El síndrome de fascitis palmar con poliartritis (PFPAS) es un trastorno paraneoplásico infrecuente. Consiste en artritis generalizada de la mano, eritema, tumefacción, rigidez y engrosamiento de la fascia palmar con contracturas progresivas en flexión de los dedos, yse asocia a una enfermedad maligna subyacente. Nodular fasciitis is a benign, reactive myofibroblastic tumor that is often mistaken for a sarcoma because of its histological appearance and rapid growth. Involvement of a finger is extremely rare. We report a case of nodular fasciitis of the thumb, accompanied by bone erosion. Magnetic resonance findings suggested the possibility of a malignancy, which could have led to misdiagnosis as a malignant soft tissue sarcoma. Instead, the lesion was treated by excisional biopsy, which confirmed nodular fasciitis. There has been no evidence of local recurrence at recent follow-up, 1 year after surgery. This case illustrates that, to avoid unnecessarily aggressive surgery, nodular fasciitis must be included in the differential diagnosis for any finger lesion that resembles a sarcoma, even if bone erosion is present.

The aim of this study is to review the spectrum of primary periarthropathies. Rheumatology clinic, Ibn Sina teaching hospital, Mosul, Iraq. Prospective non-experimental clinical study. Two hundred patients having primary periarthropathy were included in the study. Secondary cases of periarthropathy were excluded. Patients with multiple periarthropathy were recorded. Two hundred patients were studied, 114 were females and 33 were males who had one type of periarthropathy; the remaining had combinations of periarthropathies. The mean age for periarthropathy was more than 40, except for tendinitis [36.8 +/- 10.5]. The most common periarthropathies is the shoulder 93 [46.5%], followed by Carpal tunnel syndrome 57 [28.5%] and planter fasciitis 34 [17%], history was significantly high in these three conditions. Various combinations of periarthropathies were seen in the study group. Shoulder periarthropathies and CTS were the most common, 46.5% and 28.5% respectively. The concept of oligoarthropathy and possibly poly-periarthropathy needs further research. Constitutional factors, for example, gender and pro-inflammatory mechanisms need further studies

Introdução: a fasciíte nodular é um processo reacional benigno que ocorre em decorrência da proliferação miofibroblástica, de etiologia desconhecida. É um tumor solitário, indolor e de crescimento rápido. O diagnóstico, geralmente, é um desafio, pois pode ser confundido com tumores malignos, devido o seu comportamento clínico agressivo associado aos achados histológicos. É uma doença auto-limitada e o diagnóstico adequado é essencial para evitar tratamentos agressivos desnecessários. Localiza-se, preferencialmente, nas extremidades superiores e no tronco, sendo a cabeça e pescoço menos acometidos. A maioria dos casos origina-se nos tecidos moles, como fascia, músculos e tecido subcutâneo. O tratamento de escolha é a exérese cirúrgica. Objetivo: relatar o caso de uma paciente com apresentação não usual dessa doença, como seja, na região cervical.


Introduction: nodular fasciitis is a benign reactive myofibroblastic proliferative process of unknown etiology. It presents as a solitary painless nodule with rapid growing. The condition is self-limited, and the proper diagnosis is essential in order to avoid unnecessary aggressive treatment. The diagnosis is often a challenge because it may be confused with a malignant tumor due to its aggressive clinical behavior and histological features. Although most commonly located em the upper extremities and the trunk, in the head and neck region it occurs less frequently. The majority of cases arise in the soft tissue, i. e. fascia, muscle, or subcutaneous tissue. The treatment of choice is the surgical excision. Objective: it is described a case report of a 42-year-old woman with an unusual presentation of that illness.

RESUMO

A 31-yr-old Korean woman was presented with 4-month history of bilateral hand swelling and stiffness. On clinical examination, she had a painful synovitis of both hands, wrists, knees and ankles. The radiologic and histological examinations confirmed it with palmar fasciitis and polyarthritis syndrome (PFPAS). PFPAS is an uncommon disorder characterized by progressive flexion contractures of both hands, inflammatory fasciitiis, fibrosis, and a generalized inflammatory arthritis. Although most reported cases of PFPAS have been associated with various malignancies, our patient have not been associated with malignancy during 24 months follow up period from her first symptom onset. Her symptoms were improved with moderate dose of corticosteroid and she is currently taking prednisone 5 mg daily without any evidence for internal malignancy. We present here in a young Korean patient with idiopathic PFPAS who was successfully treated with administration of corticosteroid.

RESUMO

Eosinophilic fasciitis (EF) is scleroderma-like disease without Raynaud's phenomenon or visceral involvement. It is characterized by painful swelling of the extremities, accompanied by rapid weight gain, fever and myalgia. The acute state of disease is associated with significant peripheral blood eosinophilia, an elevated erythrocyte sedimentation rate and hypergammaglobulinemia. EF is also frequently associated with hematological abnormalities, including malignant lymphoproliferative diseases, but rarely associated with autoimmune thyroiditis. In the present study we report a case of eosinophilic fasciitis associated with autoimmune thyroiditis.

Eosinophilic fasciitis presents with skin thickening involving the extremities, and sparing the hands. Raynaud's phenomenon is usually absent. It can be precipitated by undue exertion and is characterized by eosinophilia and infiltration of skin and subcutaneous tissue by mixed inflammatory infiltrate including eosinophils. We present a young man who developed eosinophilic fasciitis following severe gymnastic activity.
