RESUMO

Rectus sheath hematoma is a well documented clinical entity, though uncommon and often clinically misdiagnosed cause of acute abdomen. The non-specific nature of presentation combined with a lower incidence of the disorder leads to difficulty in diagnosing. Our patient presented with rectus sheath hematoma, following caesarean section on 9th post-operative day. She presented with wound discharge and lower abdominal pain. The case report is presented to increase the awareness in considering this entity in the differential diagnosis and management of acute lower abdominal pain. Rectus sheath hematoma’s early diagnosis and appropriate treatment may help to prevent complications.

RESUMO

The incidence of ovarian tumor complicating caesarean section was about 1 in 200 caesarean births while ovarian tumors complicated termination of pregnancy in 1 of 594 procedures. Any type of Ovarian mass can be encountered during pregnancy, but the most common are cystic. Because pregnant women are usually young, malignant tumors are relatively uncommon. A patient third gravida with previous 2 caesarean sections presented in an emergency with complaints of persistent pain in abdomen for last 7 days with gestational age of 35 weeks and 3 days. Emergency Caesarean section was done to deliver an alive male baby of 2.25 Kg with good Apgar score. Intraoperative finding was suggestive of a huge multiloculated left ovarian mass of approximately. 40×30×15 cm weighing 20 Kg with straw coloured mucinous fluid. Up to 3 liters of fl uid aspirated before delivering the cyst out of the abdomen. Left salpingoopherectomy was done and specimen sent for histopathological examination that showed Mucinous cystadenoma of the ovary. Post operative period remain uneventful.

RESUMO

Incidental vaginotomy, a potentially severe complication, has occasionally been reported in cesarean sections performed after prolonged second stage labor. There is controversy on the significance of vaginotomy and its consequences in cesarean section. Ureteral injury has been reported as a possible complication of cesarean section. Herein, we present a case of anterior vaginal incision instead of lower uterine segment, where the right ureter had been obstructed by sutures. The injury was detected and corrected during the operation. Thus incidental vaginotomy in cesarean section must be taken seriously and avoided as far as possible. It is also crucial to inspect and preserve the integrity of ureters in such cases

Cesarean is performed as a routine surgery nowadays and not only many facilities, hospital beds and equipments and professional personnel are involved but also mortality rate and pregnancy complications are more common in women who have undergone cesarean than those who have vaginal deliveries. The aim of this study was to evaluate and compare the frequency of vaginal delivery, cesarean and its causes in Shahroud, so that finding would be used for proper planning to promote vaginal delivery. This is a cross-sectional study. Data gathering instruments are two questionnaires, which were prepared base on objectives of research. When their validity was determined, necessary information was gathered in 5 months. Results showed that total number of delivery in Shahroud were 1221 cases which 1.3% of deliveries were with vaccum 42.1% cesarean and 56.6% vaginal deliveries at this period. The most common causes of cesarean were previous history of cesarean [26.1%] willing to perform tubectomy [9.5%] and cephalopelvic disproportion [8.4%]. Cesarean frequency in Shahroud is higher than accepted international statistics [20-22%]. Considering that the most common cause of cesarean is history of previous cesarean and since many researches believe that vaginal delivery is safe after cesarean, we hope this outbreak of cesarean rate decreases in this town and consequently all over the country by public training to promote vaginal delivery

RESUMO

Foram analisadas prospectivamente, quanto a complicaçöes, 309 pacientes submetidas a operaçäo cesariana. A mais freqüente complicaçäo foi a dificuldade de extraçäo fetal (8,3%). Constituíram fatores de risco a esta complicaçäo a repetiçäo da operaçäo (a partir da terceira) e a ausência de trablaho de parto. Além disso, verificou-se que fetos com menos de 1500g apresentam a complicaçäo com maior freqüência e, por serem mais sensíveis à mesma, devem ser considerados como especialmente em risco com a dificuldade de extraçäo
