Background: Lungs are the second location in frequency of hydatidosis or cystic echinococcosis. Aim: To characterize the clinical manifestations, diagnosis, findings and treatment of patients hospitalized for a pulmonary hydatid cyst (PHC). Materials and Methods: Review of databases, medical records, operation notes and death certificates of patients admitted to a Chilean regional hospital with a PHC. Results: Four hundred twenty-nine cases were identified and complete data was obtained from 368 patients aged 34 ± 19 years, 224 (60,9%)men. The most common clinical manifestations were cough in 269 (73%) and chest pain in 217 (59%). The most frequent locations were the right lung in 210 (57%) and lower lobes in 218 (59%). One hundred eighty-seven cysts (51%) were complicated. Conservative surgery (cystectomy) was performed in 308 (84%). Postoperative morbidity was observed in 77 (21%) and mortality in 6 (2%) patients. Recurrence was observed in 28 (8%) patients. There was a significant reduction in morbidity, mortality, reoperations, and postoperative days over time. Conclusions: In these patients, pulmonary hydatidosis was diagnosed mainly during working ages and half of patients had a complicated cyst.

RESUMO

Abstract Hepatopulmonary hydatidosis in young children is a rare and atypical presentation of Echinococcus granulosus infection. We report the first case of cystic echinococcosis caused by a microvariant of E. granulosus sensu stricto. Chemotherapy and systemic corticoids were administered before curative surgery was performed. Recurrence was not observed for more than 24 months of follow-up.

RESUMO

Hydatid disease is a zoonosis caused by Echinococcus granulosus, and represents a global public health problem. In children the pulmonary location is the most frequent. The hydatid cyst can be asymptomatic and present symptoms when breaking into the bronchial tree or pleura. We present a case of complicated pulmonary hydatid cyst in a 14-year-old girl from Morocco, where this disease is endemic. The initial clinical picture simulated a lung abscess. The management was surgical, associated with albendazole treatment, with a good clinical evolution.


La enfermedad hidatídica es una zoonosis producida por el Echinococcus granulosus, y representa un problema mundial de salud pública. En niños la localización pulmonar es la más frecuente. El quiste hidatídico puede ser asintomático y presentar síntomas al romperse hacia el árbol bronquial o pleura. Presentamos un caso de quiste hidatídico pulmonar complicado en una niña de 14 años residente en Marruecos, donde esta enfermedad es endémica. El cuadro clínico inicial simuló un absceso pulmonar. El manejo fue quirúrgico, asociado a tratamiento con albendazol, con una buena evolución clínica.

RESUMO

Abstract Cardiac hydatid cyst is an uncommon but potentially fatal disease. In cystic Echinococcus humans are an accidental host. Liver and lungs are the most frequently involved organs. Herein a unique case of intramyocardial hydatid cyst of left ventricle along with pulmonary hydatid cyst in a 38-year-old lady is reported. Surgical removal of the cardiac hydatid cyst was done with the aid of cardiopulmonary bypass followed by removal of pulmonary hydatid cyst.

RESUMO

A 13-year-old girl, complained of fever and cough with mucoid expectoration associated with episodes of streaky haemoptysis. Past history was not significant. On examination vital signs were normal with a pulse oximetry saturation of 98 percent. Chest examination revealed decreased breath sounds in the left mammary and infra-mammary area.

Due to the diversity of surgical techniques and great differences in the incidence of pulmonary hydatid cysts around the world, the most appropriate surgical technique has not yet been substantiated. We presented the results of a single surgical technique in a consecutive group of patients and described the technical details. The study was conducted during an 8-year period on 125 patients with a mean age of 33.1 yrs that were suffering from pulmonary hydatid cysts. The surgical procedure included: thoracotomy, opening the cyst, removing all its contents, removal and suturing the bronchial openings. The pericyst cavity left open into the pleural space. Surgical complications, morbidity and mortality rates were evaluated. In addition, the recurrence rate was assessed post-operatively by periodic chest radiographs. There were a total of 181 cysts in 125 patients; 156[86.2%] cysts were operated via the above-mentioned technique and for 25 cysts due to destruction of parenchyma, lobectomy [n=9] or segmentectomy [n=2] was performed. Complications included prolonged air leakage in 4, persistent pleural effusion in 1 and pulmonary embolism in 1patient. There were five recurrences [2.8%] and 1 death due to pneumonia and sepsis. Thoracotomy, evacuation of the endocyst and closure of the bronchial openings comprise an appropriate surgical technique for the treatment of hydatid cysts of the lung

Hydatid disease is a parasitic infestation which is endemic in many sheep and cattle raising areas and is still an important health hazard in the world. The aim of this study was to compare the clinical features, radiologic accuracy, and the surgical approaches of pulmonary hydatid cysts among children and adults in Northwestern Iran. 445 patients treated for pulmonary hydatid cysts over the last ten years, were retrospectively evaluated. The patients were arranged in two groups. There were 383 adults [86%] and 62 children [14%] and their clinical, radiological and surgical characteristics of the pulmonary hydatid cysts were compared. Unlike adults, in children the frequency of pulmonary hydatid cysts in males was significantly [p=0.0036] higher than in females [65% vs. 36%]. Cough was the most common symptom in both groups. The frequency of hemoptysis was more common [p=0.0042] in adults [38%] than in children [19%]. However, giant pulmonary hydatid cysts were more commonly found in children [p < 0.05]. Cystotomy with or without capitonnage was the most preferred method used in both groups. Postoperative complications [pneumothorax] developed in five adults and one child, and were more frequent in noncapitonnage procedures. Only one recurrence was observed in the adult group. The accuracy of chest x-ray was 84% and 74% in children and adults, respectively. Due to the high accuracy of chest x-ray in diagnosis of hydatid cysts of the lung in both groups, it is recommended as the preferred method of diagnosis in endemic regions

RESUMO

In this case report we present a case of large solitary hydatid disease of the lung which was discovered incidentally. The lesion was similar to bullous emphysema with its thin wall and absence of calcification. CT scan examination was useful especially the CT attenuation number of its internal structure which was nearly one Hounsfield unit [HU] indicating the serous nature of its content as compared to -100 HU which was characteristic of air in emphesematous bullae
