Background: The study on self-perceived uselessness may help health-care workers to provide opportunities for intervention for increased quality of health and greater survival rate in elderly. Objectives: To find the prevalence of self-perceived uselessness and its determinants among the elderly residents of old age homes of North Bengaluru. Methods: A cross-sectional descriptive study was undertaken on 129 elderly aged 60 years and above in the old age homes of North Bengaluru. Self-perceived uselessness, sociodemographic variables, morbidity status, functional status, financial status, and personal habits were assessed using a semi-structured questionnaire. Proportions, percentages, and Chi-square test were used for analysis. Results: Nearly half (45%) of the elderly had agreed that they had perceived themselves as useless as they age. Majority of those who perceived uselessness had one or >1 chronic morbidities and were functionally dependent (81.2%). Significant association was seen between self-perceived uselessness and functional dependency, gender, education, and having some form of activities during their leisure time. Conclusions: It can be concluded that self-perceived uselessness is quite prevalent in nearly one-half of the elderly in the old age homes in North Bengaluru having significant association with variables such as gender, education, leisure activities, and functional status.

Los avances en biomedicina amplían cada vez más las posibilidades de curar a enfermos críticos. Las decisiones sobre terapias por aplicar deben tener presente el riesgo de caer en el llamado "encarnizamiento terapéutico". Aunque ya se ha escrito bastante sobre los criterios básicos que se deben tener en cuenta, aún quedan numerosas dudas a la hora de tomar las decisiones. Un aspecto no menor es la necesidad de objetivar los distintos elementos por ponderar, sin olvidar que hay aspectos subjetivos que son muy importantes. En el presente artículo se presenta una manera de combinar los aspectos objetivos y subjetivos para llegar a un equilibro adecuado y práctico, que sea razonable y respete la autonomía del paciente.


Advances in biomedicine increasingly expand the possibilities of curing those who are critically ill. Decisions on what therapies to apply should consider the risk of falling into so-called therapeutic obstinacy. Although much has been written already about the fundamental criteria to be considered, there are still many questions when it comes to making decisions. Not the least of them is the need to objectify the different elements to be weighed, without forgetting there are subjective aspects that are very important as well. This article outlines the objective and subjective aspects in a combined way to strike an appropriate and practical balance that is reasonable and respects the patient's autonomy.

As decisões sobre que tipo de terapia aplicar devem considerar o risco de cair no chamado "encarniçamento terapêutico". Embora já se tenha escrito bastante sobre os critérios básicos que devem ser considerados, ainda restam numerosas dúvidas na hora de tomar as decisões. Um aspecto de não menos importância é a necessidade de objetivar os diferentes elementos por ponderar, sem esquecer que há aspectos subjetivos que são muito importantes. Neste artigo, apresenta-se uma maneira de combinar os aspectos objetivos e subjetivos para chegar a um equilíbrio adequado e prático, que seja razoável e respeite a autonomia do paciente.
