ObjectiveBased on a demand survey， to put forward the idea of family doctor service health service packages for people in functional communities， and provide suggestions for the implementation of family doctor health service in such communities. MethodsThrough the stratified cluster sampling survey of a science and innovation bearing functional community， combined with literature research， current situation survey， case analysis and interviews， this paper proposes the service content of the service packages. ResultsOn the basis of demand survey， the mode of "specified action + optional action" should be adopted. Community health service center should first provide basic service， and then expand their services in diagnosis， treatment， and health management in line with local demand， and provide optional multi-level health service packages for scientific and innovative functional community residents， which should be divided into basic health service， value-added health service and high-end health service. ConclusionSorting out the content of health service packages of family doctor service in functional communities can provide the basis for further improving the allocation of medical service resources， further optimizing the design of the financing and compensation mechanism， and further standardizing the contract service of family doctors.

Objective:To investigate the health resources utilization efficiency from the perspective of the end-of-life medical costs in advanced cancer patients,estimate the method of calculating the price range for medical service package in community health service center.Methods:Taking the medical cost of cancer patients in final year of the end of life in Hubei under the dual-referal policy of high leveled hospitals and community medical institutions,it estimated the pricing range for palliative care medical package and designed the medical payment policy for hierarchy diagnosis.Results:With the new medical service package policy,maintained the same level of medical service quantity,the total medical cost would save a great amount.Conclusion:Based on the accurate budget calculation of end-of-life costs,it had great significance for the implementation of classified diagnosis policy,rationally design the link and pricing range between hospitals and community medical service package.

Objective To construct a home-care service package for the urban disabled elderly that satisfy their demands and improve the efficiency of service. Methods From August to October, 2014, a total of 285 disabled (mild, medium and severe) elderly from a cluster sampling were investigated with the self-designed questionnaire about home-care demands among them. An initiatory service package was drawn to meet the demands reported by 40% of them or above. The package was consulted by a group of 15 experts using Delphi method. Results The final service package included 11, 19, 20 items of care for the mild, medium and severe disabled elderly, respectively. Conclu-sion The disabled elderly home-care package in Zhengzhou has been created based on the demands among disabled elderly.

To achieve the goal of universal healthcare coverage,and the objective of the ongoing healthcare reform to establish an essential healthcare system,the study proposed a financial framework for building the essential medical service package,covering medical services offered by primary medical institutions,treatment of major diseases,and essential medical services offered by secondary and tertiary hospitals.With data over the years of the total medical expense and medical service usage as the basis,and in the principles of affordability and cost-effectiveness,the total financing quota of essential medical services is expected to reach 1 940.846-2 1 62.41 7 billion,accounting for 30.66%-34.1 6% of the total healthcare expenditure.75% of the financing load should be carried by the government and society, focusing on financing medical services offered by primary institutions and lowering out-of-pocket burden of residents.

Objectives:To formulate the specific basic medical services package that adapts to China’s health-care situations. Methods:With the optional items which are covered by medical insurance and the medical services price regulation in 2012 , the 2 phase and 5 steps experts consultation methods were applied according to the princi-ples of meeting characteristics and the floor level of basic medical services. Results:1343 items are included in the basic medical service package and finally 713 items in the supplementary package. Discussion and suggestion: The screening strategy combining medical insurance system ensures the chosen items’ economical efficiency. The basic medical service providers are not just restricted in primary medical institutions. The government responsibility should be highlighted and the basic medical insurance system should be developed to be the main financial subsidy re-sources.
