INTRODUCCIÓN: La endometriosis es una patología benigna, dependiente de estrógenos, en la que el tejido que normalmente crece dentro del útero aparece fuera de este. Su localización habitual es en la pelvis, pero en ocasiones puede aparecer en otras áreas, como es el caso de la endometriosis umbilical. OBJETIVO: Familiarizar al ginecólogo con esta patología y entregar una serie de herramientas para diagnosticar, tratar y seguir a las pacientes que la presentan. CASOS CLÍNICOS: Se presentan dos casos clínicos de endometriosis umbilical primaria diagnosticados en el Hospital La Paz, en Madrid (España), entre los años 2018 y 2019. Las pacientes, de 30 y 34 años, consultaron por dolor o sangrado umbilical durante la menstruación. Ninguna tenía antecedentes de patología ginecológica ni cirugía abdominal previa. Tras una exhaustiva exploración física y una ecografía de alta resolución, se decidió extirpar la lesión con la colaboración del servicio de cirugía plástica. En ambos casos, el estudio anatomopatológico confirmó que se trataba de tejido endometriósico. Las dos pacientes presentaron una buena evolución posquirúrgica, sin recidivas hasta la fecha. CONCLUSIONES: La endometriosis umbilical primaria es una patología infrecuente, pero es necesario incluirla en el diagnóstico diferencial de una mujer con un nódulo umbilical. Siempre deben realizarse una exploración física exhaustiva y una ecografía ginecológica, para descartar posibles patologías concomitantes. El tratamiento de elección es la extirpación quirúrgica de la lesión y el diagnóstico final se establece con el estudio anatomopatológico.


INTRODUCTION: Endometriosis is an estrogen-dependent benign pathology in which endometrial tissue develops outside the uterus. Its most frequent location is the pelvis, although it can appear in other areas such as the umbilicum. OBJECTIVE: To familiarize the gynecologist with this pathology and provide a series of tools to diagnose, treat and provide continued care to these patients. CASE REPORTS: Retrospective study of two clinical cases of primary umbilical endometriosis diagnosed at La Paz University Hospital, in Madrid (Spain), between 2018 and 2019. Both patients (30 and 34 years old respectively) presented with pain and/or bleeding around the umbilical area during menstruation. Neither of them had any previous gynecologic conditions or abdominal surgeries. After exhaustive physical examination and a high-resolution ultrasound, lesions were surgically removed in collaboration with the plastic surgery department. In both cases, histology confirmed the presence of endometrial tissue. Both patients made a full recovery after surgery and havent had a recurrence of said lesions. CONCLUSIONS: Primary umbilical endometriosis is an infrequent disease. However, it must be included in the differential diagnosis of umbilical nodes in women. Exhaustive physical examination and gynecologic ultrasound should always be performed to rule out any other pathologies. Surgical removal of the nodes is the preferred treatment, and the final diagnosis is reached through histology.

The objective of this study was to determine the types of calve housing used in dairy farms, the prevalence of umbilical disorders and related risk factors. The 16 farms studied were visited to characterize the types of installation and possible risk factors, as well as information obtained from a questionnaire applied to the farmers. 806 Holstein calves were physically examined, in addition to collecting blood samples for the evaluation of Failures in Passive Immunity Transfer (FPIT), in animals that manifested inflammatory omphalopathies, and were also submitted to ultrasound examination. The prevalence of omphalopathies was assessed by Fisher's test, and multivariate logistic regression to assess risk factors. Eight types of installation were found: tropical house, suspended cage, collective stall, collective picket, Argentinean type, single-story cage, individual stall, and collective picket with chain. Omphalopathies accounted for 6.45% of the calves. Small size farms (up to 99 lactation cows) had high risk for umbilical disorders, ground floor collective calves, without side protection, with sand floor, in closed sheds and without heatstroke were considered risk factors for omphalopathies. Adequate colostrum and umbilical antisepsis are not associated with disease, its appearance being related to the housing conditions of the animals.(AU)


