RESUMO

Neoplastic lipomatous lesions of the salivary glands constitute ?0.5% of all the salivary gland tumors. Oncocytic sialolipoma of the parotid glands is extremely uncommon. We report a case of oncocytic sialolipoma of the parotid gland in a 59-year-old male who presented with a gradually increasing swelling of the right parotid. Excisional parotid biopsy performed in view of possible pleomorphic adenoma as suggested on ultrasonography showed histological features consistent with oncocytic sialolipoma. We also described the characteristics of 24 previously reported cases of oncocytic sialolipoma of the parotid gland. The median age of the patients including the present case was 56 years (range 7–89), and 14 were male. The largest and the least reported sizes of the tumor were 7.0 and 1.4 cm, respectively. The left-sided parotid gland was more commonly involved (14/23). Despite its rarity, oncocytic sialolipoma should be considered in lipomatous parotid lesions showing epithelial components with oncocytic changes.

RESUMO

This report describes a 14-year-old Pit Bull dog presenting with a soft tissue swelling of 3-month progression in the right sublingual region. Histopathological analysis of the surgically resected specimen revealed large numbers of mature adipocytes and islets consisting of mucin-containing atrophic acini and dilated ducts surrounded by a thin fibrous capsule. Findings were consistent with sialolipoma of the sublingual salivary gland. To the authors' knowledge, this is the first case of sialolipoma affecting the sublingual salivary gland in dogs.(AU)


O presente relato descreve um cão Pit Bull, de 14 anos, com aumento de volume de consistência macia em região sublingual direita com evolução de 3 meses. Após excisão cirúrgica, a análise histopatológica revelou grande número de adipócitos maduros, tecido glandular composto por ductos dilatados e ácinos atróficos contendo mucina, circundados por fina cápsula fibrosa, achados compatíveis com sialolipoma da glândula salivar sublingual. Pelo conhecimento dos autores, este é o primeiro relato de sialolipoma acometendo glândula salivar sublingual em cão.(AU)

RESUMO

Sialolipoma is a rare tumour found within both major and minor salivary glands. A total of 35 sialolipoma cases have been reported 18 within major salivary gland and 17 within minor salivary gland. Major gland sialolipoma most often are presented in the parotid gland (77%) and those from minor salivary glands were often seen in the palate (41%). All lesions were well circumscribed and contained mature adipose tissue admixed with benign salivary gland component. Treatment is surgical excision. We report a case of 52 years old male who presented with the complaint of swelling in the left parotid region for 22 years duration. Apart from cosmetic reason he has no other complain. All specific investigations done were inconclusive and suggested a benign tumor. Histopathological examination after superficial parotidectomy suggested sialolipoma.

Sialolipoma, a rare tumor of the salivary gland, is a recently described variant of salivary gland lipoma. Oncocytic sialolipoma was first described by Pusiol et al. in 2009. We report the case of an oncocytic sialolipoma of the submandibular gland in a 43-year-old female. Excision of the tumor was performed with preservation of the submandibular gland. The tumor had a thin, fibrous capsule and consisted of abundant adipose tissue, an oncocytic nodule, and scattered normal glandular structures surrounded by adipose tissue. Four cases of sialolipoma of the submandibular gland, including the present case, were reviewed. All 4 tumors were developed on the right submandibular glands, with a composition of adipose tissue as high as that of sialolipoma of the parotid gland; in contrast to previous reports, three cases were in females. As newly described tumor type, care should be taken to distinguish oncocytic sialolipoma from other salivary gland neoplasms such as simple lipoma, pleomorphic adenoma, or oncocytoma.

El sialolipoma es una variante de un lipoma de glándulas salivales que fue descrito por vez primera en el 2001 por Nagao; actualmente se tienen publicados sólo 18 casos en la literatura, siendo la mayoría de ellos en la glándula parótida. El caso que se presenta es de un paciente masculino de 57 años de edad que presenta un aumento de volumen en la región de labio inferior de lado izquierdo de 5 años de evolución, tratado con una biopsia excisional, la cual como resultado histopatológico nos arroja un diagnóstico de sialolipoma.


Sialolipoma is a salivary gland lipoma variant first described in 2001 by Ngao. Presently, there are only 18 documented cases in scientific literature. Most of them were located in the parotid gland. The case here presented is that of a 57 year old male patient, presenting volume increase in the region of the left side of the lower lip. The lesion presented a 5 year evolution. It was treated with excision biopsy. Histopathological results rendered a sialolipoma diagnosis.

Case Report: We report the case of an oncocytic sialolipoma of the submandibular gland with sebaceous differentiation in a 73-year-old man. The initial symptom was a right submandibular painless mass. Ultrasonography showed a hypoechoic oval mass posterior to the submandibular gland. The tumorectomy was performed with preservation of the salivary gland. The tumor was composed of mature adipose tissue surrounded by a thin fibrous capsule, multiple nodules of oncocytes, normal ductal-acinar units with focal ductal sebaceous differentiation. Discussion: We reviewed literature of the reported cases of mixed tumors of the salivary glands composed of mature adipose tissue with oncocytosis, salivary ducts, and acini with sebaceous differentiation. Conclusions: Sialolipoma and lipoadenoma with or without oncocytosis and/or sebaceous differentiation should be considered organ-specific tumors with a distinct histological appearance and specific terminology.

Sialolipoma is a new variant of salivary gland lipoma containing secondary entrapment of acini and ducts of normal salivary gland, first described by Nagao et al. in 2001. We present the clinicopathologic features of two cases, with a review of 18 cases of sialolipoma reported in the literature so far. In our cases, both 59-year-old and 74-year-old men had painless, movable, and soft mass. The finding of neck enhanced computed tomography was a well-demarcated hypodense mass within the superficial lobe of left parotid gland in both cases. So we performed a superficial parotidectomy with preservation of the facial nerve. Grossly, this tumor was a well-demarcating, soft, greasy, yellowish mass, and microscopically, it consisted of mature adipose tissues and normal entrapped glandular epithelium of acini and ducts. This had been recently recognized as a separate entity under the term, "sialolipoma". We report two cases with a review of the literature.
