadura, cuadro clínico, tratamiento, estancia médica, referencia a unidad de mayor complejidad, morbilidad y mortalidad, durante el periodo entre enero de 2016 y noviembre de 2018 en la población amazónica del cantón Taisha, provincia de Morona Santiago, Ecuador. Resultados: Se evaluaron 134 picaduras de escorpión, las cuales predominaron en personas adultas (70,9 %). La región corporal más frecuente de picadura fue en las extremidades superiores e inferiores (92,5 %) y el nivel de intoxicación grave correspondió al 12,7 %. La estacionalidad de las picaduras de escorpión predominó en los meses de octubre, abril, diciembre y marzo. La presentación de la morbilidad anual fue mayor en el año 2017 (52,9 %). Conclusiones: Existe una alta incidencia de picaduras de escorpión en la zona de estudio. En el Ecuador la accesibilidad al antídoto es limitada, por lo que es necesario realizar más investigaciones en el tema, incluyendo intervenciones preventivas y curativas.
Introduction: In Ecuador, the frequency, risk factors, precise effects, toxins, pathophysiology and interaction with the population of scorpion stings are unknown. Objective: To report on the incidence of scorpion stings, its clinical picture and management, and the limitations for obtaining an antidote in Ecuador. Methods: A retrospective cohort study was conducted and data was collected on the age, gender, place of residence, site of sting, clinical picture, treatment, hospital stay, referral to higher complexity unit, morbidity, and mortality from January 2016 to November 2018 in the Amazonian population from Taisha canton, Morona-Santiago province in Ecuador. Results: 134 scorpion stings were evaluated. Most cases were adults (70.9%). The most frequent region stung by scorpions were the upper and lower extremities (92.5%) and severe intoxication level accounted for 12.7% of cases. The highest incidence of scorpion stings occurred in October, April, December and March. Annual morbidity was higher in 2017 (52.9%). Conclusions: There is a high incidence of scorpion Sting in the area under study. Accessibility to an antidote is limited in Ecuador; therefore, further research on this topic and on preventive and curative interventions are necessary.

Introduction: Minas Gerais is the Brazilian state with the highest rate of scorpionism, 223,033 cases were recorded between 2010-2017, so reflections on how to reduce this public health problem are necessary. Objective: to describe the epidemiological profile of accidents with scorpions that occurred in the municipality of Patos de Minas (MG), from 2013 to 2017, and to develop an intervention project based on it. Methods: First, the number of accidents by venomous animals in Patos de Minas (MG) was analyzed using the variables included in the SINAN notification and investigation form, available on DATASUS. Additionally, two databases, CAFE and Pubmed, were used for the theoretical framework, with the following descriptors: "escorpião", "Minas Gerais", "envenenamento", "prevenção", and their respective translations into English: "scorpion", "Minas Gerais", "envenomation" and "prevention". Results: Scorpion sting accidents were the most recorded among venomous animals, and were increased annually during the study period, reaching, in 2017, the number of 274 reported cases. In addition, the frequency of accidents was recorded in greater numbers in males (51.18%) and the most affected age group was between 20 and 39 years of age (32.64%). It is also observed that most of the victims were classified as mild cases and treated in the first hour of the sting (97.92%), with no deaths in the period. The results found are in agreement with the references used, being more common, in studies, the registration of mild cases, in male adults. For the basis of the intervention proposal, five articles were used, and based on them, preventive, corrective environmental and educational actions are proposed. Conclusion: The study allowed to identify the target audience of the intervention proposals and when they should be intensified, to try to contain, thus, the constant increase in cases of scorpionism in the analyzed municipality (AU)
Introdução: Minas Gerais é o estado brasileiro com a maior taxa de escorpionismo, foram registrados 223.033 casos entre 2010-2017, dessa forma se faz necessária reflexões sobre como reduzir esse problema de saúde pública. Objetivo: O presente estudo visa descrever o perfil epidemiológico de acidentes oriundos do escorpionismo ocorridos no município de Patos de Minas (MG) no período de 2013 a 2017 e desenvolver um projeto de intervenção a partir do mesmo. Métodos: Primeiramente, o número de acidentes por animais peçonhentos em Patos de Minas (MG) foi analisado utilizando-se as variáveis que contemplam a ficha de notificação e investigação do SINAN, disponíveis no DATASUS. Além disso, foram utilizadas duas bases de dados, CAFE e PUBMED, para o referencial teórico, com os seguintes descritores: escorpião, Minas Gerais, envenenamento e prevenção e suas respectivas traduções para o inglês. Resultados: Acidentes por picada de escorpião foram os mais registrados dentre os animais peçonhentos, além de terem aumentado anualmente no período de estudo, atingindo, em 2017, o número de 274 casos notificados. Ademais, a frequência dos acidentes foi registrada em maior número no sexo masculino (51,18%) e a faixa etária mais atingida foi entre 20 a 39 anos (32,64%). Observa-se também que os acidentados foram classificados como casos leves e atendidos na primeira hora da picada (97,92%), com nenhum óbito no período. Os resultados encontrados estão em concordância com as referências utilizadas, sendo mais comum, em estudos, o registro de casos leves, em adultos, do sexo masculino. Para o embasamento da proposta de intervenção, foram utilizados cinco artigos e baseado neles propõem-se o desenvolvimento de ações preventivas, corretivas do ambiente e educacionais. Conclusão:Com o estudo, foi possível identificar o público-alvo das propostas de intervenção e quando elas deveriam ser intensificadas, para tentar conter, assim, o aumento constante dos casos de escorpionismo no município analisado (AU)