O objetivo deste estudo foi determinar os tipos de alojamento para bezerros leiteiros, a prevalência de onfalopatias e os fatores de risco relacionados. As 16 fazendas estudadas foram visitadas buscando-se caracterizar os tipos de instalação e os possíveis fatores de risco, além de informações obtidas de um questionário aplicado aos fazendeiros. Foram examinados fisicamente 806 bezerros da raça Holandesa, além da coleta de amostras de sangue, para avaliação da falha de transferência de imunidade passiva (FTIP), nos animais que manifestaram onfalopatias inflamatórias, sendo submetidos também ao exame ultrassonográfico. A prevalência das onfalopatias foi avaliada por teste de Fisher, e foi feita regressão logística multivariada a fim de se avaliarem os fatores de risco. Verificou-se oito tipos de instalação: casinha tropical, gaiola suspensa, baia coletiva, piquete coletivo, bezerreiro tipo argentino, gaiola térrea, baia individual e piquete coletivo com corrente. As onfalopatias corresponderam a 6,45% dos bezerros. Os bezerreiros coletivos térreos, sem proteções laterais, com piso de areia, borracha, concreto ou madeira, em galpões fechados, sem insolação, com alta densidade animal, antissepsia umbilical realizada por três dias e FTIP acima de 50% foram considerados fatores de risco para onfalopatias e possuem relação com o bezerreiro, sendo decisivas para evitar essas condições a colostragem e a antissepsia umbilical adequadas.(AU)

Resumen Introducción: El quiste pilonidal del ombligo (QPO) es una entidad muy infrecuente y por eso no es reconocida precozmente. El objetivo de este manuscrito es dar a conocer esta patología. Reporte de casos: Presentamos dos pacientes referidos por un proceso inflamatorio del ombligo, con descarga de mal olor. El primer paciente, luego de dos meses de tratamiento local sin resolución, es intervenido resecando la lesión umbilical, comprobando en ese momento la existencia de contenido piloso en el quiste. En el segundo paciente se plantea el diagnóstico durante la anamnesis y se confirma con el examen físico. En ambos casos se efectuó una resección parcial del ombligo incluyendo la lesión pilonidal. La biopsia confirmó el diagnóstico. El resultado posoperatorio ha sido satisfactorio y sin recaídas. Discusión y Conclusión: Hay escasa literatura relacionada con el QPO. Se proponen el tratamiento conservador y la opción de resección quirúrgica del quiste. En nuestra limitada experiencia se procedió a resecar la lesión y recomendar la depilación de la región periumbilical. No podemos descartar la alternativa de manejo conservador en futuros casos, antes de proponer la cirugía.


Introduction: Umbilical pilonidal sinus (UPS) is a rare condition and is therefore not detected early. The aim of this document is to discuss this pathology. Case report: We present two patients referred due to an inflammatory process of the navel, with the presence of a malodor. The first patient, after two months of unsuccessful local treatment, was intervened by a resection of the umbilical lesion, verifying hair content in the sinus. In the second patient the diagnosis was evident during anamnesis and confirmed with the physical examination. In both cases, a partial resection of the umbilicus was performed, including the pilonidal sinus. Biopsy confirmed the diagnosis. The postoperative result has been satisfactory, without relapse. Discussion and Conclusion: There are not much literature related to UPS. Conservative treatment or surgical resection of the sinus is proposed. In our limited experience we proceeded to a surgical resection of the lesion and recommend eliminating the hairs of the periumbilical region. We cannot rule out the conservative management option in future cases, before proposing surgery.

