RESUMO

Pulmonary capillary haemangiomatosis (PCH) is a rare disorder of unknown aetiology, characterised by proliferating capillaries that invade the pulmonary interstitium, alveolar septae and the pulmonary vasculature. It is often mis-diagnosed as primary pulmonary hypertension and pulmonary veno-occlusive disease. Pulmonary capillary haemangiomatosis is a locally aggressive benign vascular neoplasm of the lung. We report the case of a 19-year-old female who was referred to us in the early post-partum period with severe pulmonary artery hypertension, which was diagnosed as PCH by open lung biopsy.

RESUMO

Infantile hemangiomas are the most common benign tumors of infancy. While most of them have an uncomplicated course, between 10 and 15


are at risk for complications, especially during the proliferative phase, and can cause impairment of vital functions or produce permanent cosmetic disfigurement. In this update we delineate the natural history of infantile hemangiomas, with special emphasis on identifying those that require special diagnostic evaluations and multidisciplinary and closer follow-up, reviewing the study and treatment indications and current treatment modalities.

Periocular infantile capillary hemangiomas do not always respond well to conventional treatment modalities such as systemic or intralesional corticosteroids, radiotherapy or debulking surgery. The authors describe the use of intralesional bleomycin injections (IBIs) to treat potentially amblyogenic lesions in two cases where other modalities have failed. In both cases monthly IBIs successfully cleared the visual axis of the affected eye before the age of 1 year thus preventing permanent sensory deprivation amblyopia. A total of five and nine injections, respectively, were used and no significant side effects were noted. IBI appears to be a useful alternative in the treatment of periocular capillary hemangiomas refractory to more conventional modalities.

Los hemangiomas de la infancia (HI) son los tumores vasculares benignos más frecuentes de la niñez. Ocasionalmente requieren tratamiento y, en aquellos en que esté indicada la intervención, se utilizan como terapias sistémicas los corticoides y, más recientemente, el propranolol. En el año 2010, se publicó por primera vez un trabajo sobre el uso de timolol tópico para HI no complicados, superficiales y de tamaño pequeño. Se presentan tres pacientes con HI que fueron tratados con timolol tópico, en los cuales se obtuvo un resultado satisfactorio


Infantile hemangiomas (IH) are the most common vascular benign tumors of childhood. Occasionally, treatment is required and in those in which intervention is indicated, the use of oral steroids and -most recently- propranolol, are established systemic therapies. In 2010 the use of topical timolol for the treatment of uncomplicated, superficial and small sized IH, was first published. We report 3 patients with IH treated with topical timolol showing satisfactory results

RESUMO

A 21-year-old male with von Hippel-Lindau syndrome was treated for a retinal hemangioblastoma with photodynamic therapy [PDT]. We employed the standard PDT protocol with doubled treatment duration. Seven days after the procedure, there were some perilesional intra-and subretinal hemorrhages and significant exudative retinal detachment extending inferiorly. Four weeks later, the hemorrhages had mostly cleared, and diminished subretinal fluid and partial regression of the hemangioblastoma were noted. PDT might induce temporary subretinal fluid accumulation in eyes with retinal hemangiomas

RESUMO

Capillary hemangiomas are the most common periocular and orbital tumors of childhood that typically arise in infancy. Though the diagnosis is frequently made on clinical examination, various diagnostic modalities may be helpful in initial evaluation and follow-up. Tests may be necessary in diagnosing suspect cases or aid in the differentiation of potential malignant tumors. In the vast majority of cases these tumors undergo spontaneous involution without sequelae. However, some periocular and orbital capillary hemangiomas require intervention to prevent serious complications. Other tumors require treatment to lessen the surgical burden for cosmetic repair. When treatment is necessary, there are a number of therapeutic options available. As there is no standard, potential risks and benefits must be discussed with the family and treatment should be specific in each case. A complete understanding of the natural history of the tumor, indications for treatment, and response to different therapies is imperative in managing this common lesion

Enfatizar en la correcta clasificación es aún una alta prioridad en la literatura ya persiste una gran desinformación al respecto. Nuevos progresos en la ciencia han permitido una mejor comprensión de estas lesiones. El avance en el diagnóstico precoz y la experiencia terapéutica han mejorado nuestra habilidad para tratar lesiones extensas y mejorar la calidad de vida de los pacientes.


Emphasis on correct classification is still a high priority in the literature and yet there remains a great deal of misinformation. Many new developments in the basic science of these lesions are allowing better understanding of why these lesions occur while improving our management in these patients. Advances in early diagnosis and therapeutic experience have improved our ability to treat extensive lesions and also improve patients' quality of life.

