Resumen: Objetivo: Describir las características clínicas, resultados operatorios inmediatos y a 5 años de la cirugía de reparación valvular mitral. Material y métodos: Estudio retrospectivo de la cohorte de pacientes operados de reparación valvular por insuficiencia mitral (IM) en el Hospital Guillermo Grant Benavente desde el 2009 hasta marzo 2020 (N=206). Se comparan los pacientes con IM primaria y secundaria en sus características clínicas, ecocardiográficas, técnicas quirúrgicas empleadas, cirugías asociadas, morbimortalidad operatoria, sobrevida y reintervenciones hasta el 30 de mayo de 2020. Resultados: 124(60,2%) hombres. Edad media 62,6±10,5 años. La IM fue primaria en 134 y secundaria en 72. En comparación con los pacientes portadores de IM primaria, aquellos con IM secundaria tuvieron más enfermedad coronaria (69,4% versus 11,9%; p<0,001) y mayor riesgo operatorio (EuroSCORE logístico 7,7±6,7 versus 5,2±7,3; EuroSCORE II 3,4±4,8 versus 2,4±4,7; p<0,001). El mecanismo más frecuente de IM primaria fue tipo II (65,7%) y en las secundarias fue el tipo III (48,6%) seguido del tipo I (30,6%). Las IM primarias se corrigieron principalmente con procedimientos para disminuir el prolapso (76,1%). En las secundarias la técnica más utilizada fue el implante de un anillo exclusivo (76,4%). Hubo 116 cirugías asociadas y 10 (4,9%) conversiones a reemplazo valvular. Hubo 57 complicaciones operatorias y fallecieron 12 (5,8%) pacientes, 5 (3,7%) con IM primaria y 7(9,7%) con IM secundaria. La sobrevivencia global a 5 años fue 83,5% (90% en las primarias y 78% en las secundarias) y hubo 6 reintervenciones. Conclusiones: La cirugía de reparación valvular, tanto en pacientes con IM primaria como secundaria, tuvo una baja mortalidad operatoria y excelentes resultados a 5 años.


Abstract: Aim: To describe the clinical characteristics, operative and long term results of surgical mitral valve repair. Material and methods: Retrospective study of the cohort of patients undergoing valve repair due to mitral regurgitation (MR) at the Guillermo Grant Benavente Hospital from 2009 to March 2020 (N = 206). Patients with primary and secondary MR were compared on clinical and echocardiographic characteristics, surgical techniques, associated surgeries, operative morbidity and mortality, survival and reinterventions up to May 30, 2020. Results: 124 (60.2%) were men. The average age was 62.6±10.5 years. Type of MR was primary in 134 and secondary in 72. Compared to patients with primary MR, those with secondary MR had more coronary artery disease (69.4% versus 11.9%; p <0.001) and greater operative risk (logistic EuroSCORE 7.7±6, 7 versus 5.2±7.3; EuroSCORE II 3.4±4.8 versus 2,4±4.7; p<0.001). The most frequent mechanism of MR was type II in primary (65.7%) and type III (48.6%) followed by type I (30.6%) in secondary MR. Primary MR was corrected mainly with procedures to decrease prolapse (76.1%). In secondary MR the main technique used was the implantation of an exclusive ring (76.4%). There were 116 associated surgeries and 10(4.9%) conversions to valve replacement. There were 57 operative complications and 12(5.8%) patients died, 5 (3.7%) with primary MR and 7 (9.7%) with secondary MR. Overall survival at 5 years was 83.5% (90% in primary MR and 78% in secondary MR) and there were 3 reoperations. Conclusions: Valve repair surgery in both primary and secondary MR patients was associated to a low operative mortality and excellent results at 5 year post surgery.