Association between local anesthestic resistance and scorpion sting have been reported in literatures. Scorpion venom is known to affect the sodium channels in the nerve fibres and local anesthetics also acts on the same. But we report a case of successful spinal block in 23 years old second gravida posted for elective caesarean section with past history of failed spinal block and multiple scorpion sting 7 years and 2 years back. Last exposure was 4 months before previous caesarean section, thus failed spinal block may be due to recent exposure. Our conclusion is that, there is a direct relationship between duration of exposure to scorpion venom and susceptibility to local anesthetics.

Abstract Accidents caused by scorpions represent a major health problem on planet, both in individual terms and in the public health sphere. Morbid events can occur in different manifestations, and different clinical conditions must be differentiated for adequate treatment. In addition to the potential injury to human hosts, scorpion venom has been investigated for the therapy of several illnesses. Based on these aspects, the present article aims to present the clinical, diagnostic, therapeutic and prophylactic aspects of scorpionism.
Resumen Los accidentes causados por escorpiones representan un problema mayor de salud pública tanto en términos de daños individuales como en la esfera de la salud pública. Los eventos mórbidos pueden ocurrir con diferentes manifestaciones y condiciones clínicas que deben ser diferenciadas para un manejo adecuado. Ade más del daño potencial que ocasionan a los humanos, el veneno del escorpión ha sido estudiado con el fin de analizar su potencia aplicación para le tratamiento de algunas enfermedades. Este articulo presenta una actualización sobre los aspectos clínicos, terapéuticos y profilácticos del escorpionismo.

Abstract Objectives: to analyze the epidemiological and clinical aspects of accidents caused by venomous animals in children under 15 years old. Methods: a cross-sectional study with an analytical component using secondary data from Centro de Informação e Assistência Toxicológica de Pernambuco (CIATox-PE), (Poison Center in Pernambuco)), in 2017 to 2019. Notifications of accidents caused were included in the studied age group and evaluated the characteristics of poisoning (animal classification, exposure zone, place and time of the occurrence and specific use of serum therapy), and of the patient (sociodemographic variables, clinical condition and evolution). The analysis performed in STATA® 13.1 presents frequency distribution tables and Pearson's chi-square for comparison. Results: of the 2678 notifications, 82,8% were scorpionism and 10, 8% snakebite. The age group of1 to 9 years old (70.5%) and being male (54.1%) were predominant. Most of the cases occurred in urban area (80.9%), in Recife (67.3%), inside the victim's residence (83.9%) and at night (47.3%). The majority (87.1%) were classified as 'mild severity', 10% received antivenom therapy and one died (by scorpionism). Two cases of snakebite in the workplace were registered. Conclusion: there was a high frequency of accidents caused in the urban area, which may be related to the lack of urban planning and sanitary education. The accidents caused among children in the household environment and the suspicion of child labor in the age group of 10 to 14 years old were also highlighted which favors the development and habits of the venomous animal.
Resumo Objetivos: analisar aspectos epidemiológicos e clínicos dos acidentes por animais peçonhentos em menores de 15 anos. Métodos: estudo transversal com componente analítico utilizando dados secundários do Centro de Informação e Assistência Toxicológica de Pernambuco (CIATox-PE), no período de 2017 a 2019. Foram incluídas as notificações dos acidentes na faixa etária estudada e avaliadas características do envenenamento (classe do animal, zona de exposição, local/turno da ocorrência e uso soroterapia específica) e do paciente (variáveis sociodemográficas, gravidade clínica e evolução). A análise realizada no STATA® 13.1 apresenta tabelas de distribuição de frequência e qui-quadrado de Pearson para comparação. Resultados: das 2678 notificações, 82,8% foram de escorpionismo e 10,8% de ofidismo. Predominaram na faixa etária de 1a 9 anos (70,5%) e sexo masculino (54,1%);ocorreram principalmente na zona urbana (80,9%),no Recife(67,3%), na residência da vítima (83,9%), durante horário noturno (47,3%). A maioria (87,1%) foi classificada como de gravidade leve, 10% receberam soroterapia e ocorreu um óbito (escorpionismo). Registraram-se dois casos de ofidismo em ambiente de trabalho. Conclusão: ressaltam-se os acidentes intradomiciliares entre crianças e a suspeita de trabalho infantil na faixa etária de 10 a 14anos.Os acidentes ocorreram sobretudo na zona urbana provavelmente associado à falta de planejamento e educação sanitária que favorece o desenvolvimento e hábitos do animal peçonhento.

