RESUMO

The new Standardization Law, implemented in 2018, has added a standard post-implementation evaluation system, aiming to continuously improve the quality of standards through post-implementation evaluation. Standards in the forensic science field are closely related to accreditation activities. Forensic science standards are not only the criteria on which accreditation activities are carried out, but also one of the key contents of the inspection of forensic science institutions in accreditation activities. Since 2018, the certification and accreditation policies in the forensic science field have also been changed, which has brought impacts on the construction of a standard system based on accreditation.This paper analyzes the standard data from China National Accreditation Center from Conformity Assessment on forensic clinical identification accreditation assessment. It points out that the current coverage of laboratory accreditation activities is limited, the development in different provinces is unbalanced, and there is overlap and crossover in the standards in use. It is emphasized that the construction of the national forensic science standardization technical committee, the improvement of the forensic science standard system, the establishment of the standard implementation evaluation index system, and promotion of the coordination of standards, and the certifications and accreditations should be accelerated, in order to continue to promote the standardization and accreditation activities in the field of forensic science.

El trabajo realizado por los comités de Bioética es muy complejo, la solución de conflictos donde están involucrados aspectos relacionados con la ética de la vida o de la salud, no siempre tienen una sola solución. Es importante que estos comités se constituyan en las instituciones, para garantizar los derechos y el trato humanista de los profesionales o instituciones dedicadas al sector de la salud. Sin embargo, la posibilidad de acreditar estos comités resultaría muy beneficiosa para las comunidades donde actuarían, ya que además de garantizar el cumplimiento de normas y procedimientos, se podría crear una nueva instancia deliberativa, que ampliaría el proceso de conocimiento y toma de decisiones en el trato y en la resolución de conflictos. Asimismo, en estas reflexiones se reconoce la necesidad de que una participación racional y pluralista permitiría promover soluciones acordes a los problemas, y no solo desde el ámbito de determinado comité, sino de una instancia superior; que trabaje acorde a los mismos mecanismos y principios.


The work done by the committees of Bioethics is very complex, conflict resolution which are involved ethical aspects of life or health, do not always have a single solution. It is important that these committees are formed in institutions, to ensure the rights and humanistic treatment professionals or institutions dedicated to the health sector. However, the ability to establish these committees would be very beneficial to the communities where they act, as well as ensuring compliance with rules and procedures; you could create a new instance deliberative process would expand the knowledge and decision-making in the treatment and resolution of conflicts. Also, in these reflections recognizes the need for rational and pluralistic participation would promote consistent solutions to problems, and not only from the field of specific committee, but a higher court, which works according to the same mechanisms and principles.

On July 23rd, 2010 a revised medical law (Article 58) was passed to change existing evaluation system of medical institutions to an accreditation system. The new healthcare accreditation system was introduced to encourage medical institutions to work voluntarily and continuously to improve patient safety and medical service quality. Changes regarding the healthcare accreditation system included the establishment of an accreditation agency, the voluntary participation of medical institutions, accreditation standards centering on the treatment process and patient safety, tracing methodology, and the announcement of comprehensive results concerning accreditation. Despite varying views on the healthcare accreditation system, including some that are critical, it is meaningful that the voluntary nature of the system acknowledges that the medical institutions must be active agents in improving medical service quality. Healthcare quality is not improved instantaneously, but instead gradually through continuous communication within the clinical field. For this accreditation system to be successful, followings are essential: the accreditation agency becomes financially independent and is managed efficiently, the autonomy and regulation surrounding the system are balanced, the professionalism of the system is ensured, and the medical field plays an active role in the operation of the system.

RESUMO

Recently, there have been many changes in the area of healthcare. There is no certainty how these changes will affect the healthcare system and public health. However, to at least have these changes positively implemented, it is clear that evaluation through continuous monitoring is necessary. The enforcement of the Medical Institution Accreditation and Medical Dispute Mediation Law as well as legal revisions regarding the public healthcare system are changes to improve the quality of healthcare, while at the same time, provide penalties for infractions of the new law such as medicine/medical device rebates; moreover, legal revisions regarding telemedicine are anticipated to impartially vitalize technical development as well as the pharmaceutical industry. For these changes to have a positive effect on the medical field and people's lives, an accurate comprehension of the system and understanding of the details is necessary to be able to respond sensitively to any changes in the future. Therefore, this paper examined the background information on the current discussion on the changes in the healthcare system, examined the detailed content of the system, and reviewed the areas that were in dispute as well as the main issues to contemplate the expected effects of the changes and future tasks that may be generated as a result. These considerations will act as foundation for an in depth understanding of recent trends in the healthcare system.

