Se expone el caso de un recién nacido que desarrolló sepsis connatal precoz a Streptococcus agalactiae, con meningitis aguda supurada y osteoartritis de rodilla izquierda. Como factor de riesgo la madre no tenía realizado el exudado rectovaginal, pesquisa que detecta la colonización por estreptococo del grupo B (EGB). Se aisló el germen en hemocultivo y en líquido de punción articular. Recibió tratamiento antibiótico adecuado a la sensibilidad del microorganismo y según pauta de sepsis con meningitis, evolucionando favorablemente. En este trabajo se describe la epidemiología de la sepsis neonatal y los cambios ocurridos luego de la implementación de la profilaxis antibiótica en el preparto.


We hereby present the case of a newborn with early connatal sepsis due to Streptococcus agalactiae, with acute suppurative meningitis and left knee osteoarthritis. As a risk factor, the mother had not performed the rectus vaginal exudate screening that detects colonization by Group B Streptococcus (GBS). The germ was isolated in blood culture and in joint puncture fluid. The patient received germ-sensitive antibiotic treatment for meningitis sepsis and evolved favorably. This paper describes the epidemiology of neonatal sepsis and the changes that have occurred after the administration of the antibiotic prophylaxis during pregnancy.


Apresentamos o caso de um recém-nascido com sepse neonatal precoce por Streptococcus agalactiae, com meningite supurativa aguda e osteoartrite de joelho esquerdo. Como fator de risco, a mãe não realizou teste de exsudato vaginal do reto que detecta a colonização por estreptococos do grupo B (SGB). O germe foi isolado em hemocultura e líquido de punção articular. A paciente recebeu tratamento com antibióticos germinativos para padrão meningite sepse e evoluiu favoravelmente. Este artigo descreve a epidemiologia da sepse neonatal e as mudanças ocorridas após a administração da profilaxia antibiótica durante a gravidez.

BACKGROUND: Osteoarthritis (OA) is a health problem affecting millions of individuals worldwide. Aim: To evaluate risk factors for hip and knee osteoarthritis (OA) in women aged 40 to 59 years. MATERIAL AND METHODS: Analysis of a prospective cohort of 1159 women attending preventive health care programs and followed during 28 years. They underwent a clinical and laboratory evaluation from 1990 to 1993. The diagnosis of OA was retrieved from registries of a special program for osteoarthritis in 2020. RESULTS: Twenty four percent of participants developed osteoarthritis during the follow-up. At the beginning of the study and compared with women without OA, they were older (median [interquartile range or IQR]: 49.6 [8.5] and 47.2 [8.2] years respectively), had a higher body mass index (26.3 [5.3] and 25.1 [5.3] respectively), and a higher frequency of jobs with low qualification (76 and 62% respectively). The presence of type 2 diabetes mellitus, chronic hypertension, a previous history of alcohol or cigarette consumption, postmenopausal status and lipid and glucose blood levels did not differ between women with or without OA. Cox regression showed a final model that incorporates body mass index (hazard ratio (HR): 1.04; 95% confidence intervals (CI): 1.01-1.07), age (HR: 1.05; 95% CI: 1.03-1.08) and having an unqualified job (HR: 1.88; 95% CI: 1.43-2.47) as risk factors for OA. CONCLUSIONS: Obesity and the type of job are the most relevant risk factors found for OD: both may be modified with proper care.

