RESUMO

O presente trabalho busca detalhar uma experiência pioneira de desenvolvimento de um sistema de informação em saúde voltado para subsidiar a gestão com base em evidências junto à atenção primária à saúde (APS). O processo de estruturação de programas e políticas públicas muitas vezes se dá sem acesso às melhores evidências científicas. Nesse contexto, o Sistema Integrado de Informações Mais Médicos (SIMM), aqui descrito, materializou um esforço para suprir a lacuna de informações para a gestão em saúde. Criado com objetivo de integrar os dados das principais fontes de informação relativas ao Projeto de Cooperação entre governo brasileiro e Organização Pan-Americana da Saúde/Organização Mundial da Saúde (OPAS/ OMS) para provimento de médicos no Programa Mais Médicos (PMM), o SIMM buscou otimizar a geração e a consolidação de informações que possam subsidiar decisões, além de apoiar o controle administrativo e financeiro. A experiência ilustra o desenvolvimento de uma solução auxiliar que pode ser ampliada para monitorar parâmetros de recursos humanos em saúde não apenas no PMM, mas na APS como um todo. A expertise desenvolvida criou condições para que o sistema pudesse ser configurado como um bem público, para além do suporte à gestão de um projeto em específico. O sistema pode ser adaptado e replicado em outros países das Américas para melhorar a disponibilidade e a qualidade das informações utilizadas por tomadores de decisão.(AU)


The present work provides details about a novel experience involving the development of a health information system focused on supporting evidence-based management at the primary health care (PHC) level. The process of structuring public health programs and policies is often developed without access to the best available scientific evidence. In this context, the proposed system (Sistema Integrado de Informações Mais Médicos, SIMM) materializes the effort to bridge the information gap for health management. Created with the aim of integrating data from the main information sources associated with the Cooperation Project between the Brazilian federal government and the Pan American Health Organization/World Health Organization (PAHO/ WHO) for provision of physicians for the More Doctors Program, SIMM strove to optimize the generation and consolidation of information to support decision-making, in addition to providing support for administrative and financial control. The experience illustrates the development of an ancillary solution that can be expanded to monitor human resources for health parameters across the entire PHC setting, far beyond the More Doctors Program. The resulting expertise created conditions for the system to be configured as a public asset, rather than being restricted to providing managerial support for a specific project. SIMM may be adapted and replicated in other American countries to improve the availability and quality of the information used by decision makers.(AU)


En el presente trabajo se detalla una experiencia pionera de desarrollo de un sistema de información de salud centrado en apoyar la gestión basada en la evidencia en el nivel de la atención primaria de salud. Muchas veces, el proceso de estructuración de los programas y políticas públicas se realiza sin acceso a las mejor evidencia científica. En ese contexto, el sistema integrado de información de Mais Médicos (SIMM) que se describe aquí materializó un esfuerzo para suplir la carencia de información para la gestión en materia de salud. El SIMM, creado con el objetivo de integrar los datos de las principales fuentes de información relativas al proyecto de cooperación entre el Gobierno del Brasil y la Organización Panamericana de la Salud/Organización Mundial de la Salud (OPS/OMS) para la provisión de médicos destinados al programa Más Médicos, se utilizó para optimizar la generación y consolidación de información a fin de sustentar las decisiones, además de servir de apoyo para el control administrativo y financiero. La experiencia ilustra el desarrollo de una solución complementaria que puede ampliarse para observar los parámetros de recursos humanos en salud no solo en el programa Más Médicos, sino en todo el ámbito de la atención primaria de salud. La pericia adquirida creó condiciones propicias para poder configurar el sistema como un bien público, con miras a ampliar su alcance más allá del apoyo a la gestión de un proyecto específico. Es posible adaptar y reproducir el sistema en otros países de la Región de las Américas para mejorar la disponibilidad y la calidad de la información utilizada por los responsables de tomar decisiones.(AU)

RESUMO

JCI standard is the most professional and authoritative certification standard of medical quality and safety system in the world. HRP information system is a systematic hospital resource management platform which integrates the existing hospital information resources and establishes a set of unified, efficient, interconnected and information sharing operation and management of the hospital. In order to meet the requirements of closed-loop management of implantable medical devices in the JCI standard, our hospital has established a set of standards which based on the HRP system platform, including access permission, purchase application, entering and leaving the warehouse, bookkeeping charge, cost accounting and postoperative traceability of implanted medical device management system. HRP system improves the management level of implantable medical devices, and realizes the closed-loop management of the whole process of implantable medical devices.

