ABSTRACT Purposes: To evaluate the effects of nalbuphine 1% on the expression of metalloproteinase 1 (MMP-1), metalloproteinase 9 (MMP-9), and opioid growth factor (OGF) in rabbit corneas after lamellar keratectomy. Methods: The rabbits were assigned to two groups: group nalbuphine (GN, n=30), which received 30 µL of nalbuphine 1% in 4 daily applications at regular intervals until corneal epithelialization, and group control (GC, n=30), which received physiological saline solution under the same conditions adopted in GN. The corneas were collected for immunohistochemistry on days 1, 3, 5, 7, and 9 after lamellar keratectomy, and the expressions of MMP-1, MMP-9, and OGF were analyzed. Results: The expressions of MMP-1 and MMP-9 increased until day 5 of the evaluation, with no differences observed between GN and GC (p>0.05). On days 7 and 9, significant reductions were observed in the expression of MMP-1 (p<0.01), with no differences observed between GN and GC (p>0.05). The expression of OGF was constant in all periods (p>0.05), restricted to the corneal epithelium, and there was no difference between the groups (p>0.05). Conclusions: The study results showed that nalbuphine 1% did not alter the expression patterns of MMP-1, MMP-9, and OGF in rabbit corneas after lamellar keratectomy. .


RESUMO Objetivos: Avaliar os efeitos da nalbufina 1% sobre a expressão da metaloproteinase 1 (MMP-1), da metaloproteinase 9 (MMP-9) e do fator de crescimento opióide (OGF), em córneas de coelhos submetidas à ceratectomia lamelar. Métodos: Constituíram-se dois grupos: grupo nalbufina (GN, n=30), que recebeu 30 µL de nalbufina 1% em 4 aplicações diárias, a intervalos regulares, até a epitelização corneal; controle (GC, n=30), que recebeu solução salina nas mesmas condições adotadas no GN. As córneas foram colhidas para imuno-histoquímica decorridos 1, 3, 5, 7 e 9 dias das ceratectomias lamelares, visando a se avaliarem as MMP-1, MMP-9 e OGF. Resultados: A expressão das MMP-1 e de MMP-9 se elevou até o quinto dia de avaliação, sem diferença entre GN e GC (p>0,05). Nos dias 7 e 9, observou-se redução significativa na expressão das enzimas (p<0,01), sendo que diferenças não foram observadas entre os grupos (p>0,05). O OGF exibiu imunomarcação constante em todos os períodos (p>0,05), restrita ao epitélio corneal. Não foram encontradas diferenças entre os grupos (p>0,05). Conclusões: Com base dos resultados obtidos, há como admitir que a nalbufina 1% não alterou o padrão de expressão da MMP-1, da MMP-9 e do OGF em córneas de coelhos submetidas à ceratectomia lamelar. .

A pilot investigation to transfer the established corneal collagen crosslinking [CXL] procedure in European eyes into clinically affected African eyes and to optimize the treatment by adapting the riboflavin composition. CXL was performed in 15 eyes [11 patients] with advanced stages of keratoconus in the Eye Clinic of Bafoussam in the West Region of Cameroon. The following six riboflavin compositions with different portions of active swelling additives were applied-Solution 1 [0.5% methylhydroxypropylcellulose [MHPC]], solution 2 [1.0% MHPC], solution 3 [1.7% MHPC], solution 4 [5% dextran], solution 5 [10% dextran] and solution 6 [no active swelling ingredient]. The central corneal thickness [CCT] was measured by ultrasound pachymetry before and after de-epithelialization and at least every 10 min during CXL. The application of the riboflavin solutions resulted in the following mean final CCT values: 172 +/- 15% using solution 1 [60 min/n = 5]; 183 +/- 8% using solution 2 [60 min/n = 5]; 170% using solution 3 [60 min/n = l]; 80% using solution 4 [45 min/n = l]; 99% using solution 5 [45 min/n = l] and 150 _ 13% using solution 6 [50 min/n = 2]. The combination of riboflavin compositions with swelling and stabilizing effects on the corneal stroma seems necessary in African eyes with advanced keratoconus. Further studies are required to confirm these primary results

