RESUMO

Obesity is a chronic, recurrent, and progressive metabolic disorder characterized by the abnormal or excessive accumulation of body fat caused by multiple factors such as genetics, dietary structure, lifestyle and behavior, psychology, environment, and society, leading to an energy surplus. Obesity is a major risk factor that increases the risk of developing various chronic diseases, including type 2 diabetes, hypertension, cardiovascular diseases, stroke, and certain malignancies. The global incidence of obesity is increasing year by year. With the continuous improvement of people's living standards, more than half of adults in China are now overweight or obese, posing a serious threat to people's health and increasing the social and economic burden. It has become a pressing major public health issue that needs to be addressed urgently. The concept of obesity can be traced back to the Huangdi’s Internal Classic (Huang Di Nei Jing), which describes it as "the problems in fat and affluent people are caused by excessive taking of rich food", and suggests that ''frequent intake of rich and greasy foods can produce interior heat. Sweet flavor causes chest fullness. That is why its spleen-Qi flows upwards and changes into consumption-thirst disease. It can be treated by Eupatorii Herba which is used to remove stagnant Qi''. The stagnant qi is caused by the transformation failure of rich and greasy food and wine, so obesity is the disease of stagnant qi. Obesity is caused by indulging in rich and greasy food, wine, spicy and flavorful foods, raw and cold foods, and sweet and greasy foods, or overeating and leading a sedentary lifestyle, staying up late, or experiencing emotional imbalances such as excessive joy, anger, worry, pensiveness, and fear. It can also be caused by congenital abnormalities, leading to improper functioning of the spleen and stomach, dysregulation of the absorption and secretion of the small intestine, and the accumulation of stagnant Qi in the organs and muscles, resulting in a plump physique. The intake of food and drink depends on the functions of the stomach in receiving and decomposing, the small intestine in absorbing and secreting, and the spleen in transforming and transporting. The affected organs in obesity are the spleen, stomach, and small intestine. Orchids, specifically Eupatorii Herba and Lycopi Herba, are aromatic herbs that can regulate the smooth flow of Qi, eliminate stagnation, and cleanse impurities. In a broader sense, any aromatic and pungent substance that can invigorate the spleen, promote clarity, harmonize the stomach, reduce turbidity, and assist in the normal secretion and absorption functions of the small intestine, thereby eliminating excess, is referred to as orchid. Therefore, the treatment principle for obesity is to use ''orchids to eliminate stagnant Qi'', aiming to regulate the functions of the spleen, stomach, and small intestine using aromatic and pungent substances, gradually eliminating excessive dampness, phlegm, turbidity, and heat, and restore the balance of the middle energizer. This way, individuals who are obese can achieve a non-obese state.

Objective:Data mining technology was used to analyze the regulation of food therapy prescriptions in treating children′s stagnation.Methods:Collect the therapy prescriptions used for regulating children's stagnation in the Dictionary of Traditional Chinese Medicine Prescriptions, the Complete Record of Dietary Therapy Prescriptions of Traditional Chinese Medicine and the Dictionary of Chinese Medicinal Diet, extract the information of prescription name, composition, etc, and use SPSS 22.0 for frequency analysis, and use Weka for correlation analysis. Results:A total of 99 dietary prescriptions for children with hysteria were included, involving a total of 62 foods, with a total use frequency of 224 times, among which the food with high use frequency were chicken gizzard, japonica rice, hawthorn, etc. The four characteristics of food were mainly concentrated in the flat, the five tastes were mainly concentrated in the sweet, the return channel was mainly concentrated in the spleen and stomach channel, and the effect was mainly concentrated in the absorption of food and tonic deficiency. The main symptoms of the therapeutic prescription for children's accumulation of stagnation were internal accumulation of milk and food and combination of spleen deficiency. The commonly used food combination for children's accumulation of stagnation of milk and food was "fructus amomi - chicken gizzard". The commonly used food combination of children with spleen deficiency and accumulation of stagnation was "lentil bean-yam-japonica rice" and "millet-yam".Conclusions:Traditional Chinese medicine diet prescription for the treatment of children's accumulation of stagnation pay attention to harmony and regulation, sweet and slow tonifying, emphasizing the adjustment of the spleen and stomach, taking into account the regulation of lung, following the "eliminating and supplementing both, according to the cause of treatment" rule, advocate syndrome differentiation of food.