SUMMARY: Osteoarthritis (OA) is an inflammatory disease that damages the joints and affects millions of people worldwide. The potential inhibitory effects of the antidiabetic drug metformin combined with captopril, the angiotensin-converting enzyme inhibitor, on diabetes-induced damage to the knee joint articular cartilage associated with the inhibition of glycemia, dyslipidemia, and inflammation has not been investigated before. Therefore, we induced diabetes in rats using high carbohydrate and fat diets and a single injection of streptozotocin (50 mg/kg). The protective group of rats was pre-treated with combined daily doses of metformin (Met; 200 mg/kg body weight) and captopril (Cap; 150 mg/kg body weight) for 14 days before diabetic induction and continued on metformin and resveratrol until the end of the experiment at week 12. Harvested tissues obtained from knee joints were prepared for basic histology staining with haematoxylin and eosin (H&E) and examined under light microscopy. Representative H&E images showed that OA was developed in the diabetic rats as demonstrated by a profound damage to the knee joints such as irregular eroded and a sharp decrease in the thickness of the articular cartilage surface and abnormal remodeling of the subchondral bone that were substantially ameliorated by Met+Cap. Met+Cap also significantly (p< 0.05) reduced blood levels of glucose, glycated hemoglobin (HbA1c), dyslipidemia, and the inflammatory biomarkers, high sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), and tumour necrosis factor-alpha (TNF-α) induced by diabetes. In addition, a significant (p≤ 0.0014) correlation between the articular cartilage thickness and the blood levels of glucose, HbA1c, triglyceride (TG), low density lipoprotein-cholesterol (LDL-C), high density lipoprotein- cholesterol (HDL-C), and hs-CRP were observed. Thus, we demonstrate that Met+Cap effectively protect the knee joint against injuries induced secondary to diabetes in rats, possibly due to the inhibition of glycemia, dyslipidemia, and biomarkers of inflammation.


RESUMEN: La osteoartritis (OA) es una enfermedad inflamatoria que daña las articulaciones y afecta a millones de per- sonas en todo el mundo. No se han investigado los posibles efectos inhibidores del fármaco antidiabético metformina combinado con captopril, el inhibidor de la enzima convertidora de angiotensina, sobre el daño inducido por la diabetes en el cartílago articular de la articulación de la rodilla asociado con la inhibición de la glucemia, dislipidemia e inflamación. En este estudio fue inducida la diabetes en ratas con dietas altas en carbohidratos y grasas y una sola inyección de estreptozotocina (50 mg / kg). El grupo protector de ratas se pretrató con dosis diarias combinadas de metformina (Met; 200 mg / kg de peso corporal) y captopril (Cap; 150 mg / kg de peso corporal) durante 14 días antes de la inducción diabética. El tratamiento se continuó con metformina y resveratrol hasta el final del experimento en la semana 12. Los tejidos obtenidos de las articulaciones de la rodilla se prepararon para la tinción de histología básica con hematoxilina y eosina (H&E) y se examinaron con microscopía óptica. Imágenes representativas de H&E mostraron que la OA se desarrolló en las ratas diabéticas, como lo evidencia un daño profundo en las articulaciones de la rodilla, como la erosión irregular y una fuerte disminución en el grosor de la superficie del cartílago articular y remodelación anor- mal del hueso subcondral que fueron mejorados sustancialmente por Met + Cap. Met + Cap. También redujo significativamente (p <0.05) los niveles sanguíneos de glucosa, hemoglobina glicosilada (HbA1c), dislipidemia y los biomarcadores inflamatorios, proteína C reactiva de alta sensibilidad (hs-CRP), interleucina-6 (IL-6), y factor de necrosis tumoral alfa (TNF-α) inducido por diabetes. Además, una correlación significativa (p≤ 0,0014) entre el grosor del cartílago articular y los niveles sanguíneos de glucosa, HbA1c, triglicéridos (TG), lipoproteínas-colesterol de baja densidad (LDL- C), lipoproteínas de alta densidad-colesterol (HDL-C) ) y hs-CRP. Así, demostramos que Met + Cap protege eficazmente la articulación de la rodilla contra lesiones inducidas por diabetes en ratas, posiblemente debido a la inhibición de la glicemia, dislipidemia y biomarcadores de inflamación.

Background: Obesity is associated with pain, reduction of function and quality of life in patients with osteoarthritis (OA). Aim: To describe the clinical profile of women with knee OA according to their body mass index (BMI). Material and Methods: Observational study in 308 women with knee OA. According to their BMI, they were classified as normal-weight, overweight and obese. The primary outcome measure was functionality evaluated with the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Secondary outcomes were sleep quality evaluated using the Pittsburgh Sleep Quality Index (PSQI) and quality of life assessed with the European Quality of Life Five Dimension (EuroQol-5D). Results: WOMAC, PSQI and EuroQol-5D scores were significantly higher in obese women. Conclusions: Overweight and obese women with OA have more sleep disorders, reduction on functionality and quality of life compared to their normal weight counterparts.