El envenenamiento debido a picadora de escorpiones es reportado en diferentes países de Suramérica, en algunos, como México, es un problema de salud pública. En Colombia el accidente por escorpiones no es de reporte obligatorio, por lo que hay subregistro de casos. Los accidentes por este tipo de animales ponzoñosos se asocian a manifestaciones locales, y en casos de gravedad moderada a severa, a problemas cardiovasculares y neurológicos. Sin embargo, es importante reconocer manifestaciones que, aunque reportadas con menos frecuencia, implican gran morbilidad, como la pancreatitis y las alteraciones renales, entre las que está la nefritis. Se reporta el caso de un varón de 46 años, que presentó un accidente por Tytus, con nefritis intersticial. Este es el primer reporte de esta complicación en Colombia. Nuestro objetivo principal es resaltar la importancia de una pronta identificación de casos y un oportuno inicio del tratamiento por parte del personal de la salud, que permitan disminuir la morbilidad y mortalidad de estos pacientes.
Scorpion poisoning is reported in different countries in South America, especially in Mexico, where it has become a public health issue. In Colombia this type of accident is not mandatory to report, therefore, there is underreporting of cases. Accidents caused by these poisonous animals are mainly associated with local manifestations and, in cases of moderate to severe severity, to cardiovascular and neurological problems. Nevertheless, it is important to recognize other manifestations which, despite being reported less frequently, are associated with high morbidity such as pancreatitis and renal alterations, among which is nephritis. The case of a 46-year-old male patient is reported, who had an accident due to Tytus and presented interstitial nephritis. This is the first report of this complication in Colombia, and based on this, the main objective is to highlight the importance of prompt identification and treatment by health personnel, in order to reduce morbidity and mortality in these patients.
O envenenamento devido a picadora de escorpiões é reportado em diferentes países da América Latina, em alguns, como México, é um problema de saúde pública. Na Colômbiao acidente por escorpiões não é de reporte obrigatório, porque há sub-registro de casos. Os acidentes por este tipo de animais peçonhentos se associam a manifestações locais, e em casos de gravidade moderada a severa, a problemas cardiovasculares e neurológicos. Entretanto, é importante reconhecer manifestações que, embora reportadas com menos frequência, implicam grande morbilidade, como a pancreatite e as alterações renais, entre as que está a nefrite. Se reporta o caso de um homem de 46 anos, que apresentou um acidente por Tytus, com nefrite intersticial. Este é o primeiro reporte desta complicação na Colômbia. Nosso objetivo principal é ressaltar a importância de uma breve identificação de casos e um oportuno início do tratamento por parte do pessoal da saúde, que permitam diminuir a morbilidade e mortalidade destes pacientes.

Scorpion sting can present with multiple clinical presentations ranging from local swelling to cardiogenic shock. Scorpion sting affects many organ which can cause myocarditis, cardiogenic shock, acute renal dysfunction and respiratory collapse. Central nervous system involvement can cause encephalopathy, seizures, subarachnoid haemorrhage, and ischemic stroke. Cerebral bleed is a rare presentation of scorpion sting.Here we present a case of scorpion sting with unusual central nervous system (CNS) presentation of gangliocapsular bleed.

In this manner, the research aimed to analyze the occurrence of scorpionic accidents in a municipality in the northeastern of Brazil. An exploratory, descriptive study was made, with a quantitative approach, using secondary data which was gotten from the Notifiable Diseases Information System (SINAN), from 2008 to 2018. Data such as neighborhood, presence of street markets were also used, and the existence of sanitation and climatic data such as temperature and season. Geoprocessing was used to identify possible changes in the environment. In the analyzed period, 9,330 cases of scorpion accidents were recorded, with an average of 848 annual notifications. Scorpionic accidents occurred more frequently in women (5,686; 60.94%). Individuals aged 20 to 29 years (1.727; 18.51%) were more frequent to scorpion stings. Regarding the body parts where the stings were made, the highlights were on the foot (3.515; 37.67%) followed by the hand (2.818; 30.20%). No statistically significant relation was observed between climatic factors and scorpionic accidents. However, the high number of cases of scorpionic accidents was observed in the last 11 years studied. It was evident that during the study period there was no statistical relationship when climatic factors were correlated to scorpionic accidents. On its turn, when it was verified the results of the geoprocessing analysis, it was seen that anthropic factors have been motivating the potentiation of the occurrence of these accidents.