Objective:To investigate the clinical efficacy of microacupotomy combined with Du pulse acupuncture in the treatment of lumbar and dorsal myofascial pain syndrome. Methods:110 patients with lumbar and dorsal myofascial pain syndrome who received treatment between February 2019 and August 2020 in Haining People's Hospital were included in this study. They were randomly assigned to receive either electroacupuncture (control group, n = 55) or microacupotomy combined with Du pulse acupuncture (observation group, n = 55). Therapeutic effects were compared between the control and observation groups. Results:Before treatment, there were no significant differences in tenderness and Visual Analogue Scale scores between the control and observation groups (both P > 0.05). After treatment, tenderness and VAS scores in the observation group were (0.81 ± 0.11) points and (2.36 ± 0.25) points, respectively, which were significantly lower than those in the control group [(1.31 ± 0.10) points, (3.34 ± 0.19) points, t = 24.943 and 23.146, both P < 0.001]. Effective rate in the observation group was significantly higher than that in the control group [98.18% (54/55) vs. 81.82% (45/55), χ2 = 8.182, P < 0.05]. After treatment, tenderness scores of the gluteus medius, psoas quadratus and multifidus muscles in each group were increased, and these scores in the observation group were significantly higher than those in the control group ( t = 3.937, 2.963 and 3.633, all P < 0.05). Conclusion:Microacupotomy combined with Du pulse acupuncture in the treatment of lumbar and dorsal myofascial pain syndrome can effectively decrease the degree of pain and strengthen clinical therapeutic effects.

Objective:To observe the clinical efficacy of Shenxie Zhitong capsule in the treatment of diabetic peripheral neuropathy(DPN) of stagnant blockade of collaterals, and evaluate its effectiveness and safety. Method:The 104 patients were randomly divided into the Shenxie Zhitong capsule treatment group (the treatment group, 53 patients) and the alpha lipoic acid group (control group, 51 patients), and two groups were compared by random and contrast test. The changes of the Toronto clinical scoring system (TCSS), utah early neuropathy scores (UENS), traditional Chinese medicine(TCM)syndrome scores, visual analysis scale (VAS), ankle brachial index (ABI), vibrating perception threshold (VPT) before and after treatment were compared between two groups, and the endpoint events, such as foot ulcers, percutaneous coronary intervention (PCI), death and composite endpoint events, related indicators of glucose and lipid metabolism and safety indicators were recorded among patients. Result:Compared with the data before treatment, the scores of TCSS, UENS, and TCM syndromes in two groups were significantly reduced (<italic>P</italic><0.01) after treatment, and VAS and glycosylated haemoglobin A1c (HbA1c) were significantly reduced (<italic>P</italic><0.05), during follow-up visit, the levels of right ABI,total cholesterol (TC), and low-density lipoprotein (LDL) in two groups were significantly reduced (<italic>P</italic><0.05), and high-density lipoprotein (HDL) level was significantly increased (<italic>P</italic><0.05). control group in control group, the 2-hour postprandial plasma glucose (2 h PG) and HbA1c levels were significantly increased (<italic>P</italic><0.05). Compared with control group, the VAS of the treatment group after treatment was significantly reduced (<italic>P</italic><0.05). After treatment and during follow-up visit, compared with control group, the 2 h PG levels of the right toe in the treatment group were significantly reduced (<italic>P</italic><0.05). There was no statistically significant difference in endpoint events and safety indicators between two groups, but the incidence trend of composite endpoint events in the treatment group was lower than that in control group. Conclusion:Shenxie Zhitong capsule has definite clinical curative effect in treating diabetic peripheral neuropathy, which is more safe and effective than alpha lipoic acid in improving pain symptoms.

We experienced a case of palmoplantar pustulosis in which the patient's dermatological symptoms im­proved after treatment with a combination of bofutsushosan and keishibukuryogan. The patient was a 42-year-old woman. She had been prescribed biotin and a steroid ointment, but her condition did not improve. At her first visit to our Kampo clinic, we observed impetigo, cracks, and scaling on both palms and plantar surfaces. We determined that the patient was of the poisoned organ (zodoku) and stagnant blood (oketsu) constitutions,as outlined in the Ikkando school of medical thought. We prescribed bofutsushosan and keishibukuryogan as fundamental treatment and then in a few days, the patient's dermatological symptoms resolved. The patient later developed hyperthyroidism, which caused her symptoms to worsen again, we therefore switched to shofusan and eppikajutsuto as local and symptomatic treatment, and then she once again improved. Soon after, we returned to keishibukuryogan and bofutsusho for her to maintain remission. Since poisoned organ and stagnant blood constitutions contribute greatly to the Kampo pathology of palmoplantar pustulosis, we believe bofutsushosan and keishibukuryogan are effective formulations for the treatment of this condition. As far as we searched, there were no papers that selected bofutsushosan or blood stasis agents as the main cure from the viewpoint of the poisoned organ constitution and the stagnant blood constitution. Therefore, this case was considered to be a valuable case.