Introducción: la artrosis de rodilla puede llegar a ser una patología muy invalidante por sus síntomas, caracterizados por dolor, inseguridad y pérdida funcional. Es una patología degenerativa que ha aumentado su prevalencia en las últimas décadas, muy ligada al envejecimiento poblacional. Objetivo: validar la técnica de la ostectomía del peroné en el genu varo doloroso como nueva opción de tratamiento. Materiales y Métodos: se realizó un estudio, prospectivo y lineal en el Hospital Militar "Dr. Mario Muñoz Monroy" de Matanzas, desde abril del 2016 hasta agosto del 2017. Se aplicó la nueva técnica del Dr. Ying-Ze Zhang, del Departamento de Cirugía Ortopédica del Tercer Hospital de la Universidad Médica de Hebei en Shijiazhuang, China. En la actualidad ya existe una casuística de 75 pacientes, con 84 rodillas operadas de los cuales se presentan los resultados de los primeros 11 pacientes, posterior al año de operado. Resultados: el promedio de edad fue de 64,3 años, (45 años el menor y 84 años el mayor); de los cuales 8 eran hombres y 3 mujeres. Se aplicó la escala visual analógica para el dolor pre y post-operatorio en cuanto a la marcha y al subir escalones; estando todos los pacientes al caminar, por encima de 6 y al subir escaleras por encima de 7. Después de un año de operados 10 pacientes se encontraban evaluados al caminar entre 0 y 3 puntos, y al subir escalones 9 en igual puntuación; 1 en 5, y solamente 1 paciente mantuvo igual puntuación antes de operarse. Valorando estas puntuaciones se evaluaron 9 pacientes de bien, 2 de regular, y se presentaron 3 complicaciones. Conclusiones: al año de seguimiento ocurre gran mejoría del dolor, mejorando la seguridad del paciente durante la marcha (AU).


Introduction: the fibular osteoarthritis could be a very invalidating disease due to its symptoms, characterized by pain, insecurity and functional loss. It is a degenerative disease whose prevalence has increased during the last decades, tightly linked to population ageing. Objective: to validate the technique of fibular ostectomy in the painful genu varum as a new treatment option. Materials and Methods: a prospective, lineal study was carried out in the Military Hospital "Dr. Mario Muñoz Monroy", of Matanzas, from April 2016 to August 2017. It was used the new technique of Dr. Ying-Ze Zhang, from the Department of Orthopedic Surgery of the Third Hospital of Hebei Medical University in Shijiazhuang, China. Currently there is a series of cases of 75 patients, with 84 operated knees; the results of the first 11 operated patients, are presented here after a year of the surgery. Results: the average age was 64.3 years, 45 years the youngest and 84 years the eldest; from them, 8 were men and 3 women. The analogical visual scale for the pre and post-surgical pain was applied during the gait and when going upstairs. During the gait all the patients were above 6, and when climbing upstairs above 7. After a year from the operation 10 patients got an evaluation between 0 and 3 points during the gait, and 9 got the same score when climbing steps; one got 5, and only 1 patient kept the same score than before the operation. Taking into account these scores, 9 patients were evaluated as good, 2 regular, and there were 3 complications. Conclusions: after a one-year follow-up, the pain greatly improves, improving patients' security during the gait (AU).

