RESUMO

We report a rare case of optic nerve atrophy with severe disc cupping resulting from methanol poisoning. A 30-year-old man presented to the hospital complaining of decreased visual acuity in both eyes a day after drinking alcohol containing methanol. His initial visual acuity allowed for only visualizing hand motion and not corrected in either eye. Initial intraocular pressure was within normal limits in both eyes. Initial fundus examination showed optic disc swelling in both eyes. Four years later, he visited our hospital for an eye evaluation. Visual acuity in both eyes still only allowed for visualizing hand motion. No nystagmus was observed in either eye during the optokinetic nystagmus test, and no waves were found in a visual evoked potential test. No specific change was noted on brain magnetic resonance imaging. On fundus examination, there was disc pallor in both eyes and disc cupping with a high cup/disc (C/D) ratio above 0.9 in the left eye. C/D ratio of the right eye was 0.5. Methanol poisoning may induce glaucomatous disc cupping in the late stage as well as optic atrophy. One possible mechanism of disc cupping is ganglion cell loss due to acute demyelination of the retrobulbar optic nerve. This report is the first photographic evidence of methanol induced optic disc cupping in Korea.

OBJETIVO: Avaliar o efeito da redução da pressão intra-ocular (PIO) obtido com o uso de terapia medicamentosa na espessura da camada de fibras nervosas da retina medida pela polarimetria de varredura a laser (PVL) em pacientes glaucomatosos ou hipertensos oculares. MÉTODOS: Trinta e sete olhos de 37 pacientes foram prospectivamente incluídos no estudo e avaliados com a PVL sem uso de medicação ocular hipotensora e num período entre 15 e 30 dias após a instituição de medicação ocular hipotensora, que resultou em redução da PIO de pelo menos 25 por cento. Os parâmetros medidos pela PVL antes e após a redução da PIO foram comparados com o teste t de Student pareado. RESULTADOS: A PIO média dos 37 pacientes diminuiu significativamente de 26,57±4,23 mmHg para 16,54±2,92 mmHg (p<0,05) após terapia medicamentosa. Não houve diferença estatisticamente significativa entre os valores dos 10 parâmetros do PVL medidos antes e após a administração de medicação ocular hipotensora (p>0,05). CONCLUSÃO: A redução da PIO com o uso de medicação ocular hipotensora não altera a medida da espessura da camada de fibras nervosas da retina pela PVL em pacientes com glaucoma ou hipertensão ocular.


PURPOSE: To evaluate changes in retinal nerve fiber layer thickness as measured by scanning laser polarimetry (SLP) after the use of medication to reduce intraocular pressure (IOP) in glaucomatous or ocular hypertensive patients. METHODS: The authors prospectively enrolled 37 eyes of 37 patients in whom IOP was reduced by more than 25 percent after the use of medication. The images were obtained before and 15 to 30 days after the introduction of medication. The SLP parameters measured before and after the use of medication were compared using paired Student's t Test. RESULTS: The mean IOP was significantly reduced from 26.57±4.23 mmHg to 16.54 ±2.92 mmHg after the use of medication (p<0.05). None of the 10 SLP analyzed parameters was significantly affected by the reduction of IOP with medication (p>0.05). CONCLUSION: The retinal nerve fiber layer thickness, as measured by SLP, is not affected by the reduction of IOP with medication in patients with glaucoma or ocular hypertension.

The purpose of this study was to evaluate the neuroprotective effect of memantine, a N-methyl-D-aspartate antagonist, in an experimental optic nerve ischemia. Endothelin-1 (ET-1) in a dosage of 0.1 microg/day was delivered to the perineural region of the anterior optic nerve by osmotically driven minipumps for 8 weeks in 10 rabbits. In 5 rabbits, 1 mg/kg memantine was administered concurrently by intramuscular injection once a daily. Morphologic optic nerve head changes were monitored with a confocal scanning laser ophthalmoscope. Multivariate statistical analysis showed a significant change in topometric parameters (cup area, cup depth and rim volume), indicating an increase in optic nerve head cupping and a decrease of neural rim volume in the ET-1 administered eyes (P < 0.0001). In rabbits where memantine was given concurrently with ET-1, no significant change in topometric parameters was observed after ET-1 administration (P = 0.78). The current results suggest that memantine has a neuroprotective effect in optic nerve ischemia. Memantine may potentially be useful in the management of various ischemic disorders of the optic nerve, including glaucoma.

RESUMO

Complications of amiodarone therapy have been reported in different structures of the eye such as the lids, adnexa, cornea and lens. In a few cases, the optic disc has been affected. These usually have a unilateral effect. We present a rare case of amiodarone therapy with bilateral disc edema, and flame-shaped hemorrhages with classic amiodarone keratopathy [cornea verticillata]. Only a few cases have been reported of Amiodarone toxicity to the optic disc