Pathological changes in the umbilical region are common in calves. Among such alterations, omphalitis is included. This term is used to define inflammation and infection of the external structures of the umbilicus. According to the affected structures, it can be subclassified into omphalophlebitis, omphaloarteritis, omphalourachitis and panvasculitis. These inflammations are usually associated with bacterial infections. There are predisposing conditions that include inadequate handling such as poor hygiene and neglect of primary care. Omphalitis can affect the animal in a multisystemic way, compromising its well-being and bringing economic losses. In treatment, the use of antimicrobials does not always solve the problem. Thus, surgical treatment can be used, which has good results and should be the choice in the disease. The objective of this work is to report 30 cases of omphalitis in calves, submitted to surgical or conservative treatment. Thirty cases of omphalitis in calves treated in the routine of the Veterinary Hospital of the Paranaense University was analyzed. On physical examination, the animals presented fever, apathy, hyporexia or anorexia and increase of umbilical volume, usually with purulent secretion. Some animals had sepsis and arthritis. In animals with sepsis, hyperemia of the episcleral vessels, dehydration and severe apathy were observed. In calves with arthritis, increased joint volume, pain on palpation and lameness were observed. In animals where the owners did not authorize the surgery, treatment was instituted with sulfadoxine and flunixim meglumine. In dehydrated calves, fluid therapy was used. Animals that were surgically treated received the same clinical treatment protocol as non-operated animals. The surgical procedure was performed under general anesthesia and consisted of resection of the affected umbilical structures. Omphalophlebitis was the most common illness. The most frequent complication was sepsis. Calves treated surgically had a higher survival rate (86.66%) than those treated clinically (46.67%). The clinical signs presented by all animals converged with the literature, allowing for clinical diagnosis. Clinical examination is essential for diagnosis in omphalitis cases. Complementary methods include ultrasound, thermography and laparoscopy, which are important to identify changes in intra-abdominal umbilical structures. Accurate diagnosis of the involved structures was only possible in animals surgically, as well as alterations in organs such as the liver and bladder. There is great variability related to the umbilical structures involved, according to initial care, breeds, seasonality or even the method of conception. Unlike what is observed in the literature, in the present study, there was a higher prevalence of omphalophlebitis, demonstrating variability in relation to the umbilical structures involved. Sepsis, observed in 16.7% of cases, results from bacterial ascension of the umbilical structures. Lameness due to polyarthritis was found in 10% of animals. Meningoencephalitis was observed in 3.3%. Hepatic and retroperitoneal abscedation were observed in 6.7% and 3.3% of cases, respectively. Conservative treatment with antibiotics and local antiseptics has a limited effect on this type of condition, which was proven in the present study, since the survival rate was statistically higher in animals surgically treated.(AU)

Resumen: Introducción: el hamartoma fibroso de la infancia (HFI) es un tumor benigno de partes blandas que se presenta generalmente en población infantil y que posee una morfología histológica característica. Objetivo: describir un caso de HFI congénito de características clínicas e histológicas atípicas. Caso Clínico: recién nacido de término, sexo masculino, sin antecedentes mórbidos perinatales, es deriva do a dermatología por placa eritematosa congénita en región umbilical. Estudio histológico evidenció proliferación fusocelular en dermis e hipodermis, de morfología bifásica, con un patrón arremolina do infiltrante y otro de bandas de células fusadas con hábitos fibroblásticos y miofibroblásticos, aso ciada en profundidad a un componente de tejido adiposo maduro. El estudio inmunohistoquímico mostró positividad difusa a CD34 y focalmente para FXIIIa, con ausencia de inmunoreactividad a ac- tina, desmina, MyoD1, S100, HMB45, Melan A y EMA. Fluorescent in situ hybridization (FISH) para platelet-derived growth factor beta (PDGF beta) y para el gen ETV6 negativos, presentes en el dermatofibrosarcoma protuberans congénito y fibrosarcoma infantil, respectivamente. Estos antecedentes, sumado a los hallazgos histológicos previos, apoyaron el diagnóstico de HFI. Se realizó extirpación quirúrgica, sin signos de recidiva durante el seguimiento clínico. Conclusión: es importante consi derar el HFI dentro del diagnóstico diferencial de tumores subcutáneos infantiles, principalmente en población menor de 2 años. Si bien su comportamiento es benigno, presenta similitud con múltiples lesiones benignas y malignas, lo que hace imperativo realizar un estudio histológico exhaustivo ante lesiones clínicas sospechosas.