ABSTRACT Background: High body mass index, as well as maintaining this condition for a long period of time, are important risk factors for the development of osteoarthritis. Aim: To determine joint pain and osteoarthritis prevalence in patients referred to bariatric surgery. Methods: Morbidly obese patients referred to bariatric surgery responded to the visual analogue pain scale (VAS) and the WOMAC questionnaire. X-rays of the hips and knees were evaluated. The primary endpoints were self-reported joint pain and the diagnosis of osteoarthritis by clinical and radiological criteria of the American College of Rheumatology. Results: 141 patients were interviewed (85.1% women) with a mean age of 40 years. The mean body mass index was 46. The lumbar spine and knee joint were the most commonly reported as painful (77.9% and 73.2% respectively). Prevalence of knee osteoarthritis was 63.1% and hip osteoarthritis was 40.8%. Age, mean VAS and WOMAC scores were higher in the osteoarthritic individuals. Conclusion: There is prevalence of 90.1% of pain symptoms in morbidly obese patients referred to bariatric surgery. The prevalence of knee osteoarthritis was 63.1% and hip osteoarthritis was 40.8% in this sample.


RESUMO Racional: Alto índice de massa corpórea, assim como a manutenção desta condição por longo período de tempo, são importantes fatores de risco para o desenvolvimento de osteoartrite. Objetivo: Determinar a prevalência de dor articular e osteoartrite em pacientes aguardando cirurgia bariátrica. Métodos: Pacientes obesos mórbidos responderam à escala e questionário (VAS e WOMAC) de dor e função. Radiografias dos quadris e joelhos foram avaliadas. Os desfechos primários foram dor articular referida nos questionários e o diagnóstico de osteoartrite feito através dos critérios clinicoradiológicos do Colégio Americano de Reumatologia. Resultados: Cento e quarenta e um pacientes foram entrevistados (85,1% mulheres) com idade média de 40 anos. A média do índice de massa corpórea foi de 46. Coluna lombar e joelhos foram as regiões mais comumente referidas com dor (77,9% e 73,2% respectivamente). A prevalência de osteoartrite dos joelhos foi de 63,1% e dos quadris foi de 40,8%. Idade, média da escala visual de dor e resultados do questionário de WOMAC foram maiores nos indivíduos com osteoartrite. Conclusão: Há prevalência de 90,1% de sintomas dolorosos nos pacientes obesos mórbidos encaminhados para cirurgia bariátrica. A osteoartrite dos joelhos foi de 63,1% e de quadris de 40,8%.

Summary Objective: To evaluate the impact of morbid obesity on physical capacity, joint-related symptoms, and on the overall quality of life. Method: Cross-sectional study carried out at a university hospital, enrolling 39 individuals admitted to a bariatric surgery service. Physical capacity was assessed by Six-Minute Walk Test (SMWT) and the Borg rating of perceived exertion (RPE). Knee-related symptoms were evaluated by Knee Injury and Osteoarthritis Outcome Score (KOOS) and the Lysholm Score. Quality of life was evaluated by Short Form 36 Health Questionnaire (SF-36). Results: On SMWT, the mean distance walked was 374.1±107.5 m. The mean Borg score was 12.9±2.4. KOOS questionnaire found the following scores: pain (64.3±24), other symptoms (67.2±25.5), function in daily living (60.4±26.8), function in sport and recreation (28.5±32.2), knee-related quality of life (35.9±33.5), mean Lysholm scale score (55.3±25.4). SF-36 provided the following scores: physical functioning (41±27.4), physical role functioning (34.6±39.2), bodily pain (45.7±23.6), general health perceptions (63.1±26.2), vitality (53.5±12.1), social role functioning (52.6±29.3), emotional role functioning (41±44.9), mental health (55±27.7). Conclusion: Obesity led to significant loss of physical capacity, gait impairment, knee-related symptoms, and a negative impact on the overall quality of life.


