Background: Endoscopic ultrasound training has a learning curve greater than the other endoscopic therapeutic techniques. One of the preclinical teaching methods is the use of ex vivo porcine models. Aim: To describe five ex vivo porcine models for training in therapeutic echoendoscopic procedures. Materials and methods: Using porcine digestive tract containing esophagus, stomach, duodenum, spleen, liver and gallbladder, five models for therapeutic echoendoscopy training were described. With other segments of the porcine pieces (such as the bladder, spleen segment and omentum segment) and with easily accessible materials (such as grape and ultrasound gel), lesions were simulated to be treated. These models were applied in the Hands on course at the IRCAD (Institut de recherche contre les cancers de l'appareil digestif) Barretos of 2017. Endoscopic equipment and instruments are the same as those used in clinical practice. Result: The models are easily reproducible and do not require exchange during the hands on course period. Endoscopic and echographic imaging and tactile sensitivity are similar to the real one. Conclusion: The models described in this study demonstrated to be realistic, easy to reproduce and allow repetition during the same session. However, comparative studies are necessary to verify the real impact on teaching


Racional: El entrenamiento de la ecoendoscopía tiene una curva de aprendizaje mayor que las demás técnicas endoscópicas terapéuticas. Uno de los métodos de enseñanza preclínica es el uso de modelos porcinosex vivos. Objetivo: Describir cinco modelos porcino sex vivo para entrenamiento de procedimientos ecoendoscópicos terapéuticos. Materiales y método: Utilizando el tracto digestivo porcino, que contiene esófago, estómago, duodeno, delgado, hígado y vesícula biliar, se han descrito cinco modelos para el entrenamiento de ecoendoscopía terapéutica. Con otros segmentos de la pieza porcina (como vejiga, segmento de delgado, bazo y omento) y con materiales de fácil acceso (como uva y gel de ecografía), se simularon lesiones a ser tratadas. Estos modelos se aplicaron en el curso Handsonenel IRCAD (Institut de recherche contre les cancers de l'appareil digestif) Barretos de 2017. Los aparatos e instrumentos endoscópicos son los mismos utilizados en la práctica clínica. Resultado: Los modelos forman de fácil reproducibilidad, no siendo necesario el cambio de la pieza porcina durante el período del curso Handson. La imagen endoscópica y ecográfica y la sensibilidad táctil son similares a la real. Conclusión: Los modelos descritos en este trabajo han demostrado ser realistas, de fácil reproducción y permiten repetición durante la misma sesión. Sin embargo, los estudios comparativos son necesarios para verificar el impacto real en la enseñanza

Abstract Purpose: To examine a correlation of micro-PET images with photographic images of the digestive organs in abdominal sepsis model. Methods: Male Wistar rats weighing 265±18g were used. Abdominal sepsis was induced by ligature and cecal puncture. Micro-PET Images from abdominal cavity septic foci were obtained using 18-Fluoro-deoxyglucose, looking for a correlation with photographic images of abdominal cavity organs. Pearson's correlation test was used. Results: The mean standard uptake values (SUV) and lesion areas were 2.58±0.63SUVbwg/ml and 546.87±300.95mm2, respectively. There was a strong positive correlation between the two variables (r=0.863, p=0.137), which resulted in a coefficient of determination r2?0.75, meaning that 75% of SUV variation is explained by the lesion areas of digestive organs. Conclusion: Micro-PET allows high throughput assessment of lesion count and volume in pre-clinical rat model of CPL abdominal sepsis.

BACKGROUND: Dyspepsia is common in clinical practice with frequent relapses and often requires multiple investigations to assess intraluminal and extraluminal etiologies. Endoscopic ultrasound (EUS) has the potential of serving both purposes in a single setting. MATERIAL AND METHOD: Patients with dyspepsia who underwent EUS in four-year period were retrospectively reviewed. Diagnostic findings of US, final diagnoses were noted and compared with the reference standards. RESULTS: 131 patients with a mean age +/- SD of 50 +/- 12.7 years were included. The common diagnoses were non-ulcer dyspepsia in 56%, symptomatic gallstone (GS) in 22.9%. EUS detected two GS missed by transabdominal ultrasound (TUS). EUS missed one GS documented by surgery. EUS detected seven cases of ERCP-proven CBD stones undetected by TUS and had a sensitivity, specificity, positive predictive value and negative predictive value for CBD stones of 87.5%, 91.7%, 87.5%, and 91.7% respectively. CONCLUSION: EUS is a potential investigation for the management of dyspepsia.

To compare preoperative ultrasonographic findings with operative findings in hepatobiliary surgery. Design and Place of study: A descriptive study was conduced at Unit II, Dept. of Surgery, CHK. Place and Duration: The study was conducted in Surgical Unit-I, Ward-3 of Jinnah Postgraduate Medical Centre, Karachi, Pakistan between February, 2000 to January, 2004. Patient And Hundred patients admitted over a period of two years from January, 2002 to December 2003, for hepatobiliary diseases were included in the study. Ultrasonography for their basic problem was done. All these patients underwent surgery. Operative findings were compared with preoperative ultrasonography findings and the accuracy of ultrasonography in hepatobiliary diseases was evaluated. In 34 cases, ultrasonographic findings as compared to operative findings were inaccurate while in 55 patients these were comparable to operative findings. Generally accepted overall accuracy rate of ultrasonography in hepatobiliary diseases is 86%. Accuracy rate in this study was only 66% showing that ultrasonography is an operator dependant procedure

RESUMO

The objective is to compare the endoscopic findings before and after initiation of open access and its effect on gastrointestinal radiology services. The data of endoscopic findings before open access endoscopy [july, 1989-June, 1992] and after open access endoscopy [July, 1992-June, 1995] was collected from the records of the endoscopy unit. Similarly, data of barium meals for the same periods was collected from the radiology department. An X2 test was used to compare the endoscopic findings before and after open access policy. It is found that open access endoscopy increased the workload in the endoscopy unit but at the same time reduced the number of gastrointestinal radiological procedures. It reduced the wailing period for endoscopy and this helped in making early diagnosis and initiation of appropriate treatment

RESUMO

Forty-two children with ingestion of foreign bodies (FB) were managed conservatively. Thirty one (74%) of them were under 5 years of age. At presentation history revealed accidental ingestion in 95% and of being put in the oral cavity by elder sibs in 5% patients. Fifty seven per cent had respiratory distress, 38% had dysphagia and 12% had hematemesis. Foreign bodies were located in the gastrointestinal tract in the stomach (40%), esophagus (26%), small intestine (19%), duodenum (12%) and rectum (2%). A large majority of the FB were constituted by household objects. All the FB above the duodenojejunal junction and one in the rectum were retrieved successfully with fiberoptic endoscopes. In 19% patients, the FB had crossed duodenojejunal junction, and had come out in the stools during 4-5 days observation and these were mostly round in shape. Endoscopic procedures were carried out under intravenous diazepam or ketamine sedation. On endoscopic examination, 21% of them showed erosions in stomach and/or esophagus. No complications of endoscopic procedure or sedation were observed and none of the patients required surgical removal. Removal of FB with flexible fiberoptic endoscopes is less invasive and the best therapeutic option to avoid preventable complications of FB ingestion. In this procedure there is need of a trained and skilled Pediatric endoscopist with lot of patience and a good team work.