Abstract Introduction: Acute aortic dissection (AAD) is a devastating surgical emergency, with high operative mortality. Several scoring algorithms have been used to establish the expected mortality in these patients. Our objective was to define the predictive factors for mortality in our center and to validate the EuroSCORE and Penn classification system. Methods: Patients who underwent surgery for AAD from 2006 to 2016 were retrieved from the institution's database. Preoperative, operative and postoperative variables were collected. Observed and expected mortality was calculated by EuroSCORE. Logistic regression analysis and Cox regression analysis were performed to find predictors of operative mortality and survival, respectively. The receiver operating characteristic (ROC) curves were plotted for logistic EuroSCORE, and the area under the ROC curve (AUC) was calculated. Results: 87 patients (27.6% female) underwent surgery for AAD. The mean age was 58.6±9.7 years. Expected and observed operative mortality was 25.8±15.1% and 20.7%, respectively. Penn Aa, Ab and Abc shared similar observed/expected (O/E) mortality ratio. The only independent predictor of operative mortality (OR: 3.63; 95% CI: 1.19-11.09) and survival (HR: 2.6; 95% CI: 1.5-4.8) was female gender. EuroSCORE showed a very poor prediction capacity, with an AUC=0.566. Conclusion: Female gender was the only independent predictor of operative mortality and survival in our institution. EuroSCORE is a poor scoring algorithm to predict mortality in AAD, but with consistent results for Penn Aa, Ab and Abc.

Introdução: o número de crianças que nascem com algum tipo de cardiopatia congênita é elevado e a correção cirúrgica é o principal tratamento nesses casos. A fisioterapia atua com técnicas que auxiliam na recuperação e atenuam disfunções decorrentes do internamento. Objetivo: descrever o perfil clínico de crianças no pós-operatório de cirurgia cardíaca e as principais técnicas fisioterapêuticas utilizadas na Unidade de Terapia Intensiva de um hospital de referência pediátrico do município de Salvador. Metodologia: estudo transversal, com 49 pacientes em hospital de referência em pediatria, submetidos à cirurgia cardíaca no período de novembro de 2016 a agosto de 2017, em hospital de referência em Pediatria. Foram coletados dados dos prontuários referentes a gênero, idade, presença de outras malformações associadas, tipo de cardiopatia, tempo de ventilação mecânica, tempo de internamento na unidade de tratamento intensivo e óbito durante a estada nessa unidade. Resultados: do total da amostra, 53% eram do sexo feminino. A média de idade foi de 13 meses. Quanto ao tipo de cardiopatia, 85,7% foram acianóticas, em relação às doenças associadas, 24,4% eram portadores da Síndrome de Down. Dentre as técnicas fisioterapêuticas mais utilizadas estavam a terapia de higiene brônquica e a cinesioterapia passiva. Conclusão: este estudo demonstrou o perfil clínico dos pacientes no pósoperatório de cirurgias cardíacas, evidenciando uma média de idade de aproximadamente um ano, com discreto predomínio do sexo feminino. As técnicas fisioterapêuticas mais utilizadas foram a terapia de higiene brônquica e a cinesioterapia.


Introduction: the number of children who are born with some kind of congenital heart disease is high and the surgical correction is the main treatment in such cases. Physiotherapy works with techniques that help in the recovery and reduce malfunctions arising from the hospitalization. Objective: describe the clinical profile of children in the postoperative period of cardiac surgery and physiotherapy techniques used in the Intensive Care Unit (ICU) of a reference pediatric hospital of the municipality of. Methodology: cross-sectional study, with 49 patients in pediatric reference hospital who underwent heart surgery in the period from November 2016 to August 2017, in pediatric reference hospital. Data were collected from medical records relating to gender, age, presence of other associated malformations, cardiopathy type, time of VM, time of hospitalization in the ICU and death during a stay in the ICU. Results: of the total sample, 53% were of female sex. The average age was 13 months. As for the type of cardiopathy, 85.7% were acyanotic, in relation to associated diseases, 24.4% were carriers of Down Syndrome. Among the physiotherapeutic techniques used were bronchial hygiene therapy (BHT) and passive kinesitherapy. Conclusion: this study demonstrated the clinical profile of patients in the postoperative period of cardiac surgery, showing an average age of about a year, with discreet predominance of the female sex. The most used physiotherapeutic techniques were the BHT and kinesitherapy.