Este artigo discute o desenvolvimento de uma versão inicial do Sistema de Informação para a Gestão do Cuidado na Rede de Atenção Domiciliar (SI GESCAD) que auxilia na clínica ampliada, na coordenação e na continuidade assistencial ao paciente em atenção domiciliar (AD), sua família e ao cuidador. Objetiva-se, ainda, refletir sobre a contribuição, os limites e as possibilidades do SI GESCAD na perspectiva da Rede de Atenção Domiciliar do SUS (RAS-AD). Pesquisa de produção tecnológica referenciada em protocolo de estudo multimétodo. Engenharia de software e Interação Humano-Computador-IHC com a adoção do design centrado no usuário, modelos evolucionários e interativos de processo de software (prototipagem e espiral). Finalizou-se um protótipo funcional do GESCAD que permite a gestão do cuidado ao paciente em AD nas dimensões do contexto sociofamiliar e do cuidador; da Atividade de Vida Diária (AVD); da clínica e do monitoramento de variáveis associadas ao tipo de AD2. O SI GESCAD permite a horizontalidade dos processos de trabalho das equipes de AD nos níveis de atenção da RAS-AD/SUS, com repercussões positivas à coordenação do cuidado e à continuidade assistencial.


The present article describes the development of the initial version of the Brazilian Care Management Information System for the Home Care Network (SI GESCAD). This system was created to enhance comprehensive care, care coordination and the continuity of care provided to the patients, family and caretakers of the Home Care (HC) program. We also present a reflection on the contributions, limitations and possibilities of the SI GESCAD within the scope of the Home Care Network of the Brazilian Unified Health System (RAS-AD). This was a study on technology production based on a multi-method protocol. It discussed software engineering and human-computer interaction (HCI) based on user-centered design, as well as evolutionary and interactive software process (prototyping and spiral). A functional prototype of the GESCAD was finalized, which allowed for the management of HC to take into consideration the patient's social context, family and caretakers. The system also proved to help in the management of activities of daily living (ADLs), clinical care and the monitoring of variables associated with type 2 HC. The SI GESCAD allowed for a more horizontal work process for HC teams at the RAS-AD/SUS level of care, with positive repercussions on care coordination and continuity of care.

RESUMO

Medical equipments are essential supplies to carry out medical work. How to ensure the safety and reliability of the medical equipments in diagnosis, and reduce procurement and maintenance costs is a topic of concern to everyone. In this paper, product lifecycle management (PLM) and enterprise resource planning (ERP) are cited to establish a lifecycle management information system. Through integrative and analysis of the various stages of the relevant data in life-cycle, it can ensure safety and reliability of medical equipments in the operation and provide the convincing data for meticulous management.

RESUMO

OBJECTIVES: The purpose of this study was to review evaluation studies of nursing management information systems (NMISs) and their outcome measures to examine system effectiveness. METHODS: For the systematic review, a literature search of the PubMed, CINAHL, Embase, and Cochrane Library databases was conducted to retrieve original articles published between 1970 and 2014. Medical Subject Headings (MeSH) terms included informatics, medical informatics, nursing informatics, medical informatics application, and management information systems for information systems and evaluation studies and nursing evaluation research for evaluation research. Additionally, manag* and admin*, and nurs* were combined. Title, abstract, and full-text reviews were completed by two reviewers. And then, year, author, type of management system, study purpose, study design, data source, system users, study subjects, and outcomes were extracted from the selected articles. The quality and risk of bias of the studies that were finally selected were assessed with the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS) criteria. RESULTS: Out of the 2,257 retrieved articles, a total of six articles were selected. These included two scheduling programs, two nursing cost-related programs, and two patient care management programs. For the outcome measurements, usefulness, time saving, satisfaction, cost, attitude, usability, data quality/completeness/accuracy, and personnel work patterns were included. User satisfaction, time saving, and usefulness mostly showed positive findings. CONCLUSIONS: The study results suggest that NMISs were effective in time saving and useful in nursing care. Because there was a lack of quality in the reviewed studies, well-designed research, such as randomized controlled trials, should be conducted to more objectively evaluate the effectiveness of NMISs.

Based on the analysis of the present status of medical devices management, this paper optimized management process, developed a medical devices management system with Web technologies. With information technology to dynamic master the use of state of the entire life-cycle of medical devices. Through the closed-loop management with pre-event budget, mid-event control and after-event analysis, improved the delicacy management level of medical devices, optimized asset allocation, promoted positive operation of devices.

Resumen:


Introducción: el estudio de los sistemas de información se produjo como una sub-disciplina de las ciencias de la computación en un intento por entender y racionalizar la administración de la tecnología dentro de las organizaciones, sin embargo, constituye una herramienta tecnológica útil en la toma de decisiones.