Abstract: Introduction: Fibrous hamartoma of infancy (FHI) is a benign, soft tissue tumor that usually oc curs in children and has a characteristic histological morphology. Objective: To describe a case of congenital FHI with atypical histological and clinical characteristics. Clinical case: Full-term male newborn, with no perinatal morbid history was referred to dermatology due to a congenital erythe matous plaque in the umbilical region. The histological study showed a fusocelullar proliferation in dermis and hypodermis of biphasic distribution, with an infiltrative, swirling pattern and bundles of spindle fibroblast-like and myofibroblast-like cells, associated in depth with a mature adipose tissue component. The immunohistochemical study revealed diffuse positivity for CD34, and focal posi tivity for FXIIIa, without immunoreactivity for actin, desmin, MyoD1, S100, HMB45, Melan-A, or EMA. Fluorescent in situ hybridization (FISH) was negative for platelet-derived growth factor recep tor beta (PDGFR-beta) and for ETV6 gene. PDGFR-beta and ETV6 gene are present in congenital dermatofibrosarcoma protuberans and infantile fibrosarcoma, respectively. This history, in addition to previous histological findings, supported the diagnosis of FHI. Surgical resection was performed, without signs of recurrence during clinical follow-up. Conclusion: It is important to consider the FHI within the differential diagnosis of subcutaneous tumors in children, especially in those under two years of age. Although its behavior is benign, it is similar to multiple benign and malignant le sions, which makes it imperative to perform a histological study in front of suspicious clinical lesions.

Abstract: Primary cutaneous endometriosis is a rare condition. It appears without a prior history of surgical procedure and the umbilicus is the most frequently involved area. Primary umbilical endometriosis, or Villar's nodule, usually presents as a painful nodule. Its differential diagnosis may be challenging. Although histopathological assessment represents the gold standard for diagnosis, cutaneous ultrasonography may be useful in guiding the surgical treatment. Ultrasonographic features of cutaneous endometriosis have not yet been fully explored in the literature. Hence, we report peculiar ultrasonographic findings of primary umbilical endometriosis

ABSTRACT Objective: to report the characteristics, evolution and outcome of patients with primary umbilical endometriosis. Methods: an observational and descriptive study of patients with primary umbilical endometriosis diagnosed between 2014 and 2017. The clinical variables evaluated were age, clinical picture, lesion characteristics, diagnostic methods, treatment and recurrence. Results: six patients diagnosed with primary umbilical endometriosis aged 28 to 45 years were operated on during the study period. They had lesions ranging from one to 2.5cm in diameter, violet in five patients and erythematous-violaceous in one. The duration of the symptoms until diagnosis ranged from one to three years and in all the cases studied the diagnosis was made through the clinical manifestations and confirmed by histopathological analysis. No case was associated with neoplastic alterations. All patients evaluated had pain and umbilical bleeding in the menstrual period. Conclusion: umbilical endometriosis is an uncommon disease and should be included in the differential diagnosis of women as umbilical nodules. The treatment of choice is the total exeresis of the lesion.


RESUMO Objetivo: relatar as características, evolução e desfecho de pacientes portadoras de endometriose umbilical primária. Métodos: estudo observacional e descritivo de pacientes portadoras de endometriose umbilical primária diagnosticada entre 2014 e 2017. As variáveis clínicas avaliadas foram: idade, quadro clínico, características das lesões, métodos diagnósticos, tratamento e recidiva. Resultados: seis pacientes com diagnóstico de endometriose umbilical primária, com idades entre 28 e 45 anos foram operadas no período do estudo. Elas apresentavam lesões que variavam de 1,0cm a 2,5cm de diâmetro, de cor violácea em cinco pacientes e eritemato-violácea em uma. O tempo de duração dos sintomas até o diagnóstico variou de um a três anos e em todos os casos estudados o diagnóstico foi feito por meio das manifestações clínicas e confirmado por meio da análise histopatológica. Nenhum caso foi associado com alterações neoplásicas. Todas as pacientes avaliadas apresentavam como manifestação clínica dor e sangramento umbilical no período menstrual. Conclusão: a endometriose umbilical é uma doença pouco frequente e deve ser incluída no diagnostico diferencial de mulheres como nódulo umbilical. O tratamento de eleição é a exérese total da lesão.

Introdução: Endometriose primária de umbigo é uma afecção rara que acomete mulheres em idade fértil. A endometriose umbilical espontânea apresenta-se com nódulo em umbigo, dor cíclica, sangramento local discreto durante o período menstrual, sem história de cirurgia prévia. O tratamento consiste na excisão em bloco da lesão e cordão umbilical, com margens para evitar recorrência e a reconstrução umbilical imediata. Métodos: Apresentamos a técnica cirúrgica para excisão circular do endometrioma umbilical e a neo-onfaloplastia em um só estágio. Conclusão: A possibilidade de endometriose umbilical deve ser considerada quando da presença de nodulações e hemorragias umbilicais, mesmo sem cirurgia prévia. A técnica cirúrgica proporciona remissão total da lesão e uma cicatriz umbilical de aspecto natural.