Antecedentes: Desde una prespectiva histórica se comentan los sistemas de evaluación y acreditación de carreras de medicina a nivel mundial y nacional hasta la actualidad. Objetivos: Analizar los estándares y procedimientos de aplicación de la acreditación de las carreras de medicina de Argentina a la luz de los resultados a fin de proponer correcciones al sistema. Lugar de aplicación: inicialmente en la sede de las carreras y posteriormente en la sede de la acreditadora (CONEAU). Diseño: Estudio retrospectivo. Resultados: Se acreditaron: con 6 años: 2 carreras; con 3 años sin requerimientos: 2 carreras; con 3 años y requerimientos desde la autoevaluación: 3 carreras; con 3 años y requerimientos de los pares evaluadores: 14 carreras; con autorización de iniciar actividades: 3 Proyectos Universitarios. Conclusiones: Se destaca el valor de la acreditación de la carrera de Medicina y se propone una visión sobre aspectos no analizados, como sería asegurar el seguimiento del graduado y definir las limitaciones y los alcances del título de médico, destacándose que esta primera experiencia de acreditación carga la denominada curva del aprendizaje

La asistencia y la docencia son actividades inseparables y complementarias de la Atención de Salud, que se potencian mutuamente. Los Centros de Atención de Salud, de todos los niveles de complejidad, deben ser acreditados en el doble aspecto asistencial y docente. En ambos aspectos debe considerarse: La existencia de un Programa de actividades con objetivos bien definidos que establezcan obligaciones, exigencias y responsabilidades claramente definidas. La disponibilidad de recursos humanos, materiales y financieros suficientes. La integración en una malla o red docente-asistencial operante. Hay Establecimientos preferentemente docentes, responsables de la realización de actividades educativas orientadas básicamente a la entrega de conocimientos teóricos. Otros, en cambio, esencialmente asistenciales pero tienen una importante función formativa, consistente en el desarrollo de destrezas prácticas; en la adquisición de experiencia y, muy especialmente, en la creación de hábitos y actitudes positivas y duraderas. En otras palabras, los llamados Centros Asistenciales tienen la misión de transformar en los integrantes de todos sus estamentos, sean ellos profesionales o no, los conocimientos virtuales en destrezas reales. Por último, diremos que, en nuestra opinión, en las últimas décadas, se ha avanzado mucho más en el campo de la Acreditación y Certificación de los médicos (CONACEM) y de los Centros docentes formadores de profesionales de la Salud (ASOFAMECH) que en la de los profesionales no médicos y en particular de los Centros asistenciales. Estos últimos son, no obstante, los campos de práctica clínica en los que se desarrollan y adquieren destrezas, habilidades, actitudes y hábitos que condicionan, en gran medida, la calidad de la Atención de Salud.

La autoevaluación y la autorregulación sirven para determinar el funcionamiento y los logros del programa, que se revierten en la capacidad de retroalimentar su funcionamiento y procurar su excelencia, cuyo fin es demostrar que el programa que se somete a la acreditación tiene calidad, la cual es verificada por pares externos a la institución o programa.El Programa de Enfermería de la Universidad de La Sabana, desde el inicio de sus actividades, ha realizado autoevaluaciones que le han servido para mejorar sus procesos y retroalimentar sus acciones en la formación de estudiantes y profesores. No ajena a lo determinado por la Ley, se inscribió en el proceso de acreditación, y para tal fin, siguiendo los lineamientos dados por la Dirección de Acreditación y Currículo, realizó una autoevaluación del programa en los diez factores propuestos por el CNA; analizó cada una de las 66 características y las 200 variables, y emitió juicios basados en documentos de análisis y soporte y en encuestas de opinión aplicadas a estudiantes, profesores, directivos y egresados, acerca de la realidad académica y administrativa, lo cual permitió determinar las fortalezas y debilidades y proponer planes de mejoramiento en el fortalecimiento del programa

RESUMO

We report the experience of the re-valuation committee of the University of Chile Medical School, in the period 1984-1999. This experience is relevant to the licensing processes in Latin America. We report the political, legal and regulatory backup and the academic criteria used to esatblish different evaluation methodologies. We also communicate the main results obtained. Factors such as the country and University conferring a title, nationality and length of service of the applicant, influence in the percentage of licensure exammination approval. Some modifications to the licensure process should be incorporated but, the overall results obtained, fully justify the existence of this licensure committee

Since the law of medical specialities was sanctioned in the province of Tucumán we started to work on the instrumentation of the statutes the law provides. The goal of our work was to determine if health professionals and other people involved had the necessary knowledge of the topic, its implications and possible consequences, as well as to establish the policies for its execution. With this purpose we carried out a descriptive study of transversal type by creating a questionnaire to be filled by people responsible in the following areas : Si.Pro.Sa, university, medical college, scientific societies, resident physicians and the professionals chosen at random, and a selection of national expert as a part of the Delphi method. Both questionnaires took into consideration the following aspect : accurate knoledge of the topic; quality of the medical assistance; population access to super specialism; struggle for power, efficiency and effectiveness and recognizing new specialities, among others. By considering the data and the local and national legislation we confirmed the hypothesis that our health professionals are not deeply concerned with the topic and its implications. As a matter of fact, we could detect a hard strunggle for personal interests among them. We conclude with a number of proposals for the regulation of this law.
