RESUMO

Sparganosis is a rare parasitic disease caused by migrating plerocercoid tapeworm larva of the genus Spirometra. Infection in humans is mainly caused by the ingestion of raw or inadequately cooked flesh of infected frogs, snakes, and chickens. Here, we report a rare case of a 45-year-old man who was admitted to our hospital with left lower chest pain. The chest radiograph and computed tomography (CT) scan revealed localized pleural effusion in the left lower lobe; further, peripheral blood eosinophilia and eosinophilic pleural effusion were present. Percutaneous catheter drainage was performed, which revealed long worm-shaped material that was identified as a sparganum by DNA sequencing. The patient showed clinical improvement after drainage of the sparganum. This study demonstrates the importance of considering parasitic diseases in the differential diagnosis of eosinophilic pleural effusion.

RESUMO

Differential diagnosis between Tuberculosis pleurisy and Non Tuberculoses pleural effusion can be sometimes difficult and needs invasive diagnostic methods. In recent years, several biological markers have been found to be diagnostic markers of Tuberculosis pleurisy. The aim of this study was assessment of the value of Gamma-Interferon in the diagnosis of TB pleurisy. We studied 70 patients with pleural effusion who had been admitted to Ghaem hospital, Mashhad University of Medical Sciences, Mashhad, Iran. Interferon gamma level was measured by "ELISA' method. The diagnosis of tuberculosis was based on microbiologic study of pleural fluid plus pleural biopsy. Altogether Twenty patients had tuberculosis pleurisy [44.44%], 21 patients had malignant pleural effusion [46.67%] and 4 patients had pleural effusion with various non TB non malignant etiologies [8.9%]. The sensitivity, specificity of Gamma-Interferon in TB pleurisy were 60% and 96% respectively. INF gamma is a valuable indicative marker for diagnosis of tuberculosis pleurisies' but we should consider situations with high level of INF gamma without any evidence of TB and also Tuberculosis pleurisy patients with INF gamma level lower than diagnostic threshold

RESUMO

La tuberculosis (TBC) es producida por el Mycobacterium tuberculosis. Es escrofuloderma es una de sus formas cutáneas e integra el grupo de las TBC colicuativas. El empiema tuberculoso puede ocasionar inflamación de los tejidos de la peripleura o “peripleuritis”, con formación de abscesos fríos peripleuríticos; en su evolución puede abrirse camino a través de los planos profundos hacia piel, constituyendo el denominado empiema por necesidad, que se manifiesta clínicamente con tumoraciones cutáneas que pueden evocar lipomas. Se presenta el caso de un paciente con coexistencia de escrofuloderma a focos múltiples (óseo y ganglionar) y empiema por necesidad.

Pasteurella multocida tiene cuatro diferentes tipos capsulares (A, B, D y E); A y D son los que caudan problemas en cerdos. El tipo A causa neumonía y el D, rinitis; este tipo capsular produce tpxona, que destruye los cornetes nasales. En 1983, se aislaron dos cepas de P. multocida tipo A en dos granjas con problemas respiratorios, como pleuritis, pericarditis, neumonía fibrinosa y pequeños abscesos. Estas cepas de P. multocida se identificaron con A52 y A59, las que se observaron también al microscopio electrónico observándose pequeñas microvellocidades en la superificie. En inoculaciones experimentales con estos tipos, en relación con los que normalmente ocasionan problemas respiratorios en cerdos, se observó que tales cepas desencadenan problemas más severos de tipo pleuroneumónico abscedativo.

RESUMO

Fifteen out of 612 acute myocardial infarction patients who had onset of features of post-myocardial infarction (Dressler's) syndrome between the 3rd and 21st post-infarction days were studied. All had pericardial rub and raised ESR, 86.6% had fever, 73.3% had arthralgia and 66.7% had pleuro-pericardial pain. Early onset Dressler's syndrome (EODS) was found to be more common in anterior wall myocardial infarction, to occur earlier in second infarctions and in diabetics, and to occur most commonly between the 6th and 10th post-infarction days. It was concluded that EODS is an occasional cause for recurrent chest pain after acute myocardial infarction.

RESUMO

Se describe un caso de artritis reumatoide juvenil poliarticular seronegativo con neumopatía restrictiva como manifestación de la primera. Se revisa literatura médica al respecto y métodos para determinar el diagnóstico, haciendo hincapié en la necesidad de investigar la función pulmonar en todo paciente con artritis reumatoide, aun cuando permanezca asintomático. No es frecuente sin embargo la presentación de esta variedad clínica