Desarrollo: el Sistema de Información Administrativa de los Recursos Humanos de Enfermería (SIARHE) es uno de los proyectos más ambiciosos de la Comisión Permanente de Enfermería, mediante el cual se pretende contar con información confiable, oportuna y completa para detectar y atender con oportunidad las necesidades de recursos de enfermería y su mejor distribución a nivel nacional e institucional y en consecuencia, lograr el equilibrio entre la oferta y la demanda de estos recursos. Conclusiones: la información reportada por el SIARHE, está enfocada a lograr la caracterización del personal de enfermería desde el punto de vista académico, laboral y profesional para coadyuvar en la formulación de políticas que permitan conformar la fuerza de trabajo de enfermería suficiente y calificada, para responder de manera efectiva a las demandas de salud del país. Los avances al momento son significativos gracias a la contribución de las instituciones que conforman el sistema nacional de salud, en la consolidación del SIARHE, recurso administrativo necesario para asegurar la continuidad en la operación del sistema.


Abstract:


Introduction: the study of information systems occurred as a sub-discipline of computer science in an attempt to understand and rationalize the administration of technology within organizations; however, it is a useful technological tool in decision making.


Development: management Information System of Nursing Human Resources (Abbreviated in Spanish as SIARHE) is one of the most ambitious projects of the Standing Nursing Committee, through which it aims to have reliable, timely and complete information to detect and respond to the needs of nursing resources and for a better distribution to national and institutional level, thus achieving balance between supply and demand for these resources.


Conclusions: the information reported by the SIARHE, is focused on achieving nursing characterization from the academic, professional and work-setting standpoint to assist in the formulation of policies that will shape the skilled and enough nursing workforce to effectively respond to the health needs of the country. Progress to date are significant due to the contribution of the institutions of the national health system in the consolidation of SIARHE, administrative device necessary to ensure the continued operation of the system.

Inclusive, sufficient, precise and up to date information is necessary for planning in a health information system. Health Population Information Management System [HPIMS] is a system that collects, stores, retrieves, analyzes, interprets, and distributes health population information for prevention and treatment. The aim of this study was first; to compare Health Population Information Management System in health ministry, Iran statistics center, vital registry organization and United Nations, second, to propose a suitable model for Iran. This study was conducted to compare the features of Iran Health Population Information Management System with the United Nations standards. This was an applied cross-sectional study. The population included Iran Health Population Information Management Systems. The instruments of the study were a questionnaire and a cheek list. The data were collected through observation and an interview. The sources of information consisted of the staff, scientific documentation and the Internet. Criteria Rating Technique and descriptive statistics were used for data analysis. The findings showed an absence of timely, relevant, comprehensive and sufficient reporting in data registration system in Iran. Moreover, the organizations in charge of HPIMS had parallel functions which would lead to contradictory reports. Health Population Information, published by the Ministry of Health differed from that of Iran Statistics Center and other data registry organizations. Overall, HPIMS in the Ministry of Health was ranked as "very good" while the other organizations were ranked as "good". The findings suggest that the information systems can only influence decisions if they are relevant, trustworthy and available for decision makers. However, information systems in Iran are inadequate to satisfy the needs of management. Therefore, it seems necessary to reform and design a suitable model representing accountability of organizations in regard to their duties and workflow. It is also suggested to decrease parallel functions and resources through a sharing system

"The study aimed to document the kinds of leadership styles are practiced at health centres (H/C) and how these styles can be explained by the contexts; characteristics of the health centre in charge (IC) and subordinate trained health staff (STHS). Methods A well-researched leadership style model was applied; which included task; relation and change styles. This is a cross-sectional study using self-administered questionnaires in 47 H/C in 3 districts. 347 STHSs (95) and 46 ICs (98) responded. Questions explored background data and perceived leadership behaviour. Style items were factor analysed; and bivariate analyses and hierarchical regressions determined how styles could be explained. Results Two leadership styles were revealed: ""Trans""style contained all relation and the majority of task and change items; ""Control"" style focused on health statistics (Health Management Information System); reporting and evaluation. STHS and IC had a median age/median work experience of 34/5 years and 38;5/2 years; respectively. 48"

RESUMO

Trauma is a leading cause of death and disability around the world. Injuries are responsible for about six million deaths annually, of which ninety percent occur in developing countries. In Iran, injuries are the most common cause of death among age groups below fifty. Trauma system development is a systematic and comprehensive approach to injury prevention and treatment whose effectiveness has been proved. The present study aims at designing a trauma system management model as the first step toward trauma system establishment in Iran. In this qualitative research, a conceptual framework was developed based on the public health approach and three well-known trauma system models. We used Benchmarks, Indicators and Scoring [BIS] to analyze the current situation of Iran trauma care system. Then the trauma system management was designed using the policy development phase of public health approach The trauma system management model, validated by a panel of experts, describes lead agency, trauma system plan, policy-making councils, and data-based control according to the four main functions of management: leading, planning, organizing and controlling. This model may be implemented in two phases: the exclusive phase, focusing on resource integration and the inclusive phase, which concentrates on system development. The model could facilitate the development of trauma system in Iran through pilot studies as the assurance phase of public health approach. Furthermore, the model can provide a practical framework for trauma system management at the international level