Aim@#The aim of this study was to monitor the restricted water systems during the period of COVID-19 lockdown for the waterborne pathogen Legionella pneumophila. Selected water systems were monitored to evaluate the effect of stagnation in terms of Legionella colonization.@*Methodology and results@#A total of 160 water samples were taken from buildings which were temporarily shut down or had restricted usage between April-December 2020. None of the sampling points had Legionella positive history in the last three years. All water samples were tested using classic microbiological culture as the gold standard and rapid Vermicon ScanVIT Legionella test kit combined with epifluorescence microscopy. Sixteen (10%) out of 160 water samples were recorded as Legionella-positive by both methods, where none of them was positive before the COVID-19 lockdown. All positives were tested as L. pneumophila serogroup 1 with latex agglutination kit.@*Conclusion, significance and impact of study@#Colonization of Legionella in stagnant water systems occurs rapidly. Before reopening, routine monitoring in buildings is extremely important especially under restricted use or closure is in case. Flushing, disinfecting and testing are crucial for minimizing the health risks for the public health.

[Objective] To introduce the experience of Professor ZHANG Biao in the treatment of infantile dyspepsia. [Methods] Through the long-term study with Professor ZHANG, large amounts of medical records are collected and generalized, so that the experiences concerning infantile dyspepsia are summarized, when and how to choose activation, tonification or digestion depends on excess and deficiency,and proven cases are presented. [Results] It is thought that infantile dyspepsia mostly belongs to excess syndrome, or excess syndrome combined with deficiency syndrome, so the treatment is heiping digestion, tonifying spleen and harmonizing stomach. Professor ZHANG thinks that the cause of children's dyspepsia is the weakness in spleen and stomach eventually. It is characterized by excess in syndromes and deficiency in root causes. Its clinical manifestations are concurrence of excess and deficiency. As for the treatment, there's no need to wait for the pathogenic qi to disappear. The basic principle of treatment is always to activate the spleen, regulate qi and help digestion. Professor ZHANG raises that syndromes with more deficicency than excess are treated by activating spleen, which is complemented by tonification. That is to say, activation and tonification are combined, while helping digestion is considered, also. But as to syndromes with more excess than deficiency, the treatment will be activating spleen combined with helping digestion, also with regulating qi. He also cares for children 's regulating and giving reasonable advice. [Conclusion]According to the children's characteristics of physiology and pathology, we should pay more attention to excess and deficiency to choose better method, at the same time, proper feeding and teaching are also important.

Objective: To evaluate effect of the morphology of simulated S-shaped root canals with Reciproc or Mtwo instruments on root canal irrigation. Methods: A total of 40 simulated S-shaped resin blocks were randomly divided into 2 groups (n=20), which was prepared by Reciproc or Mtwo. Blue ink was injected with constant speed and volume into the canals using 30G lateral opening syringe. hTe distance from needle tip to apex and vapor lock length were measured. A type of 25# 0.04 gutta-percha point was used to stir and remove the vapor lock. hTe time for removal of the vapor lock was recorded. Another syringe was used to inject saline into the canals and to measure the length of stagnant water. Results: hTe distance from needle tip to apex in the Reciproc group was signiifcantly longer than that in the Mtwo group [(4.276 ± 0.221) mm vs (3.459 ± 0.205) mm,P<0.05]. The vapor lock length in the Reciproc group was signiifcantly longer than that in the Mtwo group [(4.472 ± 0.230) mm vs (3.668±0.217) mm,P<0.05]. hTe time to remove vapor lock was signiifcantly shorter in the Reciproc group than that in the Mtwo group [(10.13 ± 1.79) s vs (15.29 ± 2.15) s,P<0.05]. The length of stagnant water was also shorter in the Reciproc group than that in the Mtwo group[(1.351 ± 0.142) mm vs (2.245 ± 0.206) mm,P<0.05]. Conclusion: hTe morphology of S-shaped root canal affects the root canal irrigation. hTe effect of root canal irrigated by Reciproc is better than that by Mtwo.

