RESUMO

Objective To evaluate the clinical value of the combination of S-shaped urethral dilator and nephro-scope for the treatment of male urethral stricture disease. Methods Guidewires were inserted into bladder through the nephroscope under direct vision. The urethral dilation with S-shaped urethral dilator was carried out by nephroscope in 41 male patients. All catheters were located across the strictures and remained for 4-6 weeks. The regular follow-up was done on all cases to assess the clinical effect on urine flow. Results All surgeries were successful without serious complications. The mean operative time was (40.16 ± 5.78) min. The voiding symptoms were significantly improved after catheter removal compared with those of preoperation in all cases. Patients were followed up after surgery and were regularly urethral dilation at least 6 weeks. The mean follow-up time was (34.75±6.42) weeks.There were incontinence, diminished sexual function and other complications after operation in all cases. Conclusion Nephroscope combined with S-shaped urethral dilator for the treatment of male urethral stricture disease is feasible, minimally invasive and safe, which is worthy of recommendation.

RESUMO

Objective To discuss the feasibility and safety of a novel urethral dilation procedure using antegrade insertion of guide wire through suprapubic fistula (retrograde insertion failed) to facilitate retrograde dilation for treating early stricture following urethroplasty. Methods Thirteen patients, who failed to receive retrograde insertion, developed early stricture following urethroplasty. The guide wire was inserted through a pre-made suprapubic fistula to pass the stricture with the help of flexible cystoscope. Then the guide wire was used to guide polytef catheter to perform retrograde dilation. Results The procedures were successfully done in all the 13 patients. The mean operation time was (10. 7 ± 3. 18) min (8-17 min), and there were no serious complications. Conclusion The present dilation procedure is an effective and safe method for management of early stricture recurrence following urethroplasty.

RESUMO

A clinical survey has been undertaken of 50 patients with urethral stricture who had been treated with intermittent dilatation in the Department of Urology, Kyungpook National University Hospital during 8 years periods from January, 1977 to December l984. These patients had history of urethral stricture for 1-15 years periods prior to attending this hospital and had been attended with intermittent dilatation for l-6 years. The interval between dilatation was a range from 2 days to 2 years. Sixty-two percent had infected urine but the apparent decreasing of the incidence of infected complications following dilatation may be attributed to the use of prophylactic chemotherapeutic agents. Associated diseases and complications related to instrumentation were noticed in 20 cases of these patients but half of this was healed during intermittent dilatation and use of chemotherapeutic agents. In the follow up of 50 patients with intermittent dilatation, 37 cases remained trouble free whereas 13 cases had repeat complications or required another management.