Resumo Objetivo: Avaliar o impacto da obesidade mórbida sobre a capacidade física, sintomas osteoarticulares e qualidade de vida global. Método: Estudo transversal realizado em hospital universitário, envolvendo 39 indivíduos admitidos em um serviço de cirurgia bariátrica. A capacidade física foi avaliada através do teste de caminhada de 6 minutos e pela escala de percepção de esforço de Borg. Os sintomas relacionados ao joelho foram avaliados pelos escores de KOOS e Lysholm; a qualidade de vida foi avaliada por meio do questionário SF-36. Resultados: No teste de caminhada de 6 minutos, a distância média foi de 374,1±107,5 m. O escore médio de Borg foi 12,9±2,4. Os seguintes escores foram observados no KOOS: dor (64,3±24); outros sintomas (67,2±25,5); atividades da vida diária (60,4±26,8); atividades esportivas e lazer (28,5±32,2); qualidade de vida (35,9±33,5); o escore de Lysholm médio foi 55,3±25,4. O SF-36 mostrou estes escores: capacidade funcional (41±27,4); limitação por aspectos físicos (34,6±39,2); dor (45,7±23,6); estado geral de saúde (63,1±26,2); vitalidade (53,5±12,1); aspectos sociais (52,6±29,3); aspectos emocionais (41±44,9); saúde mental (55±27,7). Conclusão: A obesidade levou à perda significativa de capacidade física, a prejuízo à marcha, a sintomas relacionados ao joelho e a impacto negativo sobre a qualidade de vida global.

CONTEXT AND OBJECTIVE: The development of a slow and progressive mechanical model for osteoarthritis is important for correlation with clinical practice, and for evaluating the effects of disease-modifying medications. A mechanical osteoarthritis model was developed to evaluate the effects of intra-articular hyaluronic acid (HA) injection and oral diacerein administration. DESIGN AND SETTING: Experimental study at the Department of Orthopedics and Traumatology, Universidade de São Paulo. METHOD: Total medial meniscectomy was performed on seven groups of ten Wistar rats each, comprising four control groups (C) and three study groups (S). C.I: operated, non-medicated; C.II: operated, injections of HA vehicle; C.III: non-operated, non-medicated; C.IV: operated, non-medicated, sacrificed three months post-meniscectomy; S.I: operated, receiving intra-articular HA injections; S.II: operated, oral diacerein from the third to the seventh postoperative month; S.III: operated, received both medications. All the animals (except C.IV) were sacrificed seven months post-meniscectomy. All femurs and tibias were assessed histologically. CONTEXTO E OBJETIVO: Desenvolver um modelo osteoartrítico mecânico lento e progressivo é importante para correlação com a prática clínica e para avaliar os efeitos de medicamentos modificadores da doença. Um modelo mecânico de osteartrite foi desenvolvido para avaliar os efeitos de injeção intra-articular de hialuronato de sódio (AH) e de administração de diacereína oral. DESENHO E LOCAL: Estudo experimental no Departamento de Ortopedia e Traumatologia, Universidade de São Paulo. MÉTODO: Meniscectomia medial total foi feita em sete grupos de dez ratos Wistar, sendo quatro grupos controle (C) e três grupos estudo (E). C.I: operado, não medicado; C.II: operado, recebendo injeções do veículo do AH; C.III: não operado, não medicado; C.IV: operado, não medicado, sacrificado três meses pósmeniscectomia; EI: operado, recebendo injeções de AH intra-articular; E.II: operado, recebendo diacereína oral do terceiro ao sétimo mês pós-operatório; E.III: operado, recebeu ambas medicações. Introducción: la artrosis del compartimento patelofemoral es observada con frecuencia en pacientes con gonartrosis, entre sus síntomas y signos destacan el dolor y la sensación de inestabilidad. Objetivo: actualizar y profundizar en el conocimiento de aspectos esenciales como: epidemiología, factores predisponentes, imagenología, diagnóstico diferencial y tratamiento en pacientes con artrosis patelofemoral. Mètodos: se revisaron 200 artículos publicados en Pubmed, Hinari y Medline mediante el localizador de información Endnote con la palabra clave patellofemoral osteoarthritis, se utilizaron 52 citas, 46 de ellas de los últimos cinco años. Resultados: se abordan aspectos de gran interés desde el punto de vista epidemiológico acerca de la frecuencia con que se afecta el compartimento patelofemoral y la diferencias entre la condromalacia de rótula y la artrosis patelofemoral. Se describen los factores predisponentes entre los que se destaca, la displasia patelofemoral. Los exámenes imagenológicos se describen en detalle, en especial los relacionados con las vistas axiales de rótula y se menciona la importancia de la gammagrafía ósea. Se nombran las modalidades más usadas y sus principales indicaciones. Conclusiones: la presente revisión aporta elementos actuales para el tratamiento de enfermos con artrosis patelofemoral y aclara las diferencias entre dos entidades que se confunden con frecuencia en la práctica médica(AU)


