RESUMO

Objective To propose the orientation,goals and some key issues for future research in the field of war wound/trauma through reviewing the updated literature.Methods The latest literature in the field of war wound/trauma were reviewed.Results The notion "medicine always be with soldiers" had been raised by America and the Europe military circles,with special emphasis on emergency techniques and equipments,which were used in the front of battle field.This forwarded the medical aids as quick as possible and the formation of medical rescue system without crack.Meanwhile,much attention had been paid to the pathogenesis and prevention of post-traumatic complications,wounding characteristics and mechanisms of high-tech new weapons,as well as repair and regeneration of injured tissues.In China,a great advances had been made in the filed of emergency rescue,shock resuscitation,complication treatment and wound repair.Conclusions During the coming "twelfth five-year program",the research standpoint is to meet the needs of Chinese troops to perform better in modern wars and non-military actions,and the aim is to significantly reduce the rate of mortality and disability of trauma as well as to improve the whole army's health.To fulfill this aim,a lot of work should be done to enhance the trauma emergency ability at the battle front,the organization and remedy ability at conditions of combined operations of armed forces,and the translational medical research for rescue of trauma,so as to unceasingly increase the medical service ability of our army to cope with secure threaten and to accomplish multiple military missions.

RESUMO

The morbidity of shock in conventional combat is about 10%-15%. It is estimated that during the high-tech combat in the future it will be up to 25%-30%. Data from many sources showed that approximately 50% of combatants who died were as a result of hemorrhage. So it is very important to implement early management for war wound (or traumatic )shock. Based on the profound research and understanding of the pathogenesis of shock, the metabolism of tissue fluid and oxygen in recently years, government and military departments in many countries have made many beneficial explorations in resuscitation time,resuscitation approaches and principles, resuscitation endpoints and criteria, and the choice of fluids for war wound (or traumatic)shock, some new concepts and viewpoints have been raised.

RESUMO

Objective To approach the directive function of time efficiency of first-aid to war wound and trauma. Methods To analyse the relationship between time and war wound or trauma death based on the statistical data of those wounds, then to define the time window of first aid aiming at the peak value of death. Results Bleeding and asphyxia should be virtually controlled in “emergency platinum 10 minutes”, resuscitation from shock should be within 30 minutes, at the same time, a definite operation to save life should be performed in “golden an hour”. Conclusions It is important to raise the time efficiency in each stage by training and improving the military affairs. To spread the first-aid knowledge may decrease cripples and mortality.

RESUMO

After looking back the history of research on wound ballistics and discussing the present situation of research on ballistic characteristics of wounding agents, mechanism of woundins, assessment of wounding capacity and devitalized tissues, etc. the following suggestions are put forward: 1. The study of wound ballistics, blast injury, and impact injury is able to be combined and complemented each other. 2. Interdisciplinary cooperation for elucidating the mechanism of wounding is necessary. 3. Based on the animal experiment, the clinical study on war wounds should be streshtened. 4. Some theories or points of view on wound ballistics need to be reviewed or even corrected.