Introduction: Umbilical primary endometriosis is a rare condition that affects women of childbearing age. Spontaneous umbilical endometriosis presents lump in the navel, cyclic pain, discreet local bleeding during menstruation, with no history of previous surgery. The treatment is block excision of the lesion and umbilical cord with edges margin to avoid recurrence and immediate umbilical reconstruction. Methods: We describe the surgical technique for circle excision of the umbilical endometrioma and neo-omphaloplasty in one stage. Conclusion: The possibility of umbilical endometriosis should be considered when the presence of nodules and umbilical bleeding, even without previous surgery. The surgical technique provides total remission of the lesion and an umbilical natural scar.

As afecções umbilicais merecem destaque entre as enfermidades que acometem bezerros no primeiro mês de vida, pela alta incidência durante essa fase da vida dos neonatos e pelas relevantes perdas econômicas, decorrentes da mortalidade, custos de tratamento e atendimento veterinário, além de sequelas que poderão determinar menores ganho de peso e produção leiteira. O diagnóstico dessas enfermidades muitas vezes é prejudicado pela dificuldade em se detectar os acometimentos das estruturas intra-abdominais do umbigo, o que afetará negativamente a escolha do tratamento mais adequado e o estabelecimento do real prognóstico para cada animal. No presente estudo, realizou-se a análise ultrassonográfica dos animais com inflamação/infecção dos componentes umbilicais sendo possível observar algumas características em relação a mensurações e ecogenicidade, presentes nos componentes umbilicais acometidos. A partir dessas avaliações concluiu-se que a medida mais adequada para identificar animais com possíveis processos inflamatórios nos componentes umbilicais é a espessura da parede dos vasos umbilicais quando comparada ao diâmetro dos mesmos. Ainda, o estudo possibilitou verificar algumas particularidades do comportamento dos componentes do cordão umbilical em bezerros sadios da raça Holandesa, bem como determinar que os vasos umbilicais intra-abdominais iniciam uma involução em porção mais interna e progridem para as extremidades próximas ao anel umbilical com o evoluir da idade. Destaca-se que o confronto desses resultados com aqueles poucos descritos na literatura disponível mostrou-se com notáveis diferenças.(AU)


Umbilical disorders deserve mention among the illnesses affecting calves in the first month of life. The high incidence during this phase of life of newborns and relevant economic losses arising from mortality, cost of treatment and veterinary care, beyond sequelae may determine lower weight gain and milk production. The diagnosis of these diseases is often hampered by the difficulty to detect the involvement of intra-abdominal umbilical structures, which negatively affect the choice of the most appropriate treatment and the actual outcome for each calf. In the present study, we performed ultrasound examinations of calves with inflammation/infection in the umbilical components and observed some characteristics related with measurements and echogenicity present in umbilical affected components. From these evaluation it was concluded that the wall thickness of the umbilical vessels is the more reliable standard to determine changes in these components, as compared with that of the diameter of the umbilical vessels. Furthermore, from this study we observed some peculiarities of involution of the intra-abdominal umbilical cord components in healthy Holstein calves used for the control group to characterize the behavior of the umbilical structures during the progress of the age in the newborns. It is noteworthy that the comparison of these results with those found in the rare literature available showed notable differences.(AU)

El cordón umbilical, tan vital en la vida intrauterina, no tiene una función fisiológica en el ser humano después del nacimiento. Entre los primeros 10-20 días de vida del recién nacido requiere de cuidados que el pediatra y personal de salud que realiza los controles debe indicar a la madre para asegurar que el proceso de desecación y eliminación del cordón se complete de forma apropiada. Sin embargo, aun cuando la madre haya seguido correctamente las indicaciones, existen posibilidades de que la evolución clínica no sea la normal y la cicatrización se retarde o complique. Las posibilidades que el clínico debe sospechar son bastante variadas dentro de un espectro que va desde patologías leves hasta aquellas más complejas que incluyen alteraciones del desarrollo intrauterino con resultado de malformaciones congénitas. El propósito de este artículo es revisar la literatura publicada al respecto y actualizar aspectos clínicos importantes que el pediatra o médico que atiende niños debe conocer para orientar correctamente el diagnóstico y oportunidad terapéutica.


