RESUMO

This study was aimed to demonstrate preliminarily the effects and mechanisms of uremic clearance granule (UCG) ameliorating renal interstitial fibrosis (RIF) by regulating transforming growth factor (TGF)-β1/SnoN/Smads signaling pathway in vivo. Fifteen rats were randomly divided into 3 groups:the normal group,the model group and the UCG group. The rats with renal failure were induced by intragastric administration of adenine and unilateral ureteral obstruction (UUO). After modeling,the rats in the UCG group and in the other groups were intervened by intragastric administration of UCG and distilled water respectively during 3 weeks. The body weight and 24 h urinary protein excretion (Upro) in all rats were tested after drug administration. All rats were killed after drug administration for 3 weeks,blood and kidneys were collected and weighted,kidney appearance and renal morphological characteristics were observed. In addition,serum biochemical indices and the protein expressions of TGF-β1,SnoN,phosphorylated Smad2/3 (p-Smad2/3) and Smad7 in the kidney were evaluated respectively. The results indicated that,after the intervention of UCG,the general state of health,kidney appearance,serum creatinine (Scr),blood urea nitrogen (BUN),uric acid (UA),albumin (Alb),Upro and renal morphological change in model rats were improved in different degrees,respectively. Moreover,UCG down-regulated the protein expressions of TGF-β1 and p-Smad2/3,and up-regulated the protein expressions of SnoN and Smad7 in the kidney. In conclusion,UCG reduces extracellular matrix (ECM) synthesis and delays the progression of renal failure via possibly multi-targeting at regulating TGF-β1/SnoN/Smads signaling pathway in vivo.

Objective To observe the effects of Uremic Clearance Granule combined with angiotensin receptor blockers (ARB) or angiotensin converting enzyme inhibitors (ACEI) in treating different period of diabetic nephropathy (DN ) .Methods 46 patients with DN were divided into the control group and the treatment group ,23 cases in each group .The two groups were given the rou-tine hypoglycemic therapy .The control group was treated with ACEI or ARB ,while the treatment group was given Uremic Clear-ance Granule 5 g each time ,four times daily for successive 4 weeks .Then the indexes of serum creatinine ,blood urea nitrogen ,cysta-tin-C ,microalbumin and 24 h urine total protein at 24 h before and after treatment were observed .Results The microalbumin and 24 h urine total protein after treatment in the two groups were reduced compared with before treatment (P0 .05) . In the patients with the stage Ⅳ of DN ,on the basis of ACEI or ARB adding Uremic Clearance Granule could significantly decrease the urine protein(P<0 .05) .Conclusion ACEI or ARB combined with Uremic Clearance Granule has definite effect for treating massive proteinuria of DN .

RESUMO

OBJECTIVE:To observe the efficacy of small dose of uremic clearance granule combined with colon enema on aged patients with chronic renal failure(CRF) . METHODS:60 aged CRF patients were randomly divided into treatment group(n=30) and control group(n=30) . Two groups were given essential treatment. Control group were treated with medicinal carbon additionally. Treatment group were additionally treated with uremic clearance granule(p.o.) combined with colon enema. The course of treatment was 15 days and curative effects were compared between two groups after 30 days of treatment. RESULTS:30 days later clinical symptom of patients in treatment group was obviously improved and the levels of blood urea nitrogen,serum creatinine,blood uric acid and serum potassium were lower than before treatment with significant difference. CONCLUSION:Small dose of uremic clearance granule combined with colon enema show satisfactory effect on CRF in aged patients as well as simple,practical and good compliance. This therapy is worth of spreading and putting into application.