Introduction: arthritis of the patellofemoral compartment is frequent in patients with gonarthrosis. Its signs and symptoms include pain and a feeling of instability. Objective: update and broaden knowledge on essential aspects such as epidemiology, predisposing factors, imaging, differential diagnosis and treatment of patients with patellofemoral osteoarthritis. Methods: a review was conducted of 200 papers published by Pubmed, Hinari and Medline using the information locator EndNote with the key term "patellofemoral osteoarthritis". Fifty-three citations were used, 46 of them from the last five years. Results: a discussion is provided of topics of great epidemiological interest, such as the frequency with which the patellofemoral compartment is involved, and the differences between patellar chondromalacia and patellofemoral osteoarthritis. A description is included of predisposing factors, among them patellofemoral dysplasia. Imaging studies, especially those based on axial patellar views, are described in detail. Reference is made to the importance of bone scintigraphy. A description is presented of the most common treatment modes and their main indications. Conclusions: the present review provides updated information about the treatment of patients with patellofemoral osteoarthritis, and sheds light on the differences between two conditions which are often confused in medical practice(AU)


Introduction: on trouve d'habitude une arthrose du compartiment fémoro-patellaire chez les patients atteints de gonarthrose. Les symptômes et signes comprennent la douleur et la sensation d'instabilité. Objectif: le but de cette révision est d'actualiser et d'approfondir les connaissances sur des aspects essentiels tels que l'épidémiologie, les facteurs de prédisposition, l'imagerie, le diagnostic différentiel et le traitement de l'arthrose fémoro-patellaire. Méthodes: On fait une révision de 200 articles publiés à Pubmed, Hinari et Medline, en utilisant le localisateur d'information Endnote avec le mot clé patellofemoral osteoarthritis. Cinquante-trois, dont 46 ont été publiés dans ces derniers cinq ans, sont cités. Résultats: On aborde des aspects très intéressants du point de vue épidémiologique à propos de la fréquence dans laquelle le compartiment fémoro-patellaire est touché, des différences entre la chondromalacie de la rotule et l'arthrose fémoro-patellaire. Les facteurs de prédisposition, tels que la dysplasie fémoro-patellaire, sont décrits. Les examens d'imagerie sont décrits en détail, notamment ceux qui sont en rapport avec les vues axiales de la rotule. On fait mention de l'importance de la gammagraphie osseuse. Quant au traitement, les techniques les plus fréquemment utilisées et leurs indications principales sont définies. Conclusions: La présente révision met en relief les éléments actuels sur le traitement des malades atteints d'arthrose fémoro-patellaire, et fait des distinctions entre les deux conditions fréquemment confondues dans la pratique médicale(AU)

OBJECTIVE: Body mass index (BMI) and knee osteoarthritis have a strong association, but other anthropometric measures lack such associations. To date, no study has evaluated non-obese knee osteoarthritis to negate the systemic and metabolic effects of obesity. This study examines the validity of the contention that BMI and other anthropometric measures have a significant relationship with knee osteoarthritis. METHODS: In total, 180 subjects with a diagnosis of knee osteoarthritis were recruited and classified according to Kellgren-Lawrence (KL) grades. Body mass index, mid-upper arm circumference, waist-hip ratio and triceps-skinfold thickness were recorded by standard procedures. Osteoarthritis outcome scores (WOMAC) were evaluated. RESULTS: (1) In both genders, the BMI was significantly higher for KL grade 4 than for grade 2; triceps-skinfold thickness was positively correlated with the joint space width of the tibial medial compartment. (2) In males, triceps-skinfold thickness significantly increased as the KL grades moved from 2 to 4; the significantly higher BMI found in varus aligned knees was positively correlated with WOMAC scores. (3) In females, the waist-hip ratio was significantly higher for KL grade 4 than for grade 2; a significant correlation was found between BMI and WOMAC scores. The waist-hip ratio was significantly associated with varus aligned knees and it positively correlated with WOMAC scores and with the joint space width of the tibial medial compartment. The mid-upper arm circumference demonstrated no correlation with knee osteoarthritis. CONCLUS'ON: This study validates the contention that BMI and other anthropometric measures have a significant association with knee osteoarthritis. Contrary to common belief, the triceps-skinfold thickness (peripheral fat) in males and the waist-hip ratio (central fat) in females were more strongly associated with knee osteoarthritis than BMI.