Resumen Objetivo: Analizar las causas de mortalidad hospitalaria de pacientes operados de cirugía cardiaca en México en el año 2015. Identificar los factores de riesgo de mortalidad y realizar una correlación con el tiempo de estancia hospitalaria en terapia intensiva cardiovascular. Método: Estudio de casos y controles. Se estudió la base de datos de la Terapia Intensiva Cardiovascular del Instituto Nacional de Cardiología. Se incluyeron todos los pacientes adultos operados de cirugía cardiaca en el año 2015. Resultados: Se operaron 571 pacientes. La indicación quirúrgica predominante fue la cirugía de cambio valvular único o múltiple, seguida de la cirugía de revascularización coronaria y corrección de cardiopatías congénitas del adulto. La mortalidad global fue de un 9.2% y el 8% falleció en terapia intensiva. Los principales factores de riesgo de muerte fueron la presencia de falla orgánica o hipertensión pulmonar prequirúrgica, y el tiempo prolongado de circulación extracorpórea. La principal causa de muerte fue el choque cardiogénico. La mortalidad hospitalaria observada en esta población fue mayor para los operados de tromboendarterectomía pulmonar, cirugía de enfermedad aórtica compleja y cirugía valvular. Conclusiones: La mortalidad de los pacientes operados de cirugía cardiaca en México difiere levemente de la reportada en la literatura mundial porque se trata mayormente de cirugía multivalvular y de procedimientos quirúrgicos mixtos complejos.


Abstract Objective: To analyse hospital mortality in patients subjected to cardiac surgery in Mexico during the year 2015, and identify the mortality risks factors, and its correlation with days of hospital stay in the cardiovascular intensive care unit. Method: The database of Cardiovascular Intensive Care of the National Institute of Cardiology was examined for this cases and controls study that included only adult patients subjected to cardiac surgery during the year 2015. Results: A total of 571 patients were subjected to a surgical procedure. The predominant indication was single or multiple valve replacement surgery, followed by coronary revascularisation surgery, and correction of adult congenital heart disease. Overall mortality was 9.2, and 8% died in intensive care. The main risk factors for death were preoperative organ failure or pulmo- nary hypertension, and prolonged time with extracorporeal circulation. The primary cause of death was secondary to cardiogenic shock. The hospital mortality observed in this population was higher for patients undergoing pulmonary thromboendarterectomy, complex aortic disease surgery, and valvular surgery. Conclusions: The mortality of patients undergoing cardiac surgery in Mexico differs slightly from that reported in the world literature, primarily because there were more multivalvular surgeries and mixed complex procedures performed.

Abstract Introduction: The outcomes of Jehovah's Witness (JW) patients submitted to open heart surgery may vary across countries and communities. The aim of this study was to describe the morbidity and mortality of JW patients undergoing cardiac surgery in a tertiary hospital center in Southern Brazil. Methods: A case-control study was conducted including all JW patients submitted to cardiac surgery from 2008 to 2016. Three consecutive surgical non-JW controls were matched to each selected JW patient. The preoperative risk of death was estimated through the mean EuroSCORE II. Results: We studied 16 JW patients with a mean age of 60.6±12.1 years. The non-JW group included 48 patients with a mean age of 63.3±11.1 years (P=0.416). Isolated coronary artery bypass graft surgery was the most frequent surgery performed in both groups. Median EuroSCORE II was 1.29 (IQR: 0.66-3.08) and 1.43 (IQR: 0.72-2.63), respectively (P=0.988). The mortality tended to be higher in JW patients (18.8% vs. 4.2%, P=0.095), and there was a higher difference between the predicted and observed mortality in JW patients compared with controls (4.1 and 18.8% vs. 2.1 and 4.2%). More JW patients needed hemodialysis in the postoperative period (20.0 vs. 2.1%, P=0.039). Conclusion: We showed a high rate of in-hospital mortality in JW patients submitted to cardiac surgery. The EuroSCORE II may underestimate the surgical risk in these patients.