To review the salient features of periocular capillary hemangioma, provide the ophthalmologist with clinical, diagnostic and histological features characteristic of the tumor and discuss various methods of management. Literature review of periocular capillary hemangioma, diagnostic evaluation with emphasis on treatment through the presentation of illustrative clinical cases. Capillary hemangioma is the most common benign vascular tumour found on the head and neck area including eyelids and orbit. The lesion typically manifests within the first few weeks of life, grows rapidly in the first year during the proliferative phase, then invariably and slowly regresses over the next 4 to 5 years during the patients, however, the functional defects in the form of amblyopia, squint, facial disfigurement and rarely optic atrophy may persist long after complete resolution of the tumor. the diagnosis of capillary hemangioma requires a combination of clinical and imaging studies such as ultrasonography, computerized tomography, magnetic resonance imaging, and angiography in selected cases. With the advent of less invasive diagnostic techniques, the need for biopsy in capillary hemangiomas has decreased. Nevertheless, it should be differentiated from other periocular tumour such as rhabdomyosarcoma, lymphangioma, chloroma, neuroblastoma, orbital cyst, and orbital cellulitis. Treatment is indicated to prevent amblyopia or cosmetic disfigurement. If indicated, intra-lesional corticosteroids may be used to enhance resolution of the tumor. Other forms of treatment tried with variable success include systemic and topical corticosteroids, radiation, surgical excision and intravenous embolization of the tumor. In recent years, laser ablation of the tumor has been found to be effective in some cases. Interferon-u' has been utilized effectively in cases of capillary hemangiomas that are refractory to other forms of therapy. Early treatment of amblyopia is essential to restore good vision in the affected eye. Eyelid and orbital capillary hemangiomas are treatable vascular lesions. Ophthalmologists should be aware of the ocular complicationns of this tumor. Early recognition and timely diagnosis and treatment may prevent amblyopia and cosmetic disfigurement

Introdução: Os hemangiomas são lesões vasculares proliferativas, benignas e relativamente freqüentes na clínica estomatológica. Cerca de 83% dos casos acometem região de cabeça e pescoço. Quando localizados na cavidade bucal, comumente acometem lábios, língua, mucosa jugal e palato. O tipo mais comum é o hemangioma capilar que raramente provoca sintomas significantes. Vários são os tratamentos recomendados, tais como: esclerose química, crio terapia, remoção cirúrgica, laserterapia ou a combinação destes. Método: O trabalho tem como proposta relatar um caso clínico de hemangioma capilar localizado em mucosa jugal, cuja regressão ocorreu após aplicação de um esclerosante químico, o oleato de monoetanolamina a 5% (Ethamolin®), durante intervalos quinzenais. Conclusão: Concluiu-se que a solução possui excelente resultado terapêutico, além de facilidade de uso.


Introduction: Hemangiomas are vascular proliferate benign lesions, frequent in the stomatology clinic. It can happen to any human sex or race, and it's most common during childhood. About 83% of the cases comprise the head and neck region. When located in the buccal cavity, they commonly affect lips, tongue, buccal mucous membrane and the palate. The most common kind is the capillary hemangioma, which rarely causes significant symptoms. There are several kinds of treatments for hemangioma, such as: chemical sclerosis, cryotherapy, surgery, lasertherapy or the combination of these treatments. The purpose of this work is to relate a clinical case of located capillary hemangioma at the buccal mucous membrane, of which reversion occurred after the use of a chemical sclerosing, a monoethanolamine oleate at 5% (Ethamolin®), during biweekly periods. It can be concluded that the solution brings excellent therapeutic results, in addition to being easy to use.

Os autores descrevem um caso de uma paciente portadora de hemangioma capilar da retina que evoluiu com descolamento de retina tradicional, após crioterapia. A vitrectomia via pars plana foi efetiva nessa complicaçäo do hemangioma e a retina apresentou-se colada após 1 ano e 6 meses de seguimento. Säo feitas consideraçöes a respeito das manifestaçöes clínicas e oftalmológicas, complicaçöes e tratamento.

Los hemangiomas son tumores benignos caracterizados por la proliferación de los vasos sanguíneos. El melanoma juvenil o nevo de Spitz es una rara forma de nevo melanocítico benigno que puede ser confundido fácilmente, no sólo con un verdadero melanoma maligno, sino también con lesiones vasculares, tales como el hemangioma capilar. Presentamos el caso de un niño de 8 años con nevo de Spitz en el lóbulo nasal desde el año de edad, que fue diagnosticado y tratado como un hamangioma capilar inmaduo. El estudio histopatológico demostró células melanocíticas epiteloides sin signo de atipia. La lesión fue extirpada quirúrgicamente con resultados estéticos satisfactorios.