RESUMO

For many sub-Saharan health facilities; accurate patient identification remains a challenge. Poor national person identification systems; inefficient identification procedures; the use of weak search criteria and sometimes fraudulent practice consist some of the underlying causes. In this study; patient identification effectiveness has been compared between 27 sub-Saharan hospitals using paper based procedures and 6 health facilities in the same region that had implemented a hospital information management system. Based on a simple metric; results show a significant (p0.001) improvement reducing identification errors from 64.6 before to 2.3 after information system implementation in a sample of 1 private and 5 public hospitals in Rwanda and Burundi

OBJECTIVES: In this research, the hospital information system of Gachon University Gil hospital is introduced and a future strategy for hospital information systems is proposed. METHODS: This research introduces the development conditions of hospital information system at Gachon University Gil hospital, information about the development of the enterprise resource planning (ERP), a medical service process improvement system, and the personal health record (PHR) system. RESULTS: The medical service process and work efficiency were improved through the medical service process improvement system, which is the most common hospital information system at Gachon University Gil hospital and which includes an emergency medical service system, an online evaluation system and a round support system. CONCLUSIONS: Gachon University Gil hospital developed medical service improvement systems to increase work efficiency of medical team and optimized the systems to prove the availability of high-quality medical services for patients and their families. The PHR-based personalized health care solution is under development and will provide higher quality medical service for more patients in the future.

<p><b>OBJECTIVE</b>To form a wound information management scheme with objectivity, standardization, and convenience by means of wound information management system.</p><p><b>METHODS</b>A wound information management system was set up with the acquisition terminal, the defined wound description, the data bank, and related softwares. The efficacy of this system was evaluated in clinical practice.</p><p><b>RESULTS</b>The acquisition terminal was composed of the third generation mobile phone and the software. It was feasible to get access to the wound information, including description, image, and therapeutic plan from the data bank by mobile phone. During 4 months, a collection of a total of 232 wound treatment information was entered, and accordingly standardized data of 38 patients were formed automatically.</p><p><b>CONCLUSIONS</b>This system can provide standardized wound information management by standardized techniques of acquisition, transmission, and storage of wound information. It can be used widely in hospitals, especially primary medical institutions. Data resource of the system makes it possible for epidemiological study with large sample size in future.</p>

O Presente trabalho trata de um Plano de Intervenção, tendo em vista a Implantação da Sala de Situação e Matriz de Monitoramento da Diretoria Geral de Gestão do Trabalho da Secretaria de Saúde do Recife. Partindo do pressuposto de que o planejamento estratégico garante ao gestor uma maior possibilidade de acerto na tomada de decisão, no intuito de garantir ao cidadão uma maior qualidade nos serviços ofertados e ao profissional uma melhor condição de trabalho. (MORAES, 1994). Desta forma propõe-se a criação de Grupo de Trabalho para analisar a base de Matriz de Monitoramento, sugerida neste Plano, definindo-se os responsáveis pelas informações/Fontes e prazos necessários para alimentação dos dados/informações. Para a DGGT a Matriz de Monitoramento vai funcionar como uma coletânea de informações, indicadores alimentados periodicamente pelo Grupo de Trabalho específico, e posteriormente analisadas para subsidiar o Gestor na Tomada de decisões. As novas ferramentas dos sistemas de informação na Saúde alinhados ao novo modelo de gestão participativa impulsionam o gestor a novos modelos de governar, dentro desta nova proposta surge a Sala de Situação, que terá a Matriz de Monitoramento como uma importante ferramenta. A Sala de Situação, a ser implantada nesta Diretoria tem o propósito de elencar os problemas, avaliar situações passadas, também avaliar as situações presente e ainda, elencar propostas de soluções calculando impactos/situações futuras. A partir deste mapa de informações e visões, o gestor terá subsídios para antever situações, planejar para o futuro, evitando surpresas e crises insolúveis. (MATTUS, 1987)...

According to hospital medical equipment, information equipment and water, electricity and other equipment maintenance procedures, this paper planned and developed a comprehensive maintenance management system for hospitals. The system implements equipment maintenance, maintenance applications, maintenance registration, preventive maintenance, data quantitative analysis and other functions.
