RESUMO

Objective To design an assisted patient conveying and vibration damping system to solve the problems of operator fatigue and patient bump during casualty evacuation.Methods The assisted patient conveying and vibration damping system was composed of several conveying straps and a vibration damping mechanism.The conveying straps were made up of a waist strap,two shoulder straps,a chest strap,adhesive straps and joint components,and the joint components included adjusting buckles,big buckles,small buckles,connecting buckles and hook mechanisms;the vibration damping mechanism adopted the technical form of extension handle combined with vibration absorber,in which the extension handle was made of rigid material and the vibration absorber was equipped with a scissor guiding mechanism.Tests were carried out on the system to record the operating time of the operators and to analyze the system's vibration damping characteristics.Results The system developed extended the operating time of the stretcher conveyers while reduced the vibration during casualty transport,with a maximum vibration reduction of 71.73%.Conclusion The system developed gains advantages in low vibration and low workload,and can be used for casualty conveying in poor road conditions.[Chinese Medical Equipment Journal,2024,45(1):15-24]

BACKGROUND: Calf muscle tightness is a very common occurring condition in athlete. Stretching is form of physical exercise in which a specific muscle or tendon (muscle group) is deliberately flexed or stretched in order to improve the muscle's elasticity and achieve comfortable muscle tone. Flexibility helps with injury prevention, reduction of soreness following a workout, and a general sense of well-being. There are different stretching technique and protocol for improvement in calf extensibility and flexibility. The purpose of this study was to investigate the effectiveness of two techniques PNF and SELF STRAP STRETCHING for improving calf flexibility among collegiate athletes. MATERIALS AND METHOD: The selected 30 college athletes were randomly allocated to 2 groups equally. Group1 (n= 15) were given PNF and Group2 (n=15) were given self-strap stretching technique. Calf flexibility for each group was measured using goniometer and knee to wall test.Treatment was given for four days a week for 4 weeks. RESULT: The collected values were analyzed using paired 't' test and independent 't' test. The result showed an increase in the values of goniometric measurement and knee to wall test of the PNF technique (group A) compared to baseline value of self-strap stretching (group B) 16.4 and 10.88 at the end of 4 weeks. CONCLUSION: This study concluded that PNF technique is found to bemore effective in improving the calf muscle among collegiate athletes.

OBJECTIVE: To explore the feasibility of different operative approaches for the treatment of upper thyroid gland. METHODS: The clinical data of 87 patients underwent lobectomy in Department of Oncological Surgery, the First Affiliated Hospital of Bengbu Medical College from January 2016 to December 2018 were analyzed retrospectively.Preoperative ultrasound was used to evaluate the location of the tumor in the upper thyroid gland or thyroid gland hypertrophy.According to the operative approach,87 cases were divided into two groups including 42 cases of the cricothyroid approach and 45 cases of viacervical strap muscles approach. The intraoperative exposure of superior laryngeal nerve and postoperative complications of superior laryngeal nerve function and parathyroid gland were compared. RESULTS: Compared with the superior laryngeal nerve finding rate(83.3%)in the cricothyroid approach,the superior laryngeal nerve finding rate was higher(95.6%)(P0.05). CONCLUSION: The viacervical strap muscles approach compares the cricothyroid approach to the laryngeal nerve with a high rate of finding and a short searching time. Applying viacervical strap muscles approach in the case of an upper thyroid tumor and glandular hypertrophy helps to expose the upper pole and protect the external nerve branch of the larynx.

In one of the rarest form of freak accident, a young woman surrendered to her death in a manner that belie the reality only to be played back in a slow motion sequence. History is replete of instances in which death struck individuals in otherwise innocent forms. In this case – elaborated below – a helmet tucked under the crook of an elbow became the undoing causing the woman to topple backward in a slow-motion, the head striking the hard surface of the road and precipitating her death later in a hospital.

Objective Observe the clinical effect of the controllable airbag elastic chest strap after the breast cancer operation,studying the clinical value of the controllable airbag elastic chest strap.Methods Between May.2009 and Oct.2012,71 women having surgery for breast cancer at The Affiliated Second Hospital of Nan Hua University Oncology Center.They were to receive either a common thoracic girdle compressed binding(n =35),or the controllable airbag elastic chest strap compressed binding (n =36) at the postoperation.The following complications were evaluated:the incidence of seroma formation and duration,the incidence of skin flap swelling,and the dressing time.Results Thirty-five patients with common thoracic girdle is 14 cases of subcutaneous effusion,fluid rate was 40.0%,the effusion duration 7 to 31 days,an average of 17.6 ±7.3 days,15 cases with skin redness (disc),swelling rate is 42.9%,the average treatment time 18.7 ±3.5 minutes; With the controllable airbag elastic chest strap group of six cases of subcutaneous effusion,fluid rate was 16.7%,the duration of effusion 5-14days,an average of 10.5 ± 3.9 days,skin redness (disc),swelling rate is 13.9%,the average treatment time 12.2 ± 2.4 minutes.Conclusions The controllable airbag elastic chest strap can reduce subcutaneous effusion,reduce skin flapswelling,saving the treatment time,is worth promoting in the clinical medical supplies.