Abstract Objective: To test the capacity of the Logistic CASUS Score on the second postoperative day, the total serum bilirubin dosage on the second postoperative day and the extracorporeal circulation time, as possible predictive factors of long-term stay in Intensive Care Unit after cardiac surgery. Methods: Eight-two patients submitted to cardiac surgery with extracorporeal circulation were selected. The Logistic CASUS Score on the second postoperative day was calculated and bilirubin dosage on the second postoperative day was measured. The extracorporeal circulation time was also registered. Patients were divided into two groups: Group A, those who were discharged up to the second day of postoperative care; Group B, those who were discharged after the second day of postoperative care. Results: In this study, 40 cases were listed in Group A and 42 cases in Group B. The mean extracorporeal circulation time was 83.9±29.4 min in Group A and 95.8±29.31 min in Group B. Extracorporeal circulation time was not significant in this study (P=0.0735). The level of P significance of bilirubin dosage on the second postoperative day was 0.0003 and an area under the ROC curve of 0.708 with a cut-off point at 0.51 mg/dl was registered. The level of P significance of Logistic CASUS Score on the second postoperative day was 0.0001 and an area under the ROC curve of 0.723 with a cut-off point at 0.40% was registered. Conclusion: The Logistic CASUS Score on the second postoperative day has shown to be better than the bilirubin dosage on the second postoperative day as a predictive tool for calculating the length of stay in intensive care unit during the postoperative care period of patients. Notwithstanding, extracorporeal circulation time has failed to prove itself as an efficient tool to predict an extended length of stay in intensive care unit.

ABSTRACT Introduction: In patients with mechanical prosthetic heart valves or atrial fibrillation requiring anticoagulation to prevent thromboembolic events, several factors influence adherence and anticoagulation complications. Objective: To evaluate the factors that interfere with the quality and complications of anticoagulation with vitamin K antagonists. Methods: A retrospective cohort study of 100 patients, in the period from 2011 to 2014, was performed. Anticoagulation conditions in the last year, regarding the presence of complications (embolisms/bleeding) and inadequate treatment were assessed: achievement of less than 8 annual prothrombin times and International Normalized Ratio outside therapeutic target in more than 40% of prothrombin times. Results: There were 31 complications (22 minor bleeding without hospitalization and 9 major complications: 7 bleeding with hospitalization and two emboli); 70 were with International Normalized Ratio outside the target in more than 40% of the tests and 36 with insufficient number of prothrombin times. Socioeconomic factors, anticoagulant type and anticoagulation reason had no relationship with complications or with inadequate treatment. There were more complications in patients with longer duration of anticoagulation (P=0.001). Women had more International Normalized Ratio outside the target range (OR 2.61, CI:1.0-6.5; P=0.04). Patients with lower number of annual prothrombin times had longer times of anticoagulation (P=0.03), less annual consultations (P=0.02) and less dose adjustments (P=0.003). Patients with longer duration of anticoagulation have more complications (P=0.001). Conclusion: There was a high rate of major complications and International Normalized Ratio was outside the goal. Less annual prothrombin times was related to longer duration of anticoagulation, less annual consultations and less dose adjustments. More major complications occurred in patients with longer duration of anticoagulation.

ABSTRACT Introduction: Risk Adjustment for Congenital Heart Surgery 1 (RACHS-1) score is a simple model that can be easily applied and has been widely used for mortality comparison among pediatric cardiovascular services. It is based on the categorization of several surgical palliative or corrective procedures, which have similar mortality in the treatment of congenital heart disease. Objective: To analyze the in-hospital mortality in pediatric patients (<18 years) submitted to cardiac surgery for congenital heart disease based on RACHS-1 score, during a 12-year period. Methods: A retrospective date analysis was performed from January 2003 to December 2014. The survey was divided in two periods of six years long each, to check for any improvement in the results. We evaluated the numbers of procedures performed, complexity of surgery and hospital mortality. Results: Three thousand and two hundred and one surgeries were performed. Of these, 3071 were able to be classified according to the score RACHS-1. Among the patients, 51.7% were male and 47.5% were younger than one year of age. The most common RACHS-1 category was 3 (35.5%). The mortality was 1.8%, 5.5%, 14.9%, 32.5% and 68.6% for category 1, 2, 3, 4 and 6, respectively. There was a significant increase in the number of surgeries (48%) and a significant reduction in the mortality in the last period analysed (13.3% in period I and 10.4% in period II; P=0.014). Conclusion: RACHS-1 score was a useful score for mortality risk in our service, although we are aware that other factors have an impact on the total mortality.

