ABSTRACT
Doctors make decisions all the time. Occasionally they are simple, easy and definitely correct. But we almost always have to deal with a certain degree of uncertainty that causes us to question whether or not we have made the best decision, until the final result appears. Sometimes, to these decisions are added several factors (causal, confusing or distracting) that can determine an unfavorable outcome for the patient, for the system or for both. The systematic evaluation, not only of the results, but also of the process that led to them, is one of the most valuable tools for continuous improvement in medical practice and patient care