The umbilical cord, is a vital structure in utero, but it has no one physiological function in humans after birth. Among the first 10-20 days old newborn baby requires care pediatrician and health staff should learn to the mother how she must be care to make the drying process and complete removal of the cord can be appropriate. However, even if the mother has correctly followed the instructions, there are chances that the clinical course than the normal healing is delayed or complicated. The chances that the clinician should suspect are quite varied within a spectrum from mild to the most complex pathologies including disturbances of intrauterine development resulting in congenital malformations. The purpose of this article is to review the published literature and update important clinical aspects pediatrician or physician who treats children should learn to correctly orient the diagnosis and therapeutic opportunity.

Pilonidal sinus is common in the sacrococcygeal region. It is rare at the umbilicus. Incidence being 0.6%, we report a case of umbilical pilonidal sinus treated in our hospital. A 20-year-old male patient was admitted to our hospital with complaints of purulent discharge from the umbilicus since 3 months. Patient was posted for omphalectomy. Intraoperatively, a tuft of hair was found in a sinus tract communicating with the umbilicus. Histopathological evaluation was compatible with pilonidal sinus disease. There are only a few reports of the umbilical pilonidal sinus in the literature. Umbilical sinus tract differs from the sacrococcygeal variety in the absence of multiple tracts and low recurrence rates. Treatment options and differential diagnosis are discussed in the article.

Umbilical endometriosis is an important differential diagnosis of any umbilical lesion. A 35-yearold type 2 diabetic woman presented with intermittent umbilical discharge which failed to respond to various antibiotics. An ultrasound scan and MRI scan failed to show any obvious abnormality. The umbilicus was excised and histology confirmed endometriosis. Surgical excision provides a definitive diagnosis and curative treatment for isolated endometriosis

The umbilicus can be affected by various skin diseases including metastasis of internal malignancies, at which point it is known as Sister Mary Joseph's Nodule. The authors report a case of Sister Mary Joseph's Nodule as the first sign of internal malignancy and highlight the importance for the dermatologist to recognize the condition.

O pseudoxantoma elástico perfurante periumbilical (PEPP), ou elastose cálcica perfurante, é distúrbio raro e sua patogênese está associada a alteração da fibra elástica, tendo o trauma provável participação. Apresenta-se caso de paciente de 70 anos com quadro de máculas enegrecidas e pápulas encimadas por crostas na região periareolar e periumbilical há mais de dez anos. O exame histopatológico revelou fibras elásticas alteradas, repletas de cálcio, e formação de pertuito na derme com fibras elásticas degradadas no seu interior. O caso descrito corresponde ao PEPP, que nesta paciente também acomete área periareolar bilateralmente.


The periumbilical perforating pseudoxanthoma elasticum or perforating calcific elastosis is a rare disorder and its pathogenesis is associated with the alteration of elastic fibers, which may probably result from trauma. We present the case of a 70-year-old woman with long-time blackish maculas and keratotic surface papules on the periareolar and periumbilical regions. The histopathological examination revealed altered elastic fibers, replete with calcium and formation of a path in the dermis with elastic fibers degraded in their interior, confirming the clinical diagnosis.