Doenças cardiovasculares representam a maior carga de morbimortalidade para o sistema de saúde e a cirurgia cardíaca desempenha importante impacto na sua resolutividade. O agrupamento das informações demográficas e clínicas relevantes dos pacientes acometidos, no nível de estratos específicos e em correlação com os conjuntos de recursos requeridos, representa a possibilidade de adaptar, aprimorar e inovar nos programas assistenciais. Este projeto tem por objetivo remodelar o escore de risco "InsCor" para formulação do SP-SCORE (São Paulo System for Cardiac Operative Risk Evaluation), visando refletir melhor a complexidade da assistência cirúrgica cardíaca. Os hospitais participantes integram os Núcleos de Avaliação de Tecnologias da Secretaria de Estado da Saúde de São Paulo (NATSs/SES-SP). Para a elaboração do SP-SCORE, serão utilizadas as 10 variáveis do modelo InsCor e 8 outras com suposta influência no Brasil. Os desfechos primários consistem na morbidade e na mortalidade hospitalar. A técnica de bootstrap junto a procedimentos de seleção automatizada de variáveis "stepwise" será utilizada para desenvolver um modelo parcimonioso por meio da regressão logística múltipla. Este projeto visa subsidiar a sustentabilidade e o financiamento do SUS-SP para as Redes Regionalizadas de Atenção à Saúde (RRAS) de cirurgias de coronária e/ou valva, promovendo alocação equitativa, incremento do acesso e efetividade, bem como caracterizar a magnitude dos recursos disponíveis e seu impacto.


Cardiovascular diseases represent the greatest burden of morbidity and mortality for the health system and cardiac surgery has an important impact on their resolutivity. The association and correlation of patients' demographic and clinical relevant information with the resources required for each stratum represent the possibility to adapt, improve and innovate into the healthcare programs. This project aims to remodel the "InsCor" risk score for the formulation of the SP-SCORE (Sao Paulo System for Cardiac Operative Risk Evaluation) in order to better reflects the complexity of cardiac surgical care. The participating hospitals include the Health Technology Assessment Centers in of the Health Secretariat' HTA Network of São Paulo State (HTA-NATSs / SES-SP). The SP-SCORE will use 10 variables of the InsCor model and others 8 variables with presumed influence in Brazil. The primary endpoints are morbidity and mortality. Bootstrap technique besides automated selection of variables (stepwise) will be used to develop a parsimonious model by multiple logistic regression. This project will contribute for the SUS-SP regionalized health-care (RRAS) sustainability and financing of the CABG and/or heart valve surgery programs promoting equitable allocation, increasing access and effectiveness, as well as characterizing the magnitude of available resources and its impact.

Objetivo: Caracterizar os pacientes submetidos à cirurgia cardíaca em hospital de referência da região norte do Ceará. Método: Pesquisa quantitativa, descritiva, de análise documental realizado pela análise de 176 prontuários de acordo com os aspectos éticos da resolução 196/96. Resultados: Identificou-se predominância de pacientes masculinos, casados, residentes em municípios distantes do serviço de saúde com predomínio da faixa etária acima de 40 anos. A permanência hospitalar foi baixa e por isso, percebe-se redução de riscos relacionados à internação e também dos custos com estes serviços, os quais influenciam para o aumento da rotatividade e disponibilização de leitos. Discussão: Foi evidenciada grande rotatividade e efetividade do serviço, possibilitando também vislumbrar que a principal cirurgia realizada é a revascularização do miocárdio em homens com a duração de internação em torno de quatro dias. Conclusão: Apesar dos riscos de uma cirurgia cardiovascular no hospital estudado, os pacientes recuperam-se com grande sucesso.


Objective: To characterize patients undergoing cardiac surgery at a referral hospital in northern of Ceara. Method: This is a quantitative and descriptive study using documentary analysis carried out by 176 medical records in accordance with the ethical aspects of Resolution 196/96. Results: We identified predominantly men, married, living indistant cities of the health service and a greater tendency for people over 40 years old. The hospital stay was low, contributing to reduction of risks related to hospitalization, also reducing the cost of these services, for influencing turnover and availability of beds. Discussion: This study shows high turnover and effectiveness of the service, allowing also envision that the primary surgery is coronary artery bypass surgery in men with the duration of hospitalization in about four days. Conclusion: We found that despite the risks of a cardiovascular surgery in the studied hospital, the patients recover with a great success.