ObjectiveTo investigate the clinical effect ofcarboprost tromethamine combined with uterine strap suture in hemorrhage during cesarean section.MethodsSixty patients with hemorrhage during cesarean section were selected and divided by random digits table method into observation group and control group with 30 cases each.The patients in observation group were treated with carboprost tromethamine combined with uterine strap suture,while the patients in control group were treated with carboprost tromethamine alone.The clinical effect,blood loss,blood transfusion cases,transfusion volume and the occurrence of recurrent hemorrhage and complication of two groups were compared.ResultsCompared with control group,the efficiency in observation group was significantly higher [93.33%(28/30) vs.70.00% (21/30),P< 0.05];blood loss,blood transfusion cases,transfusion volume and recurrent hemorrhage rate of observation group were obviously lower [ (603.35 ± 52.62) ml vs.(689.17 ± 50.36) ml,3 cases vs.10 cases,(640.21 ±63.58) ml vs.(726.17 ±68.74) ml,3.33%(1/30) vs.26.67%(8/30),P <0.05];the occurrence rate of complication of two groups had no statistical significance (P > 0.05).ConclusionsCarboprost tromethamine combined with uterine strap suture can improve the clinical effect of patients with hemorrhage during cesarean section,decrease blood loss and transfusion volume,and reduce the recurrent hemorrhage rate.

Objective: To study the effect of night-time hallux valgus strap usage on decreasing the progression of hallux valgus angle. Methods: Patients, who were older than eighteen years old with moderate to severe degree of hallux valgus, were randomized into 2 groups: the study group (prescribed to use night-time hallux valgus strap for 8 hours per night for 12 months), and the control group. Patients in both groups were advised to have proper foot care with proper shoes. Results: There were 25 patients in the study group and 22 patients in the control group. No statistical difference was found in demographic data between both groups. The hallux valgus angle, which was obtained through radiographic measurement, was decreased in both groups. However, there were no statistically significant differences in the decrease of hallux valgus angle between the two groups (p>0.05). Conclusion: Prescribing to use night-time hallux valgus strap for 12 months cannot decrease the progression of the hallux valgus angle more than the control group.

Objective To evaluate the advantages and disadvantages of cervical strap muscles longitudinal incision and linea alba cervicalis incision in thyroid surgery.Methods The clinical data of 91 patients with thyroid surgery wefe retrospectively analyzed.They were divided into two groups by random digits table,46 patients with cervical stsap muscles longitudinal incision as the observation group,45patients with linea alba cervicalis incision as the control group,the surgery effect of two groups was compared. Results The operative time,blood loss and postoperative complications between the two groups,the differences were not statistically significant(P>0.05).Compared with control group,the exposure evaluation scores in the upper pole[(3.88±0.32)points vs.(2.15±0.22)points],under pole[(3.68±0.46) points vs.(2.18±0.42) points]and lateral side[(3.55±0.47)points vs.(2.18±0.39)points]of thyroid in observation group were significantly better (P<0.01),but the exposure effect scores in thyroid gorge were signilicantly lower [(2.31±0.39) points vs.(3.48±0.52) points](P<0.01),SF-36 score was better [(77.1±7.3) points vs.(66.4±6.8)points](P<0.05).Conclusion The effects of cervical strap muscles longitudinal incision and linea alba cervicalis incision in thyroid surgery are satisfactory,but the exposure effect of two incision are different,thus it is required to choose the applications according to the conditions ofpatients.

Objective To explore the efficacy of a new elastic suspension strap orthosis, designed by the authors, in improving the motor function and walking ability of stroke patients. Methods Thirty-nine stroke pa-tients less than 1 month after stroke were recruited and divided randomly into a treatment group and a control group. The patients in the treatment group accepted walking training with the assistance of an elastic suspension strap orthosis, 5 times a week for 8 weeks, while the control group accepted the same training without the assistance of the elastic suspension strap orthosis. Results The treatment group improved significantly more than the control group with regard to its average Fugl-Meyer score, the functional ambulation categories of its members, and their walking speeds. Conclusions The elastic suspension strap orthosis could help correct abnormal gait patterns, improve walking speed, and enhance the recovery of walking ability in stroke patients.

We reported three types of newly designed arm slings (the "V" strap pouch arm sling, the Modified hemisling, the Tripod arm sling). The three slings and the conventional hemisling were all tested by a single stroke patient for the correction of shoulder subluxation. Sling effectiveness was evaluated by simple shoulder AP X-rays, quantitative electromyography (root mean square; RMS), strap tension, and preference of the patient. Of the four types, the "V" strap pouch arm sling reduced the vertical displacement the most. RMS values of the supraspinatus muscle tended decrease and strap tension was also lower after using the "V" strap pouch arm sling compared with others. The patient also preferred the "V" strap pouch arm sling to the other slings. We suggested that "V" strap pouch arm sling was the most effective among four types of arm slings (including conventional hemisling) for the correction of shoulder subluxation, and patient's preference.

