Effect of Dahuang Xiezhuo Prescription on Renal Tubular Function in Patients with IgA Nephropathy Complicated with Hyperuricemia / 中国实验方剂学杂志

ObjectiveTo observe the effect of Dahuang Xiezhuo prescription on the clinical symptoms， blood uric acid， and renal tubular function of patients with immunoglobulin A （IgA） nephropathy in stages 1-2 of chronic kidney disease （CKD） complicated with hyperuricemia （HUA）. MethodSixty patients with IgA nephropathy in stages 1-2 of CKD complicated with HUA of spleen and kidney deficiency and combined turbidity and blood stasis syndromes were randomly divided into an observation group and a control group， with 30 cases in each group. The patients in the control group received basic treatment， i.e.， losartan potassium tablets 50-100 mg/time， once per day， and sodium bicarbonate tablets 0.5 g/time， three times per day by oral administration， combined with low-salt， low-fat， and low-purine diet. The patients in the observation group received Dahuang Xiezhuo prescription on the basis of basic treatment， one dose per day， twice a day in the morning and evening with warm water. Both groups were treated for two months. The total scores of traditional Chinese medicine（TCM）syndrome， blood pressure， 24 h urinary protein （24 h UTP）， blood urea nitrogen （BUN）， serum creatinine （SCr） ［glomerular filtration rate （eGFR） was calculated by CKD-epidemiology collaboration （CKD-EPI） formula］， serum uric acid （SUA）， and renal tubular function indexes ［urinary α1-microglobulin （α1-MG）， urinary β2-microglobulin （β2-MG）， urinary kidney injury molecule-1 （KIM-1）， and neutrophil gelatinase-associated lipocalin （NGAL）］ of the two groups before treatment and two months after treatment were recorded. The clinical efficacy of the two groups was evaluated two months after treatment. ResultAfter 2 months of treatment，the total effective rate in the observation group was 81.48%（22/27），higher than 50.00%（14/28） in the control group（χ2 =6.661，P<0.05）. The total scores of TCM syndrome， 24 h UTP， and SUA in the observation group and the observation group were lower than those before treatment （P<0.05）， and compared with the control group after treatment， the observation group decreased more significantly （P<0.05）. After treatment， the blood pressure in the observation group and the observation group was lower than that before treatment （P<0.05）， and there was no significant difference in blood pressure between the two groups after treatment. After treatment， the levels of urinary α1-MG， β2-MG， KIM-1， and NGAL in the two groups were lower than those before treatment （P<0.05）， and the observation group was lower than the control group after treatment （P<0.05）. There were no significant inter-group and intra-group differences in BUN， SCr， and eGFR levels before and after treatment. There were no obvious abnormalities in blood routine， liver function， and electrolytes before and after treatment in the two groups， and no adverse reactions such as allergies occurred. ConclusionDahuang Xiezhuo prescription can effectively improve the clinical symptoms of IgA nephropathy with HUA （CKD1-2） patients with spleen and kidney deficiency and combined turbidity and blood stasis syndromes， reduce blood uric acid level， alleviate renal tubular injury， and protect the kidney. The curative effect is better than that of basic treatment.

Effect of Bushen Kangshuai Tablets on Chronic Fatigue Syndrome with Syndrome of Spleen and Kidney Deficiency / 中国实验方剂学杂志

Objective: To observe the effect of Bushen Kangshuai tablets combined with abdominal massage on chronic fatigue syndrome (CFS) with syndrome of spleen and kidney deficiency, and investigate its antioxidant stress and immune regulation effects. Method: One hundred and sixty patients were randomly divided into control group and observation group by random number table, 80 cases in each group. Patients in control group got abdominal massage, 1 time/day, 5 times/week, for a total of 3 treatment courses, 15 times in one course. Based on the treatment in control group, patients in observation group also received Bushen Kangshuai tablets, 6 tablets/time, 3 times/days, for a total of 12 weeks in treatment. Before and after treatment, symptom scores of spleen and kidney deficiency, fatigue scale-14 (SF-14), self-rating anxiety scale (SAS), self-rating depression scale (SDS) and Pittsburgh sleep quality index (PSQI) were graded; and levels of serum immunoglobulin (IgG, IgA and IgM), natural killer cells (NK), T lymphocyte subsets (CD3+, CD4+, CD8+ and CD4+/CD8+), malondialdehyde (MDA), superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) were detected. Result: In the rank sum test, the clinical efficacy in observation group was better than that in control group (Z=1.982, PPPP+, CD4+, CD4+/CD8+ were also higher then those in control group (PP+ (PPConclusion: Bushen Kangshuai tablets combined with abdominal massage to CFS with syndrome of spleen and kidney deficiency can significantly relieve fatigue, improve spleen and kidney deficiency, and improve immune function and antioxidant stress, superior to abdominal massage therapy alone in the clinical Efficacy.

Research Progress in Relationship Between Treating Steroid-induced Osteonecrosis of Femoral Head by Deficiency and OPG/RANK/RANKL / 中国中医药信息杂志

Recent studies have shown that TCM treatment for bone metabolic diseases, such as steroid-induced avascular necrosis of the femoral head, has definite efficacy and has received extensive attention. However, due to lack of molecular biological basis of pharmacodynamics mechanism, it has not yet reached the standard of scientific treatment. The discovery of OPG/RANK/RANKL system has become a new breakthrough point in the prevention and treatment of bone metabolic diseases. The pathogenesis of deficiency of spleen and kidney - blood stasis - caused by phlegm arthralgia caused by blood stasis is closely related to the system. The effect of the final control of the system from the deficiency theory can be expressed through the axial micro-information. This article discussed the TCM syndrome differentiation of steroid-induced osteonecrosis of the femoral head and the relation of regulation of bone metabolism combined with the OPG/RANK/RANKL system, and provided the basis for prevention and treatment of this disease.