AIM: To assess the results of corneal collagen cross-linking with riboflavin using ultraviolet - A light for keratoconus at one year in Indian eyes. MATERIALS AND METHODS: Sixty-eight eyes of 41 patients with progressive keratoconus were included in this retrospective study. All eyes completed was 12 months of follow-up and 37 eyes had a one-year follow-up. The maximum follow-up was 16 months. Ocular examinations including refraction, best corrected visual acuity (BCVA), corneal topography, were recorded at each visit. RESULTS: The mean age was 16.9 +/- 3.5 years (range 12-39 years) and the mean follow-up was 10.05 +/- 3.55 months (range six to 16 months). Thirty seven eyes with a follow-up of at least 12 months were analyzed. The preoperative values on the day of treatment were compared with postoperative values of the 12-month examination. This showed that BCVA improved at least one line in 54% (20/37) of eyes and remained stable in 28% (10/37) of eyes ( P =0.006). Astigmatism decreased by a mean of 1.20 diopter (D) in 47% (17/37) of eyes ( P =0.005) and remained stable (within +/- 0.50 D) in 42% (15/37) of eyes. The K value of the apex decreased by a mean of 2.73 D in 66% (24/37) of eyes ( P =0.004) and remained stable (within +/- 0.50 D) in 22% (8/37) of eyes. The maximum K value decreased by a mean of 2.47 D in 54% (20/37) of eyes ( P =0.004) and remained stable (within +/- 0.50 D) in 38% (14/37) of eyes. Corneal Wavefront analysis revealed that spherical and higher-order aberrations did not show significant variations in the follow-up period. The coma component showed a very significant reduction at six months after treatment and persisted throughout the follow-up period ( P =0.003) CONCLUSION: The results show a stabilization and improvement in keratoconus after collagen cross-linking in Indian eyes. This suggests that it is an effective treatment for progressive keratoconus.

OBJETIVO: Apresentar os resultados visuais e ceratométricos, seis meses após tratamento foto-terapêutico com luz ultravioleta (UV) e vitamina B2 (Ultra B2), em pacientes com ceratocone progressivo. MÉTODOS: Vinte e cinco olhos de 20 pacientes (15 homens e 5 mulheres) com ceratocone progressivo, determinado pelo aumento de curvatura em exames seriados de topografia corneal, nos últimos seis meses foram avaliados. Acuidade visual não corrigida (UVA), acuidade visual melhor corrigida com óculos (BSCVA), equivalente esférico (SEQ), cilindro refrativo manifesto e a curvatura máxima (max K) pré e pós-operatórios (1, 3 e 6 meses) foram determinadas. Todos os pacientes foram submetidos ao tratamento Ultra B2 usando riboflavina (vitamina B2) e a luz ultravioleta (UV, 370 nm). O epitélio corneal foi removido após assepsia, colocação de blefarostato e anestesia tópica com proparacaína, por meio de solução de álcool hidratado (20 por cento) utilizada por 30 segundos. A córnea foi saturada com vitamina B2 por 15 minutos; em seguida, foi irradiada por luz UV por 30 minutos. Ao final do procedimento, foi colocada lente de contato terapêutica (LCT), mantida até a epitelização total. RESULTADOS: Houve melhora na UVA após o primeiro mês (de 0,15 ± 0,15 para 0,23 ± 0,20), com contínua mudança no terceiro e sexto mês pós-operatório, atingindo a diferença estatisticamente significante nesse período (p=0,025 e p=0,037 respectivamente). BSCVA melhorou de 0,41 ± 0,27 para 0,49 ± 0,29 no sexto mês, sem atingir a diferença estatisticamente significante. A progressão do ceratocone após o procedimento não foi notada em nenhum paciente, em comparação com o avanço topográfico nos 6 meses precedentes. Após 6 meses do procedimento, max K diminuiu em mais que 2,00 D (de 53,02 ± 8,42 para 50,88 ± 6,05 D), SEQ em menos que 1 D (de -3,27 ± 4,08 para -2,68 ± 3,02 D) e o cilindro refrativo em menos que 0,5 D (de -2,29 ± 1,77 para -1,86 ± 0,92), sem atingir diferença estatisticamente...