The effect of a newly developed abdominal webbing strap for pain relief after exploratory laparotomy in gynaecological patients was studied using a quasi-experimental method. The subjects consisted of 60 patients in gynaecological wards and were randomly allocated into 2 groups as a control group and an experimental group. The experimental group was given the webbing strap when they gained consciousness after operation until 72 hours later while the control group was not. A visual analogue scale was used to evaluate the pain level experienced by the patient. The percentage changes in pain level while resting, coughing and walking were evaluated in both groups at 24, 48, and 72 hours after operation. The data were then analysed using the Student t-test. The results of the study showed no statistical difference in the decrease of pain level for either group while they were resting (P > 0.05) at every time of evaluation. On the contrary, at every time of evaluation, the pain level of the experimental group showed a greater decrease than the control group when coughing or walking which was statistically significant (P < 0.001). The quantity of analgesic usage was also not statistically different for either group (P > 0.05). As a result of this, we suggest our new developed webbing strap should be used routinely in gynaecological patients after exploratory laparotomy.

PURPOSE: Voice change after thyroidectomy has generally been the result of damage to the recurrent or superior laryngeal nerve. But many patients complain voice alteration without laryngeal nerve injury after thyroidectomy. The purpose of this study is to investigate whether strap muscle division results in any subjective or objective functional sequelae in voice, through long-term follow-up prospectively. METHODS: Twenty-two female patients who had undergone thyroid surgery between July 1998 and December 1999, were studied. The patients who were planned for neck dissection, who had benign laryngeal disease or vocal cord paresis, and whose vocal cord paresis were developed after thyroid surgery, were excluded from this study. Twelve patients had undergone thyroidectomy via retraction of strap muscle and ten patients had undergone thyroidectomy via cutting of strap muscle. For evaluation of voice, questionnaires for changes of voice, acoustics (fundamental frequency, jitter, shimmer, signal to noise ratio, noise to harmonic ratio, voice range), and aerodynamic (maximal phonation time) analyses were done. RESULTS: The subjective voice symptoms after thyroidectomy were disturbances of high pitch, singing, loud voice, and easy fatigue at phonation. There were no significant differences in voice parameters on acoustic and aerodynamic analyses between the strap muscle retraction group and the cutting group through long-term follow-up. CONCLUSION: We conclude that strap muscle division does not result in any subjective or objective functional problems in voice. We suggest that surgical division and reconstruction of these muscles should be employed routinely when operating on large, toxic or neoplastic glands.

BACKGROUND AND OBJECTIVES: A voice analysis was carried out before and after thyroid surgeries without laryngeal nerve injury, between strap muscle retraction and cutting group in thyroid surgery. The study was done by means of questionaires, acoustics and aerodynamic analysis. MATERIALS AND METHODS: A total of fourty-two patients of whom thirty-eight are females and four are males were analyzed; hemithyroidectomy (19 cases) and total thyroidectomy (23 cases). Twenty-seven patients had undergone thyroidectomy via retraction of strap muscle and 15 patients had undergone thyroidectomy via cutting of strap muscle. For evaluation of voice, questionaires for changes of voice, acoustics (fundamental frequency, jitter, shimmer, noise to harmonic ratio, voice intensity, vocal range), and aerodynamic (maximal phonation time, mean flow rate and subglottal pressure) analyses were done. RESULTS: The subjective voice symptoms after thyroidectomy were disturbances of high pitch, loud voice and singing voice, and easy fatigue at phonation. These voice symptoms were related to the extent of surgery. Vocal range was the only diminished parameter in acoustic and aerodynamic analyses after thyroidectomy. There were significant differences in jitter on acoustic and aerodynamic analyses between the strap muscle retraction group and the cutting group. CONCLUSION:Voice alteration after thyroidectomy without laryngeal nerve injury may be associated with the disturbance of the extralaryngeal skeleton- laryngotracheal fixation with impairment of vertical movement or by temporal malfunction of the strap muscles. These voice problems were related to the extent of surgery and cutting of strap muscles.

Objective To analyze the effect of Smad7 on the expression of TGF-?R,R Smads and STRAP in the process of malignant transformation of immortalized human bronchial epithelial cells BEP2D.Methods Cells were cultured and protein were extracted from BEP2D,BERP35T2,BS7 and RS7 cells,then Western blotting was performed to examine the expression level of TGF-?RⅠ,TGF-?RⅡ,Smad2/3,Smad4 and STRAP.(Results) Stable transfection of Smad7 gene led to decreased expression of TGF-?RⅡ and increased expression of Smad2 and STRAP in both BEP2D and BERP35T2 cells.There were no changes in the expression of Smad3 and Smad4.Conclusion Overexpression of Smad7 leads to decreased expression of TGF-?R Ⅱ and increased expression of STRAP in BEP2D and BERP35T2 cells,which could be one of the mechanisms of the malignant transformation of BEP2D cells.