Clinical Efficacy of Yiqi Huoxue Decoction in the Treatment of Children with Spleen and Kidney Deficiency Type Nephrotic Syndrome / 现代生物医学进展

Objective:To study the clinical efficacy ofYiqi Huoxue Decoction in the treatment of children with spleen and kidney deficiency type nephrotic syndrome.Methods:A total of 80 children with spleen and kidney deficiency type nephrotic syndrome in our hospital from June 2014 to October 2016 were enrolled in this study.The subjects were divided into control group (n=40) and treatment group (n=40) according to the random number table method.The control group was treated with prednisone,and the treatment group was treated with prednisone combined with Yiqi Huoxue Decoction,the two groups were treated for 4 months.The clinical efficacy of the two groups were evaluate.The 24h proteinuria,total cholesterol (TC),plasma albumin (Alb),and renal function parameters including blood urea nitrogen (BUN),serum creatinine (Scr),and serum creatinine clearance (Ccr) of the two groups before and after treatment were compared.Results:The total effective rate of the treatment group was 95.00％,which was significantly higher than 67.50％ of the control group,the difference was statistically significant (P＜0.05).The 24h proteinuria,TC,BUN and Scr of the two groups after treatment were significantly lower than before treatment,the Alb,Ccr of the two groups were significantly higher than before treatment,and above indexes in treatment group were better than the control group,the differences were statistically significant (P＜0.05).Conclusion:Yiqi Huoxue Decoction have good clinical efficacy in the treatment of children with spleen and kidney deficiency type nephrotic syndrome,which can obviously improve children renal function,and is worthy of clinical application.

Qiu Changlin's Experience of Syndrome Differentiation and Treatment on Pregnancy Complicating Myasthenia Gravis / 浙江中医药大学学报

Objective] To conclude the teacher Qiu's clinical experience and academic points on diagnosing and treating pregnancy with myasthenia gravis(MG). [Method] By reviewing relevant literature of traditional Chinese medicine, studying in clinic with teacher and tidying medical records, systematic mining and constructing teacher Qiu's clinical experience and academic points on etiology and pathogenesis, syndrome differentiation, medication thinking, drug characteristics, the taboo of drugs and combining traditional Chinese and western medicines treatment of pregnancy with myasthenia gravis(MG).[Results]Teacher Qiu thinks, pregnancy with MG involving spleen and kidney deficiency is the pathological process of pregnancy and physio-logical process of MG; Clinical syndrome differentiations can be divided into three types like temper deficiency, spleen and kidney deficiency, qi and Yin deficiency; establish the therapeutic rules of nourishing qi to invigorate spleen, tonifying the spleen and kidney and tonifying qi and yin. Respectively adding and subtracting Buzhong Yiqi Decoction with Taishan Panshi San, Buzhong Yiqi Decoction with aconitum lizhong pill and Taishan Panshi San, Buzhong Yiqi Decoction with sengmai san and Taishan Panshi San for syndrome differentiation treating the disease, nourishing qi to invigorate spleen and fetal irritability are the main treatments of the disease. Buzhong Yiqi Decoction and Taishan Panshi San are the basic prescriptions. Large dose of the astragalus membranaceus, atractylodes macrocephala koidz, Codonopsis pilosula,epimedium gradiflorum morr to tonify kidney strengthening spleen and supporting Qi in the prescription, combining traditional Chinese medicine with western medicine can reduce the use of western medicine and reduce the adverse reaction of western medicine. [Conclusion]The teacher Qiu's experience of syndrome differentiation and treatment on pregnancy complicating myasthenia gravis is rich, efficacy is got and has less side effect. It states that Chinese medicine treatment of pregnancy with MG has certain advantages and is worth being popularized and inheriting.

Clinical observation of Yishen-Jianpi-Huoxue Decoction on the treatment of adolescent patients with polycystic ovary syndrome / 国际中医中药杂志

Objective To observe the clinical therapeutic effects of Yishen-Jianpi-Huoxue Decoction on the adolescent patients with Polycystic Ovary Syndrome (PCOS).Methods 42 patients diagnosed of PCOS of spleen and kidney deficiency syndrome were recruited in to traditional Chinese medicine group and treated with Yishen-Jianpi-Huoxue Decoction,and 35 patients of PCOS were recruited into control group and guided with the healthy lifestyle.The course of the treatment was 3 months.The menstrual cycle,body mass index (BMI),F-G score,GAGS score,basal body temperature (BBT),and endocrine related inspection were analyzed before and after the treatment.Results The clinical symptoms and related laboratory tests were improved in both groups after the treatment,but the effects of traditional Chinese medicine group was much better than the control group in BMI(23.11+3.64),GAGS score(7.33±7.06),FINS(14.73+8.97) mmol/L,and HOMA-IR (3.13 ±1.94),P＜0.05 or ＜0.01.Conclusion Yishen-Jianpi-Huoxue Decoction can improve the clinical symptoms and endocrine condition of edolescent PCOS patients.