Alguns problemas têm sido observados nos bezerros produtos da técnica fertilização in vitro, dentre esses a elevada casuística de onfalopatias. A partir dessa observação, objetivou-se com este trabalho realizar um estudo retrospectivo da correlação entre os métodos de concepção e a ocorrência de onfalopatias em bovinos e descrever os resultados obtidos a partir dos tratamentos conservativo e cirúrgico. Foram utilizados 44 bovinos atendidos no Hospital Veterinário da Unesp, Campus de Araçatuba, com idade variando de um dia a 12 meses entre os anos de 2003 e 2007. Desses bovinos 27 eram provenientes de fertilização in vitro (FIV), 12 de inseminação artificial (IA), dois de monta natural (MN) e três de transferência de embriões (TE). O diagnóstico clínico-cirúrgico revelou que todos apresentavam afecções umbilicais, sendo 22 casos de persistência de úraco, oito de onfaloflebite, oito de hérnias umbilicais, cinco de onfalites e um de fibrose umbilical. Inicialmente e no pós-operatório administrou-se em todos os animais, uma vez ao dia, durante dez dias, 3mg/kg de ceftiofur sódico pela via intravenosa (IV). Nos casos de infecção grave ou irresponsiva a terapia antimicrobiana inicial, acrescentou-se 6,6mg/kg, durante sete dias de sulfato de gentamicina IV. A antissepsia do umbigo, com tintura de iodo a 2 por cento, foi instituída duas vezes ao dia, nos casos tratados conservativamente, enquanto que os bovinos submetidos à cirurgia receberam 1,1mg/kg de flunixin meglumine IV, uma vez ao dia, por cinco dias consecutivos. Dos 22 animais diagnosticados com persistência de úraco, 10 apresentavam drenagem de urina pelo umbigo e receberam 2mL de tintura de iodo 10 por cento no interior do úraco, sendo 15 tratados com a excisão cirúrgica, especialmente, devido à formação de divertículo vésico-uracal. Todos os animais que apresentavam onfaloflebite e hérnia umbilical foram submetidos à cirurgia...


Some problems have been observed in bovine products of the in vitro fertilization technical, among these, the high umbilical diseases casuistry. From this observation, the aim of this work was to accomplish a retrospective study of the correlation between the conception methods and the occurrence of umbilical diseases in bovines and describe the results obtained from the conservative and surgical treatments. For this 44 animals attended at Veterinary Teaching Hospital of Unesp Araçatuba, with age varying from one day to 12 months among the years of 2003 and 2007 were used, twenty seven were from in vitro fertilization (IVF), twelve were from artificial insemination (AI), two were from natural mounts and three were from embryo transfer. The clinical-surgical diagnosis of the animals revealed that all of them presented umbilical disorders, being 22 cases of urachus patent, eight omphalophlebitis, eight umbilical hernia, five omphalitis and one umbilical fibrosis. Before and at the postoperative period was administered in all animals once a day for ten days 3mg/kg of ceftiofur sodium IV. In cases of serious infection or lack of response of the initial antimicrobial therapy a complementary treatment during seven days with 6.6 mg/kg of gentamicin sulfate IV was carried out. The antisepsis of the umbilicus with tincture of iodine to 2 percent was carried out twice daily in cases treated clinically, with out surgery, while the animals underwent surgery received 1.1mg/kg of flunixin meglumine IV once daily for five consecutive days.Among the 22 animals diagnosed with urachus patent, 10 had urine drainage from the umbilicus and received 2mL of 10 percent tincture of iodine in the urachus, and 15 was treated with surgical excision, due to formation of diverticulum from the blader to urachus. Objective : The aim of this analysis was to evaluate our management and outcome of eight cases of Sister Mary Joseph's Nodule of the umbilicus (SMJN). Methods : Between 2002 and 2009, we collected eight cases of SMJN and their clinical data were studied in a retrospective manner. Results : The mean age of the patients was 50.6 years (range, 34-58). There were three males and five females. Five cases were discovered preoperatively during an abdominal examination and in the remaining three patients, the umbilical nodule was noticed during surgery. Most patients presented with abdominal pain. Various signs were discovered during clinical examination. Exhaustive investigations done included ultrasound, computed tomography, magnetic resonance imaging, endoscopic retrograde cholangiopancreatography, magnetic resonance cholangiopancreatography, gastrointestinal endoscopy, mammogram, bone scan, and a panel of tumor markers. The primary tumors were gastric adenocarcinoma (two patients), ovarian adenocarcinoma (two patients), pancreatic carcinoma (one patient), colonic adenocarcinoma (one patient), gallbladder adenocarcinoma (one patient), and cholangiocarcinoma (one patient). As the disease was advanced and metastatic in all patients, only palliative therapy was offered. All patients died within a short period after clinical presentation, ranging from four to 58 weeks (mean, 20 weeks). Conclusion : SMJN is a rare manifestation of a variety of advanced malignancies, usually of gastrointestinal origin. A majority of the patients present at a late stage and many harbor distant metastases. The patient's survival is very short (mean, 20 weeks) leading to a poor outcome. We believe that if the primary cancer is discovered at an early stage, the prognosis may improve.