Objective: To evaluate the efficacy and safety of Diterpene Ginkgolides Meglumine Injection (DGMI) in the treatment of recovered stroke with syndrome of stagnant phlegm blocking collaterals, especially the clinical efficacy in improving the function of language and movements. Methods: The clinical trial was carried out by the methods of stratification and r andomization (416 cases of patients with atherosclerotic thrombotic cerebral infarction (ATCI) were r andomly divided into experimental and control groups by the ratio of 3:1), blindness, and positive parallel control of Shuxuening Injection (SI), and multi-center clinical study. DGMI (25 mg, 5 mL/amp) or SI (5 mL/amp) was diluted into 250 mL physiological saline, iv drip, once daily for 14 d. The dropping speed must be controlled as 10-15 drops/min for the first infusion. Results: There was statistical difference (P=0.000 1) for the variations of inducing rate of the defect extent of nervous functions before and after the treatment in the experimental and control groups. The experimental group was superior to the control group. There was no statistical difference for the variation of scales of the patient living ability in the two groups (P>0.05). For the comprehensive efficacy of cerebral infarction, the total effective rates were 85.39% and 73.27% in the experimental and control groups, respectively, with statistical difference (P=0.0001). For the efficacy of syndrome of traditional Chinese medicine (TCM), the total effective rates were 62.99% and 40.59% in the experimental and control groups, respectively, with the statistical difference (P=0.0001). For the single indicator of nervous functions, such as arm movements, h and movements, extremity movements, walking, as well as the syndromes of TCM, such as upper limb disable, lower limb disable, deviated tongue, aphasia, abate or deficiency of feeling, dizziness, more and turbid phlegm, abnormal tongue and pulse manifestation, the experimental group was superior to the control group with statistified difference. Conclusion: It is safe and effective for DGMI in the treatment of the syndrome of stagnant phlegm blocking collaterals in convalescence of ATCI. Meanwhile, it is also manifested that DGMI has the certain superiorities in the fields, such as improving the total score of nervous function deficiency for the patients with stroke, the nervous function, the total score of TCM syndromes, including upper limb disable, deviated tongue, dizziness, more and turbid phlegm, white coat of tongue, abnormal pulse, etc.

Objective To investigate the effect of Yishen-Daotan decoction on TCM syndrome elements of infertility caused by kidney deficiency and phlegm stagnant type polycystic ovary syndrome.Methods 60 patients of kidney deficiency and phlegm stagnant type polycystic ovarian syndrome infertilities were administrated with modified Yishen-Daotan decoction for 3 months and the changes of TCM syndrome elements,menstrual cycle and serum sex hormone were compared before and after the treatment.Results ① TCM syndrome elements had a statistical changes after the treatment compared with those before the treatment(t=62.79,P<0.01).②menstrual cycle showed statistical difference after the treatment compared with that before the treatment,(x2=21.64,P<0.01).③Serum FSH,E2 were increased,while LH,LH/FSH and T were decreased after the treatment,showing a significant difference compared with those before the treatment (t=2.67,P<0.05).④The total effective rate was 91.67% and pregnancy rate was 30%.Conclusion Yishen-Daotan decoction significantly improve clinical symptoms in patients with kidney deficiency and phlegm stagnant type PCOS infertility.

Naegleria fowleri a free-living amoeba commonly found in moist soil and fresh water, enters the body via the nasal mucosa and migrates along the olfactory nerve to the brain, where it causes fulminant meningoencephalitis that is generally fatal. From 1988 to 1992, a survey of pathogenic Naegleria sp. was carried out in the industrial areas of three provinces. Water samples from warm and fresh ponds were collected from Pathumthani (34 samples), Samutprakan (150 samples) and Lopburi (162 samples). AII specimens were cultured in non-nutrient agar Escherichia coli media (NNE) AT 37oC. The amoebae obtained from water samples were in amoeboid, flagellate and cyst form. The amoeboflagellate transformation was occurred in distilled water and amoebae saline. The amoebae from water samples in Pathumthain (70.59 per cent) and Samutprakan (69.30 per cent) were cultured in Chang’s axenic media (SCGYEM) at 37oC. Non-pathogenic and pathogenic Naeglenia sp were distinguished by the characteristics of growth properties since the pathogenic form can grow only in the SCGYEM axenic media. The percentage of amoebae from Pathumthani and Samutprakan in SCGYEM axenic media at 37oC. were 11.46% and 12.00% respectively. The amoebae from Lopburi cultured in NNE 45oC (10.49 per cent) was inoculated through nasal cavity of Rattus rattus. The result was 10.49 per cent of the amoebae also determined for the virulence and the rats were died in 5-7 days. The histological studies of mouse brain tissue sections revealed the large central karysome and halo around the nucleus of the organisms, the amount of amoebae were accumulated around perivascular space. The characteristics, pathogenicity and pathology of pathogenic Naegleria sp. are the same as Naegleria fowleri.
