RESUMO

BACKGROUND: Rehabilitation costs borne by the service institution are part of the total cost incurred when a patient is admitted to hospital. The total costs cannot directly represent the rehabilitation costs. When considering the funding allocated for specific services, it is useful to have figures that represent the actual costs of those services. OBJECTIVE: Study the unit cost of rehabilitation. MATERIAL AND METHOD: The data were collected from March to December 2006. Three hundred twenty seven patients from nine collaborating centers, including 18 patients from Sirindhron National Medical Rehabilitation Center (SNMRC) participated in this study. Descriptive analysis produced results in percent, mean standard deviation, and p-value. One unit of rehabilitation treatment is equal to twenty minutes. RESULTS: The average unit cost of rehabilitation treatment among the nine collaborating centers was 94.56 units per week and 33.78 from rehabilitation nursing. At SNMRC, the average rehabilitation unit was 32.67 units per week and the cost for rehabilitation was 11,170.56 +/- 5641.73 baht. CONCLUSION: The calculated unit cost was 60 baht/20 minutes service time.

RESUMO

This paper aimed to clarify the effectiveness and the cost of the community-based rehabilitation service in Klong Toey slum after a three year study period. One hundred and seventy eight patients used community-based rehabilitation during the three year period. One hundred and fifty-seven patients (86.5%) reported that their problems/conditions were cured or improved. Only nine patients (5.1%) reported that they stopped using community-based rehabilitation because their problems/conditions did not improve. A statistically significant improvement in pain level and walking velocity assessment, in 105 and 78 patients respectively, was demonstrated. Total cost and cost per patient-day of the community-based rehabilitation were Bt 559,920 and Bt 111.1 respectively. Cost per-patient-day of this community-based rehabilitation service was compared with an estimated cost per patient-day of using rehabilitation services at Chulalongkorn University Hospital and was found to be cheaper. This study supported the role of community-based rehabilitation in Thailand. The need for a health service study in rural areas was also noted.

RESUMO

Mediante 3 modelos de intervençäo foram estudadas 817 crianças, sob diferentes condiçöes nutricionais, com idade de 6 a 60 meses. Destas, 509 foram inscritas no Programa de Suplementaçäo Alimentar (PSA), 200 no Centro de Educaçäo e Recuperaçäo Nutricional (CERN) e 108 no Instituto Materno Infantil de Pernambuco (imip) (modelo hospitalar). O estado nutricional das crianças foi discriminado pelas classificaçöes de Gomez, ARIZA-MACIAS E SEOANE-LATHAM (modificada). Os resultados indicaram que o modelo P.S.A. praticamente näo altera a situaçäo inicial encontrada. Os dados do hospital säo melhores que os obtidos no CERN, a um custo 28 vezes maior. Discute-se o papel provável da história natural na evoluçäo espontânea de muitos casos, a despeito das intervençöes de caráter técnico, operados pelos Serviços de Saúde. De modo geral, os resultados das intervençöes näo säo promissores