PURPOSE: To present early visual and keratometric results for corneal cross-linking with riboflavin and UV irradiation in patients with progressive keratoconus. METHODS: Twenty-five eyes of twenty patients (15 males and 5 females) with a progressive keratoconus in the previous 6 months were followed. Unaided visual acuity (UVA), best spectacle corrected visual acuity (BSCVA), spherical equivalent (SEQ), manifest cylinder, and maximal corneal curvature (max K) values were followed at 1, 3 and 6 months. All patients were submitted to corneal cross-linking using riboflavin (vitamin B2) as the photosensitizer and ultraviolet light (UV, wavelength 370 nm). Epithelium was removed with 20 percent alcohol, cornea was soaked with vitamin B2 for 15 min, and then irradiated with UV light for 30 min, after which a bandage contact lens (BCL) was placed. RESULTS: UVA increased after one month (from 0.15 ± 0.15 to 0.23 ± 0.20), and went on increasing at 3 and 6 months, reaching statistical significance (p=0.025 e p=0.037, respectively). BSCVA increased from 0.41 ± 0.27 to 0.49 ± 0.29 at month six, without reaching statistical significance at any time point. Progression of keratoconus stopped in all patients, in contrast with progression in all of them in the six-month period prior to the surgery. Max K decreased by more than 2 D (from 53.02 ± 8.42 to 50.88 ± 6.05 D), SEQ less that 1 D (from -3.27 ± 4.08 to -2.68 ± 3.02 D), while refractive cylinder decreased less than 0.5 D (from -2.29 ± 1.77 to -1.86 ± 0.92 D), without reaching a statistically significant difference. None of the eyes lost any line of BSCVA, 12 maintained the preoperative BSCVA, 7 gained one line, 5 gained two lines, and 1 patient gained three lines of BSCVA. CONCLUSIONS: Corneal cross-linking with riboflavin and UV light seems to be a safe (no loss of BSCVA) and effective (anatomical and optical properties maintained) procedure, which has shown to stop the progression of the keratoconus...

Corneal transparency is attributed to the regular spacing and diameter of collagen fibrils, and proteoglycans may play a role in fibrillogenesis and matrix assembly. Corneal scar tissue is opaque and this opacity is explained by decreased ultrastructural order that may be related to proteoglycan composition. Thus, the objectives of the present study were to characterize the proteoglycans synthesized by human corneal explants and to investigate the effect of mechanical epithelial debridement. Human corneas unsuitable for transplants were immersed in F-12 culture medium and maintained under tissue culture conditions. The proteoglycans synthesized in 24 h were labeled metabolically by the addition of 35S-sulfate to the medium. These compounds were extracted by 4 M GuHCl and identified by a combination of agarose gel electrophoresis, enzymatic degradation with protease and mucopolysaccharidases, and immunoblotting. Decorin was identified as the main dermatan sulfate proteoglycan and keratan sulfate proteoglycans were also prominent components. When the glycosaminoglycan side chains were analyzed, only keratan sulfate and dermatan sulfate were detected (~50 percent each). Nevertheless, when these compounds were 35S-labeled metabolically, the label in dermatan sulfate was greater than in keratan sulfate, suggesting a lower synthesis rate for keratan sulfate. 35S-Heparan sulfate also appeared. The removal of the epithelial layer caused a decrease in heparan sulfate labeling and induced the synthesis of dermatan sulfate by the stroma. The increased deposit of dermatan sulfate proteoglycans in the stroma suggests a functional relationship between epithelium and stroma that could be related to the corneal opacity that may appear after epithelial cell debridement