Objetivo: Caracterizar los pacientes sometidos a cirugía cardíaca en un hospital de referencia en el norte de Ceará. Método: Estudio cuantitativo, descriptivo de análisis documental llevada a cabo por 176 historias clínicas de acuerdo con los aspectos éticos de la Resolución 196/96. Resultados: Se identificaron un predominio del sexo masculino, casado, viviendo en ciudades lejos de los servicios de salud con la edad predominante por encima de 40 años. La estancia hospitalaria fue baja y por lo tanto, realizar una reducción de los riesgos relacionados con la hospitalización y los costos de estos servicios, que influyen para aumentar el volumen de negocios y la disponibilidad de camas. Discusión: Este estudio muestra una alta rotación y la eficacia del servicio, lo que permite también vislumbrar que la cirugía primaria es la cirugía de revascularización coronaria en los hombres con la duración de la hospitalización en unos cuatro días. Conclusión: A pesar de los riesgos de la cirugía cardiovascular en el hospital, los pacientes se recuperan con éxito.

Objectives: To report the results of 13 years worth of epidemiologic surveillance of ventilator-associated pneumonia (VAP) following heart surgery and the main interventions applied in order to reduce VAP incidence. Methods: This is a retrospective and descriptive study of active epidemiologic surveillance of VAP. National diagnostic criteria were used. Interventions associated with a decrease in VAlP incidence in adults who underwent heart surgery are described. Results: A significant and sustained reduction was observed in the rate of VAP; being 56.7 per 1,000 ventilator-days in 1998 vs 4.7 per 1,000 ventilator-days in 2010 (p < 0.001). The strongest reduction was observed following 2003 (34.4 to 14.8 per 1,000 ventilator-days in 2004, p < 0.001). The interventions with greatest impact were the implementation of an early-weaning protocol, the introduction of trained nurses to perform the mechanical ventilator equipment management and the routine use of alcohol-based hand rubs. Conclusion: Epidemiologic surveillance associated with the establishment of a multifactorial intervention program applied in collaboration with the attending team, have demonstrated a significant reduction of VAP incidence after heart surgery.


Objetivos: Comunicar los resultados de 13 años de vigilancia epidemiológica de neumonía asociada a ventilación mecánica (NAVM) post cirugía cardíaca y las principales intervenciones implementadas para reducir su incidencia. Metodología: Estudio retrospectivo, descriptivo, de vigilancia epidemiológica activa de NAVM utilizando los criterios del ]Ministerio de Salud (MINSAL) y de las intervenciones asociadas con una disminución de la tasa de NAVM en adultos operados de cirugía cardíaca. Resultados: Se observó una reducción significativa y sostenida de la tasa de NAVM, siendo 56,7 por 1.000 días de ventilación mecánica (VM) en 1998 vs 4,7 por 1.000 días de VM en 2010 (p < 0,001). La mayor reducción fue observada a partir de 2003 (desde 34,4 a 14,8 por 1.000 días de VM en 2004, p < 0,001). Las intervenciones con mayor impacto fueron la implementación de un protocolo de extubación precoz, la incorporación de enfermeras capacitadas en el manejo de los equipos de VM y el uso rutinario de alcohol gel. Conclusión: La vigilancia epidemiológica asociada a un programa de intervención multifactorial aplicado en conjunto con el equipo tratante permitió reducir significativamente la incidencia de NAVM post cirugía cardíaca.

Background: Rates ofmorbidity and mortality in Infective Endocarditis (IE) remain high and prognosis in this disease is still difficult and uncertain. Aim: To study IE in Chile in its active phase during inpatient hospital stay and long term survival rates. Material and Methods: Observational prospective national cohort study of 506 consecutive patients included between June 1,1998 and July 31, 2008, from 37 Chilean hospitals (secondary and tertiary centers) nationwide. Results: The main findings were the presence of Rheumatic valve disease in 22.1 % of patients, a history of intravenous drug abuse (IVDA) only in 0.7%, the presence of Staphylo-coccus aureus in 29.2% of blood cultures, negative blood cultures in 33.2%, heart failure in 51.7% and native valve involvement in 86% ofpatients. Echocardiographic diagnosis was achieved in 94% of patients. Hospital mortality was 26.1% and its prognostics factors were persisting infection (Odds ratio (OR) 6.43, Confidence Interval (CI) 1.45-28.33%), failure of medical treatment and no surgical intervention (OR 48.8; CI 6.67-349.9). Five and 10 years survival rates were 75.6 and 48.6%, respectively. The significant prognostic factors for long term mortality, determined by multivariate analysis were the presence of diabetes, Staphylococcus aureus infection, sepsis, heart failure, renal failure and lack of surgical treatment during the IE episode. Conclusions: The microbiologic diagnosis of IE must be urgently improved in Chile. Mortality rates are still high (26.1%) partly because of a high incidence of negative blood cultures and the need for more surgical valve interventions during in-hospital period. Long term prognostic factors for mortality should be identified early to improve outcome.