O envelhecimento tem sido motivo de preocupação e questionamentos constantes dentro de um contexto sócio-econômico e cultural mundial. No Brasil, devido o 2 chatamento da pirâmide demográfica consequente ao declive na taxa de mortalidade e também à diminuição na taxa de natalidade, a política de saúde tem sido levada a enfocar atenção integral à saúde do idoso dentro do sistema de saúde brasileiro, mesmo considerando suas limitações. Além disso, há um constante progresso em relação à busca de altemativas para que se melhorem as condições de vida ou de sobrevivência dos idosos. A osteoartrose (DA) em joelhos em indivíduos acima de 60 anos é fator límitante na qualidade de vida dos mesmos, impossibilitando-os de realizar suas atividades de vida diária de forma qualitativa e quantitativa. Sendo assim, nesta pesquisa foi estudada a prevalência de DA de joelho em uma amostra populacional de idosos residentes em Botucatu, São Paulo, com o intuito de possibilitar uma avaliação da sua autonomia em relação a esse aspecto. Para tanto, utilizou-se instrumento validado na literatura e adaptado para a nossa realidade. Foram entrevistados 355 idosos. O processo amostra foi o de sorteio dos idosos para prevalência desconhecida a partir de familias previamente selecionadas na amostragem sistemática, após atualização do censo do ano de 2002, com a localização das famílias (uma a cada quatro) através do mapa residencial da cidade. Essa população de idosos foi entrevistada três anos após o sorteio e, por este motivo, foi feita a reposição de perdas e óbitos de 80 idosos, estratificados por idade e sexo. Os idosos que na entrevista domiciliar referiram dor em joelhos foram encaminhados para exame clínico. Foi encontrado que 64% dos idosos referiram dor em joelhos e, destes, 98,6% apresentaram dor ou rigidez matinal e protocinética, com dificuldades acentuadas para realizar as suas atividades de vida diária...

A osteoartrite é a forma mais prevalente de doença articular. Os agentes físicos como o gelo e o calor podem combater o processo álgico quando corretamente indicados e utilizados. O objetivo deste trabalho foi a comparação de protocolos de tratamento fisioterapêutico, que envolveram o uso de cinesioterapia, crioterapia e ondas curtas, em indivíduos com osteoartrite de joelho. Em um estudo randomizado prospectivo cego foram tratados 25 indivíduos (25 joelhos), de 58 a 78 anos. GRUPO A: cinesioterapia e ondas curtas (n=9). GRUPO B: cinesioterapia e gelo (n=6). GRUPO C: cinesioterapia (grupo controle) (n=9). As variáveis analisadas foram: percepção subjetiva de dor (Borg), qualidade funcional (Lequesne), amplitude de movimento, flexibilidade e força muscular. A dor melhorou significativamente somente no grupo B, com qualidade funcional e flexibilidade positiva em todos grupos. O ganho de amplitude foi semelhante nos grupos B e C. Houve manutenção da força flexora nos grupos A e B e ganho nos indivíduos do grupo C. Na musculatura extensora observou-se ganho de força no grupo B e C e perda no grupo A. O melhor protocolo foi aquele que envolveu a aplicação de gelo e cinesioterapia para analgesia; não houve relação de ganho de amplitude, flexibilidade e força associado a termoterapia.


