RESUMO

Objective To investigate the prevalence of healthcare-associated infection(HAI)in a children's spe-cialty hospital in Guangzhou in 2014.Methods A cross-sectional survey was undertaken to investigate the preva-lence of HAI among all hospitalized patients on September 17,2014.Results A total of 997 patients were investiga-ted,30 patients developed 32 times of HAI,HAI rate and HAI case rate were 3.01% and 3.21% respectively. The main infection sites were upper respiratory tract(n= 11,34.38% );the departments with higher HAI prevalence rates were intensive care unit(ICU,3 .99% )and pediatric internal medicine department(3 .60% );the rate of etio-logical examination in patients with HAI accounted for 93 .33% ,a total of 24 strains of pathogens were isolated,6 of which were virus (25 . 00% ),6 were fungi (25 . 00% ),11 were bacteria (45 . 83% ),and 1 was Chlamydia (4.17% ). The main bacteria were Staphylococcusaureus (n= 3,27.27% )and Staphylococcusepidermidis (n= 3, 27.27% );a total of 451 (45.24% )patients received antimicrobial agents on the investigation day,67.41% of whom received therapeutic use of antimicrobial agents,19.73% received prophylactic use,and 12.86% received both therapeutic and prophylactic use;mono-drug application accounted for 82 .26% ,the percentage of bacterial de-tection among patients with therapeutic antimicrobial use (including combination of therapeutic and prophylactic use)was 82.60% .Conclusion Management of key departments and key sites of HAI should be strengthened in children's specialty hospital,antimicrobial agents should be used rationally,so as to protect the safety of children.

RESUMO

PURPOSE: Aim of this study is to investigate the characteristics and performance of colorectal-anal specialty vs. general hospitals for South Korean inpatients with colorectal-anal diseases, and assesses the short-term designation effect of the government's specialty hospital. MATERIALS AND METHODS: Nationwide all colorectal-anal disease inpatient claims (n=292158) for 2010-2012 were used to investigate length of stay and inpatient charges for surgical and medical procedures in specialty vs. general hospitals. The patients' claim data were matched to hospital data, and multi-level linear mixed models to account for clustering of patients within hospitals were performed. RESULTS: Inpatient charges at colorectal-anal specialty hospitals were 27% greater per case and 92% greater per day than those at small general hospitals, but the average length of stay was 49% shorter. Colorectal-anal specialty hospitals had shorter length of stay and a higher inpatient charges per day for both surgical and medical procedures, but per case charges were not significantly different. A "specialty" designation effect also found that the colorectal-anal specialty hospitals may have consciously attempted to reduce their length of stay and inpatient charges. Both hospital and patient level factors had significant roles in determining length of stay and inpatient charges. CONCLUSION: Colorectal-anal specialty hospitals have shorter length of stay and higher inpatient charges per day than small general hospitals. A "specialty" designation by government influence performance and healthcare spending of hospitals as well. In order to maintain prosperous specialty hospital system, investigation into additional factors that affect performance, such as quality of care and patient satisfaction should be carried out.

Peking University Hospital of Stomatology plays an important role in the advancement of oral science and oral health in China.Management for scientific research is an imperative part of hospital management in an effort to maintain and improve hospital's core competence in scientific research.Hence,the management system should be continuously strengthened in a systematic manner to create a people-orientated and positive working environment.