The aim of this article is to review the local experience over the last 18 years (1994-2011) in providing surgical intervention for children with heart conditions in Jamaica. Eight hundred and sixty-three children received cardiac surgery during this period, 441 were done with the assistance of visiting overseas teams and 422 by the local surgical teams. The majority of cases receiving cardiac surgery were for congenital heart defects; however, 3% of cases were for rheumatic heart disease. The overall survival rate was 94% for the period 1994 to 2008.


El objetivo de este artículo es pasar examen a la experiencia local durante los últimos 18 años (1994-2011) en cuanto a brindar intervención quirúrgica a niños con problemas cardíacos en Jamaica. Ochocientos sesenta y tres niños recibieron cirugía cardíaca durante este periodo. De estas, 441 se realizaron con ayuda de equipos formados por personal médico visitante extranjero, y 422 por equipos de cirujanos locales. La mayoría de los casos que recibieron cirugía cardíaca presentaban defectos cardíacos congénitos. Sin embargo, un 3% de los casos presentaban la enfermedad reumática del corazón. La tasa de supervivencia global fue 94% para el periodo 1994 a 2008.

Introducción: Para disminuir la lista de espera para la cirugía cardiaca electiva, en 1999 el Instituto Nacional de Cardiología Ignacio Chávez inició un programa de vía rápida para casos de muy bajo riesgo quirúrgico. En 2004, este programa se extendió a pacientes con riesgo intermedio. Objetivos: Estudio prospectivo, descriptivo, para evaluar las características clínicas y demográficas de los pacientes del programa de vía rápida en cirugía cardiaca electiva. También se analizaron la estancia hospitalaria, mortalidad, complicaciones y reingresos. Métodos: De marzo de 2004 a febrero de 2009 incluimos pacientes adultos con indicación de cirugía cardiaca y con riesgo quirúrgico de bajo a intermedio, con requisitos preoperatorios completos antes del internamiento. Resultados: De un total de 598 pacientes ingresados al programa, se analizaron 533, con edad de 47 ± 14 años, 62.5% mujeres. Se clasificaron en cuatro grupos: valvulares (68%), congénitos (25%), isquémicos (5%) y mixtos (2%). Los promedios de días de estancia hospitalaria fueron: preoperatoria 1.9, terapia tres, postoperatoria en piso 6.9 y total 11.9 días. Se evidenció que 17.8 % estuvieron más de 14 días por: reoperaciones, complicaciones pulmonares, arritmias, o infecciones. La mortalidad fue de 4.1% y hubo 2.8% de reingresos en los primeros tres meses posteriores a la cirugía. Conclusiones: Este programa conduce a bajos índices de mortalidad, estancia hospitalaria y reingresos.


Background: In 1999 so as to decrease the list of cardiac surgery the "fast track" program was started for patients with very mortality low risk. In 2004, this program was extended to moderate risk patients. Objectives: A prospective, descriptive study to evaluate the clinical and demographic characteristics of "fast track" program patients for elective cardiac surgery. We also analyzed the hospital stay, mortality, complications and readmissions. Methods: From March 2004 to February we included adult patients with indications for cardiac surgery, low to intermediate risk of mortality and complete preoperative requirements. Results: From a total of 598 patients, 533 were analyzed, aged 47±14 years, 62.5% female. They were classified in four groups: valvular (68%), congenital (25%), coronary artery disease (5%), and mixed (2%). The average hospital stay was: preoperative 1.9, intensive care unit three, postoperative in hospitalization 6.9 and total 11.9 days. We found that 17.8% had a hospital stay longer than 14 days due to: reoperations, pulmonary complications, arrhythmias or infections. The mortality was 4.1 and 2.8% had readmissions three months after surgery. Conclusions: This program shows a low rate of mortality, hospital stay and readmissions.