Osteoarthritis is the most prevalent form of joint disease. Physical agents such as ice and heat can fight the pain process, when correctly indicated and used. The objective of this study was to compare physiotherapy protocols involving the use of exercise, cryotherapy and short waves in individuals with knee osteoarthritis. In a prospective randomized study, 25 individuals were treated (25 knees), with ages ranging from 58 to 78 years. GROUP A: exercise and short-waves (n=9). GROUP B: exercise and ice (n=6). GROUP C: exercise alone (control) (n=9). Analyzed variables were: subjective perception of pain (Borg), functional quality (Lequesne), range of motion, flexibility and muscular strength. Pain significantly improved only for group B, with positive functional quality and flexibility in all groups. The improvement in range of motion was similar for groups B and C. Flexion strength was maintained for groups A and B, and strength gain for individuals in group C. In the extensors muscles, strength gain was seen in groups B and C, while strength loss was reported for group A. The best protocol was that one involving cryotherapy and exercises for pain relief. All groups showed improved functional quality; no correlation was found for range of motion gain, flexibility and strength gain associated with thermotherapy.

The etiology of posttraumatic arthritis has not been studied well. Although it has long been assumed that articular incongruity leads to post traumatic arthritis, clinical and basic sciences evidence to support such claim are not sufficient. On the other hand, the relationship of instability due to anterior cruciate ligament deficiency and Arthrosis has not been clearly defined and the relative importance of instability and incongruity is unknown. ACL tearing were created in right knees of 9 rabbits [group A] and ACL tearing and displaced lateral condylar fracture with 1 mm step were created in another 9 rabbits [group B]. After 6 months, osteoarthritis changes were compared between control knee and injured knee of each rabbit and between the two groups. In both groups, osteoarthritis changes in injured knees were more comparing to the control knee and in whole osteoarthritis changes were significantly more in group B compared to group A. The unstable knee due to ACL deficiency goes toward osteoarthritis faster than normal knee. Incongruity in unstable joint increases intensity of osteoarthritis changes. It shows the importance of achieving anatomic reduction in intra articular fractures before ligament reconstruction

To find out the prevalence and relation between osteoarthritis of knees and obesity in Al-Ahsa region, Kingdom of Saudi Arabia [KSA]. The study included 243 male and female patients diagnosed with osteoarthritis of knees between June 2001 to March 2003. All patients were recruited from the Physical Therapy Department, King Fahd Hofuf Hospital, Hofuf, KSA. The clinical diagnosis was supported by plain x-rays of knees, and of other joint if needed. The weight and height of all patients were taken using one standard weight and height scale, and body mass index was also calculated and recorded. More than 90.53% of the patients referred with osteoarthritis of the knees were obese or overweight. The mean body weight of all patients was 84.61 kg and the mean height was 1.59 meters. Osteoarthritis of the knees was more common in obese female than male patients with a female to male ratio of 2.37:1. Obesity is a disease. The aim of all health professionals and others in the community should be directed to the prevention of this disease and its risk to develop multiple complications

to describe the pattern of primary osteoarthritis of the knee in Jordanians, including age, sex, and relation to obesity and to assess the severity of pain and extent of disability in these patients. this is prospective study involving 74 patients with primary osteoarthritis of the knee, who met the American College of Rheumatology criteria for the classification and reporting of osteoarthritis of the knee. The patients were seen over the period between June 1999 and February 2000, in the Rheumatology and Internal Medicine Clinics at Prince Ali Hospital. These patients were evaluated in relation to age, sex, body weight, bilaterality, and the severity of pain, which was assessed according to Hernborg and Nilsson criteria. The effect of osteoarthritis on normal joint function and the extent of disability were assessed by a local modified classification used by other authors. the majority of patients [77.1%] were above 55 years of age. Most [75.7%] were females in a ratio of 3.1: 1 [females:male], and 90.5% were overweight and obese. Only four patients [males] had unilateral knee involvement. Almost half of the patients achieved four or more scores out of five, on the Hernborg and Nilsson criteria for pain. Most [86.5%] of the patients could walk outside unaided but with pain. Half of the patients could do their daily activity but with difficulty. the pattern of osteoarthritis of the knee in Jordanians is almost similar to that in other parts of the world. There are, however, some differences including the earlier age of onset of the disease, the earlier peak age of prevalence and the variation in the female to male ratio
